Comparison of Intracorporeal and Extracorporeal Esophagojejunostomy after Laparoscopic Total Gastrectomy for Gastric Cancer： A Meta-Analysis Based on Short-Term

Background： Laparoscopic total gastrectomy （LTG） is increasingly performed in patients with gastric cancer. However, the usage of intracorporeal esophagojejunostomy （IEJ） following LTG is limited, as the safety and efficacy remain unclear. The present meta-analysis aimed to evaluate the feasibility and safety of IEJ following LTG. Methods： Studies published from January 1994 to January 2017 comparing the outcomes ofl EJ and extracorporeal esophagojejunostomy （EEJ） following LTG were reviewed and collected from the PubMed, EBSCO, Cochrane Library, Embase, and China National Knowledge lntemet （CNKI）. Operative results, postoperative recovery, and postoperative complications were compared and analyzed. The weighted mean difference （WMD） and odds ratio （OR） with a 95% confidence interval （CI） were calculated using the Review Manager 5.3. Results： Seven nonrandomized studies with 785 patients were included. Compared with EEJ, IEJ has less blood loss （WMD： -13.52 ml; 95% CI： -24.82-2.22; P = 0.02）, earlier time to first oral intake （WMD： -0.49 day; 95% CI： -0.83-0.14; P 〈 0.01 ）, and shorter length of hospitalization （WMD： -0.62 day; 95% CI： - 1.08-0.16; P 〈 0.01 ）. There was no significant difference between IEJ and EEJ regarding the operation time, anastomotic time, number of retrieved lymph nodes, time to first flatus, anastomosis leakage rate, anastomosis stenosis rate, and proximal resections （all P 〉 0.05）. Conclusions： Compared with EEJ, IEJ has better cosmesis, milder surgical trauma, and a faster postoperative recovery. IEJ can be performed as safely as EEJ. IEJ should be encouraged to surgeons with sufficient expertise.

Optimization of multi-model ensemble forecasting of typhoon waves

Accurately forecasting ocean waves during typhoon events is extremely important in aiding the mitigation and minimization of their potential damage to the coastal infrastructure, and the protection of coastal communities. However, due to the complex hydrological and meteorological interaction and uncertainties arising from different modeling systems, quantifying the uncertainties and improving the forecasting accuracy of modeled typhoon-induced waves remain challenging. This paper presents a practical approach to optimizing model-ensemble wave heights in an attempt to improve the accuracy of real-time typhoon wave forecasting. A locally weighted learning algorithm is used to obtain the weights for the wave heights computed by the WAVEWATCH III wave model driven by winds from four different weather models （model-ensembles）. The optimized weights are subsequently used to calculate the resulting wave heights from the model-ensembles. The results show that the opti- mization is capable of capturing the different behavioral effects of the different weather models on wave generation. Comparison with the measurements at the selected wave buoy locations shows that the optimized weights, obtained through a training process, can significantly improve the accuracy of the forecasted wave heights over the standard mean values, particularly for typhoon-induced peak waves. The results also indicate that the algorithm is easy to imnlement and practieal for real-time wave forecasting.

Indications of pro-inflammatory cytokines in laparoscopic and open liver resection for early-stage hepatocellular carcinoma

Background:Our clinical practice of laparoscopic liver resection(LLR)had achieved better short-term and long-term benefits for patients with hepatocellular carcinoma(HCC)over open liver resection(OLR),but the underlying mechanisms are not clear.This study was to find out whether systemic inflammation plays an important role.Methods:A total of 103 patients with early-stage HCC under liver resection were enrolled(LLR group,n=53;OLR group,n=50).The expression of 9 inflammatory cytokines in patients at preoperation,postoperative day 1(POD1)and POD7 was quantified by Luminex Multiplex assay.The relationships of the cytokines and the postoperative outcomes were compared between LLR and OLR.Results:Seven of the circulating cytokines were found to be significantly upregulated on POD1 after LLR or OLR compared to their preoperative levels.Compared to OLR,the POD1 levels of granulocytemacrophage colony-stimulating factor(GM-CSF),interleukin-6(IL-6),IL-8,and monocyte chemoattractant protein-1(MCP-1)in the LLR group were significantly lower.Higher POD1 levels of these cytokines were significantly correlated with longer operative time and higher volume of blood loss during operation.The levels of these cytokines were positively associated with postoperative liver injury,and the length of hospital stay.Importantly,a high level of IL-6 at POD1 was a risk factor for HCC recurrence and poor disease-free survival after liver resection.Conclusions:Significantly lower level of GM-CSF,IL-6,IL-8,and MCP-1 after liver resection represented a milder systemic inflammation which might be an important mechanism to offer better short-term and long-term outcomes in LLR over OLR.

Identifying the complex genetic architecture of growth and fatness traits in a Duroc pig population

In modern pig breeding programs,growth and fatness are vital economic traits that significantly influence porcine production.To identify underlying variants and candidate genes associated with growth and fatness traits,a total of 1067 genotyped Duroc pigs with de-regressed estimated breeding values(DEBV)records were analyzed in a genome wide association study(GWAS)by using a single marker regression model.In total,28 potential single nucleotide polymorphisms(SNPs)were associated with these traits of interest.Moreover,VPS4 B,PHLPP1,and some other genes were highlighted as functionally plausible candidate genes that compose the underlying genetic architecture of porcine growth and fatness traits.Our findings contribute to a better understanding of the genetic architectures underlying swine growth and fatness traits that can be potentially used in pig breeding programs.

The distributed permutation flowshop scheduling problem（DPFSP） has important applications in modern industries.This paper presents a discrete artificial bee colony（DABC） algorithm for solving the DPFSP with total flow time criterion. We use the job permutation based representation to represent the candidate solutions. The insertion operator is adapted to generate neighboring solutions in the search process. We re-design the search phases including employ bees, onlooker bees, and scout bees according to the problem-specific knowledge. A comprehensive computational campaign based on the 720 benchmark instances demonstrates the effectiveness of the proposed DABC algorithm for solving the DPFSP with the total flowtime criterion.

Dexmedetomidine Attenuates High Glucose-induced HK-2 Epithelial-mesenchymal Transition by Inhibiting AKT and ERK

Objective To explore the protective effects of dexmedetomidine(Dex)against high glucose-induced epithelial-mesenchymal transition in HK-2 cells and relevant mechanisms.Methods HK-2 cells were exposed to either glucose or glucose+Dex for 6 h.The production of ROS,morphology of HK-2 cells,and cell cycle were detected.Moreover,the expression of AKT,p-AKT,ERK,pERK,PI3 K,E-Cadherin,Claudin-1,andα-SMA were determined and compared between HK-2 cells exposed to glucose and those exposed to both glucose and Dex with or without PI3 K/AKT pathway inhibitor LY294002 and ERK pathway inhibitor U0126.Results Compared with HK-2 cells exposed to high level of glucose,the HK-2 cells exposed to both high level of glucose and Dex showed:(1)lower level of ROS production;(2)cell morphology was complete;(3)more cells in G1 phase;(4)lower expression of p-AKT,p-ERK andα-SMA,higher expression of ECadherin and Claudin-1.PI3 K/AKT inhibitor LY294002 and ERK inhibitor U0126 decreased the expression of p-AKT,p-ERK andα-SMA,and increased the expression of E-Cadherin and Claudin-1.Conclusion Dex can attenuate high glucose-induced HK-2 epithelial-mesenchymal transition by inhibiting AKT and ERK.

Spinal anesthesia is a common technique which can significantly alleviate surgery patients' anguish, and can avoid the risks associated with general anesthesia. However, spinal anesthesia is associated with some other risks. The most common adverse event is hypotension resulting from a near complete sympathetic block. Nearly 1/3 of non-obstetric patients experience spinal anesthesia-induced hypotension. Moreover, in obstetric patients without pharmacological prophylaxis, the incidence rate of spinal anesthesia-induced hypotension can reach up to 70%-80%[1].

A pulmonary source of infection in patients with sepsis-associated acute kidney injury leads to a worse outcome and poor recovery of kidney function

BACKGROUND:Hospital mortality rates are higher among patients with sepsis-associated acute kidney injury(SA-AKI)than among patients with sepsis.However,the pathogenesis underlying SA-AKI remains unclear.We hypothesized that the source of infection affects development of SA-AKI.We aim to explore the relationship between the anatomical source of infection and outcome in patients with SA-AKI.METHODS:Between January 2013 and January 2018,113 patients with SA-AKI admitted to our Emergency Center were identifi ed and divided into two groups:those with pulmonary infections and those with other sources of infection.For each patient,we collected data from admission until either discharge or death.We also recorded the clinical outcome after 90 days for the discharged patients.RESULTS:The most common source of infection was the lung(52/113 cases,46%),followed by gastrointestinal(GI)(25/113 cases,22.1%)and urinary(22/113,19.5%)sources.Our analysis showed that patients with SA-AKI had a significantly worse outcome(30/52 cases,P<0.001)and poorer kidney recovery(P=0.015)with pulmonary sources of infection than those infected by another source.Data also showed that patients not infected by a pulmonary source more likely experienced shock(28/61 cases,P=0.037).CONCLUSION:This study demonstrated that the source of infection infl uenced the outcome of SA-AKI patients in an independent manner.Lung injury may influence renal function in an asyet undetermined manner as the recovery of kidney function was poorer in SA-AKI patients with a pulmonary source of infection.

Deep insight of unique phase transition behaviors and mechanism in Zr_(2)Co-H isotope system with ultra-low equilibrium pressure

Efficient capture,safe storage and release of tritium from the international thermonuclear experimental reactor(ITER) reaction exhaust gas is a perplexing problem,and the development of an efficient tritium-getter material with ultra-low hydrogenation equilibrium pressure is considered as a reliable way.In this work,Zr_(2)Co alloy was selected as a tritium-getter material and prepared through induction levitation melting.Fundamental performance test results show that Zr_(2)Co exhibits an ultra-low hydrogenation equilibrium pressure of 3.22 × 10^(-6) Pa at 25℃ and excellent hydriding kinetics under a low hydrogen pressure of 0.005 MPa.Interestingly,unique phase transition behaviors were presented in Zr_(2)Co-H system.Specifically,Zr_(2)CoH_(5) formed by Zr_(2)Co hydrogenated at room temperature is initially decomposed into ZrH_(2) and ZrCoH_(3) at200 ℃.With the temperature increasing to 350 ℃,ZrCoH_(3)is dehydrogenated to ZrCo,and then ZrCo further reacts with ZrH_(2) at 650 ℃ to reform Zr_(2)Co and hydrogen.Among the staged phase transition pathways during dehydrogenation,the decomposition of Zr_(2)CoH_(5) occurs preferentially,which is well accordance with both the smallest reaction energy barrier and the maximum reaction spontaneity that are determined respectively from kinetics activation energy and thermodynamics Gibbs free energy.Furthermore,first principles calculation results indicate that the stronger binding of hydrogen in interstitial environments of ZrCoH_(3)and ZrH_(2) triggers the hydrogen-stabilized phase transformation of Zr_(2)CoH_(5).The unique phase transition mechanisms in Zr_(2)Co-H system can shed light on the further exploration and regulation of analogous staged phase transition of hydrogen storage materials.
