Conversion therapy of a giant hepatocellular carcinoma with portal vein thrombus and inferior vena cava thrombus:A case report and review of literature

BACKGROUND The prognosis of hepatocellular carcinoma(HCC)combined with portal and hepatic vein cancerous thrombosis is poor,for unresectable patients the combination of targeted therapy and immune therapy was the first-line recommended treatment for advanced HCC,with a median survival time of only about 2.7-6 months.In this case report,we present the case of a patient with portal and hepatic vein cancerous thrombosis who achieved pathologic complete response after conversion therapy.CASE SUMMARY In our center,a patient with giant HCC combined with portal vein tumor thrombus and hepatic vein tumor thrombus was treated with transcatheter arterial chemoembolization(TACE),radiotherapy,targeted therapy and immunotherapy,and was continuously given icaritin soft capsules for oral regulation.After 7 months of conversion therapy,the patient's tumor shrank and the tumor thrombus subsided significantly.The pathology of surgical resection was in complete remission,and there was no progression in the postoperative follow-up for 7 months,which provided a basis for the future strategy of combined conversion therapy.CONCLUSION In this case,atezolizumab,bevacizumab,icaritin soft capsules combined with radiotherapy and TACE had a good effect.For patients with hepatocellular carcinoma combined with hepatic vein/inferior vena cava tumor thrombus,adopting a high-intensity,multimodal proactive strategy under the guidance of multidisciplinary team(MDT)is an important attempt to break through the current treatment dilemma.

Effects of targeted muscle reinnervation on spinal cord motor neurons in rats following tibial nerve transection

Targeted muscle reinnervation(TMR)is a surgical procedure used to transfer residual peripheral nerves from amputated limbs to targeted muscles,which allows the target muscles to become sources of motor control information for function reconstruction.However,the effect of TMR on injured motor neurons is still unclear.In this study,we aimed to explore the effect of hind limb TMR surgery on injured motor neurons in the spinal cord of rats after tibial nerve transection.We found that the reduction in hind limb motor function and atrophy in mice caused by tibial nerve transection improved after TMR.TMR enhanced nerve regeneration by increasing the number of axons and myelin sheath thickness in the tibial nerve,increasing the number of anterior horn motor neurons,and increasing the number of choline acetyltransferase-positive cells and immunofluorescence intensity of synaptophysin in rat spinal cord.Our findings suggest that TMR may enable the reconnection of residual nerve fibers to target muscles,thus restoring hind limb motor function on the injured side.

Thirty-day outcomes of in-hospital multi-vessel versus culpritonly revascularization strategy for ST-segment elevation myocardial infarction with multivessel coronary disease

BACKGROUND Many studies have demonstrated the benefit of complete multivessel revascularization versus culprit-only intervention in patients of ST-segment elevation myocardial infarction(STEMI)and multivessel coronary artery disease.However,only a few single-center retrospective studies were performed on small Chinese cohorts.Our study aims to demonstrate the advantage of multivessel percutaneous intervention(PCI)strategy on 30-day in-hospital outcomes to patients with STEMI and multivessel disease in larger Chinese population.METHODS From the Improving Care for Cardiovascular Disease in China-Acute Coronary Syndrome(CCC-ACS)project,5935 patients with STEMI and multivessel disease undergoing PCI and hospitalized for fewer than 30 days were analyzed.After 5:1 propensity score matching,3577 patients with culprit-only PCI and 877 with in-hospital multivessel PCI were included.The primary outcome was major adverse cardiovascular and cerebrovascular event(MACCE),defined as a composite of myocardial infarction,all-cause death,stent thrombosis,heart failure,and stroke.RESULTS Multivariable logistic regression analysis revealed that in-hospital multivessel PCI was associated with lower risk of 30-day MACCE(adjusted OR=0.75,95%CI:0.57-0.98,P=0.032)than culprit-only PCI and conferred no increased risk of allcause death,myocardial infarction,stent thrombosis,stroke,or bleeding.Subgroup analysis showed that MACCE reduction was observed more often from patients with trans-femoral access(OR=0.34,95%CI:0.15-0.74)than with trans-radial access(OR=0.87,95%CI:0.66-1.16,P for interaction=0.017).CONCLUSIONS The in-hospital multivessel PCI strategy was associated with a lower risk of 30-day MACCE than culprit-only PCI in patients with STEMI and multivessel coronary artery disease.

Trends and sex differences in atrial fibrillation hospitalization and catheter ablation at tertiary hospitals in China from 2013 to 2016

BACKGROUND Catheter ablation for atrial fibrillation(AF)is commonly performed worldwide.However,the clinical charac-teristics of hospitalized patients with AF and national trends in catheter ablation at tertiary hospitals in China remain unreported.METHODS This study used the Chinese national database(Hospital Quality Monitoring System)from 2013 to 2016,which is a mandatory database that collects the front page of patients’medical records for hospital accreditation,to describe the clinical cha-racteristics of patients with AF as an overall cohort and as subgroups divided by catheter ablation and sex.RESULTS Of 597,919 AF patients first admitted,57,983 patients underwent catheter ablation[56,384 cases(97.2%)of radiofre-quency ablation and 1599 cases(2.8%)of cryoablation]at 746 tertiary hospitals.Nearly 10%of patients hospitalized with AF at ter-tiary hospitals in China underwent catheter ablation,and the percentage of patients undergoing catheter ablation was on the rise bet-ween 2013 and 2016,and the number of cases increased by 2.5 times.Compared with AF patients who did not undergo catheter ablation,those who did were younger,more frequently male,and had fewer baseline comorbidities.Although the overall CHA_(2)DS_(2)-VASc score revealed over half of the patients were high-risk,patients who underwent catheter ablation were mostly low-risk(71.2%of males and 59.1%of females).Considering in-hospital adverse events,the overall pericardial tamponade and all-cause death incidences were 0.2%(0.6%in the ablation group)and 1.2%(0.1%in the ablation group),respectively;both of which were higher in females than males.CONCLUSIONS In this study,AF patients who underwent catheter ablation were relatively young,had a low thrombosis risk,and had few comorbidities and adverse events.Females were older and experienced more complications than males.

Dear editor,Wellens syndrome is characterized by electrocardiography(ECG)patterns of the biphasic or inverted T wave in the precordial leads with normal R wave progression and absence of Q waves,[1,2]which indicates occlusive lesions of the proximal left anterior descending(LAD)coronary artery and warrants.[1]These ECG changes usually occur during a pain-free interval and turn to a fatal consequence if without urgent intervention therapy.[1,3]However,in some cases,coronary stenosis was absent and ECG patterns manifested as Wellens syndrome,which was defined as pseudo-Wellens syndrome.

Stock market forecasting has drawn interest from both economists and computer scientists as a classic yet difficult topic.With the objective of constructing an effective prediction model,both linear and machine learning tools have been investigated for the past couple of decades.In recent years,recurrent neural networks(RNNs)have been observed to perform well on tasks involving sequence-based data in many research domains.With this motivation,we investigated the performance of long-short term memory(LSTM)and gated recurrent units(GRU)and their combination with the attention mechanism;LSTM+Attention,GRU+Attention,and LSTM+GRU+Attention.The methods were evaluated with stock data from three different stock indices:the KSE 100 index,the DSE 30 index,and the BSE Sensex.The results were compared to other machine learning models such as support vector regression,random forest,and k-nearest neighbor.The best results for the three datasets were obtained by the RNN-based models combined with the attention mechanism.The performances of the RNN and attention-based models are higher and would be more effective for applications in the business industry.

Unusual phenomenon of optimizing the Griewank function with the increase of dimension

The Griewank function is a typical multimodal benchmark function,composed of a quadratic convex function and an oscillatory nonconvex function.The comparative importance of Griewank's two major parts alters in different dimensions.Different from most test functions,an unusual phenomenon appears when optimizing the Griewank function.The Griewank function first becomes more difficult and then becomes easier to optimize with the increase of dimension.In this study,from the methodology perspective,this phenomenon is explained by structural,mathematical,and quantum analyses.Furthermore,frequency transformation and amplitude transformation are implemented on the Griewank function to make a generalization.The multi-scale quantum harmonic oscillator algorithm(MQHOA)with quantum tunnel effect is used to verify its characteristics.Experimental results indicate that the Griewank function's two-scale structure is the main reason for this phenomenon.The quantum tunneling mechanism mentioned in this paper is an effective method which can be generalized to analyze the generation and variation of solutions for numerous swarm optimization algorithms.

Glass-forming ability and corrosion performance of Mn-doped Mg-Zn-Ca amorphous alloys for biomedical applications

In the present work, ribbon and 2-mm rod samples of Mg-Zn-Ca-Mn alloys were prepared by meltspinning and copper mold injection methods, respectively. Effects of Mn doping on glass-forming ability and corrosion performance in simulated body fluid of Mg65Zn30Ca5 alloy were studied through X-ray diffraction, scanning electron microscopy, differential scanning calorimeter, and electrochemical and immersion tests. Results show that with the Mn addition increasing, all the ribbon samples are completely in amorphous state. However, the microstructure of 2-mm rod samples transfers from fully amorphous for the Mn-free alloy to almost polycrystalline state with precipitated Mg, Mn, and MgZn phases. Glass-forming ability of Mg65Zn30Ca5 alloy is decreased by Mn addition. Results of electrochemical and immersion tests demon- strate that the Mn-doped samples exhibit more negative corrosion potential and larger corrosion current density, suggesting that the corrosion resistance decreases with doping amount of Mn element increasing.

Prevalence, genetic diversity, and antimicrobial susceptibility profiles of Staphylococcus aureus isolated from bovine mastitis in Zhejiang Province, China

This study was conducted to determine genetic diversity and antimicrobial susceptibility profiles of Staphylococcus aureus recovered from bovine mastitis in Zhejiang Province, China. Out of 3178 quarter milk samples from 846 lactating cows, among which 459 cows (54.3%) were found HMT positive, 890 quarters (28%) were found having subclinical mastitis. From 75 representative S. aureus isolates, 16 distinct types were identified by pulsed-field gel electrophoresis (PFGE). Four major PFGE types (A, B, C, and D) accounted for 82.7% of all isolates, and type A (41.3%) was observed in multiple herds across the studied areas. Each region was found to have a predominant type: Hangzhou type A (64.1%), Ningbo type C (34.5%) and type B (23.1%), Jinhua type D (53.3%), and Taizhou type C (62.5%). Results of antimicrobial susceptibility tests showed that 90.7% of the isolates were resistant to at least one antimicrobial. Resistance to penicillin and ampicillin (77.3%), tetracycline (60.0%), or erythromycin (48.0%) was observed. The bacteria resistant to multiple antibiotics such as penicillin, ampicillin, tetracycline, and erythromycin were commonly found. The information obtained from this study is useful for designing specific control programs for bovine S. aureus mastitis in this region.

Effect of HIP Treatment on Fatigue Notch Sensitivity of Ti-6Al-4V Alloy Fabricated by Electron Beam Melting

The effect of hot isostatic pressing(HIP)treatment on the fatigue notch sensitivity of Ti-6A1-4V alloy fabricated by electron beam melting(EBM)was investigated.The results indicate that the fatigue notch sensitivity of the as-fabricated samples is much lower than that of HIPed samples.The variation of α lamella thickness and the distribution of pore defects have an effect on the fatigue strength in smooth and notched EBM Ti-6A1-4V samples,resulting in the divergence of the as-fabricated and HIPed samples on fatigue notch sensitivity.
