Risk factors for hepatocellular carcinoma in patients with drug-resistant chronic hepatitis B

AIM:To investigate the risk factors and characteristics of hepatocellular carcinoma(HCC) in the patients with drug-resistant chronic hepatitis B(CHB).METHODS:A total of 432 patients with drug-resistant CHB were analyzed retrospectively from January 2004to December 2012. The patients were divided into two groups:the HCC group(n = 57) and the non-HCC group(n = 375). Two groups compared using logistic regression for various patients and viral characteristics in order to identify associated risk factors for ***,patient and tumor characteristics of HCC patients with na ve CHB(n group,n = 117) were compared to the HCC group(R group,n = 57) to identify any difference in HCC characteristics between ***:A significant difference was found for age,platelet count,alpha-fetoprotein(AFP),positivity of HBeAg,seroconversion rate of HBeAg,virologic response,the Child-Pugh score,presence of rtM204I,and the duration of antiviral treatment in non-HCC and HCC group. Cirrhosis,age(> 50 years),HBeAg(+),virologic non-responder status,and rtM204I mutants were independent risk factors for the development of HCC. The R group had lower serum C-reactive protein(CRP) and AFP levels,earlier stage tumors,and a shorter mean tumor surveillance period than the n group. However,the total follow-up duration was not significantly different between the two ***:13.2% of patients with drug-resistant CHB developed HCC. Age,cirrhosis,YIDD status,HBeAg status,and virologic response are associated with risk of HCC. Patients with drug-resistant CHB and these clinical factors may benefit from closer HCC surveillance.

AIM:To clarify the role of surgical resection for multiple hepatocellular carcinomas(HCCs)compared to transarterial chemoembolization(TACE)and liver transplantation(LT). METHODS:Among the HCC patients who were managed at Yonsei University Health System between January 2003 and December 2008,160 patients who met the following criteria were retrospectively enrolled:(1) two or three radiologically diagnosed HCCs;(2)no radiologic vascular invasion;(3)Child-Pugh class A;(4) main tumor smaller than 5 cm in diameter;and(5) platelet count greater than 50 000/***-term outcomes were compared among the following three treatment modalities:surgical resection or combined radiofrequency ablation(RFA)(n=36),TACE(n=107), and LT(n=17).The survival curves were computed using the Kaplan-Meier method and compared with a log-rank *** identify the patients who gained a survival benefit from surgical resection,we also investigated prognostic factors for survival following surgical *** analyses of the prognostic factors for survival were performed using the Cox proportional hazard model. RESULTS:The overall survival(OS)rate was significantly higher in the surgical resection group than in the TACE group(48.1%vs 28.9%at 5 years,Pn that of the surgical resection group,although the difference was not statistically significant(80.2%vs 48.1%at 5 years,P=0.447).The disease-free survival rates were also significantly higher in the LT group than in the surgical resection group(88.2%vs 11.2%at 5 years, Pnly significant prognostic factor for poor OS after surgical resection. AIM： To determine if the fraction of nardostachysjata- mansi （n J） has the potential to ameliorate the severity of acute pancreatitis （AP）. METHODS： Mice were administered the biologically active fraction of n J, i.e., the 4th fraction （n J4）, intra- peritoneally, and then injected with the stable chole- cystokinin analogue cerulein hourly for 6 h. Six hours after the last cerulein injection, the pancreas, lung, and blood were harvested for morphological examination,measurement of cytokine expression, and examination of neutrophil infiltration. RESULTS： n J4 administration attenuated the sever- ity of AP and lung injury associated with AP. It also reduced cytokine production and neutrophil infiltration and resulted in the in vivo up-regulation of heine oxy- genase-1 （HO-1）. Furthermore, nJ4 and its biologically active fraction, n J4-2 inhibited the cerulein-induced death of acinar cells by inducing HO-1 in isolated pan- creatic acinar cells. COnCLUSIOn： These results suggest that n J4 may be a candidate fraction offering protection in AP and n J4 might ameliorate the severity of pancreatitis by induc- ing HO-1 expression.

Sleep disordered breathing (SDB) is an independent risk factor for cardiovascular disease. It is known to be associated more frequently with men than women, particularly in the premenopausal age range. The goal of this study is to evaluate gender differences among Korean patients diagnosed with SBD. This study included 309 patients who visited our Sleep Clinic due to sleep-related symptoms and were diagnosed with SDB by overnight polysomnography (PSG). We analyzed age, gender, body mass index, various PSG indices including sleep stages, apnea-hypopnea index (AHI) and AHI ratio in rapid eye movement (REM) versus non-REM (nREM) sleep stages (R:n ratio). Of those 309 patients diagnosed with obstructive sleep apnea, 217 (70.2%) were men (mean age 51.05 ± 12.64 years) and 92 (29.8%) were women (mean age 64.53 ± 10.43 years). The mean AHI during total sleep time was 30.34 ± 21.17 in men and 21.47 ± 17.14 in women (P P P = 0.402). REM SDB with R:n ratio higher than 2.0 was more frequently observed in women than in men, 34.8% (32/92) of women, compared with 11.9% (26/217) in men (P 60 years old. These findings suggest the possibility of different pathophysiologic mechanisms of SDB between genders and also between nREM versus REM sleep, which can be partly explained by the influence of female sex hormones.

Harmful algae blooms have become an increasing concern in context with the safety of water resources around the globe;however, little is known about the dynamics and specific causes of such blooms in the prairie ecozone in north America. The aim of this study was to research the nitrogen (n) and phosphorous (P) content and nutrient limitation (defined as n and P limitation) of growth of cyanobacteria in a northern prairies reservoir (Lake Diefenbaker [LD], SK, Canada). A combination of concentration balance analysis for n and P, controlled bioassays with the natural consortium of phytoplankton or defined monocultures of cyanobacteria, and satellite imagery was applied to address this aim. The current trophic status of Lake Diefenbaker is one of moderate eutrophication. Primary production in the lake is P-limited, and n did not represent a limiting factor for algal production. There was no significant increase in TP con- centrations between the upper and lower portions of the reservoir, indicating that most of the phosphorus in LD comes from upstream sites in Alberta. Anabaena circinalis, a species that has the potential to seriously degrade lake ecosys- tems, was identified as the predominant cyanobacteria in LD. Together with the fact that TP influxes into the reservoir primarily originate from upstream sources, these results suggest the need for remedial measures in the upstream reach of the South Saskatchewan River. Satellite imaging represented a promising approach in support of monitoring for po- tential algal blooms in LD;however, due to limited sensitivity and issues associated with atmosphere interference this methodology should only be used in combination with in situ water quality monitoring.
