目的开发胆总管结石患者经内镜逆行性胰胆管造影术(ERCP)后发生早期胆道感染(PEEBI)的预测工具,以期辅助临床进行ERCP术前决策和术后早期个性化干预。方法采用观察性双向队列研究方法,选取某医院接受ERCP的胆总管结石住院患者,分别采用有向无环图(DAGs)和最小绝对收缩与选择算子法(LASSO)进行基于logistic回归的PEEBI预测,比较预测模型,并进行内外部验证。结果2020年1月1日—2023年9月30日,共纳入接受ERCP的胆总管结石患者2121例,其中77例(3.6%)发生了PEEBI,且主要集中在术后前2天(66.2%)。影响PEEBI的主要因素为非医源的患者相关因素,即糖尿病(OR=2.43,95%CI:1.14~4.85)、胆管恶性肿瘤(OR=3.95,95%CI:1.74~8.31)和十二指肠乳头憩室(OR=4.39,95%CI:1.86~9.52)。DAGs模型较LASSO模型的综合判别能力高3.0%(P=0.007),且DAGs模型外部验证中的区分性能(D=0.133,P=0.894)和校准性能(χ^(2)=5.499,P=0.703)均表现良好。结论本研究构建的DAGs模型具有良好的预测性能,临床可在该工具的协助下采取针对性的围手术期早期预防措施,以减少PEEBI的发生。

Coastal management in China is confronted with an urgent choice between natural restoration and maintenance of existing seawalls and reclaimed land for economic development.A key criterion for making this decision is the resilience to coastal flooding,which depends on the ability to predict tidal *** duration asymmetry(TDA)is a key parameter in determination of the arrival and duration of flood *** study selected the western inner shelf of the Yellow Sea(WYS)as the study area and investigated the responses of TDA to different shoreline configurations and relative sea level *** responses of TDA to shoreline reconstruction yielded spatial variability locally and *** the nearshore area,the responses of TDA to the complex ocean environment mainly originated from the combined functions of reflection,bottom friction,and advection,which controlled the energy transfer from M2 or S2 constituents to their overtides or compound *** sensitivity of TDA to coastline typologies was not limited to coastal waters but could stretch over the entire inner *** vulnerability of tidal responses was due to the displacement of the M2 amphidrome of the Kelvin wave on the WYS,which in turn changed tidal energy fluxes over the *** relative sea level rise could intensify the feedback of TDA to seawalls and land reclamation.

目的观察天麻素注射液联合常规治疗突发性耳聋患者的临床疗效。方法回顾性分析2022年1月~12月住院收治的117例突发性耳聋患者的资料,根据治疗情况分为对照组(51例)和观察组(66例)。对照组患者参照我国2015年《突发性聋诊断和治疗指南》进行治疗;观察组同时联合应用天麻素注射液静滴治疗。比较两组患者入院时和治疗10 d后的听力、眩晕及耳鸣评分的改善情况。结果两组患者年龄、性别、患耳侧别等资料比较,差异无统计学意义(P>0.05);两组患者治疗后听力均有改善,但观察组听力提高幅度明显优于对照组,差异有统计学意义(t=4.698,P<0.05);对照组和观察组的总有效率分别为72.5%和89.4%,差异具有统计学意义(χ^(2)=5.543,P=0.019);两组患者治疗后视觉模拟量表(VAS)评分均有改善,但观察组明显优于对照组,差异有统计学意义(t=3.820,P<0.05);两组患者治疗后耳鸣评价量表(TEQ)评分均有改善,但观察组明显优于对照组,差异有统计学意义(t=7.238,P<0.05)。结论天麻素注射液可以提高突发性耳聋患者的听力,临床疗效确切,并可改善眩晕、耳鸣等伴随症状。

Recently,an environmentally friendly electrolysis process of soluble vanadates is proposed and successfully confirmed for V_(2)O_(3) *** this paper,the solubilities of various vanadates(***_(3),Na_(4)V_(2)O_(7) and Na_(3)VO_(4))in NaCl molten salt are *** dependences of V_(2)O_(3) extraction on vanadate form,anode material and molten salt system are studied.A long-term electrolysis is carried *** result indicates that the solubilities of all vanadates are high and meet electrolytic *** to Na_(4)V_(2)O_(7) and Na_(3)VO_(4),NaVO_(3) exhibits larger current efficiency and lower electricity *** using SnO_(2) anode instead of graphite anode,the current efficiency of NaVO_(3) electrolysis can be increased to 58.1%and carbon pollution is avoided at 700℃.The extraction rate of V_(2)O_(3) reaches 81.3%after long-term electrolysis of 51 *** work optimizes the electrochemical preparation process of V_(2)O_(3) from various vanadates and contributes to the improvement of current efficiency and extraction rate.

The melt memory effect is a widely observed phenomenon in semi-crystalline polymers. In practical applications, various additives are usually introduced into polymers, which may affect their melt memory behavior. In this work, the effect of talc on the melt memory effect of metallocene-made isotactic polypropylene(M-PP) was investigated in detail by using the differential scanning calorimetry. The results indicated that the introduction of talc significantly strengthened the melt memory effect of M-PP. Specifically, the upper limit temperature of Domain II increased from 161 ℃ to 174 ℃, resulting in a substantial widening of the temperature range of Domain IIa from 1 ℃ to 14 ℃. Analysis of the crystal orientation of the M-PP containing talc cooled from various Ts suggested that the remarkably enhanced melt memory effect could be ascribed to the stabilization of oriented nuclei facilitated by talc. This stabilizing effect was likely attributable to the prefreezing effect or the sorption interaction between talc and the M-PP chains.

A promising method for the preparation of pure metallic vanadium via V2CO anode electrolysis was proposed and *** simple and controllable synthesis of a molten chloride salt containing VCl2by an insitu reaction between VCl3and metallic vanadium was verified through thermodynamic analysis and ***-KCl-VCl2molten salt exhibited excellent recycling performance and could be repetitively used to prepare metallic vanadium by V2CO *** V2+ions remained in the molten salt after *** electrolysis conditions,such as the cathode diameter,current density,V2+ion concentration,and temperature,were optimised to maximise the current *** was also simulated to reveal the regulatory *** highest current efficiency was 85%.The purity of metallic vanadium was up to 99.8%.In this study,an easy and efficient preparation of pure metallic vanadium was achieved.

Tillage practices during the fallow period benefit water storage and yield in dryland wheat ***,there is currently no clarity on the responses of soil organic carbon(SOC),total nitrogen(TN),and available nutrients to tillage practices within the growing *** study evaluated the effects of three tillage practices(NT,no tillage;SS,subsoil tillage;DT,deep tillage)over five years on soil physicochemical *** samples at harvest stage from the fifth year were analyzed to determine the soil aggregate and aggregate-associated C and N *** results indicated that SS and DT improved grain yield,straw biomass and straw carbon return of wheat compared with *** contrast to DT and NT,SS favored SOC and TN concentrations and stocks by increasing the soil organic carbon sequestration rate(SOCSR)and soil nitrogen sequestration rate(TNSR)in the 0-40 cm *** SOC levels under SS and NT were associated with greater aggregate-associated C fractions,while TN was positively associated with soluble organic nitrogen(SON).Compared with DT,the NT and SS treatments improved soil available nutrients in the 0-20 cm *** findings suggest that SS is an excellent practice for increasing soil carbon,nitrogen and nutrient availability in dryland wheat fields in North China.

The screw air-source heat pump can cause incessant high noise levels during operation,which might hinder adoption of this energy-efficient heat ***,acoustic measurements and comparison testing were performed in this *** measurements revealed that the compressor is the main noise source of the heat pump,and it shows a multipeak frequency distribution and a wide frequency spectrum under different work conditions,with multiple peaks at 63,250,and 1000 ***,a compressor sound insulation cover with broadband absorption was proposed,and it was experimentally proven that the insulation cover can reduce the maximum sound pressure level of one unit from 89.8 dBA to 79.1 ***,we proposed several noise reduction strategies and compared their noise reduction effects using computer *** results showed that the noise problem can be effectively improved through the rational design of the sound barrier and the layout and opening options of heat *** distance between the sound barrier and heat pump and the sound attenuation due to diffraction ALa exhibit a U-shaped *** buildings of different heights,the optimal heights of noise barrier are *** 5.5-meter is the optimal height of the sound barrier for single-story *** conclusions can be applied to other building projects for heat pump noise reduction.
