A prospective study:intraoperative ^(125)I radioactive seed implant therapy in advanced esophageal squamous cell carcinoma

Objective： To investigated the role of intraoperative iodine-125 （125I） brachytherapy as a treatment option for advanced thoracic esophageal squamous cell carcinoma （ESCC）. Methods： Using preoperative computed tomography （CT）-based staging criteria, between 2000 and 2008, 298 patients with ESCC （stage Ⅱ-Ⅲ） were enrolled in this prospective study. With informed consent, patients were randomized into two groups： intraoperative ^125^I seed implantation and surgery alone （control group）. Twenty to forty 125I seeds （0.5 mCi per seed）, with a total activity in 10-30 mCi, and a matched peripheral dose （MPD） of 60-70 Gy, were implanted under direct visualization. The surgical procedure used in this study was either a radical resection, which involved an esophagectomy through a left thoracotomy with two-field lymphadenectomy, or palliative resection. The postoperative complications were observed and recorded. The location and quality assessment of J25I seeds were assessed using CT scans or X-ray imaging. The short-term efficacy was evaluated according to WHO criteria. The 1, 3, 5 and 7-year survival rates were determined on follow-up. Results： There was no displacement or loss of 125I seeds. The local recurrence rates in the intraoperative 125I seed implantation group and control group were 14.9% and 38.7%, respectively （P 〈 0.05）. An objective response rate of 92% was observed in the seed implant group, which was significantly higher than 0% in the control group （P 〈 0.05）. There was no significant difference between the two groups when comparing of complications （P 〉 0.05）. The 1-year survival rate of the two groups were not significantly different （P 〉 0.05）. However, the 3, 5 and 7-year survival rates in the united 125I group （64%, 55.3% and 8%, respectively） were statistically different from those in the control group （52%, 29.1% and 1.4%,respectively）（P 〈 0.05）. Conclusion： Intraoperative 125I seed implantation is safe and effective for advanced ESCC. Seed implantation

long-term efficacy of perioperative chemoradiotherapy on esophageal squamous cell carcinoma

AIM: To investigate the role of perioperative chemoradiotherapy (CRT) in the treatment of locally advanced thoracic esophageal squamous cell carcinoma (ESCC). METHODS: Using preoperative computed tomography (CT)-based staging criteria, 238 patients with ESCC (stage ⅡⅢ ) were enrolled in this prospective study between January 1997 and June 2004. With informed consent, patients were randomized into 3 groups: preoperative CRT (80 cases), postoperative CRT (78 cases) and surgery alone (S) (80 cases). The 1-, 3-, 5and 10-year survival were followed up. Progressionfree survival (PFS) was chosen as the primary endpoint by treatment arm measured from study entry until documented progression of disease or death from any cause. The secondary endpoint was overall survival (OS) determined as the time (in months) between the date of therapy and the date of death. Other objectives were surgical and adjuvant therapy ***: With median follow-up of 45 mo for all the enrolled patients, significant differences in the 1-, 3-, 5-, 10-year OS (91.3%, 63.5%, 43.5%, 24.5% vs 91%, 62.8%, 42.3%, 24.4% vs 87.5%, 51.3%, 33.8%, 12.5%, P = 0.0176) and PFS (89.3%, 61.3%, 37.5%, 18.1% vs 89.1%, 61.1%, 37.2%, 17.8% vs 84.5%, 49.3%, 25.9%, 6.2%, P = 0.0151) were detected among the 3 arms. There were no significant differences in OS and PFS between the preoperative CRT and postoperative CRT arm (P > 0.05). For the patients who had radical resection, significant differences in median PFS (48 mo vs 61 mo vs 39.5 mo, P = 0.0331) and median OS (56.5 mo vs 72 mo vs 41.5 mo, P = 0.0153) were detected among the 3 arms, but there were no significant differences in OS and PFS between the preoperative CRT and postoperative CRT arm (P > 0.05). Effect of neoadjuvant chemoradiotherapy on prognosis and surgery for esophageal carcinoma

The dynamic characteristics of different airflows on micro-scales have been explored from many perspectives since the late *** the one hand,most analytical tools and research subjects in previous contributions vary significantly:some only focus on fluctuant velocity features,while others pay attention to directional *** the other hand,despite the wide variety of existing analytical methods,they are not systematically classified and *** paper aims to establish a system including state-of-the-art tools for airflow analysis and to further design a holistic toolkit named Airflow Analytical Toolkit(AAT).The AAT contains two tools,responsible for analyzing the velocity and direction characteristics of airflows,each of which is integrated with multiple analytical *** assess the performance of the developed toolkit,we further take typical natural and mechanical winds as cases to show its excellent analytical *** the help of this toolkit,the great differences in velocity and directional characteristics among different airflows are *** comparative results reveal that not only is the velocity of natural wind more fluctuating than that of mechanical wind,but its incoming flow direction is also more *** AAT,serving as a powerful and user-friendly instrument,will hopefully offer great convenience in data analysis and guidance for a deeper understanding of the dynamic characteristics of airflows,and further remedy the gap in airflow analytical tools.

Upconverting and downshifting circularly polarized luminescence in a superstructural helix

The modulation of circularly polarized luminescence(CPl) has become an increasingly prominent area of research. In this study,we propose a unique “one-excitation-to-three-emissions” CPl system through the co-assembly of an upconversion system,incorporating one sensitizer(Pt(II) mesotetraphenyl tetrabenzoporphyrine, Pt TPBP) and two annihilators(R/S-DPA and R/SBDP) within liquid crystals. The chiral nature of the annihilators induces a transformation of the nematic liquid crystal into chiral nematic liquid crystals(N*lC), establishing an excellent chiral matrix. Upon the incorporation of the sensitizer Pt TPBP and subsequent excitation at 635 nm, the system demonstrates two independent triplet–triplet annihilation photon upconversion(TTA-UC) with the emission in blue and yellow, aided by thermally activated triplet–triplet energy transfer(TTET). This is accompanied by the simultaneous generation of upconverting circularly polarized luminescence(UC-CPl) and downshifting near-infrared circularly polarized luminescence(DS-CPl) originating from the residual luminescence of Pt TPBP. Remarkably,fine-tuning the ratio between the two annihilators allows the TTA-UC system to exhibit multicolor CPl emission with an amplified luminescence dissymmetry factor(glum, reaching up to 0.6). Our study unveils a previously unreported “one-excitation-to-three-emissions” system and provides a versatile strategy for modulating CPl emissions, surpassing conventional methodologies.

Association between Bmi1 and clinicopathological status of esophageal squamous cell carcinoma

AIM： To investigate the clinicopathological roles of Bmil in esophageal squamous cell carcinoma （ESCC）.METHODS： Quantitative real-time polymerase chain reaction and immunohistochemical staining for Broil were performed in cancerous and adjacent non-cancerous paraffin-embedded esophageal ***： The Bmil expression level was unaffected by gender and age. The level of Broil mRNA in ESCC was significantly higher than that in the adjacent non-cancerous tissues （2.181 ± 2.158 vs 0.931 ± 0.894, P = 0.0152）, and its over-expression was aggressively associated with lymph node metastasis （3.580 ± 2.487 vs 1.703 ± 0.758, P = 0.0003）, poorer cell differentiation （P = 0.0000） and advanced pathological stage （3.827± 2.673 vs 1.590 ± 0.735, P = 0.0001）. The patients were divided into high-expression and low-expression groups based on the median expression level of Bmi1 mRNA, and a shorter overall survival time in the former group was observed. Immunohistochemistry for Bmi1 oncoprotein showed diffusely positive, focally positive and negative expression in 44, 16 and 10 of 70 ESCC cases, respectively, compared with three, two and five of 10 adjacent non-cancerous cases （P = 0.027）. The positive rate of the oncoprotein in samples of histological grade Ⅲ was higher than that of grade Ⅱ（P = 0.031）, but its expression had no relation to the lymph node metastasis and pathological staging. In 70 ESCC samples, Bmi1 showed high intense expression in the cytoplasm and less or even no expression in the ***： Bmi1 was over-expressed in ESCC. Increased Bmi1 mRNA expression was significantly associated with ESCC progression, and the oncoprotein was largely distributed in the cytoplasm of tumor cells.
