Cetuximab Combination with Chemotherapy in Advanced Non-Small Cell Lung Cancer

Objective： To observe the efficacy and safety of cetuximab combined with chemotherapy in advanced non-small-cell lung cancer （NSCLC）, and to investigate the association of status of K-RAS gene mutation and epidermal growth factor receptor （EGFR） genotype with clinical outcome. Methods： Between Jan. 2006 and Sep. 2009, nineteen patients with advanced NSCLC received cetuximab （〉4 weeks） combined with chemotherapy in Department of Thoracic Oncology at Beijing Cancer Hospital. Response, survival and toxicity were retrospectively assessed, epidermal growth factor receptor （EGFR） protein expression was evaluated by ELISA Kit. The status of K-RAS gene mutation was tested by PCR-RFLP and EGFR gene amplification was measured by EGFR fluorescence in situ hybridization （FISH）. Results： Partial response（PR） was observed in 26.3%（5/19） of the patients and stable disease（SD） in 52.6%（10/19）. Median progression free survival（PFS） was 6 months （95% CI： 3.6-8.4）. Median overall survival （MST） and 1-year survival rate（SR） were 10.6 months （95% CI： 6.6-14.6） and 47.6%, respectively. Mild or moderate skin rash was the most common toxicity related with cetuximab. K-RAS gene mutation, EGFR protein level and amplification have little correlation with prognosis. Conclusion： Cetuximab combined with chemotherapy was tolerable and the skin rash related with cetuximab was mild to moderate. Cetuximab may prolong survival of the patients who failed to previous chemotherapy.

Clinical outcomes of EGFR-TKI treatment and genetic heterogeneity in lung adenocarcinoma patients with EGFR mutations on exons 19 and 21

Background:Epidermal growth factor receptor(EGFR) mutations,including a known exon 19 deletion(19 del) and exon 21 L858 R point mutation(L858R mutation),are strong predictors of the response to EGFR tyrosine kinase inhibitor(EGFR-TKI) treatment in lung ***,whether patients carrying EGFR 19 del and L858 R mutations exhibit different responsiveness to EGFR-TKls and what are the potential mechanism for this difference remain *** study aimed to investigate the clinical outcomes of EGFR-TKI treatment in patients with EGFR 19 del and L858 R mutations and explore the genetic heterogeneity of tumors with the two mutation ***:Of 1127 patients with advanced lung adenocarcinoma harboring EGFR 19 del or L858 R mutations,532 received EGFR-TKI treatment and were included in this *** 19 del and L858 R mutations were detected by using denaturing high-performance liquid chromatography(DHPLC).T790 M mutation,which is a common resistant mutation on exon 20 of EGFR,was detected by amplification refractory mutation system(ARMS).Next-generation sequencing(NGS) was used to explore the genetic heterogeneity of tumors with EGFR 19 del and L858 R ***:Of the 532 patients,319(60.0%) had EGFR 19 del,and 213(40.0%) had L858 R *** patients with EGFR 19 del presented a significantly higher overall response rate(ORR) for EGFR-TKI treatment(55.2%vs.43.7%,P = 0.017) and had a longer progression-free survival(PFS) after first-line EGFR-TKI treatment(14.4 vs.11.4 months,P = 0.034) compared with those with L858 R ***,no statistically significant difference in overall survival(OS) was observed between the two groups of patients.T790 M mutation status was analyzed in 88 patients before EGFR-TKI treatment and 134 after EGFR-TKI treatment,and there was no significant difference in the co-existence of T790 M mutation with EGFR 19 del and L858 R mutations before EGFR-TKI treatment(5.6%vs.8.8%,P = 0.554)or after treatme

High Expression of ERCC1 Is a Poor Prognostic Factor in Chinese Patients with Non-small Cell Lung Cancer Receiving Cisplatin-based Therapy

Objective： to determine the role of excision repair cross-complementing group 1 gene （ERCC1） and ribonucleotide reductase subunit M1 （RRM1） expression in predicting response and survival in Chinese patients with advanced stage non-small cell lung cancer （NSCLC） treated with platinum-based chemotherapy. Methods： Formalin-fixed, paraffin-embedded biopsy tissues were retrospectively obtained from 160 advanced NSCLC patients. mRNA expression levels of ERCC1 and RRM1 were determined by real-time PCR. Results： Associations between mRNA expression level of ERCC1 and RRM1 and clinic-pathologic parameters were analyzed. One handred and forty two （88.75%） specimens were successfully amplified. mRNA expression level of ERCC1 and RRM1 was negatively associated with tumor response. ERCC1 expression levels ranged from 0.01 to 78.79 [median 0.63, mean 4.25 and standard deviation （SD） 9.23] and RRM1 from 0.00 to 30.91 （median 0.63, mean 0.87 and SD 3.36）. By adopting cut-off values according to median expression levels, we found that MST in patients with low ERCC1 mRNA levels was significantly longer in overall population than in patients with higher levels （18.63 versus 13.69 months, log-rank 5.73, P=0.017）, but we did not found the survival benefit of the patients with low expression level of RRM1. （19.07 versus 14.88, log-rank 1.66, P=0.197）. Further, in the multivariable Cox regression model analysis we found that low level of ERCC1 expression, the presence of weight loss and target therapy, were significant prognostic factors for survival. Conclusion： ERCC1 expression is associated with patients’ survival in Chinese advanced NSCLC patients treated with platinum-based regimen and may serve as a biomarker in predicting tumor response and clinical outcome in the patient population.

Nanostructured lubricant additives for titanium alloy:Lubrication by the solid-liquid interface with Coulomb repulsion

In this work,the advantage of Coulomb repulsion in the intermolecular forces experienced by molecules on the solid–liquid nanosized contact interface is taken,and the superior friction-reducing property of Cu_(3)(PO_(4))2·3H_(2)O(CuP)oil-based additives has been confirmed for titanium ***-dimensional(3D)CuP nanoflowers(CuP-Fs)with a strong capillary absorption effect are prepared to achieve the homogeneous mixing of solid CuP and lubricating *** by CuP-Fs additives for titanium alloy,friction coefficient(COF)can be reduced by 73.68%,and wear rate(WR)reduced by 99.69%.It is demonstrated that the extraordinary friction-reducing property is due to the repulsive solid–liquid interface with low viscous shear force originating from Coulomb repulsion between polar water molecules in CuP and non-polar oil ***,any steric hindrance or connection between this repulsive solid–liquid interface will trigger the adhesion and increase the viscous shear force,for example,dispersant,hydrogen bondings,and shaky adsorbed water ***,the lamellar thickness of CuP and the molecular size of lubricant both have a great influence on tribological *** the lubrication mechanism based on interface Coulomb repulsion is proposed that may help broaden the scope of the exploration in low-friction nanomaterial design and new lubricant systems.

Immediate Versus Delayed Treatment with EGFR Tyrosine Kinase Inhibitors after First-line Therapy in Advanced Non-small-cell Lung Cancer

Objective： To analyze the outcomes of patients who received TKI immediately after the first-line without progression as maintenance treatment （immediate group） vs. those received delayed treatment upon disease progression as second-line therapy （delayed group）. Methods： The study included 159 no-small-cell lung cancer （NSCLC） patients who received gefitinib or erlotinib as maintenance treatment in the immediate group （85 patients） or as second-line therapy in the delayed group （74 patients）. The primary end point was progression-free survival （PFS）. EGFR mutation status was detected using denaturing high-performance liquid chromatography （DHPLC）. Results： PFS was 17.3 and 16.4 months in the immediate and delayed groups, respectively （hazard ratio [HR], 0.99; 95% Confidence Interval [CI]： 0.69-1.42; P=0.947）. In a subgroup analysis that included only patients with EGFR mutation, however, PFS was significantly longer in the immediate group than in the delayed group （HR, 0.48; 95% CI： 0.27-0.85; P=0.012）. In patients with wild type EGFR, the risk for disease progression was comparable between the two groups （HR, 1.23; 95% CI： 0.61-2.51; P=0.564）. No significant difference was demonstrated between the immediate and delayed group in terms of the overall survival （OS）（26.1 months vs. 21.6 months, respectively; HR=0.53; 95% CI： 0.27 to 1.06; P=0.072）. There was also no difference in the incidence of adverse events between the two groups. Conclusions： EGFR TKI maintenance improves PFS in patients with EGFR mutation. Prospectively designed clinical studies that compare TKI immediate vs. delayed treatment after first-line chemotherapy upon disease progression are needed.

Ellagic acid inhibits gastric cancer cells by modulating oxidative stress and inducing apoptosis

Ash removal from inferior coal via ammonium fluoride roasting and simultaneous yield of white carbon black

The quality upgrading and deashing of inferior coal by chemical method still faces great *** dangers of strong acid,strong alkali,waste water and exhaust gas as well as high cost limit its industrial *** paper systematically investigates the ash reduction and desilicification of two typical inferior coal utilizing ammonium fluoride roasting *** the optimal conditions,for fat coal and gas coal,the deashing rates are 69.02%and 54.13%,and the desilicification rates are 92.64%and 90.27%,*** molar dosage of ammonium fluoride remains consistent for both coals;however,the gas coal,characterized by a lower ash and silica content(less than half that of the fat coal),achieves optimum deashing effect at a reduced time and *** majority of silicon in coal transforms into gaseous ammonium fluorosilicate,subsequently preparing nanoscale amorphous silica with a purity of 99.90%through ammonia *** of the fluorine in deashed coal are assigned in inorganic minerals,suggesting the possibility of further fluorine and ash removal via *** research provides a green and facile route to deash inferior coal and produce nano-scale white carbon black simultaneously.

Distributed Cooperative Secondary Voltage Control in an Unbalanced Microgrid

The negative-sequence reactive power-conductance(Q^(−)-G) droop control strategy is a conventional method to compensate for the output unbalanced voltages in an islanded ***, the conflict between unbalanced voltage compensation and negative-sequence reactive power sharing, caused bythe impedance mismatching of distribution lines and distributedgenerators, has not been solved only by Q^(−)-G droop *** this paper, a distributed cooperative secondary unbalancedvoltage control strategy is proposed to decrease the outputvoltage unbalance factor (VUF) of each droop-controlled DG,as well as to further enhance the negative-sequence reactivepower sharing effectiveness among DGs by properly shiftingup and down the Q^(−)-G droop characteristics of each *** algorithm for adaptive VUF weight coefficient is proposedto better suppress VUF under severe imbalance ***, a negative-sequence small-signal model of an MGunder an unbalanced condition, considering the communicationdelay time of the proposed SUVC, is established to analyze thesystem’s stability and transient performance under the influenceof some critical parameters. Evaluation of lightning-induced overvoltage on a 10 kV distribution line based on electromagnetic return-stroke model using finite-difference time-domain

Accurate simulation of lightning-induced overvoltage for overhead distribution lines is helpful to prevent lightning trip *** electromagnetic return-stroke model was used to represent lightning and then a 3D finite-difference time-domain(FDTD)method was adopted to simulate the lightning-induced overvoltage on a distribution line without a field-line coupling *** lightning-induced overvoltage behave for different ground conductivity and varying distance between the distribution line and the lightning channel was *** results showed that the overvoltage waveforms at the centre point of the line corresponding to lightning strikes on the lossy ground and an ideal ground(σ=∞)were similar;however,the peak amplitudes of the waveform were affected by soil conductivity at a close *** relationship between magnitude of the overvoltage and distance can be described by a second-order exponential decay ***,the overvoltage calculated using the proposed model was compared with those obtained based on Agrawal's model and measurements made using the newly developed intelligent insulator on *** these comparisons,it could be concluded that the FDTD method with the electromagnetic return-stroke model produces reasonably accurate results of the attenuated oscillation waveform,which can better reproduce the overvoltage on operational distribution lines.
