Surgical treatment of 21 patients with spinal metastases of differentiated thyroid cancer

Background Differentiated thyroid cancer （DTC） is a common primary cancer for spinal metastases （SM）.The treatments for DTC spinal metastases （SM） have evolved from simple surgery and radiotherapy to a multidisciplinary comprehensive therapeutic strategy of combined spinal surgery,general surgery,radiotherapy,nuclear medicine and endocrinology.The purpose of this study was to discuss the efficacy and prognosis associated with different surgical treatments of SM patients with DTC.Methods A total of 21 consecutive patients with SM of DTC that were treated between 1999 and 2013 were studied.Biopsy was routinely performed to achieve the pathological diagnosis before treatment.Three patients underwent total spondylectomy intralesionally or piecemeally,and 18 had curettage.Postoperative recurrence and survival times were analyzed by the Kaplan-Meier methods.Results Nineteen patients （90％） had an average of 42.7 months （range,7-170 months） follow-up.The median visual analogue scale for pain reduced from 5 points to 1 point （P ＜0.01）,and the median Karnofsky performance score increased from 70 to 90 points after surgery （P ＜0.01）.Seventeen patients with neurological deficits attained improvements after surgeries,of at least one level according to the Frankel classification （P ＜0.01）.Eight patients with curettage had recurrence.Four patients died of DTC,12 patients lived with disease,and three patients were disease-free.No significant effects on postoperative recurrence or survival were observed between surgery combined with conservative treatment,total spondylectomy,the number of bone metastases and visceral metastasis.Conclusions DTC-SM have a relatively favorable prognosis,and curettage and stabilization can effectively relieve the pain and improve the quality of life and neurological status of the patients.For patients with Tomita scores of ＜3,total spondylectomy may have better clinical outcomes.Comprehensive therapeutic strategies including surgery,radioiodine,external beam radiation therapy and embolization should

This paper deals with single-machine scheduling problems with a more general learning effect based on sum-of-processing-time. In this study, sum-of-processing-time-based learning effect means that the processing time of a job is defined by a decreasing function of the total normal processing time of jobs that come before it in the sequence. Results show that even with the introduction of the sum-of-processing-time-based learning effect to job processing times, single-machine makespan minimization problems remain polynomially solvable. The curves of the optimal schedule of a total completion time minimization problem are V-shaped with respect to iob normal orocessinz times.

Blockades of angiotensin and aldosterone reduce osteopontin expression and interstitial fibrosis infiltration in rats with myocardial infarction

Background It has been reported that osteopontin has an important role in cardiac fibrosis and remodeling. However, its direct mechanisms remain unclear. The purpose of this study was to investigate the role of angiotensin and aldosterone blockades in cardiac osteopontin expression associated with cardiac remodeling in myocardial infarcted （MI） rats. Methods Fifty SD rats that survived 24 hours after ligating left anterior descending coronary artery were randomly divided into three groups： MI-saline group （n=15, 5 ml/d）, MI-perindopril group （n=18, perindopril 2 mg·kg^-1·d^-1） and MI-spironolacton （n=-17, spironolacton 20 mg·kg^-1·d^-1）. A sham operation group （n=15） was selected as non-infarcted control. At 6 weeks after treatment, hemodynamic pararmeters and left ventricular function were measured with catheterization, interstitial fibrosis infiltration and cardiomyocyte diameters were evaluated histologically. Myocardium osteopontin protein expression level in the non-infarcted myocardium was detected by Western blotting. Results No osteopontin protein was detected in the myocardium of sham-operation rats. High levels of osteopontin protein expression were detected in the MI-saline rats, but the levels were suppressed in the MI-perindopril and MI-spironolacton rats at 6 weeks following MI （P 〈0.01, respectively）. Compared with the sham operation group, all rats in the MI group showed marked interstitial fibrosis infiltration in the non-infarction area, higher ventricular weight/body weight ratio, significantly increased cardiomyocyte diameter （P 〈0.01, respectively）, and developed significant systolic and diastolic dysfunction as indicated by decreased left ventricular systolic pressure （LVSP） and ±dp/dt, as well as increased left ventricular end-diastolic pressure （LVEDP）（P 〈0.01, respectively）. Angiotensin and aldosterone blockades partly prevented cardiac fibrosis and systolic and diastolic dysfunction （P 〈0.01, respectively）. 近年来,稳定同位素标记化合物,尤其是同位素标记氨基酸在合成生物学、蛋白质组学、代谢组学等前沿科学研究中发挥着越来越重要的作用。建立了一种基于高分辨液质联用的双标记氨基酸同位素丰度检测方法,以^(13)C_(2),^(15)N-甘氨酸为例,考察了样品浓度、质谱扫描张数等影响因素对同位素丰度计算结果的影响,结果表明该方法具有良好的准确性和稳定性,所需样品用量少,操作简单,有望成为一种简便、通用的同位素试剂质控方法;也将为代谢组学、海洋学等同位素示踪技术应用研究提供数据支持。

Effect of current emission abatement strategies on air quality improvement in China:A case study of Baotou,a typical industrial city in Inner Mongolia

The national Air Pollution Prevention and Control Action Plan required significant decreases in PM_（2.5） levels over China.To explore more effective emission abatement strategies in industrial cities,a case study was conducted in Baotou to evaluate the current national control measures.The total emissions of SO_2,NO_X,PM_（2.5） and NMVOC（non-methane volatile organic compounds） in Baotou were 211.2 Gg,156.1 Gg,28.8 Gg,and 48.5 Gg,respectively in 2013,and they would experience a reduction of 30.4%,26.6%,15.1%,and 8.7%,respectively in 2017 and 39.0%,32.0%,24.4%,and 12.9%,respectively in2020.The SO_2,NO_Xand PM_（2.5） emissions from the industrial sector would experience a greater decrease,with reductions of 37%,32.7 and 24.3%,respectively.From 2013 to 2020,the concentrations of SO_2,NO_2,and PM_（2.5） are expected to decline by approximately 30%,10% and 14.5%,respectively.The reduction rate of SNA（sulfate,nitrate and ammonium）concentrations was significantly higher than that of PM_（2.5） in 2017,implying that the current key strategy toward controlling air pollutants from the industrial sector is more powerful for SNA.Although air pollution control measures implemented in the industrial sector could greatly reduce total emissions,constraining the emissions from lower sources such as residential coal combustion would be more effective in decreasing the concentration of PM_（2.5） from 2017 to 2020.These results suggest that even for a typical industrial city,the reduction of PM_（2.5） concentrations not only requires decreases in emissions from the industrial sector,but also from the low emission sources.The seasonal variation in sulfate concentration also showed that emission from coal-burning is the key factor to control during the heating season.

Modified frontolateral partial laryngectomy operation: combined muscle-pedicle hyoid bone and thyrohyoid membrane flap in laryngeal reconstruction

Objective: Laryngeal reconstruction is needed to preserve laryngeal function in patients who have undergone extensive vertical or frontal partial laryngectomy. However, the procedure remains a difficult challenge. Several reconstruction techniques have been described, but these techniques pose risks of complications such as laryngeal stenosis. This study aimed to evaluate the postoperative course and functional outcomes of a new technique that combined a muscle-pedicle hyoid bone and a thyrohyoid flap during laryngeal reconstruction after tumor resection. Methods: Four patients underwent extensive vertical partial or frontal partial laryngectomy for cancer. After tumor resection, laryngeal reconstruction was performed using the proposed technique. Postoperative recovery time, complications, and oncologic results were evaluated. Results: The four patients were successfully treated with the proposed technique. No dyspnea, dysphagia, or death occurred during the postoperative course. Decannulation was performed after a median of 3 days. The average postoperative hospital stay was 7 days. Short-term postoperative functional recovery was normal. No laryngeal stenosis or tumor recurrence was observed in any of the four patients after a follow-up period of more than 24 months. Conclusion: The combination of the muscle-pedicle hyoid bone and the thyrohyoid flap is a reliable procedure for laryngeal reconstruction after extensive vertical partial or frontal partial laryngectomy.

Approximate Discovery of Service Nodes by Duplicate Detection in Flows

Discovery of service nodes in flows is a challenging task,especially in large ISPs or campus networks where the amount of traffic across network is massive.We propose an effective data structure called Round-robin Buddy Bloom Filters(RBBF) to detect duplicate elements in flows.A two-stage approximate algorithm based on RBBF which can be used for detecting service nodes from NetFlow data is also given and the performance of the algorithm is analyzed.In our case,the proposed algorithm uses about 1% memory of hash table with false positive error rate less than 5%.A prototype system,which is compatible with both IPv4 and IPv6,using the proposed data structure and algorithm is introduced.Some real world case studies based on the prototype system are discussed.

视频的点击率预估是视频推荐系统中的重要任务之一,推荐系统可以根据点击率的预估调整视频推荐顺序以提升视频推荐的效果.近年来,随着视频数量的爆炸式增长,视频推荐的冷启动问题也变得愈发严重.针对这个问题,提出了一个新的视频点击率预估模型,通过使用视频的内容特征以及上下文特征来加强视频点击率预估的效果;同时,通过对冷启动场景的模拟训练和基于近邻的替代方法提升模型应对新视频点击率预估的能力.提出的模型可以同时对旧视频和新视频进行点击率预估.在两个真实的电视剧(Track_1_series)和电影(Track_2_movies)点击率预估数据集上的实验表明:提出的模型可以显著改善对旧视频的点击率预估性能,并在两个数据集上均超过了现有的模型;对于新视频,相比于不考虑冷启动问题的模型只能获得0.57左右的AUC性能,该模型在两个数据集上分别获得0.645和0.615的性能,表现出针对冷启动问题更好的鲁棒性.

One Hundred Years of Influenza Since the 1918 Pandemic—Is China Prepared Today?

Almost 100 years after the 1918 influenza pandemic,China experienced its largest,most widespread epidemic of human infections with avian influenza A(H7N9),the influenza virus with the greatest pandemic potential of all influenza viruses assessed to date by the United States Centers for Disease Control and Prevention’s Influenza Risk Assessment Tool.This historical review describes how China was affected by the 1918,1958,1968,and 2009 influenza pandemics,records milestones in China’s capacity to detect and respond to influenza threats,and identifies remaining challenges for pandemic preparedness.This review suggests that past influenza pandemics have improved China’s national capabilities such that China has become a global leader in influenza detection and response.Further enhancing China’s pandemic preparedness to address remaining challenges requires government commitment and increased investment in China’s public health and healthcare systems.
