OBJECTIVE To investigate the association between baseline hemoglobin A1c(HbA1c)levels and bleeding in patients with non-ST-segment elevation acute coronary syndrome(NSTE-ACS)who underwent percutaneous coronary intervention(PCI).METHODS This observational cohort study enrolled 6283 consecutive NSTE-ACS patients undergoing PCI from January 1,2010 to December 31,*** on baseline HbA1c levels,the patients were divided into the group with HbA1c<7%(n=4740)and the group with HbA1c≥7%(n=1543).The primary outcomes are major bleeding(BARC grades 3-5)and all-cause death during *** Of patients enrolled,4705(74.9%)were male,and 2143(34.1%)had a history of diabetes mellitus,with a mean(SD)age of 64.13(10.32)*** median follow-up duration was 3.21 *** with the patients with HbA1c<7%,the risk of major bleeding events during follow-up was higher in patients with HbA1c≥7%(adjusted hazard ratio[HR]=1.57;95%con-fidence interval[CI]:1.01-2.44;P=0.044),while the risk of all-cause death during follow-up was not associated with the higher HbA1c levels(adjusted HR=0.88;95%CI:0.66-1.18;P=0.398).CONCLUSIONS Compared with the lower baseline HbA1c levels,the higher baseline HbA1c levels were associated with an increase in long-term bleeding risk in NSTE-ACS patients undergoing PCI,though higher baseline HbA1c levels were not associ-ated with the higher risk in all-cause death.

Objective To describe the long-term antithrombotic management patterns(AMPs)and clinical outcomes of Chinese patients with acute coronary syndrome(ACS).Methods This was an observational,multicenter,longitudinal cohort extension study of Chinese patients who had completed the EPICOR Asia 2-year follow-up study post-hospitalization for an ACS *** in AMP and clinical outcomes for up to 5 years post-ACS event were *** Overall,2334 patients with ACS were enrolled at 49 *** mean age was 61.6 years and 76.3%were *** study end,2093 patients completed the 3-year *** baseline(2 years post-ACS event),72.4%of patents received one antiplatelet(AP)medication,with aspirin being the preferred one.A small proportion of patients(21.5%)was treated with two or more APs(2+AP),and even fewer patients(6.1%)did not receive any AP medication at *** study completion,the proportion of patients without AP therapy increased to 13.6%,while the percentage of patients on one AP and 2+AP decreased to 69.3%and 17.1%,***,a higher incidence of clinical events(composite of all-cause mortality,myocardial infarction,stroke)was observed for the 2+AP(13.2%)subgroup than for the no AP(10.5%)and one AP(8.6%)***,the 2+AP subgroup exhibited the greatest number of bleeding events,outpatient visits,and hospitalization *** myocardial infarction or stroke,bleeding events prompted an adjustment in *** Most patients in China received at least one AP medication up to 5 years after an ACS event.

Background： The excess volume of contrast media （CM） is a marker of a more severe coronary culprit lesion and longer intervention duration in patients undergoing cardiac procedures. However, it is unclear whether the contrast volume is directly correlated with worse clinical outcomes. The aim of this study was to investigate the association between contrast dose and the incidence of 1-year major adverse cardiac and cerebrovascular events （MACCE） and all-cause bleeding events in patients undergoing cardiac catheterization and coronary angiography （CAG）. Methods： We prospectively enrolled 10,961 consecutive patients diagnosed with coronary heart disease expecting CAG from 2012 to 2013. The study population was pursued with a follow-up duration of 1 year. The predictive value of contrast volume, divided into quartiles, for the risk of MACCE and all-cause bleeding events was assessed using logistic regression analysis. Results： The cumulative incidence of 1-year MACCE was 8.65%, which was directly associated with increasing contrast volume. In particular, MACCE was observed in 7.16%, 7.89%, 9.31%, and 11.73% of cases in the contrast volume quartile Q1 （≤100 ml）, Q2 （101-140 m）, Q3 （141-200 ml）, and Q4 （〉200 ml）, respectively （P 〈 0.001）. Moreover, the incidence of 1-year all-cause bleeding events was noted in 4.70%, 5.93%, 7.28%, and 8.21% of patients in Q1, Q2, Q3, and Q4, respectively （P 〈 0.001）. The survival analysis showed that the 1-year MACCE rate was higher in patients using greater CM volume during the CAG. CM volume used 〉140 ml was associated with the occurrence of 1-year MACCE, and the incidence was dramatically elevated in patients exceeding a contrast volume of 200 ml （P = 0.007）. Conclusion： Our data suggested that higher contrast volume was significantly correlated with an increased risk of MACCE and all-cause bleeding events in patients undergoing cardiac catheterization.

AIM:To investigate the effect of pegylated interferon (IFN) α-2b on specific CD8+ T lymphocytes in patients with chronic hepatitis B (CHB). METHODS:Twenty-one patients with CHB were treated with pegylated IFN α-2b. Periphery blood mononuclear cells were isolated from fresh heparinized blood by Ficoll-Hypaque density gradient centrifugation (density:1.077 g/L,Pharmingen) at weeks 0,4,8,12,and 24,respectively. Frequency of circulating hepatitis B virus (HBV) epitope-specific CD8 T cells was detected by flow cytometry. Cytokines were detected by cytometric bead assay. RESULTS:The frequency of circulating HBV core or env-specific CD8 T cells was higher (P < 0.05),the number of HBV core specific CD8 T cells was greater at week 24 (P < 0.05),the level of Th1-type cytokines [interleukin (IL)-12,tumor necrosis factor-α,and IFN-γ] was higher,while that of Th2-type cytokines (IL-4,IL-6,and IL-10) was lower in responders than in nonresponders (P < 0.05) after pegylated IFN α-2b treatment. The IL-6 level was correlated with HBV DNA (r = 0.597,P = 0.04),while the inducible protein-10 (IP-10) level was correlated with serum alanine aminotransferase (ALT) (r = 0.545,P = 0.005). The IP-10 level at week 8 after pegylated IFN α-2b treatment could predict the normalization of ALT in CHB patients (positive predict value = 56%,negative predict value = 92%). CONCLUSION:Pegylated IFN α-2b can enhance the immune response of CHB patients by increasing the frequency of HBV specific CD8+ T cells and regulating the Th1/Th2 cytokines.

β-ray-induced X-ray spectroscopy(BIXS)is a promising method for tritium detection in solid materials because of its unique advantages,such as large detection depth,nondestructive testing capabilities,and low requirements for sample ***,high-accuracy reconstruction of the tritium depth profile remains a significant challenge for this *** this study,a novel reconstruction method based on a backpropagation(BP)neural network algorithm that demonstrates high accuracy,broad applicability,and robust noise resistance is *** average reconstruction error calculated using the BP network(8.0%)was much lower than that obtained using traditional numerical methods(26.5%).In addition,the BP method can accurately reconstruct BIX spectra of samples with an unknown range of tritium and exhibits wide applicability to spectra with various tritium ***,the BP network demonstrates superior accuracy and stability compared to numerical methods when reconstructing the spectra,with a relative uncertainty ranging from 0 to 10%.This study highlights the advantages of BP networks in accurately reconstructing the tritium depth profile from BIXS and promotes their further application in tritium detection.

Shielding materials are critical for downhole pulsed neutron tool design because they directly influence the accuracy of formation measurements.A well-designed shield configuration ensures that the response of the tool is maximally representative of the formation without being affected by the tool and borehole *** study investigated the effects of boron-containing materials on neutron and gamma detectors based on a newly designed logging-while-drilling tool that is currently undergoing *** the boron content increased,the ability to absorb thermal neutrons increased *** simulation,it was proven that boron carbide(B_(4)C)can be used as an effective boron shielding material for thermal neutrons,and is therefore employed in this *** shield against thermal neutrons migrating from the mud pipes,the optimal shielding thicknesses for the near-and far-neutron detectors were determined to be 5 and 4 *** a porosity of 25 p.u.,near-neutron sensitivity exhibited a 5.6%***,to shield the capture gamma generated by thermal neutrons once they enter the tool from the mud pipe and formation,internal and external shields for the gamma detector were *** results show that the internal shield requires a boron content of 75%,whereas the external shield has a thickness of 14.2 mm thickness and a boron content of 25%to minimize the tool effect.
