Background Mycoplasma pneumoniae(***)is a significant contributor to community-acquired pneumonia among *** 1968,when a strain of *** resistant to macrolide antibiotics was initially reported in Japan,macrolide-resistant ***(MRMP)has been documented in many countries worldwide,with varying incidence *** infections lead to a poor response to macrolide antibiotics,frequently resulting in prolonged fever,extended antibiotic treatment,increased hospitalization,intensive care unit admissions,and a significantly higher proportion of patients receiving glucocorticoids or second-line *** 2000,the global incidence of MRMP has gradually increased,especially in East Asia,which has posed a serious challenge to the treatment of *** infections in children and attracted widespread attention from ***,there is still no global consensus on the diagnosis and treatment of MRMP in *** We organized 29 Chinese experts majoring in pediatric pulmonology and epidemiology to write the world’s first consensus on the diagnosis and treatment of pediatric MRMP pneumonia,based on evidence *** evidence searches and reviews were conducted using electronic databases,including PubMed,Embase,Web of Science,CNKI,Medline,and the Cochrane *** used variations in terms for“macrolide-resistant”,“Mycoplasma pneumoniae”,“MP”,“***”,“pneumonia”,“MRMP”,“lower respiratory tract infection”,“Mycoplasma pneumoniae infection”,“children”,and“pediatric”.Results Epidemiology,pathogenesis,clinical manifestations,early identification,laboratory examination,principles of antibiotic use,application of glucocorticoids and intravenous immunoglobulin,and precautions for bronchoscopy are *** and rapid identification of gene mutations associated with MRMP is now available by polymerase chain reaction and fluorescent probe techniques in respiratory *** the resistance rate to mac

Treatment of pediatric mild persistent asthma with low-dose budesonide inhalation suspension *** in China

Background There are limited studies comparing budesonide inhalation suspension(BIS)with montelukast in real-world settings where treatment adherence and persistency may be *** real-world study aims to investigate the control effectiveness of montelukast or BIS as a monotherapy in Chinese children with mild *** Data were derived from a retrospective questionnaire-based analysis of 2-14-year-old children with mild persistent asthma,who received either 500 pg of BIS(n=153)or 4-5 mg of montelukast(n=240)once *** indicators of asthma control,the Asthma Control Test(ACT)/Childhood ACT(C-ACT)score,and the asthma-related medical costs were *** differences between the two groups were compared using an unpaired-t-test(normally distributed).Mann-Whitney U test(non-normally distributed)or chi-squared test(categorical variables).Results Medication compliance in the past 3-month period was better in the montelukast group than in the BIS group(P=0.042).The montelukast group exhibited better asthma control in the past 4-week period,including lower percentages of asthmatic children with symptoms more than twice a week(P=0.021),had night waking or night coughing(P=0.022),or required reliever medication more than twice a week(P<0.001).The montelukast group had a lower percentage of children with an ACT/C-ACT score≤19(P=0.015).Caregivers reported a significantly better exercise tolerance in the children who received montelukast *** in the past 12 months(P<0.001).Significantly higher medical expenditures attributable to asthma in the past 12 months were observed in the BIS group *** group(P<0.001).Conclusion Both treatments provided acceptable overall asthma control in children with mild persistent asthma;however,more reliever medication and more medical expenditures attributable to asthma were needed for BIS *** in real-world settings,where factors such as compliance were also taken into account.

Hydrothermal Synthesis,Crystal Structure and Thermal Analysis of a Dinuclear Complex Cd_2(3,5-Dinitrobenzoate)_4(pyridine)_4

A dinuclear complex Cd2（dnba）4（pyridine）4 （dnba = 3,5-dinitrobenzoate） has been synthesized by hydrothermal method and characterized by X-ray single-crystal diffraction, elemental analysis, FT-IR spectroscopy, DSC and TG-DTG techniques. The complex with empirical formula C48H32Cd2NI2024 （Mr = 692.83） crystallizes in monoclinic, space group P21/n with a - 12.0344（14）, b = 10.5752（13）, c = 21.578（3） A, β = 104.150（2）°, V = 2662.8（6） A^3, Z = 2, D, = 1.728 g/cm^3,μ（MoKa） = 0.897 mm^-1, F（000） = 1384, S = 1.016 and （△/σ）max = 0.001. R = 0.0638 and wR = 0.0737 for all data; the final R = 0.0337 and wR = 0.0644. In this complex, four carboxylates are bidentate-or chelate-coordinated with the Cd（Ⅱ） centers to give the dinuclear structure. The other coordination positions of Cd（Ⅱ） are occupied by the lone pair electrons from N of four pyridines. Thermal analyses DSC and TG-DTG have been used to determine the thermal decomposition mechanism of the title complex.

Dynamic impedance of piles in visco-elastic material considering axial loads

The dynamic impedance function of pile in visco-elastie material considering axial loads under lateral dynamic force was analyzed, and the beam dynamic differential equation was used to induce the dynamic impedance function. After analyzing the edge conditions, the dynamic impedance functions were deduced. Contrasted with the result that does not consider axial loads, the axial loads have obvious influence on the dynamic impedance *** the results show that the dimensionless prarmeter of the dynamic impedance will change from 6 % to 9 % when considering axial loads, and dimensionless prarmeter of the dynamic impedance of the coupling horizontal-sway will increase by 31 %.

A novelγ-γ'(Ni solid solution—Ni_(3)Al)NiPtAl coating was produced on advanced single-crystal(SC)superalloy N5 by electroplating of a thin Pt coating with~3μm in thickness followed by heat treatment at 1000℃in *** comparison,a traditionalγ-γ'NiPtAl coating was also produced by electroplating of a thick Pt coating with~6μm in thickness followed by heat treatment at 1100℃in *** novel coating has a mass gain of~0.5 mg·cm^(-2)after 100 h 1-h cyclic oxidation at 1150℃,showing better oxidation resistance than the traditional *** oxide scale mostly consisting ofα-Al_(2)O_(3)grew on the novel coating because the element Cr from the SC alloy substrate contributes to the formation ofα-Al_(2)O_(3).Pt content in the novel coating has significant effect on the oxidation *** optimized Pt content in the coating is about 16 at%,which is helpful to reduce the diffusion of harmful elements from the substrate into the coating but also to reduce the cost of the coating.

冠状动脉慢血流患者血清Lp-PLA2、NLR水平变化及临床意义

目的探讨冠状动脉慢血流(CSF)患者血清脂蛋白相关磷脂酶A2(Lp-PLA2)及中性粒细胞与淋巴细胞比值(NLR)的表达水平及其意义。方法选取河北北方学院附属第一医院2022年12月至2023年8月经冠状动脉造影(CAG)检查确诊为CSF的患者45例作为观察组(CSF组),并选取同期行CAG检查结果提示冠状动脉血流正常的患者45例作为对照组。比较两组间血清Lp-PLA2和NLR水平差异;采用多因素logistic回归模型分析CSF发生人群的影响因素;绘制受试者工作特征(ROC)曲线,以评估血清Lp-PLA2和NLR在诊断CSF发生方面的效能。结果CSF组患者血清Lp-PLA2[(25.33±4.02)μg/L vs.(22.29±4.07)μg/L]、NLR[2.61(2.11,3.25)vs.1.54(1.19,1.86)]水平明显高于对照组(均P<0.05)。多因素logistic回归提示,血清Lp-PLA2水平升高(OR=1.210,95%CI:1.015~1.442,P<0.05)、NLR水平升高(OR=35.826,95%CI:5.565~230.638,P<0.01)是患者发生CSF的独立危险因素。进一步行ROC曲线分析结果表明,血清Lp-PLA2诊断CSF发生的临界值是23.127μg/L,ROC曲线下面积(AUC)为0.699(95%CI:0.591~0.807,P<0.05),敏感度0.756,特异度0.600。NLR诊断CSF发生的临界值是2.074,AUC为0.907(95%CI:0.845~0.969,P<0.01),敏感度0.800,特异度0.911。两者联合诊断CSF发生的AUC为0.933(95%CI:0.884~0.982,P<0.01),敏感度0.867,特异度0.889。结论CSF患者血清Lp-PLA2及NLR水平升高,两者均是影响CSF发生的独立危险因素,且对CSF有一定的诊断价值。

Attaining synergetic equilibrium of electrical conductivity and tensile strength in GQDs@GN/Cu composites through multi-scale intragranular and intergranular reinforcements

The configuration and quality of reinforcements, as well as the robustness of interfacial bonding,holding a critical significance in determining the concurrence between electrical conductivity and mechanical strength in metal matrix composites. In this study, citric acid was employed as the precursor for synthesizing multiscale carbon nanomaterials(graphene quantum dots and graphene, abbreviated as GQDs and GN). The GQDs@GN/Cu composites were fabricated through a segmented ball milling process in conjunction with subsequent spark plasma sintering(SPS). The intragranular GQDs and intergranular GQDs@GN had synergistically reinforced Cu composites through Orowan strengthening, load transfer strengthening and refinement strengthening. Furthermore,the robust interface bonding between GQDs@GN and Cu effectively mitigated interfacial impedance stemming from electron-boundary scattering. The yield strength and ultimate tensile strength of the GQDs@GN/Cu composites were recorded as 270 and 314 MPa, respectively, representing an improvement of 92 and 28% over pure Cu, while maintaining electrical conductivity at a level comparable to that of pure Cu. This study advances the understanding of the possibility of realizing a synergistic compatibility between electrical conductivity and mechanical strength in Cu composites.

Efficacy and safety of salmeterol/fluticasone compared with montelukast alone (or add-on therapy to fluticasone) in the treatment of bronchial asthma in children and adolescents: a systematic review and meta-analysis

Background:Despite the recommendation of inhaled corticosteroids(ICSs)plus long-acting beta 2-agonist(LABA)and leukotriene receptor antagonist(LTRA)or ICS/LTRA as stepwise approaches in asthmatic children,there is a lack of published systematic review comparing the efficacy and safety of the two therapies in children and adolescents aged 4 to 18 *** study aimed to compare the safety and efficacy of salmeterol/fluticasone(SFC)***(MON),or combination of montelukast and fluticasone(MFC)in children and adolescents aged 4 to 18 years with bronchial ***:A systematic search was conducted in MEDLINE,EMBASE,the Cochrane Library,China BioMedical Literature Database,Chinese National Knowledge Infrastructure,VIP Database for Chinese Technical Periodical,and Wanfang for randomized controlled trials(RCTs)published from inception to May 24,*** are as follows:SFC ***,or combination of MFC,with no limitation of dosage or *** and secondary outcome measures were as follows:the primary outcome of interest was the risk of asthma *** outcomes included risk of hospitalization,pulmonary function,asthma control level,quality of life,and adverse events(AEs).A random-effects(I^(2)≥50%)or fixed-effects model(I^(2)<50%)was used to calculate pooled effect estimates,comparing the outcomes between the intervention and control groups where ***:Of the 1006 articles identified,21 studies met the inclusion criteria with 2643 individuals;two were at low risk of *** no primary outcomes were similar after an identical treatment duration in the included studies,meta-analysis could not be ***,more studies favored SFC,instead of MON,owing to a lower risk of asthma exacerbation in the SFC *** for secondary outcome,SFC showed a significant improvement of peak expiratory flow(PEF)%pred after 4 weeks compared with MFC(mean difference[MD]:5.45;95%confidence interval[CI]:1.57-9.34;I^(2)=95%;P=0.006).
