Functional paragangliomas of the urinary bladder:a report of 9 cases

Background and Objective: Functional paraganglioma of the urinary bladder (FPUB) is a rare tumor. Misdiagnosis of FPUB before operation can lead to serious intraoperative consequences. In this article, we reported our experience in preoperative diagnosis and surgical treatment of FPUB. Methods: Clinical data of nine patients with FPUB treated between June 1985 and January 2009 at Peking Union Medical College Hospital were analyzed. Results: All patients underwent urinary catecholamine (CA) detection, B-ultrasound, CT and/or MRI scan; 5 underwent nailfola microcirculation inspection; 4 underwent 131I-metaiodobenzyl guanidine (MIBG) detection; and 6 underwent 111In-DTPA-Octreotide (OCT) scintiscan. According to the UICC bladder tumor classification, 5 patients had T2, 3 had T3, and 1 had T4 disease. All patients underwent surgical treatment, and 1 received 131I-MIBG therapy. All patients had paroxysmal hypertension and palpitation and six had cold sweat, headache, and dizziness after emphatic urination. The definitive diagnosis was made by histopathologic examination of the removed tumors and was confirmed in 7 cases by the immunohistochemical staining of chromogranin A, Ki-67 and S100 protein. The tumor consisted of discrete aggregates of zellballen cells separated by a network of vascular channels. One patient had metastases in the pelvic lymph nodes, liver and colon. Follow-up ranged from 7 to 289 months (mean, 127.2 ± 34.2). Six of the nine cases reported here were found in the usual locations. One patient had multiple tumors. The catecholamine level was elevated under basal conditions in 8 patients and during endoscopic resection of the tumor in 1 patient; it returned to normal after surgery in 8 patients. Three patients had recurrence and 1 had metastasis following surgery. Widespread soil acidification due to atmospheric acid deposition and agricultural fertilization may greatly accelerate soil carbonate dissolution and ***,to date,few studies have addressed these ***,we use meta-analysis and nationwide-survey datasets to investigate changes in soil inorganic carbon(SIC) stocks in *** observe an overall decrease in SIC stocks in topsoil(0-30 cm)(11.33 g C m-2yr-1) from the 1980 s to the 2010 *** SIC stocks have decreased by～8.99±2.24%(1.37±0.37 Pg C).The average SIC losses across China(0.046 Pg C yr-1) and in cropland(0.016 Pg C yr-1) account for～17.6%-24.0% of the terrestrial C sink and 57.1% of the soil organic carbon sink in cropland,*** deposition and climate change have profound influences on SIC *** estimate that～19.12%-19.47% of SIC stocks will be further lost by *** consumption of SIC may offset a large portion of global efforts aimed at ecosystem carbon sequestration,which emphasizes the importance of achieving a better understanding of the indirect coupling mechanisms of nitrogen and carbon cycling and of effective countermeasures to minimize SIC loss.

肾上腺皮质癌的综合治疗

目的:探讨肾上腺皮质癌临床及功能影像学和病理学特点,提高肾上腺皮质癌诊治水平。方法:分析肾上腺皮质癌患者的临床资料,根据其临床表现、肾上腺内分泌功能测定、影像学特点和病理结果做出诊断,进行手术治疗和米托坦药物治疗并随访。结果:93例患者年龄11~76岁,中位年龄48岁。男女比例1∶1.2。24 h尿游离皮质醇(urinary free cortisol,UFC)高者86例,促肾上腺皮质激素释放激素(adrenocorticotropic hormone,ACTH)下降88例,血浆皮质醇节律消失82例,醛固酮升高31例,性激素升高36例,术前神经烯醇化酶(neuron specific enolase,NSE)升高27例,胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)升高26例,76例大剂量和小剂量地塞米松抑制试验均不被抑制。高血压62例,典型Cushing综合征表现者81例。血糖升高54例,低血钾症21例,雄激素分泌者36例。肿瘤最大直径3~17 cm,伴肾上腺中央静脉、肾静脉及下腔静脉瘤栓者6例,手术至术后复发时间在1.2~5.0年。初发及随访中出现转移复发的56例,其中肺转移13例,肝转移17例,腹膜后淋巴结转移9例,腰椎转移7例,卵巢转移3例,腹壁及切口种植3例,其他部位转移4例,初发即远处转移者15例。行根治性切除术77例,侵及同侧肾者做肾和肾上腺切除术11例,肾上腺肿瘤并腔静脉癌栓切除5例,腔静脉部分切除3例。临床分期为Ⅰ期39例,Ⅱ期28例,Ⅲ期16例,Ⅳ期10例。随访8~69个月,手术5年以上的患者中有43例仍存活。结论:肾上腺皮质癌早期诊断非常关键,功能影像学检查结合临床特点及内分泌激素水平可确诊,根治性手术是唯一有效的治疗方法,米托坦可作为辅助治疗用于复发转移或无法手术治疗的患者,肿瘤恶性程度高,预后差。

It's a challenge for detecting the therapeutic targets of a polypharmacological drug from variations in the responsed networks in thedfferentiated populations with complex diseases,as stable coronary heart ***,in an adaptive,31-center,randomized,double-blind trial invoving 920 patients with moderate symptomatic stable angina treated by 14-day Danhong injection(DHI),a kind of polyphamacological drug with high quality control,or placebo(0.9%saline),with 76-day following-up,we firstly confrmed that DHl couldincrease the proportion of patients with clinically significant changes on angina-frequency assessed by Seattle Angina Questionnaire(ASAQ-AF220)(12.78%at Day 30,95%confidence interval[C]5.86-19.71%,P=0.0003,13.82%at Day 6C0,95%CI 6.82-20.82%,P=0.0001and 8.95%at Day 90,95%CI 2.06-15.85%,P=0.01).We also found that there were no significant differences in new-onset major vascularevents(P=0.8502)and serious adverse events(P=0.9105)between DHl and *** performing the RNA sequencing in 62 selectedpatients,we developed a systemic modular approach tp identfy differentilly expressed modules(DEMs)of DHI with the Z_(summay)valueless than 0 compared with the control group,calculated by weighted gene co-expression network analysis(WGCNA),and sketched out thebasic framework on a modular map with 25 functional modules targeted by ***,the effective therapeutic module(ETM),defined as the highest corelation value with the phenotype alteration(SAQ-AF,the change in SAQ-AF at Day 30 from baseline)calculatedby WGCNA,was identifed in the population with the best effect(ASAQ-AF240),which is related to anticoagulation and regulation ofcholesterol *** assessed the modular flexbility of this ETM using the global topological D value based on Euclidean distance,which is corelated with phenotype alteration(r^(2):0.8204,P=0.019)by linear *** study identified the ant-angina therapeuticmodule in the effective population treated by the multi-target

Lower hybrid current drive experiments on the HT-7 device have been carried out by scanning the following parameters: central line averaged electron density (ne = 0.6 - 2.0 × 1019m-3) and toroidal magnetic field (Bt = 1.62 - 2.0 T). The dependence of current drive efficiency on these parameters has been studied and the experimental curves of current drive efficiency as a function of ne and Bt have also been obtained. From these experimental results, it can be seen that current drive efficiency rises with the increase of toroidal magnetic field. As plasma density increases, the current drive efficiency first increases to a certain value, then gradually decreases, that is, there exists an optimized density regime where a better drive efficiency can be obtained. The analysis shows that the current drive efficiency is mainly affected by wave accessibility and impurity concentration, and the competition of these two factors determines the current drive efficiency.

The Large sky Area Multi-Object Fiber Spectroscopic Telescope（LAMOST） general survey is a spectroscopic survey that will eventually cover approximately half of the celestial sphere and collect 10 million spectra of stars, galaxies and QSOs. Objects in both the pilot survey and the first year regular survey are included in the LAMOST DR1. The pilot survey started in October 2011 and ended in June 2012, and the data have been released to the public as the LAMOST Pilot Data Release in August 2012. The regular survey started in September 2012, and completed its first year of operation in June 2013. The LAMOST DR1 includes a total of 1202 plates containing 2 955 336 spectra, of which 1 790 879 spectra have observed signalto-noise ratio（SNR） ≥ 10. All data with SNR ≥ 2 are formally released as LAMOST DR1 under the LAMOST data policy. This data release contains a total of 2 204 696 spectra, of which 1 944 329 are stellar spectra, 12 082 are galaxy spectra and 5017 are quasars. The DR1 not only includes spectra, but also three stellar catalogs with measured parameters： late A,FGK-type stars with high quality spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational and instrumental limitations, data reduction and analysis, and some caveats. A description of the FITS structure of spectral files and parameter catalogs is also provided.

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1(PD-[L]1)inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1 inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1 blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2018 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-11.0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,19.2[16.1-27.3]vs.15.7 months[13.0-20.2];adjusted HR,0.63,P=0.001;ORR,60.1%vs.32.0%;P<0.001).Grade 3/4 adverse events were observed at a rate of 15.8%and 7.5%in combination and monotherapy groups,*** results suggest that TACE plus PD-(L)1 blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.

We present the observational results from a detailed timing analysis of the black hole candidate EXO 1846-031 during its outburst in 2019 with the observations of Insight-HXMT,NICER and *** outburst can be classified roughly into four different ***-C quasi-periodic oscillations(QPOs)observed by NICER(about 0.1-6 Hz)and Insight-HXMT(about 0.7-8 Hz)are also reported in this ***,we study various physical quantities related to QPO *** QPO rms-frequency relationship in the energy band 1-10 keV indicates that there is a turning pointing in frequency around2 Hz,which is similar to that of GRS 1915+105.A possible hypothesis for the relationship above may be related to the inclination of the source,which may require a high inclination to explain *** relationships between QPO frequency and QPO rms,hardness,total fractional rms and count rate have also been found in other transient sources,which can indicate that the origin of type-C QPOs is non-thermal.

This paper describes the data release of the LAMOST pilot survey, which includes data reduction, calibration, spectral analysis, data products and data access. The accuracy of the released data and the information about the FITS headers of spectra are also introduced. The released data set includes 319 000 spectra and a catalog of these objects.

We operated a p-type point contact high purity germanium(PPCGe)detector(CDEX-1B,1.008 kg)in the China Jinping Underground Laboratory(CJPL)for 500.3 days to search for neutrinoless double beta(0νββ)decay of ^(76)Ge.A total of 504.3 kg⋅day effective exposure data was *** anti-coincidence and the multi/single-site event(MSE/SSE)discrimination methods were used to suppress the background in the energy region of interest(ROI,1989–2089 keV for this work)with a factor of 23.A background level of 0.33 counts/(keV⋅kg⋅yr)was *** lower limit on the half life of^(76)Ge 0νββdecay was constrained as T_(1/2)^(0ν)>1.0×10^(23)yr(90%C.L.),corresponding to the upper limits on the effective Majorana neutrino mass:<3.2–7.5 eV.
