STGNN-LMR:A Spatial–Temporal Graph Neural Network Approach Based on sEMG Lower Limb Motion Recognition

Lower limb motion recognition techniques commonly employ Surface Electromyographic Signal(sEMG)as input and apply a machine learning classifier or Back Propagation Neural Network(BPNN)for classification.However,this artificial feature engineering technique is not generalizable to similar tasks and is heavily reliant on the researcher’s subject expertise.In contrast,neural networks such as Convolutional Neural Network(CNN)and Long Short-term Memory Neural Network(LSTM)can automatically extract features,providing a more generalized and adaptable approach to lower limb motion recognition.Although this approach overcomes the limitations of human feature engineering,it may ignore the potential correlation among the sEMG channels.This paper proposes a spatial–temporal graph neural network model,STGNN-LMR,designed to address the problem of recognizing lower limb motion from multi-channel sEMG.STGNN-LMR transforms multi-channel sEMG into a graph structure and uses graph learning to model spatial–temporal features.An 8-channel sEMG dataset is constructed for the experimental stage,and the results show that the STGNN-LMR model achieves a recognition accuracy of 99.71%.Moreover,this paper simulates two unexpected scenarios,including sEMG sensors affected by sweat noise and sudden failure,and evaluates the testing results using hypothesis testing.According to the experimental results,the STGNN-LMR model exhibits a significant advantage over the control models in noise scenarios and failure scenarios.These experimental results confirm the effectiveness of the STGNN-LMR model for addressing the challenges associated with sEMG-based lower limb motion recognition in practical scenarios.

Background: Donor-derived cell free DNA (ddcf DNA) has been reported as a universal noninvasive biomarker for rejection monitoring in heart, kidney, liver, and lung transplantation. Current approaches based on next-generation sequencing for quantification of ddcf DNA, although promising, may be restricted by the requirement for donor material, as donor samples may not be available. Methods: We proposed a novel next-generation sequencing approach without donor-derived material and compared the non-donor-derived approach and the donor-derived approach using simulation testing and 69 clinical specimens. We also evaluated the performance for acute rejection and infection monitoring in lung transplantation. Results: The non-donor-derived approach reached similar efficacy as the donor-derived approach with a significant linear correlation of R2 = 0.98. Subsequent validation in clinical specimens demonstrated significant difference between the acute rejection group (4.83% ± 2.11%, mean ± SD) and the non-rejection group (1.61% ± 0.63%, mean ± SD) (P ’s t test). With the cut-off value of 2.999, our approach had 90.48% sensitivity (95% CI, 69.62% - 98.83%), 100% specificity (95% CI, 91.59% - 100%), and AUC 0.9266 (95% CI, 0.8277 - 1.026). The test also had the ability to simultaneously detect infectious agents, especially cytomegalovirus, as compared with the clinical test. Conclusion: The proposed approach without donor-derived material could potentially be used to monitor acute rejection and infection in lung transplantation and may be applied to other types of solid organ transplantation.

Exosome therapy holds great promise as a novel approach to improve acute skin wound healing.This review provides a comprehensive overview of the current understanding of exosome biology and its potential applications in acute skin wound healing and beyond.Exosomes,small extracellular vesicles secreted by various stem cells,have emerged as potent mediators of intercellular communication and tissue repair.One advantage of exosome therapy is its ability to avoid potential risks associated with stem cell therapy,such as immune rejection or stem cells differentiating into unwanted cell types.However,further research is necessary to optimize exosome therapy,not only in the areas of exosome isolation,characterization,and engineering,but also in determining the optimal dose,timing,administration,and frequency of exosome therapy.Thus,optimization of exosome therapy is critical for the development of more effective and safer exosome-based therapies for acute skin wound healing and other diseases induced by cancer,ischemia,or inflammation.This review provides valuable insights into the potential of exosome therapy and highlights the need for further research to optimize exosome therapy for clinical use.

Objective: To evaluate correlations between proportion and distribution of tumor stroma and MDCT early phase enhancement character in solid lung adenocarcinoma, and compare with microvessel density and histological subtypes. Methods: Thirty-one patients with lung adenocarcinoma shown as solid solitary pulmonary nodules underwent routine con-trast-enhanced MDCT followed by surgical resections. CT character included net enhancement and distribution of enhance-ment. The largest cut surface of tumor specimens were stained by hematoxylin and eosin. About 25 fields of view of each specimen were scanned as digitized pictures at low magnification. Semi-auto segmentation software was used to calculate mean stroma proportion. Pearson correlation coefficient was used to represent the relationships between extent of tumor en-hancement, proportion of tumor stroma and MVD respectively. Fisher's exact test was used to analyze statistical differences in degree of CT enhancement among groups of different histological subtypes. Results: Proportion of invasive tumor stroma (13.2%-54.5%, mean 26.2 ± 8.8%) was correlated positively with net enhancement (8-60.8 HU, mean 31.2 ± 13.6 HU; r = 0.483, P = 0.006) which was more than MVD. 58.1% cases showed homogenous enhancement, 32.3% cases showed periph-eral inhomogenous enhancement, 3.2% cases showed central inhomogenous enhancement, 3.2% cases showed asymmetri-cal inhomogenous enhancement, and 3.2% cases showed no enhancement. 58.1% cases' stroma showed mixed distribution, 35.5% cases showed peripheral distribution, 3.2% cases showed central distribution, and 3.2% cases showed asymmetrical distribution. Significantly more adenocarcinomas classified with "net enhancement > 20 HU" were found in the acinar group than in the solid with mucin subtype (P = 0.005). Conclusion: Extent of CT enhancement reflects underlying not only the tumor angiogenesis but also stroma proliferation in solid small lung adenocarcinoma. Chloride intracellular channel 1(CLIC1) is involved in the development of most aggressive human tumors, including gastric, colon, lung, liver, and glioblastoma cancers. It has become an attractive new therapeutic target for several types of cancer. In this work, we aim to identify natural products as potent CLIC1 inhibitors from Traditional Chinese Medicine(TCM) database using structure-based virtual screening and molecular dynamics(MD) simulation. First, structure-based docking was employed to screen the refined TCM database and the top 500 TCM compounds were obtained and re-ranked by X-score. Then, 30 potent hits were achieved from the top 500 TCM compounds using cluster and ligand-protein interaction analysis. Finally, MD simulations was employed to validate the stability of interactions between each hit and CLIC1 protein from docking simulation, and molecular mechanics/generalized born surface area(MM-GBSA) analysis was used to refine the virtual hits. Six TCM compounds with top MM-GBSA scores and ideal-binding models were confirmed as the final hits. Our study provides information about the interaction between TCM compounds and CLIC1 protein, which may be helpful for further experimental investigations. In addition, the top 6 natural products structural scaffolds could serve as building blocks in designing druglike molecules for CLIC1 inhibition.

近年研究表明,周细胞(PCs)与肿瘤微环境(TME)内的各类细胞成分(如癌细胞、基质细胞、免疫细胞)存在紧密的相互作用关系,可通过异常的旁分泌通路介导肿瘤偏好性的肿瘤血管新生及失能,形成免疫抑制性TME并促进肿瘤进展。结合最新的非小细胞肺癌研究进展,本文就PCs在TME中的潜在功能与作用进行了综述,并讨论了靶向PCs的肿瘤抗血管治疗及肿瘤血管正常化治疗在重塑TME中的潜在价值,即通过靶向PCs肿瘤血管正常化而突破现阶段联合抗血管治疗实体瘤的困境,该理念或将成为改善化疗、小分子抑制剂靶向治疗及免疫检查点抑制剂治疗等联合抗肿瘤治疗的新策略。

目的探讨肺移植术后新生供者特异性抗体(dnDSA)介导的急性排斥反应的治疗方法。方法回顾性分析1例肺移植术后早期出现抗体介导的急性排斥反应(AMR)受者的资料,分析其诊疗经过。结果受者因系统性硬化症相关性终末期间质性肺病接受右肺移植,术前群体反应性抗体(PRA)Ⅰ类阳性(11%),术前未行特殊预处理,手术当日及术后予以抗胸腺细胞球蛋白诱导治疗。术后早期受者康复顺利,术后13 d出现胸闷、气促,并呈进行性加重,迅速进展为Ⅰ型呼吸衰竭,PRAⅠ类上升为58%,并出现dnDSA,其位点为A24:02,平均荧光强度(MFI)值为2110,据国际心肺移植学会指南,拟诊为(可能)AMR。予血浆置换、蛋白A免疫吸附、糖皮质激素冲击、利妥昔单抗及免疫球蛋白静脉滴注等综合治疗后,PRA及DSA水平逐渐下降,术后20 d DSA MFI值为0,受者临床情况逐渐好转,呼吸困难消失,气促逐渐缓解,呼吸衰竭纠正,肺部渗出影逐渐吸收;术后45 d,受者完全康复出院。随访1年,受者状态良好,生活质量与同龄健康人相同,PRAⅠ类为5%,Ⅱ类为阴性,未出现DSA。结论在传统药物治疗基础上加用蛋白A免疫吸附治疗,能有效去除受者循环血液中的DSA,减轻靶器官损害,近期及远期治疗效果理想。针对肺移植术后AMR,采用传统药物治疗联合免疫吸附,可以达到理想治疗效果。

目的探讨高流量鼻导管(HFNC)吸氧在妊娠合并心脏病患者剖宫产中的应用。方法本研究为单中心、单盲、随机临床试验。拟行剖宫产的妊娠合并心脏病患者被随机分为2组:HFNC组接受HFNC疗法(n=27,吸气流量30 L/min,氧浓度40%),传统吸氧(COT)组接受传统氧疗(n=31,通过鼻导管输送,氧流量5L/min)。主要观察指标为母体血氧饱和度下降(SpO2her的确切检验或卡方检验。结果 HFNC组中有7.4%的孕妇(n=2/27)出现母体血氧饱和度下降,而COT组中有32.3%的孕妇(n=10/31)出现。在围手术期间,没有任何病例需要气管插管。HFNC组在术后白细胞增多的发生率较高(P0.05)。结论 HFNC疗法显著降低了妊娠合并心脏病患者围手术期母体血氧饱和度下降的发生率,并且对母体或胎儿的短期临床结果没有不良影响。
