Background:There is no consensus on whether triplet regimen is better than doublet regimen in the first-line treatment of advanced gastric cancer(AGC).We aimed to compare the efficacy and safety of oxaliplatin plus capecitabine(XELOX)and epirubicin,oxaliplatin,plus capecitabine(EOX)regimens in treating AGC.Methods:This phase III trial enrolled previously untreated patients with AGC whowere randomly assigned to receive the XELOXor EOXregimen.The primary endpoint was non-inferiority in progression-free survival(PFS)for XELOX as compared with EOX on an intention-to-treat basis.Results:Between April 10,2015 andAugust 20,2020,448AGCpatientswere randomized to receive XELOX(n=222)or EOX(n=226).The median PFS(mPFS)was 5.0 months(95%confidence interval[CI]=4.5-6.0 months)in the XELOX arm and 5.5 months(95%CI=5.0-6.0 months)in the EOX arm(hazard ratio[HR]=0.989,95%CI=0.812-1.203;P_(non-inferiority)=0.003).There was no significant difference inmedian overall survival(mOS)(12.0 vs.12.0months,P=0.384)or objective response rate(37.4%vs.45.1%,P=0.291)between the two groups.In patients with poorly differentiated adenocarcinoma and liver metastasis,the EOX arm had a significantly longer mOS(P=0.021)and a trend of longer mPFS(P=0.073)than the XELOX arm.The rate of grade 3/4 adverse events(AEs)was 42.2%(90/213)in the XELOX arm and 72.5%(156/215)in the EOX arm(P=0.001).The global health-related quality of life(QoL)score was significantly higher in the XELOX arm than in the EOX arm during chemotherapy.Conclusions:This non-inferiority trial demonstrated that the doublet regimen was as effective as the triplet regimen and had a better safety profile and QoL as a first-line treatment for AGC patients.However,the triplet regimen might have a survival advantage in patients with poorly differentiated adenocarcinoma and liver metastasis.

Selection of first-line systemic therapies for advanced hepatocellular carcinoma:A network meta-analysis of randomized controlled trials

BACKGROUND The majority of clinical trials of first-line systemic treatments for hepatocellular carcinoma(HCC)used placebo or sorafenib as comparators,and there are limited data providing a cross comparison of treatments in this setting,especially for newly-approved immune checkpoint inhibitor and vascular endothelial growth factor inhibitor combination treatments.AIM To systematically review and compare response rates,survival outcomes,and safety of first-line systemic therapies for advanced hepatocellular carcinoma.METHODS We searched PubMed,Science Direct,the Cochrane Database,Excerpta Medica Database,and abstracts from the American Society of Clinical Oncology 2020 annual congress.Eligible studies were randomized controlled trials of systemic therapy enrolling adults with advanced/unresectable HCC.Risk of bias was assessed with the Cochrane risk of bias tool for randomized controlled trials.A network meta-analysis was used to synthesize data and perform direct and indirect comparisons between treatments.P value,a frequentist analog to the surface under the cumulative ranking curve,was used to rank treatments.RESULTS In total,1398 articles were screened and 27 included.Treatments compared were atezolizumab plus bevacizumab,brivanib,donafenib,dovitinib,FOLFOX4,lenvatinib,linifanib,nintedanib,nivolumab,sorafenib,sunitinib,vandetanib,11 sorafenib combination therapies,and three other combination therapies.For overall response rate,lenvatinib ranked 1/19,followed by atezolizumab plus bevacizumab and nivolumab.For progression-free survival(PFS),atezolizumab+bevacizumab was ranked 1/15,followed by lenvatinib.With the exception of atezolizumab+bevacizumab[hazard ratios(HR)PFS=0.90;95%confidence interval(CI):0.64-1.25],the estimated HRs for PFS for all included treatments vs lenvatinib were>1;however,the associated 95%CI passed through unity for bevacizumab plus erlotinib,linifanib,and FOLFOX4.For overall survival,atezolizumab plus bevacizumab was ranked 1/25,followed by vandetanib 100 mg/d and donafinib,with lenvatin

Early Kinetics of Procalcitonin in Predicting Surgical Outcomes in Type A Aortic Dissection Patients

Background： In cardiac surgery, elevation of procalcitonin （PCT） could be observed postoperatively in the absence of any evidence of infection and also seems to be a prognostic marker. PCT levels measured in patients undergoing Type A aortic dissection （TAAD） were used to determine prognostic values for complications and surgical outcomes. Methods： Measurements of PCT, C-reactive protein （CRP）, and leukocyte count were observed in TAAD surgery patients （n = 251 ; average age： 49.02 ± 12.83 years; 78.5% male） at presurgery （TO） and 24 h （T1）, 48 h （T2）, and 7 days （T3） postsurgery. PCT clearance （PCTc） oll days 2 and 7 was calculated：（PCTd - PCTd,±,, d,,v）/PCTdx 100%. Endotracheal intubation duration, length of stay （LOS） in the Intensive Care Unit （1CU）/hospital, and complications were recorded± Results： PCT peaked 24 h postsurgery （median 2.73 ng/ml） before decreasing, Correlation existed between PCT levels at T1 and duration of cardiopulmonary bypass （P = 0.001, r = 0.278）. Serum PCT concentrations were significantly higher in nonsurvivor and multiple organ dysfunction syndrome groups on all postoperative days. PCT levels at T1 correlated with length of time of ventilation support and ICU/hospital LOS. Comparing PCT values of survivors versus nonsurvivors, a PCT cutoff level of 5.86 ng/ml at T2 had high sensitivity （70.6%） and specificity （74.3%） in predicting in-hospital death. PCTc-day 2 and 7 were significantly higher in survivor compared with nonsurvivor patients （38% vs. 8%, P = 0.012, 83% vs. -39%, P 〈 0.001）. A PCTc-day 7 cutoff point of 48.7% predicted survival with high sensitivity （77.8%） and specificity （81.8%）. Conclusions： PCT level and PCTc after TAAD surgery might serve as early prognostic markers to predict postoperative outcome. PCT measurement may help identify high-risk patients.

There is growing evidence of the high transmission potential of COVID-19 through virus-laden aerosols.Because aerosols are inhaled in various concentrations,an overall assessment of transmission risks at different indoor scales is crucial.However,a comprehensive risk assessment method that evaluates the direct link between short-range and room-scale zones is stl lacking.In this paper,a risk assessment model combining both short-range and room-scale effects is developed to obtain effective reproduction number in confined indoor environments,called Turbulent Jet Wells Riley(TJWR)model.Combined with the viral load data and aerosol generation data of different human respiratory activities,the Monte Carlo simulation method is applied to calculate the quanta emission rate,which further provides the input parameters of the TJWR model.Three known outbreaks(Hangzhou banquet hall X,Guangzhou restaurant Y,and Hong Kong restaurant Z,China)are chosen to validate the TJWR model.Results show that the TJWR model is more efficient than the original Wells-Riley model.The average relative error of the TJWR model ranges between 9%and 44%,while for the Wells-Riley model,it ranges between 57%and 78%.The TJWR model also proves that infection risk assessments using the well-mixed assumption can systematically underestimate the transmission risk for those close to the source.Additionally,there is a significant positive linear correlation between the total number of exposed individuals at the short-range and the effective reproduction number.This newly developed TJWR model has great potential for rapid and real-time overall airborne transmission risk assessment in buildings and cities.

Hemorrhagic transformation in patients with large-artery atherosclerotic stroke is associated with the gut microbiota and lipopolysaccharide

Hemorrhagic transformation is a major complication of large-artery atheroscle rotic stroke(a major ischemic stro ke subtype)that wo rsens outcomes and increases mortality.Disruption of the gut microbiota is an important feature of stroke,and some specific bacteria and bacterial metabolites may contribute to hemorrhagic transformation pathogenesis.We aimed to investigate the relationship between the gut microbiota and hemorrhagic transformation in largearte ry atheroscle rotic stro ke.An observational retrospective study was conducted.From May 2020 to September 2021,blood and fecal samples were obtained upon admission from 32 patients with first-ever acute ischemic stroke and not undergoing intravenous thrombolysis or endovascular thrombectomy,as well as 16 healthy controls.Patients with stro ke who developed hemorrhagic transfo rmation(n=15)were compared to those who did not develop hemorrhagic transformation(n=17)and with healthy controls.The gut microbiota was assessed through 16S ribosomal ribonucleic acid sequencing.We also examined key components of the lipopolysaccharide pathway:lipopolysaccharide,lipopolysaccharide-binding protein,and soluble CD14.We observed that bacterial diversity was decreased in both the hemorrhagic transformation and non-hemorrhagic transfo rmation group compared with the healthy controls.The patients with ischemic stro ke who developed hemorrhagic transfo rmation exhibited altered gut micro biota composition,in particular an increase in the relative abundance and dive rsity of members belonging to the Enterobacteriaceae family.Plasma lipopolysaccharide and lipopolysaccharide-binding protein levels were higher in the hemorrhagic transformation group compared with the non-hemorrhagic transfo rmation group.lipopolysaccharide,lipopolysaccharide-binding protein,and soluble CD14 concentrations were associated with increased abundance of Enterobacte riaceae.Next,the role of the gut microbiota in hemorrhagic transformation was evaluated using an experimental stroke rat model.In this model,t

Background:To analyze the expression of cluster of differentiation 80(CD80)/cluster of differentiation 163(CD163)in colorectal cancer(CRC)patients and their correlation with the syndrome of traditional Chinese medicine,pathology and prognosis.Methods:(1)The correlation between the pathological characteristics of 232 postoperative CRC patients and the deficiency and excess syndromes of traditional Chinese medicine was analyzed using the chi-square test,Spearman’s correlation,and Cox regression methods.(2)Immunohistochemistry and quantitative real-time PCR were used to detect the expression of CD80 and CD163 in cancer and paracancer tissues of CRC patients.(3)The relationships between the changes of CD80 and CD163 and the prognosis of CRC patients with deficiency syndrome.Patient survival were analyzed using cardinality and Cox regression proportional-hazards model regression.Results:(1)The degree of differentiation and tumor node metastasis stage of CRC were statistically different between patients with deficiency and excess syndromes(P<0.05);carcinoembryonic antigen and carbohydrate antigen 19-9 were highly expressed in the excess syndrome group,and both were significantly correlated with the distribution of traditional Chinese medicine syndromes(P<0.001);deficiency and excess syndromes,and carbohydrate antigen 19-9 were all independent factors affecting the postoperative survival of CRC patients.(2)The distribution of post-operative survival in CRC patients was significantly correlated with the distribution of the disease type(P<0.001).(3)The expression levels of CD163 protein and message RNA were significantly higher in CRC cancer tissues than in paraneoplastic tissues(P<0.001);whereas the expression of CD80 was significantly higher in paraneoplastic tissues than in cancer tissues(P<0.001).(4)The expression levels of CD80/CD163 were significantly different in different parts of the tissues of patients with deficiency and excess syndromes(P<0.001).(5)CRC patients with high CD80 expression and low CD163 expressio

Trend of Storm Surge Induced by Typical Landfall Super Typhoons During 1975–2021 in the Eastern China Sea

Climate change affects the activity of global and regional tropical cyclones(TCs).Among all TCs,typical super typhoons(STYs)are particularly devastating because they maintain their intensity when landing on the coast and thus cause casualties,economic losses,and environmental damage.Using a 3D tidal model,we reconstructed the typhoon(TY)wind field to simulate the storm surge induced by typical STYs.The TY activity was then analyzed using historical data.Results showed a downtrend of varying degrees in the annual frequency of STYs and TCs in the Western North Pacific(WNP)Basin,with a significant trend change observed for TCs from 1949 to 2021.A large difference in the interannual change in frequency was found between STYs and TCs in the WNP and Eastern China Sea(ECS).Along the coast of EC,the frequency of landfall TCs showed a weak downtrend,and the typical STYs showed reverse micro growth with peak activity in August.Zhejiang,Fujian,and Taiwan were highly vulnerable to the frontal hits of typical STYs.Affected by climate change,the average lifetime maximum intensity(LMI)locations and landfall locations of typical STYs in the ECS basin showed a significant poleward migration trend.In addition,the annual average LMI and accumulated cyclone energy showed an uptrend,indicating the increasing severity of the disaster risk.Affected by the typical STY activity in the ECS,the maximum storm surge area also showed poleward migration,and the coast of North China faced potential growth in high storm surge risks.

Binary Hybrid Artificial Hummingbird with Flower Pollination Algorithm for Feature Selection in Parkinson’s Disease Diagnosis

Parkinson’s disease is a neurodegenerative disorder that inflicts irreversible damage on humans.Some experimental data regarding Parkinson’s patients are redundant and irrelevant,posing significant challenges for disease detection.Therefore,there is a need to devise an effective method for the selective extraction of disease-specific information,ensuring both accuracy and the utilization of fewer features.In this paper,a Binary Hybrid Artificial Hummingbird and Flower Pollination Algorithm(FPA),called BFAHA,is proposed to solve the problem of Parkinson’s disease diagnosis based on speech signals.First,combining FPA with Artificial Hummingbird Algorithm(AHA)can take advantage of the strong global exploration ability possessed by FPA to improve the disadvantages of AHA,such as premature convergence and easy falling into local optimum.Second,the Hemming distance is used to determine the difference between the other individuals in the population and the optimal individual after each iteration,if the difference is too significant,the cross-mutation strategy in the genetic algorithm(GA)is used to induce the population individuals to keep approaching the optimal individual in the random search process to speed up finding the optimal solution.Finally,an S-shaped function converts the improved algorithm into a binary version to suit the characteristics of the feature selection(FS)tasks.In this paper,10 high-dimensional datasets from UCI and the ASU are used to test the performance of BFAHA and apply it to Parkinson’s disease diagnosis.Compared with other state-of-the-art algorithms,BFAHA shows excellent competitiveness in both the test datasets and the classification problem,indicating that the algorithm proposed in this study has apparent advantages in the field of feature selection.

Short-time maritime target detection based on polarization scattering characteristics

In this paper,a detection method combining Cameron decomposition based on polarization scattering characteristics in sea clutter background is proposed.Firstly,the Cameron decomposition is exploited to fuse the radar echoes of full polarization channels at the data level.Due to the artificial material structure on the surface of the target,it can be shown that the non-reciprocity of the target cell is stronger than that of the clutter cell.Then,based on the analysis of the decomposition results,a new feature with scattering geometry characteristics in polarization domain,denoted as Cameron polarization decomposition scattering weight(CPD-SW),is extracted as the test statistic,which can achieve more detailed descriptions of the clutter scattering characteristics utilizing the difference between their scattering types.Finally,the superiority of the proposed CPD-SW detector over traditional detectors in improving detection performance is verified by the IPIX measured dataset,which has strong stability under short-time observation in threshold detection and can also improve the separability of feature space zin anomaly detection.

One of the bottlenecks in the clinical treatment of small cell lung cancer(SCLC)is its susceptibility to multidrug resistance.While SCLC is sensitive to platinum-based chemotherapy at the initial stage of treatment,it can rapidly become drug-resistant,allowing tumor growth to accelerate.A significant decrease in drug sensitivity after recurrence and multidrug resistance can seriously interfere with the effectiveness of treatment,making sensitivity maintenance one of the urgent challenges to be solved.Therefore,discovering prognostic markers for cisplatin chemotherapy and understanding the mechanisms of cisplatin resistance have great practical significance for improving the outcomes of sCLC patients.
