Effect of gastric acid suppressants and prokinetics on peritoneal dialysis-related peritonitis

AIM: To investigate the effect of gastric acid suppressants and prokinetics on peritonitis development in peritoneal dialysis(PD) ***: This was a single-center, retrospective study. The medical records of 398 PD patients were collected from January 2000 to September 2012 and analyzed to compare patients with at least one episode of peritonitis(peritonitis group, group A) to patients who never had peritonitis(no peritonitis group, group B). All peritonitis episodes were analyzed to compare peritonitis caused by enteric organisms and peritonitis caused by non-enteric ***: Among the 120 patients who met the inclusion criteria, 61 patients had at least one episode ofperitonitis and 59 patients never experienced peritonitis. Twenty-four of 61 patients(39.3%) in group A and 15 of 59 patients(25.4%) in group B used gastric acid suppressants. Only the use of H2-blocker(H2B) was associated with an increased risk of PD-related peritonitis; the use of proton pump inhibitors, other antacids, and prokinetics was not found to be a significant risk factor for PD-related peritonitis. A total of 81 episodes of peritonitis were divided into enteric peritonitis(EP) or non-enteric peritonitis, depending on the causative organism, and gastric acid suppressants and prokinetics did not increase the risk of EP in PD ***: The use of H2B showed a trend for an increased risk of overall PD-related peritonitis, although further studies are required to clarify the effects of drugs on PD-related peritonitis.

We previously demonstrated that octadecylamine-functionalized nanodiamond(ND-ODA)and dexamethasone(Dex)-adsorbed ND-ODA(ND-ODA–Dex)promoted anti-inflammatory and proregenerative behavior in human macrophages in *** this study,we performed a pilot study to investigate if these immunomodulatory effects translate when used as a treatment for rheumatoid arthritis in *** local injection in limbs of mice with collagen type II-induced arthritis,microcomputed tomography showed that mice treated with a low dose of ND-ODA and ND-ODA–Dex did not experience bone loss to the levels observed in non-treated arthritic controls.A low dose of ND-ODA and ND-ODA–Dex also reduced macrophage infiltration and expression of proinflammatory mediators iNOS and tumor necrosis factor-a compared to the arthritic control,while a high dose of ND-ODA increased expression of these ***,these results suggest that ND-ODA may be useful as an inherently immunomodulatory platform,and support the need for an in-depth study,especially with respect to the effects of dose.

Measurement of stiffness and damping coefficient of rubber tractor tires using dynamic cleat test based on point contact model

The ride vibration of a tractor is affected mostly by the stiffness and damping coefficient of the seat suspension,cabin suspension,cabin rubber mounts,and rubber ***,in the case of rubber tractor tires,the stiffnesses and damping coefficients have not been researched adequately thus far,and it is not simple to measure these *** this study,a method for measuring and analyzing the stiffnesses and damping coefficients of rubber tractor tires,which were the input parameters for the tractor ride vibration simulation,was *** cleat test,proposed in this study,did not require separate and complicated test equipment,unlike the conventional *** test was conducted simply by measuring acceleration under the driving conditions of the vehicle without detaching tires from the vehicle body or setting up additional test *** on the ground-vertical acceleration data obtained,the stiffness was calculated using the logarithmic decrement method,and the damping coefficient was calculated using least squares exponential curve *** result of the cleat test indicated that the front tires had stiffnesses of 486.08-570.69 kN/m and damping coefficients of 4.02-4.52 kN·s/m;the rear tires had stiffnesses of 409.42-483.79 kN/m and damping coefficients of 2.21-2.67 kN·s/*** the test,40 mm height cleats were installed on the track and the speed of the tractor was set to 7 and 10 km/h,which were the most common speeds during the *** study is meaningful in that it has presented a new method that improves the practicality of results,reduces cost,and simplifies the test process for measuring the stiffnesses and damping coefficients of rubber tractor tires.

The miniaturization and high integration of devices demand significant thermal management *** technologies for the thermal management of electronics show some limitations in the case of multiple chip arrays.A device in multiple chip array is affected by heat from adjacent devices,along with thermal conductive *** address this problem,we present a nano composite of aligned boron nitride(BN)nanosheet islands with porous polydimethylsiloxane(PDMS)foam to have mechanical stability and non-thermal *** islands of tetrahedrally-structured BN in the composite have a high thermal conductivity of 1.219 W·m^(-1)·K^(-1) in the through-plane direction(11.234W·m^(-1)·K^(-1)in the in-plane direction)with 16 wt.%loading of *** the other hand,porous PDMS foam has a low thermal conductivity of 0.0328W·m^(-1)·K^(-1) in the through-plane direction at 70%*** pathways are then formed only in the structured BN islands of the *** porous PDMS foam can be applied as a thermal barrier between structured BN islands to inhibit thermal interference in multiple device ***,this composite can maintain selective thermal dissipation performance with 70%tensile *** beauty of the work is that it could have guided heat dissipation by assembling of multiple layers which have high vertical thermal conductive islands,while inhibiting thermal *** selective heat dissipating composite can be applied as a heatsink for multiple chip arrays electronics.

Phase diagram for Bi-site La-doped BiFeO_(3)—BaTiO_(3)lead-free piezoelectric ceramics

Three different series of lead-free ceramics,i.e.,(1-y)Bi1.03(1_x)LaxFeO_(3)-yBaTiO_(3)(y=0.27,x=0.00-0.12),(y=0.30,x=0.00-0.10),and(y=0.33,x=0.00-0.08)are prepared via a conventional solid-state reaction with water *** X-ray diffraction and electrical property measurements,two morphotropic phase boundaries(MPBs)are discovered in all three ceramic *** first MPB(MPB-Ⅰ)appeared between rhombohedral and tetragonal phases,whereas the second MPB(MPB-Ⅱ)appeared between tetragonal and cubic-like *** highest direct piezoelectric coefficients(d_(33)=201,274,and 268 pC/N)are mainly attributed to the typical MPB-I of the rhombohedral and tetragonal ***,the highest converse piezoelectric coefficients(d_(33*)=490,500,and 570 pm/V with Curie temperature>330℃)are obtained for compositions near to the MPB-II.A significant enhancement in the dielectric constant at low temperature is associated with the local structural heterogeneity by La^(3+)doping,which serves as an origin for a high piezoelectric strain *** on the crystal structure as well as on the dielectric,ferroelectric,and piezoelectric properties,a phase diagram is constructed for La-doped BiFeO3-BaTiO3 *** phase diagram reveals the relationship between piezoelectric performance and crystal structure.

Association between preoperative high sensitive troponin I levels and cardiovascular events after hip fracture surgery in the elderly

Objective Cardiovascular complications contribute to postoperative morbidity and mortality in elderly hip fracture patients. Limited data are available regarding which preoperative risk factors predict cardiovascular course following hip fracture surgery （HFS）. We used high sensitive troponin I （hs-TnI） assays and clinical parameters to identify preoperative risk factors associated with major adverse cardiac events （MACE） in elderly hip frac^u＇e patients. Method From August 2014 to November 2016, 575 patients with hip fracture were enrolled in a retrospective, single-center registry. A total of 262 of these patients underwent HFS and hs-TnI assays. MACE was defined as postoperative all-cause deaths, heart failure （HF）, new-onset atrial fibrillation （AF）, myocardial infarction （MI） and cardiovascular re-hospitalization that occurred within 90 days postoperative. Results Of 262 HFS patients, MACE developed following HFS in 65 （24.8%）. Patients with MACE were older and had higher rates of renal insufficiency, coronary artery disease, prior HF, low left ventricular ejection fraction and use of beta blockers; higher levels of hs-Tnl and N-terminal pro-brain natriuretic peptide （NT-proBNP） and higher revised cardiac risk index. A preoperative hs-TnI≥ 6.5 ng/L was associated with high risk of postoperative HF, new-onset AF and MACE. In multivariable analysis, pre-operative independent predictors for MACE were age 〉 80 years [adjusted hazard ratio （HR）： 1.79, 95% confident interval （CI）： 1.03-3.13, P = 0.04], left ventricular ejection fraction （LVEF）〈 50% （adjusted HR： 3.17, 95% CI： 1.47-5.82, P 〈 0.01） and hs-TnI 〉 6.5 ng/L （adjusted HR： 3.75, 95% CI： 2.09~5.17, P 〈 0.01）. Conclusion In elderly patients with hip fracture who undergo HFS, a preoperative assessment of hs-TnI may help the risk refinement of cardiovascular complications.

We present a 69-year-old woman with a duodenal obstruction after successful selective transcatheter arterial embolization (tae) for a duodenal diverticular hemorrhage. Two weeks after tae, the patient showed abrupt symptoms of duodenal obstruction. Resolving hematomas after successful selective transcatheter arterial embolization should be thoroughly observed because they might result in duodenal fibrotic encasement featuring inflammatory duodenal wall thickening, duodenal deformity, dysmotility, and fi nally obstruction.

Appropriate empirical antibiotic use and 30-d mortality in cirrhotic patients with bacteremia

AIM: To analyze whether prompt and appropriate empirical antibiotic(AEA) use is associated with mortality in cirrhotic patients with bacteremia. METHODS: A total of 102 episodes of bacteremia in 72 patients with cirrhosis were analyzed. AEA was defined as a using or starting an antibiotic appropriate to the isolated pathogen at the time of bacteremia. The primary endpoint was 30-d mortality. RESULTS: The mortality rate at 30 d was 30.4%(31/102 episodes). Use of AEA was associated with better survival at 30 d(76.5% vs 46.9%, P = 0.05), and inappropriate empirical antibiotic(IEA) use was an independent factor associated with increased mortality(OR = 3.24; 95%CI: 1.50-7.00; P = 0.003, adjusted for age, sex, ChildPugh Class, gastrointestinal bleeding, presence of septic shock). IEA use was more frequent when the isolated pathogen was a multiresistant pathogen, and when infection was healthcare-related or hospital-acquired. CONCLUSION: AEA use was associated with increased survival of cirrhotic patients who developed bacteremia. Strategies for AEA use, tailored according to the local epidemiological patterns, are needed to improve survival of cirrhotic patients with bacteremia.
