目的探讨经内镜逆行性胰胆管造影/乳头括约肌切开取石术(ERCP/EST)治疗80岁以上高龄胆总管结石患者的临床疗效。方法回顾性分析我院肝胆外科2016年1月~2019年12月明确诊断为胆总管结石的高龄(≥80岁)患者的临床资料,包括胆总管结石最大直径、术后并发症发生情况(胰腺炎、高淀粉酶血症、胆管炎、发热、十二指肠穿孔)、住院时间、住院总费用及手术前后相关指标。结果共有56例患者纳入研究,其中胆总管结石平均最大直径(1.14±0.42)cm,平均住院时间(9.68±4.80)d,平均住院总费用(25380.79±6622.17)元。术前谷氨酰转移酶(GGT)(466.79±397.29)U/L、凝血酶原时间(PT)(12.97±2.00)s、部分凝血活酶时间(APTT)(32.58±6.69)s;术后血清淀粉酶(172.60±239.69)U/L。术后总胆红素(TBIL)[(55.31±47.40)μmol/L vs.(33.93±41.52)μmol/L]、直接胆红素(DBIL)[36.30(52.33)μmol/L vs.14.35(15.95)μmol/L]、丙氨酸氨基转移酶(ALT)[92.25(174.98)U/L vs.47.60(59.95)U/L]、天门冬氨酸氨基转移酶(AST)[74.45(154.48)U/L vs.32.25(58.55)U/L]较术前明显降低,差异有统计学意义(P<0.05)。术后出现高淀粉酶血症12例,胰腺炎1例,无其他并发症发生。结论ERCP/EST治疗80岁以上高龄胆总管结石患者安全、有效,患者术后TBIL、DBIL、ALT、AST较术前明显降低,可作为高龄胆总管结石患者的治疗方式。

Mesoscopic superconducting islands hosting Majorana zero modes(MZMs), or Majorana islands in short, offer a prototype of topological qubits. In this work we investigate theoretically the model of a generic Majorana island tunneling-coupled to a singlepiece metallic substrate, hence an embedded Majorana island. We show the crucial consequences of an interplay between the topological ground states nonlocally addressed by the MZMs and the metallic bath with coherent electron propagation: on the one hand, the topological degeneracy on the Majorana island can be preserved, by virtue of the particle-hole symmetry, despite the apparent bath-induced coupling between MZMs; on the other hand, the electronic interference in the metallic bath may lead to profound alterations to the renormalization group behavior of the hybrid system towards low energy/temperature compared with conventional Kondo physics. This work serves to establish the model of embedded Majorana islands as an experimentally relevant and theoretically intriguing problem particularly in the direction of topological quantum computation.

Photocatalysis offers a sustainable means for the oxidative removal of low concentrations of NOx(NO,NO2,N2O,N2O5,etc.)from the *** double hydroxides(LDHs)are promising candidate photocatalysts owing to their unique layered and tunable chemical structures and abundant surface hydroxide(OH)moieties,which are hydroxyl radical(OH)***,the practical applications of LDHs are limited by their poor charge-separation ability and insufficient active ***,we developed a facile N_(2)H_(4)-driven etching approach to introduce dual Ni^(2+)and OHvacancies(Niv and OHv,respectively)into NiFe-LDH nanosheets(hereafter referred to as NiFe-LDH-et)to facilitate improved charge-carrier separation and active Lewis acidic site(Fe^(3+)and Ni^(2+)exposed at OHv)*** contrast to inert pristine LDH,NiFe-LDH-et actively removed NO under visible-light ***,Ni_(76)Fe_(24)-LDH-et etched with 1.50 mmol·L^(-1)N_(2)H_(4)solution removed 32.8%of the NO in continuously flowing air(NO feed concentration:500 parts per billion(ppb))under visible-light illumination,thereby outperforming most reported *** and theoretical data revealed that the dual vacancies promoted the production of reactive oxygen species(O_(2)·^(-)andOH)and the adsorption of NO on the *** situ spectroscopy demonstrated that NO was preferentially adsorbed at Lewis acidic sites,particularly exposed Fe^(3+)sites,converted into NO+,and subsequently oxidized to NO3without the notable formation of the more toxic intermediate NO2,thereby alleviating risks associated with its production and emission.

21世纪以来,我国教育立法进入快速发展时期,从学校教育扩展到社会教育、家庭教育等领域,数量不断上升。教育立法不断扩展的背后蕴含着一切教育活动都要有法可依、教育方方面面都要法治化的观念,呈现出过分强调法律作用的法律万能主义倾向。这种法律万能主义视立法为工具,倾向于扩张法律的功能,模糊法律与道德、政策的界限,使法律承担过多的道德和政策任务;扩张教育管理者的权力,增加管理对象的义务;扩展国家的职能范围,缩小家庭和社会的选择空间。有必要对这些倾向进行反思。教育立法应当把握法治的宗旨,注意国家权力的界限、法律的局限,把法律运用于有限的、适当的地方。

Arsenic trioxide （ATO）, a component of the traditional Chinese medicine arsenic sublimate, promotes apoptosis and induces leukemic cell differentiation. Combined with all-trans-retinotic acid （ATRA）, ATO has become the first-line induction therapy in treating acute promyelocytic leukemia （APL）. The most common side effects of ATO include hepatotoxicity, gastrointestinal symptoms, water-sodium retention, and nervous system damage. In this report, we present a rare side effect, rhabdomyolysis, in a 68-year-old female APL patient who was treated with ATO. After taking 10 mg ATO daily for 6 days, she presented shortness of breath, myodynia, elevated creatine kinase, and acute renal insufficiency. This report describes the first case of ATO-induced rhabdomyolysis.

Li-CO_(2)batteries are among the most intriguing techniques for balancing the carbon cycle,but are challenged by the annoyed thermodynamic barrier of the Li_(2)CO_(3)decomposition ***,we demonstrate the electrocatalytic performances of two-dimensional(2D)CoAl-layer double hydroxide(LDH)nanosheets can be significantly improved by trans-dimensional crosslinking with three-dimensional(3D)multilevel nanoporous(MP)-RuCoAl alloy(MP-RuCoAl alloy⊥CoAl-LDH).The MP-RuCoAl alloy⊥CoAl-LDH with multiscale pore channels and abundant nano-heterointerface is directly prepared by controllable etching Al from a Ru-Co-Al master alloy along with simultaneous partial oxidization of Al and Co *** MP-RuCoAl is composed of various intermetallic compounds and Ru with abundant grain boundaries,and forms numerous heterointerface with 2D CoAl-LDH *** multiscale porous metallic network benefits mass and electron transportation as well as discharge product storage and enables a rich multiphase reaction *** situ differential electrochemical mass spectrometry shows that the mass-to-charge ratio in the charging process is~0.733 which is consistent with the theoretical value of 3/4,stating that the reversible co-decomposition of Li_(2)CO_(3)and C can be achieved with the MP-RuCoAl alloy⊥*** Ketjen black(KB)/MP-RuCoAl⊥CoAl-LDH battery demonstrates a high cyclability for over 2270 h(227 cycles)with a lower voltage gap stabilized at~1.3 V at 200 mA·g^(−1).Our findings here provide useful guidelines for developing high efficiency transition metal based electrocatalysts by coupling with conductive porous substrate for impelling the development of practical Li-CO_(2)battery systems.

试验旨在应用胚胎移植技术提高优质老龄辽宁绒山羊母羊遗传资源利用效率。在秋季以8只平均年龄6.75岁优质辽宁绒山羊母羊做供体、28只平均年龄6.64岁辽宁绒山羊和与奶山羊杂交母羊做受体,采用公羊试情法和手术法子宫角冲取胚胎方法,比较不同超数排卵、同期发情方法对老龄绒山羊胚胎移植效果的影响。结果显示:超数排卵方法Ⅱ组供体母羊发情率、平均可用胚胎数和可利用率显著高于超数排卵方法Ⅰ组(P0.05);供体超数排卵方法Ⅱ+受体同期发情方法Ⅱ组胚胎移植受体母羊妊娠率显著高于供体超数排卵方法Ⅰ+受体同期发情方法Ⅰ组(P0.05)。根据胚胎移植受体母羊产羔情况,验证B超早期妊娠诊断准确率为91.67%(11/12),B超早期妊娠诊断检出率为88.89%(8/9)。研究表明,超数排卵方法Ⅱ组供体母羊超数排卵效果最好,超数排卵方法Ⅱ+同期发情方法Ⅱ组胚胎移植妊娠率、产羔率最高,是老龄辽宁绒山羊母羊胚胎移植最佳技术方法。
