Adolescence and the journey to adulthood involves exciting opportunities as well as psychosocial stress for young people growing *** normal experiences are potentially magnified for teenagers living with chronic illness or disability and their *** in care have improved survival for children with a variety of serious chronic medical conditions such that many who may once have died in childhood now survive well into adulthood with ongoing *** those with highly complex needs,care is often provided at major paediatric hospitals with expertise,specially trained personnel,and resources to support young people and their families for the first decades of *** the end of adolescence,however,it is generally appropriate and necessary for young adults and their caregivers to transition to the care of clinicians trained in the care of adults at general *** there are some well-managed models to support this journey of transition,these are often specific to certain conditions and usually do not involve intensive *** patients may encounter considerable challenges during this *** may include the loss of established therapeutic relationships,a perception of austerity and reduced amenity in facilities oriented to caring for adult patients,and care by clinicians with less experience with more common paediatric *** addition,there is a risk of potential conflict between clinicians and families regarding goals of care in the event of a critical illness when it occurs in a young adult with major disability and long-term health *** challenges present genuine opportunities to better understand the transition from paediatric to adult-based care and to improve processes that assist clinicians who support patients and families as they shift between healthcare settings.

The modified Reynolds equation for ultra-thin gas films between magnetic head; disk assembly is difficult to solve with conventional numerical methods, since the bearing number is very large; there exist boundary layers where pressure changes rapidly. An iterative finite difference algorithm is introduced to solve the nonlinear modified Reynolds equation, with special treatment for the abrupt change in the thickness of the gas film. The numerical results for two types of magnetic heads demonstrate that the scheme is effective.

Two plant species,Medicago truncatula (legume) and Avena sativa (non-legume),were grown in low-or high-N soils under two CO2 concentrations to test the hypothesis whether C allocation within plant-soil system is interactively or additively controlled by soil N and atmospheric CO2 is dependent upon plant species. The results showed the interaction between plant species and soil N had a significant impact on microbial activity and plant growth. The interaction between CO2 and soil N had a significant impact on soil soluble C and soil microbial biomass C under Madicago but not under Avena. Although both CO2 and soil N affected plant growth significantly,there was no interaction between CO2 and soil N on plant growth. In other words,the effects of CO2 and soil N on plant growth were additive. We considered that the interaction between N2 fixation trait of legume plant and elevated CO2 might have obscured the interaction between soil N and elevated CO2 on the growth of legume plant. In low-N soil,the shoot-to-root ratio of Avena dropped from 2.63±0.20 in the early growth stage to 1.47±0.03 in the late growth stage,indicating that Avena plant allocated more energy to roots to optimize nutrient uptake (i.e. N) when soil N was limiting. In high-N soil,the shoot-to-root ratio of Medicago increased significantly over time (from 2.45±0.30 to 5.43±0.10),suggesting that Medicago plants allocated more energy to shoots to optimize photosynthesis when N was not limiting. The shoot-to-root ratios were not significantly different between two CO2 levels.

Transcription of the Bacillus subtilis pur operon is regulated by a purine repressor (PurR)-DNA control site interaction. The pur operon control site has two PurBoxes that are re- quired for high-affinity PurR binding. An upstream, strong-binding PurBox1 is at position –81 to –68 relative to the transcription start site and a downstream weak-binding PurBox2 is at position –49 to –36. We constructed three PurBox1 mutations and the effects on binding of PurR to the control region in vitro and on regulation of pur operon expression in vivo were investigated. The mutations significantly reduced the binding of PurR to control region DNA. In strains with G-75A, G-75T and a five bp deletion (?5) pur operon repression was defective in vivo. In addition in vivo PurR titration was used to confirm that sequences flanking PurBox1 and PurBox2 are re- quired for PurR binding to the pur operon control site.

Global technologies that have made the world more interconnected have also,inadvertently,amplified the forces of stress that are now with us 24 hours a day,7 days a *** accumulated impact of this stress I call cultural stress anxiety syndrome and call on integrative medicine practitioners to recognize that it is exacerbating whatever acute stressors are also present in our patients'*** this Commentary,I outline seven major components of cultural stress(time pressure,digital intrusion,digital dependency,isolation,sedentary lifestyle,poor sleep and uncertainty),describe their health consequences,and finally,offer cultural stress-specific remedies I have utilized in my own practice,along with studies that affirm their *** hope is that we,as integrative medicine practitioners who are cognizant of the role that stress plays in disease development,will more fully appreciate the added impact of cultural stress,and advise our patients on the importance of proactive stress management.
