目的研究学龄期局灶性癫患儿发作间期睡眠结构的变化。方法应用Solar2000N神经中央监护系统对23例6～12岁局灶性癫患儿(男15例,女8例)及15例年龄、性别与之相匹配的健康儿童(健康对照组,男11例,女4例)进行全夜自然睡眠多导监测,并将23例局灶性癫患儿按照样放电出现部位(额叶癫组、颞叶癫组、枕叶癫组),发作好发时间(睡眠发作组、清醒发作组、睡眠+清醒发作组)及服用抗癫药物类型(卡马西平治疗组,卡马西平+托吡酯治疗组)进行分组,应用SPSS13.0软件分析各组患儿与健康儿童之间各项睡眠参数的差异。结果与健康对照组比较,学龄期局灶性癫患儿非快速眼动(NREM)睡眠1期比例延长(P<0.05),慢波睡眠(SWS)比例显著减少(P<0.01);额叶癫组各项睡眠参数与健康对照组比较差异无统计学意义,颞叶癫组患儿NREM睡眠1期比例及觉醒时间延长(Pa<0.05),枕叶癫组SWS比例与健康对照组比较差异有统计学意义(P<0.05);睡眠发作组觉醒次数增加(P<0.05),NREM睡眠1期比例增加(P<0.05),清醒发作组SWS比例减少(P<0.05);卡马西平+托吡酯组与未服药组比较,快速眼动(REM)睡眠时间增加(P<0.05)。结论学龄期局灶性癫患儿浅睡眠增多,深睡眠减少,以NREM1期睡眠增多为主;颞叶癫显示了更多的觉醒时间;易在睡眠中发作的癫患儿觉醒次数增加;卡马西平+托吡酯治疗可增加局灶性癫患儿的REM睡眠时间。

目的总结TRIO基因相关儿童智力发育障碍(MRD)的临床表现和基因特点。方法病例系列研究, 回顾性收集首都医科大学附属北京儿童医院神经内科及郑州大学第三附属医院儿科2019年8月至2024年3月就诊的9例TRIO基因相关MRD患儿病例资料, 包括性别、年龄、智力及运动发育、外观、癫痫发作、头颅影像学和基因结果, 总结其临床特点及基因型表型特点。结果 9例患儿中男6例、女3例, 4例患儿符合MRD63型表型, 表现为中重度发育落后伴大头畸形;5例患儿符合MRD44型表型, 表现为轻中度发育落后伴小头畸形。伴有明显特殊面容(枕部扁平、眉毛粗重、连眉、耳朵大、手指短小、肤白发黄和斜视等)5例, 伴有癫痫发作2例(肌阵挛发作和失神发作各1例), 伴有喂养困难4例, 伴有先天性白内障、先天性心脏病、反复感染、眼底改变(漆裂纹样改变)各1例。9例患儿携带的TRIO基因变异均为新生变异, 包括8个变异位点, 其中错义变异7个、移码变异1个, 分别为c.3232C>T/p.R1078W(2例)、c.3920A>G/p.Y1307C、c.4112A>T/p.H1371L、c.4283G>T/p.R1428L、c.4394A>G/p.N1465S、c.6041T>C/p.I2014T、c.6821G>A/p.R2274H、c.7027delC/p.Q2343Sfs*70。2例变异位点位于Spectrin结构域, 4例位于鸟嘌呤核苷酸交换因子(GEF)D1结构域, 2例位于GEFD2结构域, 1例(移码变异)位于PH2-SH3结构域。移码变异者表现为失神发作和轻度发育落后, 表型最轻。肌阵挛发作癫痫患儿加用丙戊酸联合左乙拉西坦发作控制, 失神癫痫患儿加用丙戊酸联合拉莫三嗪发作控制。9例患儿均进行积极康复训练, 发育缓慢进步。结论 TRIO基因相关儿童MRD表现为不同程度的发育落后, 多伴有大头或小头畸形, 特殊面容, 伴或不伴癫痫发作;变异类型以错义变异为主, 错义变异大多集中于Spectrin结构域和GEFD结构域, p.R1078W可能为相对热点变异, 移码变异者表型相对较轻。

Cattle encephalon glycoside and ignotin(CEGI)injection is a compound preparation formed by a combination of muscle extract from hea lthy rabbits and brain gangliosides from cattle,and it is generally used as a neuroprotectant in the treatment of central and peripheral nerve ***,there is still a need for high-level clinical evidence from large samples to support the use of *** therefore carried out a prospective,multicenter,randomized,double-blind,parallel-group,placebo-controlled study in which we recruited 319 patients with acute cerebral infarction from 16 centers in china.from October 2013 to May *** patients were randomized at a 3:1 ratio into CEGI(n=239;155 male,84 female;61.2±9.2 years old)and placebo(n=80;46 male,34 female;63.2±8.28 years old)*** patients were given standard care once daily for 14 days,including a 200 mg aspirin enteric-coated tablet and 20 mg atorvastatin calcium,both taken orally,and intravenous infusion of 250–500 mL 0.9%sodium chloride containing 40 mg sodium tanshinone IIA *** on conventional treatment,patients in the CEGI and placebo groups were given 12 mL CEGI or 12 mL sterile water,respectively,in an intravenous drip of 250 mL 0.9%sodium chloride(2 mL/min)once daily for 14 *** to baseline National Institutes of Health Stroke Scale scores,patients in the two groups were divided into mild and moderate *** on the modified Rankin Scale results,the rate of patients with good outcomes in the CEGI group was higher than that in the placebo group,and the rate of disability in the CEGI group was lower than that in the placebo group on day 90 after *** the CEGI group,neurological deficits were decreased on days 14 and 90 after treatment,as measured by the National Institutes of Health Stroke Scale and the Barthel *** analysis revealed that CEGI led to more significant improvements in moderate stroke *** drug-related adverse events occurred in the CEGI or

Slit-Robo GTPase-activating protein 2(SRGAP2) plays important roles in axon guidance, neuronal migration, synapse formation, and nerve regeneration. However, the role of SRGAP2 in neuroretinal degenerative disease remains unclear. In this study, we found that SRGAP2 protein was first expressed in the retina of normal mice at the embryonic stage and was mainly located in the mature retinal ganglion cell layer and the inner nuclear layer. SRGAP2 protein in the retina and optic nerve increased after optic nerve crush. Then, we established a heterozygous knockout(Srgap2+/–) mouse model of optic nerve crush and found that Srgap2 suppression increased retinal ganglion cell survival, lowered intraocular pressure, inhibited glial cell activation, and partially restored retinal function. In vitro experiments showed that Srgap2 suppression activated the mammalian target of rapamycin signaling pathway. RNA sequencing results showed that the expression of small heat shock protein genes(Cryaa, Cryba4, and Crygs) related to optic nerve injury were upregulated in the retina of Srgap2+/– mice. These results suggest that Srgap2 suppression reduced the robust activation of glial cells, activated the mammalian target of rapamycin signaling pathway related to nerve protein, increased the expression of small heat shock protein genes, inhibited the degeneration of retinal ganglion cells, and partially restored optic nerve function.

AIM:To analyse macular microvascular alterations in myopic choroidal neovascularization(m CNV)and the efficiency of anti-vascular endothelial growth factor(antiVEGF)therapy for m CNV by optical coherence tomography angiography(OCTA).METHODS:A total of 123 patients were included in this retrospective study,divided into m CNV group,high myopia(HM)group,and normal group at the affiliated Eye hospital of wenzhou medical University from January 2017 to January *** vessel density,deep capillary density,foveal avascular zone(FAZ)area,A-circularity index(AI)and vessel density around the 300μm width of the FAZ region density(FD)and the area of choroidal neovascularization(CNV)lesion(only for m CNV group)were measured on 3×3 mm2 OCTA *** area was corrected for axial *** macular thickness(CMT)was measured on OCT in m CNV *** the parameters on OCTA of 3 groups and pre-anti-VEGF and post-anti-VEGF at 1,2,3,and 6 mo follow-up in m CNV ***:There were significant differences among 3 groups in superficial vessel density,deep capillary density and FD(P<0.05).FAZ area in HM group was smaller than normal group(P<0.05),but there was no significant difference between m CNV group and the other two *** increased in m CNV group(P<0.05).The mean CMT,area and flow area of CNV lesion decreased after treatment(P<0.05),while vessel density and FAZ didn’t *** mean CMT,area and flow area of CNV lesion statistically decreased after anti-VEGF treatment in m CNV group(P<0.05),while superficial vessel density,deep capillary density and FAZ area,AI and FD didn’t *** mean reduction ratio of lesions was 50.32%(7.07%to 100%).Lesion regression 100%was observed in 2 cases(4.88%).There was a negative correlation between the CNV lesion area and reduction ratio(r=-0.380,P=0.042)and the flow lesion area and reduction ratio(r=-0.402,P=0.030).CONCLUSION:Macular vessel density decreases,FAZ turns smaller and more irregular in m CNV ***
