Clinical experience of airway management and tracheal intubation under general anesthesia in patients with scar contracture of the neck

Background Because patients with scar contracture of the neck are at a high risk of loss of the airway control after anesthesia induction, awake intubation is usually recommended. This retrospective clinical study was designed to evaluate the possibility, safety and efficacy of airway management and tracheal intubation under general anesthesia in such patients. Methods This retrospective study included 1683 patients from January 1994 to December 2006 with scar contracture of the neck, aged 1.5--67.0 years, who were scheduled for elective plastic surgery under general anesthesia in Plastic Surgery Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College. Based on the results of the preoperative airway assessment, the patients were classified into group 1 （including 1375 patients with the atlanto-occipital extension of 〉20° and the Mallampatti＇s grade I or II） and group 2 （containing 308 patients with the atlanto-occipital extension of 〈20° and the Mallampatti＇s grade III or IV. In group 1, the intravenous induction and maintenance of anesthesia and succinylcholine for muscle relaxation were used. The intubation was done using a modified Macintosh technique. In group 2, the total intravenous anesthesia （TIVA） or the sevoflurane inhalation anesthesia was chosen and the spontaneous breathing was reserved during anesthesia. The intubation was performed by a fiberoptic stylet laryngoscope （FOSL）. The number of intubation attempts, intubation time and relative complications were observed and recorded in all patients. Results In group 1, the intubation was accomplished during the first attempt in 1279 cases （93%） and the intubation time was 〈3 minutes in 1304 cases （95%）. In group 2, the intubation was completed by the first attempt in 114 patients （37%） and 123 patients had an intubation time of 〈3 minutes （40%）. A Potential Degree Sequence Problem of the Loebl-Komlós-Sós Conjecture

A non-increasing sequenceπ=(d1,..., dn) of nonnegative integers is said to be a graphic sequence if it is realizable by a simple graph G on n vertices. In this case, G is referred to as a realization ofπ. In terms of graphic sequences, the Loebl-Komlós-Sós conjecture states that for any integers k and n, if π=(d1,..., dn)is a graphic sequence with d「n/2」≥k, then every realization ofπcontains all trees with k edges as *** problem can be viewed as a forcible degree sequence problem. In this paper, we consider a potential degree sequence problem of the Loebl-Komlós-Sós conjecture, that is, we prove that for any integers k and n, ifπ=(d1,..., dn) is a graphic sequence with d「n/2」≥k, then there is a realization ofπcontaining all trees with k edges as subgraphs.

Facile preparation of a SiC@SiO_(2)nanowire-toughened ZrB_(2)-SiC/SiC bilayer coating with good interfacial bonding,high toughness,and excellent cyclic ablation resistance on C/CA composites

Preparing antioxidant coatings to address the inherent oxidation sensitivity of carbon fiber-reinforced carbon aerogel(C/CA)composites is a feasible way to promote their application in oxidizing environments as thermal insulation ***,preparing the coatings with excellent oxidation and ablation resistance while avoiding evident damage to the C/CA substrate still remains a ***,a SiC@SiO_(2)nanowire-toughened ZrB2–SiC/SiC bilayer coating with a large thickness of 500μm was prepared on C/CA using a one-step low-temperature reaction sintering method,which simultaneously formed a sintered outer layer with even-distributed nanowires and a siliconized gradient inner *** courtesy of the synergic thermal response of the layers and the crack deflection induced by the nanowires,the resulting coating has moderate residual compressive stress of 0.08–1.22 GPa in the interface,high interfacial bonding strength of 6.02 MPa,and good fracture toughness of 4.36 MPa·m^(1/2).Benefited from the optimum components and improved structure,the coating shows excellent cyclic ablation resistance with linear ablation rates of 0.1μm/s at 1650℃for 1500 s(300 s×5 cycles)and 0.4μm/s at 1850℃for 900 s(300 s×3 cycles).The one-step preparation strategy contributes to little damage to the substrate,thus showing the well-preserved mechanical and thermal insulation properties.

Forests,the largest terrestrial carbon sinks,play an important role in carbon sequestration and climate change *** forest attributes and environmental factors have been shown to impact aboveground biomass,their influence on biomass stocks in species-rich forests in southern China,a biodiversity hotspot,has rarely been *** this study,we characterized the effects of environmental factors,forest structure,and species diversity on aboveground biomass stocks of 30 plots(1 ha each) in natural forests located within seven nature reserves distributed across subtropical and marginal tropical zones in Guangxi,*** results indicate that forest aboveground biomass stocks in this region are lower than those in mature tropical and subtropical forests in other ***,we found that aboveground biomass was positively correlated with stand age,mean annual precipitation,elevation,structural attributes and species richness,although not with species *** we compared stands with the same basal area,we found that aboveground biomass stock was higher in communities with a higher coefficient of variation of diameter at breast *** findings highlight the importance of maintaining forest structural diversity and species richness to promote aboveground biomass accumulation and reveal the potential impacts of precipitation changes resulting from climate warming on the ecosystem services of subtropical and northern tropical forests in ***,many natural forests in southern China are not fully ***,their continued growth will increase their carbon storage over time.

Effects of cooling rate on microstructure and microhardness of directionally solidified Galvalume alloy

The influences of cooling rate on the phase constitution,microstructural length scale,and microhardness of directionally solidified Galvalume(Zn-55Al-1.6Si)alloy were investigated by directional solidification experiments at different withdrawal speeds(5,10,20,50,100,200,and 400μm·s^(-1)).The results show that the microstructure of directionally solidified Galvalume alloys is composed of primary Al dendrites,Si-rich phase and(Zn-Al-Si)ternary eutectics at the withdrawal speed ranging from 5 to 400μm·s^(-1).As the withdrawal speed increases,the segregation of Si element intensifies,resulting in an increase in the area fraction of the Si-rich *** addition,the primary Al dendrites show significant refinement with an increase in the withdrawal *** relationship between the primary dendrite arm spacing(λ_(1))and the thermal parameters of solidification is obtained:λ_(1)=127.3V^(-0.31).Moreover,as the withdrawal speed increases from 5 to 400μm·s^(-1),the microhardness of the alloy increases from 90 HV to 151 *** is a combined effect of grain refinement and second-phase strengthening.

Hot spot engineering in plasmonic nanostructures plays a significant role in surface-enhanced Raman scattering(SERS)for bioanalysis and cell ***,creating stable,reproducible,and strong SERS signals remains challenging due to the potential interference from surrounding chemicals and locating SERS-active analytes into hot-spot ***,we developed a straightforward approach to synthesize intra-gap nanoparticles encapsulating 4-nitrobenzenethiol(4-NBT)as a reporter molecule within these gaps to avoid outside *** made three kinds of intra-gap nanoparticles using nanorods,bipyramids,and nanospheres as cores,in which the nanorods based intra-gap nanoparticles exhibit the highest SERS *** advantage of our method is the ease of preparation of high-yield and stable intra-gap nanoparticles characterized by a short incubation time(10 min)with 4-NBT and quick synthesis without requiring an additional step to centrifuge for the purification of core *** intense localized field in the synthesized hot spots of these plasmonic gap nanostructures holds great promise as a SERS substrate for a broad range of quantitative optical applications.
