AIM:To evaluate the trends in the eradication rate of Helicobacter pylori(***) over the past 11 years in a single ***:This retrospective study covered the period from January 2000 to December *** evaluated 5746 patients diagnosed with gastric ulcers(GU),duodenal ulcers(DU),GU + DU,or nonpeptic ulcers associated with an *** *** treated them annually with the 2 wk standard first-line triple regimen,proton pump inhibitor(PPI) + amoxicilin + clarithromycin(PAC;PPI,clarithromycin 500 mg,and amoxicillin 1 g,all twice a day).The follow-up test was performed at least 4 wk after the completion of the 2 wk standard *** eradication using the PAC *** also assessed the eradication rates of 1 wk second-line therapy with a quadruple standard regimen(PPI b.i.d.,tripotassium dicitrate bismuthate 300 mg q.i.d.,metronidazole 500 mg t.i.d.,and tetracycline 500 mg q.i.d.) after the failure of the first-line *** analysis was performed with 95%CI for the differences in the annual eradication ***:A total of 5746 patients [2333 males(58.8%),1636 females(41.2%);mean age of males vs females 51.31 ± 13.1 years vs 52.76 ± 13.6 years,P < 0.05,total mean age 51.9 ± 13.3 years(mean ± SD)] were *** these patients,1674 patients were excluded:35 patients refused treatment;18 patients ceased *** eradication due to side effects;1211 patients had inappropriate indications for *** eradication,having undergone stomach cancer operation or chemotherapy;and 410 patients did not undergo the *** also excluded 103 patients who wanted to stop eradication treatment after only 1 wk due to poor compliance or the side effects mentioned ***,we evaluated the annual eradication success rates in a total of 3969 patients who received 2 wk first-line PAC *** endoscopic and clinical findings in patients who received the 2 wk PAC were as follows:gastric ulcer in 855(21.5%);duodenal ulcer in 878(22.1%);

Aortic arch calcification on chest X-ray combined with coronary calcium score show additional benefit for diagnosis and outcome in patients with angina

Background The coronary artery calcium （CAC） and aortic arch calcification （AoAC） are individually associated with cardiovascular disease and outcome. This study investigated the predictive value of AoAC combined with CAC for cardiovascular diagnosis and outcome in patients with angina. Methods A total of 2018 stable angina patients who underwent chest X-ray and cardiac multi-detector computed tomography were followed up for four years to assess adverse events, which were categorized as cardiac death, stroke, myocardial infarction, or repeated revascularization. The extent of AoAC on chest X-ray was graded on a scale from 0 to 3. Results During the four years of fol- low-up, 620 patients were treated by coronary stenting and 153 （7%） adverse events occurred. A higher grade of AoAC was associated with a higher CAC score. Cox regression showed that the CAC score, but not AoAC, were associated with adverse events. In patients with CAC score 〈 400, AoAC showed an additive predictive value in detecting significant coronary artery disease （CAD）. A gradual increases in the risk of adverse events were noted if AoAC was present in patients with similar CAC score. Conclusions As AoAC is strongly correlated with the CAC score regardless of age or gender, careful evaluation of CAD would be required in patients with AoAC on conventional chest X-rays.

Primary biliary cholangitis(PBC) is an idiopathic autoimmune liver disease characterized by chronic cholestasis and destruction of the intrahepatic bile ducts. Similar to other autoimmune diseases, the pathogenesis of PBC is considered to be a complex etiologic phenomenon involving the interaction of genetic and environmental factors. Although a number of common variants associated with PBC have been reported from genome-wide association studies, a precise genetic mechanism underlying PBC has yet to be identified. Here, we describe a family with four sisters who were diagnosed with PBC. After the diagnosis of the index patient who was in an advanced stage of PBC, one sister presented with acute hepatitis, and two sisters were subsequently diagnosed with PBC. Notably, one half-sister with a different mother exhibited no evidence of PBC following clinical investigation. Our report suggests the possibility of a maternal inheritance of PBC susceptibility. Moreover, judging from the highpenetrance of the disease observed in this family, we inferred that a pathogenic genetic variant might be the cause of PBC development. We describe a family that exhibited diverse clinical presentations of PBC that included asymptomatic stages with mildly increased liver enzyme levels and symptomatic stages with acute hepatitis or advanced liver fibrosis. Additional studies are needed to investigate the role of genetic factors in the pathogenesis of this rare autoimmune disease.

Prevalence and prognosis of acute pancreatitis in critically ill patients with COVID-19

Background:Coronavirus disease 2019(COVID-19)is a global pandemic *** addition to the well-known respiratory and fever symptoms,gastrointestinal symptoms have also been *** study aimed to evaluate the prevalence and prognosis of patients with COVID-19 infection complicated with acute pancreatitis in intensive care unit(ICU).Methods:This was a retrospective observational cohort study,and patients aged 18 years or older,ad-mitted into the ICU in a single tertiary center from January 1,2020,to April 30,2022 were *** were identified by electronic medical records and reviewed *** primary outcome was the prevalence of acute pancreatitis among ICU patients with *** secondary outcomes were the length of hospital stay,need for mechanical ventilation(MV),need for continuous renal replacement therapy(CRRT),and in-hospital ***:A total of 4133 patients,admitted into the ICU,were *** these patients,389 were infected by COVID-19,and 86 were diagnosed with acute ***-19 positive patients were more likely to present with acute pancreatitis than COVID-19 negative patients(odds ratio=5.42,95%confidence interval:2.35-6.58,P<0.01).However,the length of hospital stay,need for MV,need for CRRT,and in-hospital mortality were not significantly different between acute pancreatitis patients with and without COVID-19 ***:Severe COVID-19 infections may cause acute pancreas damage in critically ill ***-ever,the prognosis may not differ between acute pancreatitis patients with and without COVID-19 infec-tion.

Endoscopic submucosal dissection for early gastric cancer with undifferentiated-type histology:A meta-analysis

AIM: To evaluate the efficacy and safety of endoscopicsubmucosal dissection(ESD) for early gastric cancer(EGC) with undifferentiated-type ***: A systematic literature review was conducted using the core databases. Complete resection,curative resection, en bloc resection, recurrence and adverse event rate were extracted and analyzed. A random effect model was applied. The methodological quality of the enrolled studies was assessed using the Newcastle-Ottawa Scale. Publication bias was evaluated using a funnel plot, the trim and fill method, Egger's test,and a rank correlation ***: Fourteen retrospective studies between2009 and 2014 were identified(972 EGC lesions with undifferentiated-type histology). The total en bloc and complete resection rates were estimated as92.1%(95%CI: 87.4%-95.2%) and 77.5%(95%CI:69.3%-84%), respectively. The total curative resection rate was 61.4%(95%CI: 44.5%-75.9%). The overall recurrence rate was 7.6%(95%CI: 3.4%-16%).Limited to histologically diagnosed expanded-criteria lesions, the en bloc and complete resection rates were91.2% and 85.6%, respectively. The curative resection rate was 79.8%.CONCLUSION: In this analysis, ESD is a technically feasible treatment modality for EGC with undifferentiatedtype histology. Long-term studies are needed to confirm these therapeutic outcomes.

Prominent cycling reversibility and kinetics enabled by CaTiO3 protective layer on Zn metal for aqueous Zn-ion batteries

Aqueous Zn-ion batteries （AZIBs） have received considerable attention owing to their various advantages such as safety,low cost,simple battery assembly conditions,and high ionic ***,they still suffer from serious problems,including uncontrollable dendrite growth,corrosion,hydrogen evolution reaction （HER） from water decomposition,electrode passivation,and unexpected *** creation of a uniform artificial nanocrystal layer on the Zn anode surface is a promising strategy for resolving these ***,we propose the use of a perovskite CaTiO3（CTO） protective layer on Zn（CTO@Zn） as a promising approach for improving the performance of *** CTO artificial layer provides an efficient pathway for Zn ion diffusion towards the Zn metal because of the high dielectric constant （εr=180） and ferroelectric characteristics that enable the alignment of dipole moments and redistribute the Zn2+ions in the CTO *** avoiding the direct contact of the Zn anode with the electrolyte solution,the uneven dendrite growth,corrosion,parasitic side reactions,and HER are mitigated,while CTO retains its mechanical and chemical robustness during ***,CTO@Zn demonstrates an improved lifespan in a symmetric cell configuration compared with bare ***@Zn shows steady overpotential （～68 m V） for 1500 h at 1 mA cm-2/0.5 mA h cm-2,excelling bare ***,when paired with the V2O5-C cathode,the CTO@Zn//V2O5-C full battery delivers 148.4 mA h g-1（based on the mass of the cathode） after 300 *** study provides new insights into Zn metal anodes and the development of high-performance AZIBs.

Effect of friction on the contact stress of a coated polymer gear

Polymer gears are used extensively in various ***,durability issues have been emerging because of friction at gear tooth contact *** extend the lifetime of polymer gears,a low-frictional coating has been considered as a possible *** this study,a finite element simulation method was performed to investigate the contact stress between a pair of coated polymer *** simulation included various friction coefficients(COFs)for studying the effects of friction during the *** results revealed that the friction causes the contact stress to shift over the roll angle,which is attributed to the direction of the sliding friction based on a free-body *** also investigated the effects of coating and found that a thin coating has little effect on the bulk deformation behavior of the ***,the stress distribution in the coating at the pitch point was investigated as the COF *** zero friction,three notable stress regions were observed:1)the center of the surface,2)the end of the contact,and 3)the overall contact *** COF was increased in the micro-slip region of the contact interface,both tensile and compressive stresses in the coating *** study provides significant aid to engineers for understanding the stress response of the coating applied to polymer gears to achieve an optimal design.

In optics,the refractive index of a material and its spatial distribution determine the characteristics of light ***,exploring both low-and high-index materials/structures is an important consideration in this *** cavities,which are defined as low-index bases,exhibit a variety of unusual or even unexplored optical characteristics and are used in numerous functionalities including diffraction gratings,localised optical antennas and low-loss *** this report,we discuss the fabrication of hollow cavities of various sizes(O.2-5μm in diameter)that are supported by conformal dielectric/metal shells,as well as their specific applications in the ultraviolet(photodetectors),visible(light-emitting diodes,solar cells and metalenses),near-infrared(thermophotovoltaics)and mid-infrared(radiative coolers)*** findings demonstrate that hollow cavities tailored to specific spectra and applications can serve as versatile optical platforms to address the limitations of current optoelectronic ***,hollow cavity embedded structures are highly elastic and can minimise the thermal stress caused by high *** such,future applications will likely include high-temperature devices such as thermophotovoltaics and concentrator photovoltaics.

Real-world utility of serological tests in patients with suspected scrub typhus in the Republic of Korea:A single-center,retrospective,observational study

Objective:Serological tests are widely used for scrub typhus diagnosis;however,their limitations are *** study aims to assess their practical value in clinical ***:We analyzed the data of adult patients with suspected scrub typhus who visited a tertiary care hospital in the Republic of Korea from September to December from 2019 to *** included patients had an acute fever and at least one of the following ten secondary findings:myalgia,skin rash,eschar,headache,thrombocytopenia,increased liver enzyme levels,lymphadenopathy,hepatomegaly,splenomegaly,and pleural *** diagnoses were grouped as scrub typhus or other diseases by two infectious disease ***:Among 136 patients who met the eligibility criteria,109 had scrub typhus and 27 had different *** and paired total antibodies using immunofluorescence assay(IFA),and total antibodies using immunochromatography-based rapid diagnostic testing(ICT)were measured in 98%,22%,and 75%of all patients,*** using paired samples for scrub typhus was established at a median of 11[interquartile range(IQR)10-16]days following the first *** the 82 admitted patients,the median admission time was 9(IQR 7-13)*** to IFA,58(55%)patients with scrub typhus had total immunoglobulin titers≥1:320,while 23(85%)patients with other disease had titers<1:*** ICT results were observed in 64(74%)patients with scrub typhus and 10(67%)patients with other diseases showed negative ICT ***:Serological testing for scrub typhus is currently insufficient for decision-making in clinical practice.

Microstructural characteristics and low-cycle fatigue properties of AZ91 and AZ91-Ca-Y alloys extruded at different temperatures

The commercial AZ91 alloy and nonflammable SEN9(AZ91-0.3Ca-0.2Y,wt%)alloy are extruded at 300°C and 400°*** microstructure,tensile and compressive properties,and low-cycle fatigue(LCF)properties are investigated,with particular focus on the influence of the extrusion *** the AZ91 and SEN9 materials extruded at 300°C(300-materials),numerous fine Mg_(17)Al_(12)particles are inhomogeneously distributed owing to localized dynamic precipitation during extrusion,unlike those extruded at 400°C(400-materials).These fine particles suppress the coarsening of recrystallized grains,decreasing the average grain size of *** the four extruded materials have considerably different microstructures,the difference in their tensile yield strengths is insignificant because strong grain-boundary hardening and precipitation hardening effects in 300-materials are offset almost completely by a strong texture hardening effect in ***,owing to their finer grains and weaker texture,300-materials have higher compressive yield strengths *** the LCF tests,{10-12}twinning is activated at lower stresses in 400-materials than in *** the fatigue damage accumulated per cycle is smaller in 400-materials,they have longer fatigue lives than those of 300-materials.A fatigue life prediction model for the investigated materials is established on the basis of the relationship between the total strain energy density(ΔW_(t))and the number of cycles to fatigue failure(N_(f)),and it is expressed through a simple equation(ΔW_(t)=10·N_(f)-0.59).This model enables fatigue life prediction of both the investigated alloys regardless of the extrusion temperature and strain amplitude.
