Dichagelinoids A—C(1—3),three unusual dichapetalin-type triterpenoids,together with two biogenetically related derivatives dichagelinoids D and E(4 and 5)were isolated and characterized by solid data from Dichapetalum *** 1—3 possess a common rearranged C ring and different C6—C2 modified motifs at the A *** evaluation revealed that compounds 1—3 showed distinct cytotoxic activity against the tested human cancer cell lines.

Universal lesion detection(ULD)methods for computed tomography(CT)images play a vital role in the modern clinical medicine and intelligent *** is well known that single 2D CT slices lack spatial-temporal characteristics and contextual information compared to 3D CT ***,3D CT blocks necessitate significantly higher hardware resources during the learning ***,efficiently exploiting temporal correlation and spatial-temporal features of 2D CT slices is crucial for ULD *** this paper,we propose a ULD network with the enhanced temporal correlation for this purpose,named *** designed TCE module is applied to enrich the discriminate feature representation of multiple sequential CT ***,we employ multi-scale feature maps to facilitate the localization and detection of lesions in various *** experiments are conducted on the DeepLesion benchmark demonstrate that thismethod achieves 66.84%and 78.18%for FS@0.5 and FS@1.0,respectively,outperforming compared state-of-the-art methods.

The strained cyclopropane and/or cyclobutane subunits occurred in many complex natural products including terpenoids, steroids and alkaloids. Natural products with cyclopropane and/or cyclobutane motifs not only furnished fascinating structures, but also exhibited versatile biological activities, such as cytotoxic, anti-HIM antimicrobial, antiviral, and immunosuppressive effects. This review covered a large array of structurally unique natural products with strained cyclopropane and/or cyclobutane motifs and summarized their structural features, distributions, biological activities, as well as biogenetic considerations.

Effect of N-acetyl-cysteine treatment on sensorineural hearing loss: a meta-analysis

N-acetyl-cysteine (NAC) is an efficacious treatment for sensorineural hearing loss in animal models, such as noise-induced hearing loss (NIHL), however previous research into the effect of NAC on patients with hearing loss produced contradictory results. In this study, we investigated the effect of NAC treatment on sensorineural hearing loss. PubMed, Web of Science and Embase databases were searched in their entirety using the key words: hearing loss, NAC, N-acetylcysteine, and sensorineural hearing loss. Studies which included assessment of hearing loss with pure-tone threshold (PTA) data were selected. Eligible studies regarding the effects of NAC treatment on patients with hearing loss were collected by two independent reviewers. A total of 1197 individuals were included from seven published studies. Two studies reported data for a sudden idiopathic sensorineural hearing loss (SISNHL) group. Three studies reported data for a NIHL group. Other studies reported data for drug-induced hearing loss. The meta-analysis demonstrated that the overall effect of NAC treatment on sensorineural hearing loss was invalid. However, NAC treatment was linked with improved patient outcomes of hearing tests in cases of sudden hearing loss, but did not prevent hearing loss induced by noise or ototoxicity. However, there is a need for better-designed studies with larger samples to further prove the correlation between the effect of NAC and hearing loss.

Objective:The choice of chemotherapeutic regimen for triple-negative breast cancer(TNBC)remains *** recombination deficiency(HRD)has attracted increasing attention in informing chemotherapy *** study was aimed at investigating the feasibility of HRD as a clinically actionable biomarker for platinum-containing and platinum-free ***:Chinese patients with TNBC who received chemotherapy between May 1,2008 and March 31,2020 were retrospectively analyzed with a customized 3D-HRD *** positivity was defined by an HRD score≥30 or deleterious BRCA1/2 mutation.A total of 386 chemotherapy-treated patients with TNBC were screened from a surgical cohort(NCT01150513)and a metastatic cohort,and 189 patients with available clinical and tumor sequencing data were ***:In the entire cohort,49.2%(93/189)of patients were identified as HRD positive(40 with deleterious BRCA1/2 mutations and 53 with BRCA1/2 intact with an HRD score of≥30).In the first-line metastatic setting,platinum therapy was associated with longer median progression-free survival(mPFS)than platinum-free therapy[9.1 vs.3.0 months;hazard ratio(HR),0.43;95%confidence interval 0.22–0.84;P=0.01].Among HRD-positive patients,the mPFS was significantly longer in those treated with platinum rather than platinum-free therapy(13.6 vs.2.0 months;HR,0.11;P=0.001).Among patients administered a platinum-free regimen,HRD-negative patients showed a PFS significantly superior to that of HRD-positive patients(P=0.02;treatment-biomarker P-interaction=0.001).Similar results were observed in the BRCA1/2-intact *** the adjuvant setting,HRD-positive patients tended to benefit more from platinum chemotherapy than from platinum-free chemotherapy(P=0.05,P-interaction=0.02).Conclusions:HRD characterization may guide decision-making regarding the use of platinum treatment in patients with TNBC in both adjuvant and metastatic settings.

目的探讨吉西他滨为主的维持治疗对晚期乳腺癌治疗疗效的影响及影响治疗持续时间的主要原因。方法研究为前瞻性多中心研究,2017年1月至2019年1月期间从10家医疗中心选取接受含吉西他滨单药或联合其他化疗或靶向治疗的晚期乳腺癌患者,收集吉西他滨为主方案的治疗线数、剂量、周期数、不良反应、停药原因以及疗效等数据,生存分析采用Kaplan-Meier方法,预后影响因素的单因素分析采用log rank检验,多因素分析采用Cox回归模型。结果研究共筛选全国10家医院共239例晚期乳腺癌患者,最终纳入224例患者进行分析。全组患者中位年龄52岁(26~77岁),其中Luminal型83例,三阴性97例,人表皮生长因子受体2阳性型44例。55.4%(124/224)为绝经后患者。完全缓解(CR)5例,部分缓解(PR)77例,疾病稳定(SD)112例,疾病进展(PD)27例。客观有效率为36.6%(82/224)。Ⅲ级及以上不良反应主要包括白细胞下降(9例)、中性粒细胞下降(49例)、血小板下降(15例)、转氨酶升高(2例)。中位随访时间41个月(26~61个月),中位无进展生存时间(PFS)为5.6个月。引起治疗结束的主要原因包括疾病进展90例、患者个人原因终止治疗51例、严重不良反应不耐受18例、医师评估后治疗结束65例。治疗周期>6个周期较≤6个周期患者在PFS有明显的延长(8.2和5.4个月,HR=2.474,95%CI:1.730~3.538,P<0.001)。结论吉西他滨为主的治疗方案在中国人群中有较好的疗效和耐受性,长程维持治疗有利于改善生存。
