Emergency robotic colorectal surgery during the COVID-19 pandemic: A retrospective case series study

Objective: While interest in elective robotic surgery is growing, use in emergency setting remainslimited due to challenges posed by sicker patients, advanced pathology and logistical issues. During theCOVID-19 pandemic, robotic surgery could provide the benefit of having the surgeon away from thebedside and reducing the number of directly exposed medical staff. The objective of this study was toreport patient outcomes and initial learning experience of emergency robotic colorectal surgery duringthe COVID-19 pandemic.Methods: A case series study was conducted, including patients undergoing emergency robotic colorectalsurgery between February 2020 and February 2021 at Queen Alexandra Hospital in Portsmouth, UK.Patient data were collected from an ethics approved prospective database. Patient demographics,operative time, conversions and postoperative complications were recorded. In addition, readmissions,length of stay and short-term oncological outcomes were analyzed.Results: Ten patients with median age 64 y (range, 36-83 y) were included. Four patients had roboticcomplete mesocolic resection for obstructing cancers. six had colorectal resections for benign disease inemergency setting. All were R0 with a mean lymph node harvest of 54 ± 13. Mean operative time was249 ± 117 min, the median length of stay was 9.4 d (range, 5-22 d). Only one patient was given atemporary diverting ileostomy. There were no grade III/V complications and no 30-day mortality.Conclusions: Provided an experienced team and peri-operative planning, emergency robotic colorectalsurgery can achieve favorable outcomes with benefits of radical lymph node dissection in oncologicalcases and avoidance of diverting stoma.

近年来,经自然腔道取标本手术(NOsEs)治疗结直肠癌已引起了广泛关注。NOsEs应用于结直肠手术的潜在优势包括减轻术后疼痛,减少伤口并发症,减少对术后镇痛的需求,加快肠道功能的恢复,缩短住院时间,更好的美容效果,以及给患者带来了更佳的心理状态。尽管NOsEs的手术创伤显著减轻,然而该技术也存在一些潜在问题。尤其随着这项新技术的兴起,人们对其无菌的把控、肿瘤学结果和病例选择等问题表示出关切。因此,迫切需要一部操作指南来规范NOsEs手术在结直肠癌中的开展。经过国际NOsEs联盟全体成员的三轮讨论,最终形成了该部共识。这一共识有助于NOsEs在全球范围内的长远发展。

Cigarette butts(CBs)are one of the most common types of litter in the world.Due to the toxic substances they contain,the waste generated poses a harmful risk to the environment,and therefore there is an urgent need for alternative solutions to landfill storage.Thus,this work presents a possible revalorization of this waste material,which implies interesting environmental benefits.CBs were used as sacrificial templates for the preparation of CaO-based materials by impregnation with calcium and magnesium nitrates followed by flaming combustion.These materials presented enhanced porosity for their use in the Calcium Looping process applied either to thermochemical energy storage or CO_(2)capture applications.The influence of the concentration of Ca and Mg in the impregnating solutions on the multicycle reactivity of the samples was studied.An improved multicycle performance was obtained in terms of conversion for both applications.

The problem considered in this short note is the limit load determination of a vertical rock slope.The classical limit theorem is employed with the use of adaptive finite elements and nonlinear programming to determine upper and lower bound limit loads of a Hoek-Brown vertical rock slope.The objective function of the mathematical programming problem is such as to optimize a boundary load,which is known as the limit load,resembling the ultimate bearing capacity of a strip footing.While focusing on the vertical slope,parametric studies are carried out for several dimensionless ratios such as the dimensionless footing distance ratio,the dimensionless height ratio,and the dimensionless rock strength ratio.A comprehensive set of design charts is presented,and failure envelopes shown with the results explained in terms of three identified failure mechanisms,i.***.the face,the toe,and the Prandtl-type failures.These novel results can be used with great confidence in design practice,in particularly noting that the current industry-based design procedures for the presented problem are rarely found.

Predicting grain size-dependent superplastic properties in friction stir processed ZK30 magnesium alloy with machine learning methods

The aim of this work is to predict, for the first time, the high temperature flow stress dependency with the grain size and the underlaid deformation mechanism using two machine learning models, random forest（RF） and artificial neural network（ANN）. With that purpose, a ZK30 magnesium alloy was friction stir processed（FsP） using three different severe conditions to obtain fine grain microstructures（with average grain sizes between 2 and 3 μm） prone to extensive superplastic response. The three friction stir processed samples clearly deformed by grain boundary sliding（GBs） deformation mechanism at high temperatures. The maximum elongations to failure, well over 400% at high strain rate of 10-2s-1, were reached at 400 ℃ in the material with coarsest grain size of 2.8 μm, and at 300 ℃ for the finest grain size of 2 μm. Nevertheless, the superplastic response decreased at 350 ℃ and 400 ℃ due to thermal instabilities and grain coarsening, which makes it difficult to assess the operative deformation mechanism at such temperatures. This work highlights that the machine learning models considered, especially the ANN model with higher accuracy in predicting flow stress values, allow determining adequately the superplastic creep behavior including other possible grain size scenarios.

Long-term effects of gestational diabetes mellitus on the pancreas of female mouse offspring

BACKGROUND Prolonged fetal exposure to hyperglycemia may increase the risk of developing abnormal glucose metabolism and type-2 diabetes during childhood,adolescence,and adulthood;however,the mechanisms by which gestational diabetes mellitus(GDM)predisposes offspring to metabolic disorders remain unknown.AIM To quantify the nerve axons,macrophages,and vasculature in the pancreas from adult offspring born from mouse dams with GDM.METHODs GDM was induced by i.***.administration of streptozotocin(sTZ)in ICR mouse dams.At 12 wk old,fasting blood glucose levels were determined in offspring.At 15 wk old,female offspring born from dams with and without GDM were sacrificed and pancreata were processed for immunohistochemistry.We quantified the density of sensory[calcitonin gene-related peptide(CGRP)]and tyrosine hydroxylase(TH)axons,blood vessels(endomucin),and macro-phages(CD68)in the splenic pancreas using confocal microscopy.REsULTs Offspring mice born from sTZ-treated dams had similar body weight and blood glucose values compared to offspring born from vehicle-treated dams.However,the density of CGRP+and TH+axons,endomucin+blood vessels,and CD68+macrophages in the exocrine pancreas was significantly greater in offspring from mothers with GDM vs control offspring.Likewise,the microvasculature in the islets was significantly greater,but not the number of macrophages within the islets of offspring born from dams with GDM compared to control mice.CONCLUsION GDM induces neuronal,vascular,and inflammatory changes in the pancreas of adult progeny,which may partially explain the higher propensity for offspring of mothers with GDM to develop metabolic diseases.

Reuse of port industrial heritage in tourist cities:shipyards as case studies

The progressive deindustrialisation of many western cities since the 1980s has led to many industrial zones linked to port activities being abandoned or falling into disuse.Cities such as Barcelona,Naples,Lisbon,Amsterdam,and Hamburg have port industrial complexes of high tangible and intangible heritage value that could totally or partially disappear,resulting in an irreparable loss of their scientific,architectural,social,technological,and historical values.With that in mind,Adaptive Reuse(AR)of the built heritage allows the industrial memory of the ports to be preserved by turning them into new functional centres within the existing urban structure.That occurs in the context of the contemporary challenges of those cities,such as touristification,the circular economy and climate change,while guaranteeing the life cycle of those buildings.This article analyses two case studies—the Nederlandsche scheepsbouw Maatschappij(NDsM)and the Rotterdamsche Droogdok Maatschappij(RDM)shipyards,both in the Netherlands—in order to contribute to the knowledge of AR of Port Industrial Heritage.They are two examples of good practices in port industrial heritage interventions,where the factors behind their acclaim can be easily highlighted.A multi-scale methodology is therefore used and tailored to the case of port industrial heritage,based on analysing previous studies of the heritage in different spheres and on different scales.A relationship matrix tool is thus defined.It enables a comparative study to be conducted,using key variables and indicators,and considering qualitative and quantitative data.That provides extensive output information for each case study,which is summarised in the most favourable factors for the success of the AR of this port industrial heritage.

Development of endemicity and conservation importance indices for tropical forests and the floral diversity assessment of Mt.Natoo in Benguet,Philippines

The scientific community faces the challenge of measuring progress toward biodiversity targets and indices have been traditionally used.However,recent inventories in secondary tropical mountain forests using traditional biodiversity indices have yielded results that are indistinct with primary ones.This shows the need to develop complementary indices that goes beyond species count but integrates the distribution and conservation status of the species.This study developed endemicity and conservation importance index for tropical forest that incorporated the distribution and conservation status of the species.These indices were applied to Mt.Natoo,a remnant primary mossy forest in Buguias,Benguet,Philippines,that resulted to endemicity index of 81.07 and conservation importance index of 42.90.Comparing these with secondary forest sites with comparable shannon-Wiener,simpson,Evenness and Margalef’s indices,our endemicity and conservation indices clearly differentiates primary forest(our study site)with higher values from secondary forests with much lower values.Thus,we are proposing these indices for a direct but scientifically-informed identification of specific sites for conservation and protection in tropical forests.Additionally,our study documented a total of 168 vascular plant species(79 endemic and 12 locally threatened species)in Mt.Nato-o.Majority are of tropical elements for both generic and species levels with some temperate elements that could be attributed to the site's high elevation and semi-temperate climate.These are important baseline information for conservation plans and monitoring of tropical mossy forests.

Anti-amyloid antibodies in Alzheimer’s disease: what did clinical trials teach us?

Although many causes of Alzheimer’s disease(AD)may exist,both the original amyloid cascade and tau hypotheses posit that abnormal misfolding and accumulation of amyloid-β(Aβ)and tau protein is the central event causing the pathology.However,that conclusion could be only partly true,and there is conflicting evidence about the role of both proteins in AD,being able to precede and influence one another.some researchers argue that these proteins are mere executors rather than primary causes of pathology.Therefore,there have been continuing refinements of both hypotheses,with alternative explanations proposed.Aβand tau proteins may be independently involved in specific neurotoxic pathways;yet there may be other crucial processes going on in early AD.Moreover,accumulating evidence suggests that Aβand tau act synergistically,rather than additively in disease onset(Jeremic et al.,2021,2023a).
