Endoscopic nasojejunal feeding tube placement in patients with severe hepatopancreatobiliary diseases:a retrospective study of 184 patients

BACKGROUND: Total parenteral nutrition (TPN) has been recognized as the mainstay of nutritional support in patients with severe hepatopancreatobiliary (HPB) diseases for decades. However, recent studies advocate the utilization of endoscopic nasojejunal feeding tube placement (ENFTP), rather than the conventional approach. This study was designed to compare the clinical value of ENFTP and TPN in patients with severe HPB diseases. METHODS: Two groups of patients with severe HPB diseases were analyzed retrospectively. One group of 88 patients received ENFTP, and the other 96 received TPN. Routine blood levels, serum glucose and prealbumin, hepatic and renal function, serum lipid, and calcium were measured at baseline and after 1, 2, and 4 weeks of nutritional support. Also, complication rate, mortality, nutritional support time, mechanical ventilation time, mean length of time in intensive care unit, and duration of hospital stay were analyzed. RESULTS: After 4 weeks of nutritional support, the degree of recovery of red blood cells, prealbumin, and blood glucose was greater in the ENFTP than in the TPN group (P<0.05). Furthermore, the ENFTP group showed a lower incidence of septicemia, multiple organ dysfunction syndrome, peripancreatic infection, biliary infection, and nosocomial infection, in addition to shorter nutritional support time and hospital stay (P<0.05). CONCLUSIONS: ENFTP is much more effective than TPN in assisting patients with severe HPB diseases to recover from anemia, low prealbumin level, and high serum glucose, as well as in decreasing the rates of various infections (pulmonary infection excluded), multiple organ dysfunction syndrome rate, nutrition support time, and length of hospital stay. Therefore, ENFTP is safer and more economical for clinical application.

This work studied the microstructure,mechanical properties and damping properties of Mg_(95.34)Ni_(2)Y_(2.66) and Mg_(95.34)Zn_(1)Ni_(1)Y_(2.66)alloys *** difference in the evolution of the long-period stacked ordered(LPSO)phase in the two alloys during heat treatment was the *** morphology of the as-cast Mg_(95.34)Ni_(2)Y_(2.66)presented a disordered *** heat treatment at 773 K for 2 hours,the eutectic phase was integrated into the matrix,and the LPSO phase maintained the 18R *** Zn partially replaced Ni,the crystal grains became rounded in the cast alloy,and lamellar LPSO phases and more solid solution atoms were contained in the matrix after heat treatment of the Mg_(95.34)Zn_(1)Ni_(1)Y_(2.66)*** Zn and the heat treatment had a significant effect on *** dislocation internal friction peaks and grain boundary internal friction peaks were found after temperature-dependent damping of the Mg_(95.34)Ni_(2)Y_(2.66)and Mg_(95.34)Zn_(1)Ni_(1)Y_(2.66)*** heat treatment,the dislocation peak was significantly increased,especially in the alloy Mg_(95.34)Ni_(2)Y_(2).*** annealed Mg_(95.34)Ni_(2)Y_(2.66)alloy with a rod-shaped LPSO phase exhibited a good damping performance of 0.14 atε=10^(−3),which was due to the difference between the second phase and solid solution atom *** factors also affected the dynamic modulus of the *** results of this study will help in further development of high-damping magnesium alloys.

Optimal investment based on relative performance and weighted utility

This paper studies the optimal portfolio allocation of a fund manager when he bases decisions on both the absolute level of terminal relative performance and the change value of terminal relative performance comparison to a predefined reference point. We find the optimal investment strategy by maximizing a weighted average utility of a concave utility and an Sshaped utility via a concavification technique and the martingale method. Numerical results are carried out to show the impact of the extent to which the manager pays attention to the change of relative performance related to the reference point on the optimal terminal relative performance.

Regulation of Ti(C,N)deposition on hot surface of refractory in blast furnace hearth

Ti(C,N)concentration was found to be lower on the hearth sidewall of a blast furnace and increased gradually toward the bottom of the blast *** Ti(C,N)protective layer in a blast furnace is ***,the formation of a Ti(C,N)protective layer was promoted by studying the heterogeneous nucleation principle of titanium compounds on different substances and regulation measures for the deposition process of titanium compounds on refractories or *** lattice disregistry between the titanium compounds and the main components in the refractory or the main impurities in the protective layer was calculated using a two-dimensional disregistry equation to study the heterogeneous nucleation principle of titanium *** results revealed that in refractory materials,the heterogeneous nucleation of carbonitride is weak when C,SiO_(2),and Al_(2)O_(3) are used as heterogeneous nucleation substrates,and the heterogeneous nucleation of carbonitride is strong when TiO_(2) and SiC are used as heterogeneous nucleation *** nucleation phases,TiC,TiN,Ti(C_(0.3),N_(0.7)),and Ti(C_(0.5),N_(0.5))have similar heterogeneous nucleation ability in single component refractory,and the type of carbonitride has little effect on the lattice *** impurities in the protective layer as the substrate phases are not conducive to the heterogeneous nucleation of *** CaS was used as the substrate phase,the heterogeneous nucleation ability of carbonitride was the *** carbon and alumina were not conducive to the heterogeneous nucleation of carbonitride,but carbon was more unfavourable than alumina.

Mechanism of local solidification time variations with melt rate during vacuum arc remelting process of 8Cr4Mo4V high-strength steel

A 2D axisymmetric numerical model was established to investigate the variations of molten pool with different melt rates during the vacuum arc remelting of 8Cr4Mo4V high-strength steel,and the ingot growth was simulated by dynamic mesh *** results show that as the ingot grows,the molten pool profile changes from shallow and flat to V-shaped,and both the molten pool depth and the mushy width ***,the variation of both the molten pool shape and the mushy width melt rate is clarified by the thermal equilibrium *** melt rate increases,both the molten pool depth and the mushy width *** is caused by the increment in sensible heat stored in the ingot due to the limitation of the cooling capacity of the *** nonlinear increment in sensible heat leads to a nonlinear increase in the mushy *** addition,as melt rate increases,the local solidification time(LST)of ingot decreases obviously at first and then *** melt rate is controlled in a suitable range,LST is the lowest and the secondary dendrite arm spacing of the ingot is the smallest,which can effectively improve the compactness degree of 8Cr4Mo4V high-strength steel.

Effect of Platelet-rich Plasma in Stimulating Macrophage Transformation into M2 Type under AMPK Signaling Pathway

Objective: To explore the effect of platelet-rich plasma(RPR) in stimulating the transformation of pro-inflammatory(M1) macrophages into antiinflammatory(M2) under the adenosine-monophosphate-dependent protein kinase(AMPK) signaling pathway. Methods: Rat peritoneal macrophages(RAW264.7) were cultured and randomly divided into 8 groups: blank control group, LPS group, RPR group A, RPR group B, LPS+RPR(12 h) group, LPS+RPR(24 h) group A, LPS+RPR(24 h)group B, LPS+RPR(24 h) group C. RPR was prepared based on blood donors. The expressions of AMPK signaling pathway-related proteins(AMPK, ULK1, m TOR) and macrophage markers(i NOS, Arg-1) in the blank control group, LPS group, LPS+RPR(12 h)group and LPS+RPR(24 h) group were observed and compared. The expressions of macrophage markers in LPS+RPR(24 h) B and C groups were compared, and the expressions of AMPK and TGF-β in RPR A and B groups were compared. Results: The gray values of AMPK and ULK1 in LPS cells decreased significantly, while those in LPS+RPR(12 h) and LPS+RPR(24 h) A cells increased significantly. The gray values of AMPK and ULK1 in LPS+RPR(24 h) A cells were higher than those in LPS+RPR(12 h)cells(P<0.05). The m TOR gray value of LPS cells was significantly higher than that of LPS+RPR(24 h) A cells, and the m TOR gray value of LPS+RPR(24 h) A cells was significantly lower than that of LPS+RPR(12 h) cells(P<0.05). The expression of i NOS in LPS cells was significantly decreased, the expression of i NOS in LPS+RPR(12 h) and LPS+RPR(24 h) cells was significantly increased, and the expression of i NOS in LPS+RPR(24 h) cells was higher than that in LPS+RPR(12 h) cells(P<0.05). The expression of Arg-1 in LPS cells was significantly decreased, the expression of Arg-1 in LPS+RPR(12 h) and LPS+RPR(24 h) A cells was significantly increased, and the expression of Arg-1 in LPS+RPR(24 h) A cells was higher than that in LPS+RPR(12 h)cells(P<0.05). An efficient electrocatalyst is indispensable to significantly reduce energy consumption and accelerate the conversion efficiency of water *** this work,the honeycomb-like porous MoCo alloy on nitrogen-doped three-dimensional(3D)porous graphene substrate(Mo_(0.3)Co_(0.7)@NPG)has been synthesized from the annealing of layered double hydroxide(MoCo-LDH@NPG).Especially,the Mo_(0.3)Co_(0.7)@NPG exhibits low hydrogen evolution overpotential of 75 mV(10 mA·cm^(-2))and a Tafel slope of 69.9 mV·dec^(-1),which can be attributed to highly conductive NPG substrate,the unique nanostructure and the synergistic effect of Mo and ***,the Mo_(0.3)Co_(0.7)@NPG can maintain the original morphology and high catalytic activity after 50-h stability *** work proposes a general strategy to synthesize a multi-element alloy on conductive substrates with high porosity for electrocatalytic reaction.

Molecular mechanism for the unusual enhancement of the second-step chemiluminescence production from the carcinogenic tetrabromohydroquinone and H2O_(2)

We have found recently that two-step intrinsic hydroxyl radical(·OH)-dependent chemiluminescence(CL)could be produced by carcinogenic tetrahaloquinone and H_(2)O_(2).However,the first-step CL was too fast to clearly detect the stepwise generation of·OH and CL,and to distinguish the exact dividing point between the first-step and second-step *** we found that,extremely clear two-step intrinsic CL could be produced by the relative slow reaction of tetrabromohydroquinone(TBHQ)with H_(2)O_(2),which was directly dependent on the two-step·OH ***,the second-step,but not the first-step CL production of TBHQ/H_(2)O_(2)(CRET donor)was markedly enhanced by fluorescein(a typical xanthene dye,CRET acceptor)through a unique chemiluminescence resonance energy transfer(CRET)*** novel CRET system of TBHQ/H_(2)O_(2)/fluorescein was successfully applied for the sensitive detection of TBHQ with the detection limit as low as 2.5μmol/*** findings will help to develop more sensitive and highly efficient CL or CRET systems and specific CL sensor to detect the carcinogenic haloquinones,which may have broad environmental applications.
