AIM: To systematically assess the efficacy and safety of β-adrenergic blocker plus 5-isosorbide mononitrate (BB + ISMN) and endoscopic band ligation (EBL) on prophylaxis of esophageal variceal rebleeding. METHODS: Randomized controlled trials (RCTs) comparing the efficacy and safety of BB + ISMN and EBL on prophylaxis of esophageal variceal rebleeding were gathered from Medline, Embase, Cochrane Controlled Trial Registry and China Biological Medicine database between January 1980 and August 2007. Data from five trials were extracted and pooled. The analyses of the available data using the Revman 4.2 software were based on the intention-to-treat principle. RESULTS: Four RCTs met the inclusion criteria. In comparison with BB + ISMN with EBL in prophylaxis of esophageal variceal rebleeding, there was no significant difference in the rate of rebleeding [relative risk (RR), 0.79; 95% CI: 0.62-1.00; P = 0.05], bleeding-related mortality (RR, 0.76; 95% CI: 0.31-1.42; P = 0.40), overall mortality (RR, 0.81; 95% CI: 0.61-1.08; P = 0.15) and complications (RR, 1.26; 95% CI: 0.93-1.70; P = 0.13). CONCLUSION:In the prevention of esophageal variceal rebleeding, BB + ISMN are as effective as EBL. There are few complications with the two treatment modalities. Both BB + ISMN and EBL would be considered as the first-line therapy in the prevention of esophageal variceal rebleeding.

The increasing impact of the greenhouse effect on ecosystems is prompting transportation agencies to seek methods for reducing CO_(2)emissions during pavement construction and ***,the laboratory mix design process,which involves selecting aggregate gradation and binder content,is time-consuming and *** accelerate the traditional mix design procedure,this study presented a mix design procedure that can automatically determine gradation and binder content based on machine learning(ML)and a meta-heuristic ***,ML approaches were employed to model the relationship between volumetric properties(mixture bulk specific gravity(Gmb)and air void(VV))and both mixture component properties and mixture proportion,based on a dataset collected from literature with 660 mixture *** with the prediction of ML models and the modified multi-objective grey wolf optimization(MOGWO)algorithm,an automatic asphalt mix design was proposed to pursue three goals,including VV,cost,and CO_(2)*** results indicated that least squares support vector regression(LSSVR)and e Xtreme gradient boosting(XGBoost)achieved the highest prediction accuracies(correlation coefficient:0.92 for VV and 0.96 for Gmb).The MOGWO algorithm successfully found the 26 optimal mix designs for the case of VV *** ***_(2)*** to the traditional laboratory design,the optimal mixture with VV of4%achieves a cost saving of 2.46%and a reduction of 4.03%in carbon *** volumetric properties of the mixtures output by the approach also align closely with values measured in a laboratory.

Natural forests exhibit higher organic carbon concentrations and recalcitrant carbon proportions in soil than plantations: a global data synthesis

Different chemical compositions of soil organic carbon (SOC) affect its persistence and whether it significantly differs between natural forests and plantations remains unclear. By synthesizing 234 observations of SOC chemical compositions, we evaluated global patterns of concentration, individual chemical composition (alkyl C, O-alkyl C, aromatic C, and carbonyl C), and their distribution evenness. Our results indicate a notably higher SOC, a markedly larger proportion of recalcitrant alkyl C, and lower easily decomposed carbonyl C proportion in natural forests. However, SOC chemical compositions were appreciably more evenly distributed in plantations. Based on the assumed conceptual index of SOC chemical composition evenness, we deduced that, compared to natural forests, plantations may have higher possible resistance to SOC decomposition under disturbances. In tropical regions, SOC levels, recalcitrant SOC chemical composition, and their distributed evenness were significantly higher in natural forests, indicating that SOC has higher chemical stability and possible resistance to decomposition. Climate factors had minor effects on alkyl C in forests globally, while they notably affected SOC chemical composition in tropical forests. This could contribute to the differences in chemical compositions and their distributed evenness between plantations and natural stands.

Stability analysis of underground constructions requires a model study of rock masses’ long-term performance. Creep tests under different stress conditions was conducted on intact granite and granite samples fractured at 30° and 45° angles. The experimental results indicate that the steady creep strain rates of intact and fractured rock present an exponential increase trend with the increase of stress level. A nonlinear creep model is developed based on the experimental results, in which the initial damage caused by fracture together with the damage caused by constant load have been taken into consideration. The fitting analysis results indicated that the model proposed is more accurate at identifying the full creep regions in fractured granite, especially the accelerated stage of creep deformation. The least-square fit error of the proposed creep model is significantly lower than that of Nishihara model by almost an order of magnitude. An analysis of the effects of elastic modulus, viscosity coefficient, and damage factors on fractured rock strain rate and creep strain is conducted. If no consideration is given to the effects of the damage, the proposed nonlinear creep model can degenerate into to the classical Nishihara model.

Background： Some patients with pelvic organ prolapse may suffer from incontinence （SUI） named de novo SUI alter pelvic floor reconstruction of de novo SUI. ower urinary tract symptoms （LUTS）, especially stress urinary This study aimed to investigate the incidence and risk factors Methods： This is a nested case-control study of 533 patients who underwent pelvic floor reconstruction due to pelvic organ prolapse （POP） at the Department of Gynecology in Peking University People＇s Hospital from January 2011 to March 2013. According to the inclusion and exclusion criteria, 401 patients were enrolled in the study with the follow-up rate of 74.8% （101 patients lost to follow-up）. There were 75 patients with de novo SUI postoperatively. According to the ratio of 1：3, we ensured the number of control group （n = 225）. The preoperative urinary dynamics, POP-quantification scores, and LUTS were compared between the two groups by univariate and multivariate logistic regression analyses to investigate the risk factors of de novo SUI. Results： The incidence of de novo SUI was 25% （75/300）. Univariate analysis showed that the ratio of lower urinary tract obstruction （LUTO） before surgery in de novo SUI group was significantly higher than the control group （odds ratio [OR] = 2.1, 95% confidence interval [（7] [1.1-4.0], P = 0.022）. The interaction test of LUTO and other factors displayed that Aa value was an interaction factor. With the increasing score of Aa, the incidence of de novo SUI become higher （OR = 2.1, 95% CI [1.0-3.7], P = 0.045）. After multivariable adjustment, multiple regression analysis showed that LUTO was independently associated with a greater risk of de novo SUI after pelvic floor surgery （OR = 2.3, 95% CI [1.2-4.6], P = 0.013）. Conclusions： Preoperative LUTO in patients with POP is a high-risk factor of de novo SUI, and high score of Aa-point is related to the occurrence of de novo SUI, which might be due to the outlet obstruction caused by bladder prolapse.

To evaluate the longitudinal change in prostate-specific antigen （PSA） and the influence of initial PSA on the PSA change. We retrospectively analysed health examination data collected at Beijing Hospital from March 2007 to November 2011. Men with an initial PSA levels less than 4 ng ml- 1 and an annual PSA test for 5 years were enrolled into the study. The men were separated into four groups by the initial PSA level （0-0.99, 1-1.99, 2-2.99 and 3-3.99 ng ml- 1）, and the difference in PSA change among the four groups was analysed. A total of 1330 men were enrolled into the study. The mean age, initial PSA and PSA velocity （PSAV） were 58.17± 14.63 （range 24-91） years, 1.18±0.79 （range 0-4） ng m1-1 and 0.04±0.25 （range -1.34±2.02） ng m1-1 year-1, Pearson＇s correlation analysis showed no correlation between initial PSA and PSAV （r=-0.036, P=0. 189）. The PSAV of the 0-0.99, 1-1.99, 2-2.99 and 3-3.99 ng m1-1 initial PSA groups was 0.03±0.11, 0.07±0.32, 0.03±0.34 and -0.01±0.43 ng m1-1 year-1, respectively （P=0.06）. As the initial PSA increased, the percentage of having a PSAV over 0.75 ng m1-1 year-1 and a negative PSAV both significantly increased. Males with a baseline PSA of 0-0.99, 1-1.99, 2-2.99 and 3-3.99 ng ml- 1 had a 1.88%, 6.16%, 16.30% and 57.81% chance, respectively, that their PSA would increase above 4.0 ng ml- 1 over the following 4 years （P〈0.0001）. The PSAV has no correlation with the initial PSA level. However, as the initial PSA increases, the chance that males will have an abnormal PSA or PSAV in the future increases.
