Coinfection with hepatic cystic and alveolar echinococcosis with abdominal wall abscess and sinus tract formation:A case report

BACKGROUND Hepatic cystic and alveolar echinococcosis coinfections,particularly with concurrent abscesses and sinus tract formation,are extremely rare.This article presents a case of a patient diagnosed with this unique presentation,discussing the typical imaging manifestations of both echinococcosis types and detailing the diagnosis and surgical treatment experience thereof.CASE SUMMARY A 39-year-old Tibetan woman presented with concurrent hepatic cystic and alveolar echinococcosis,accompanied by abdominal wall abscesses and sinus tract formation.Initial conventional imaging examinations suggested only hepatic cystic echinococcosis,but intraoperative and postoperative pathological examination revealed the coinfection.Following radical resection of the lesions,the patient’s condition improved,and she was discharged soon thereafter.Subsequent outpatient follow-ups confirmed no recurrence of the hydatid lesion and normal surgical wound healing.Though mixed hepatic cystic and alveolar echinococcosis with abdominal wall abscesses and sinus tract formations are rare,the general treatment approach remains consistent with that of simpler infections of alveolar echinococcosis.CONCLUSION Lesions involving the abdominal wall and sinus tract formation,may require radical resection.Long-term prognosis includes albendazole and follow-up examinations.

Esophageal cancer(EC)is the seventh most common cancer worldwide,and esophageal squamous cell carcinoma(ESCC)accounts for the majority of cases of EC.To effectively diagnose and treat ESCC and improve patient prognosis,timely diagnosis in the initial phase of the illness is necessary.This article offers a detailed summary of the latest advancements and emerging technologies in the timely identification of ECs.Molecular biology and epigenetics approaches involve the use of molecular mechanisms combined with fluorescence quanti-tative polymerase chain reaction(qPCR),high-throughput sequencing technology(next-generation sequencing),and digital PCR technology to study endogenous or exogenous biomolecular changes in the human body and provide a decision-making basis for the diagnosis,treatment,and prognosis of diseases.The invest-igation of the microbiome is a swiftly progressing area in human cancer research,and microorganisms with complex functions are potential components of the tumor microenvironment.The intratumoral microbiota was also found to be connected to tumor progression.The application of endoscopy as a crucial technique for the early identification of ESCC has been essential,and with ongoing advancements in technology,endoscopy has continuously improved.With the advancement of artificial intelligence(AI)technology,the utilization of AI in the detection of gastrointestinal tumors has become increasingly prevalent.The implementation of AI can effectively resolve the discrepancies among observers,improve the detection rate,assist in predicting the depth of invasion and differentiation status,guide the pericancerous margins,and aid in a more accurate diagnosis of ESCC.

The global incidence rates of in-hospital cardiac arrest(IHCA)range from1.2 to 9.0 per1,000 hospitalized patients,as per the National Cardiac Arrest Database.[1] While IHCAs tend to exhibit superior 30-day survival rates relative to out-of-hospital cardiac arrests (OHCA) due to situational advantages,such as immediate access to medical personnel and treatments.

BACKGROUND According to clinical data,a significant percentage of patients experience pain after surgery,highlighting the importance of alleviating postoperative pain.The current approach involves intravenous self-control analgesia,often utilizing opioid analgesics such as morphine,sufentanil,and fentanyl.Surgery for colo-rectal cancer typically involves general anesthesia.Therefore,optimizing anes-thetic management and postoperative analgesic programs can effectively reduce perioperative stress and enhance postoperative recovery.The study aims to analyze the impact of different anesthesia modalities with multimodal analgesia on patients'postoperative pain.AIM To explore the effects of different anesthesia methods coupled with multi-mode analgesia on postoperative pain in patients with colorectal cancer.METHODS Following the inclusion criteria and exclusion criteria,a total of 126 patients with colorectal cancer admitted to our hospital from January 2020 to december 2022 were included,of which 63 received general anesthesia coupled with multi-mode labor pain and were set as the control group,and 63 received general anesthesia associated with epidural anesthesia coupled with multi-mode labor pain and were set as the research group.After data collection,the effects of postoperative analgesia,sedation,and recovery were compared.RESULTS Compared to the control group,the research group had shorter recovery times for orientation,extubation,eye-opening,and spontaneous respiration(P<0.05).The research group also showed lower Visual analog scale scores at 24 h and 48 h,higher Ramany scores at 6 h and 12 h,and improved cognitive function at 24 h,48 h,and 72 h(P<0.05).Additionally,interleukin-6 and interleukin-10 levels were significantly reduced at various time points in the research group compared to the control group(P<0.05).Levels of CD3+,CD4+,and CD4+/CD8+were also lower in the research group at multiple time points(P<0.05).CONCLUSION For patients with colorectal cancer,general anesthesia coupled with epidural anesthes

An integrated portable bio-monitoring system based on tough hydrogels for comprehensive detection of physiological activities

Advanced soft ion-conducting hydrogels have been developed rapidly in the integrated portable health monitoring equipment due to their higher sensitivity,sensory traits,tunable conductivity,and stretchability for physiological activities and personal healthcare detection.However,traditional hydrogel conductors are normally susceptible to large deformation and strong mechanical stress,which leads to inferior electro-mechanical stability for real application scenarios.Herein,a strong ionically conductive hydrogel(poly(vinyl alcohol)-boric acid-glycerol/sodium alginate-calcium chloride/electrolyte ions(PBG/SC/EI))was designed by engineering the covalently and ionically crosslinked networks followed by the salting-out effect to further enhance the mechanical strength and ionic conductivity of the hydrogel.Owing to the collective effects of the energy-dissipation mechanism and salting-out effect,the designed PBG/SC/EI with excellent structural integrity and robustness exhibits exceptional mechanical properties(elongation at break for 559.1%and tensile strength of 869.4 kPa)and high ionic conductivity(1.618 S·m^(-1)).As such,the PBG/SC/EI strain sensor features high sensitivity(gauge factor=2.29),which can effectively monitor various kinds of human motions(joint motions,facial micro-expression,faint respiration,and voice recognition).Meanwhile,the hydrogel-based Zn||MnO_(2)battery delivers a high capacity of 267.2 mAh·g^(-1)and a maximal energy density of 356.8 Wh·kg^(-1)associated with good cycle performance of 71.8%capacity retention after 8000 cycles.Additionally,an integrated bio-monitoring system with the sensor and Zn||MnO_(2)battery can accurately identify diverse physiological activities in a real-time and non-invasive way.This work presents a feasible strategy for designing high-performance conductive hydrogels for highly-reliable integrated bio-monitoring systems with excellent practicability.

SERPINH1 promoted the proliferation and metastasis of colorectal cancer by activating PI3K/Akt/mTOR signaling pathway

BACKGROUND Serpin peptidase inhibitor clade H member 1(SERPINH1)was initially recognized as an oncogene implicated in various human malignancies.Nevertheless,the clinical relevance and functional implications of SERPINH1 in colorectal cancer(CRC)remain largely elusive.AIM To investigate the effects of SERPINH1 on CRC cells and its specific mechanism.METHODS Quantitative real-time polymerase chain reaction,western blotting analysis,The Cancer Genome Atlas data mining and immunohistochemistry were employed to examine SERPINH1 expression in CRC cell lines and tissues.A series of in-vitro assays were performed to demonstrate the function of SERPINH1 and its possible mechanisms in CRC.RESULTS SERPINH1 demonstrated elevated expression levels in both CRC cells and tissues,manifested at both mRNA and protein tiers.Elevated SERPINH1 levels correlated closely with advanced T stage,lymph node involvement,and distant metastasis,exhibiting a significant association with poorer overall survival among CRC patients.Subsequent investigations unveiled that SERPINH1 overexpression notably bolstered CRC cell proliferation,invasion,and migration in vitro,while conversely,SERPINH1 knockdown elicited the opposite effects.Gene set enrichment analysis underscored a correlation between SERPINH1 upregulation and genes associated with cell cycle regulation.Our findings underscored the capacity of heightened SERPINH1 levels to expedite G1/S phase cell cycle progression via phosphatidylinositol 3-kinase/AKT/mechanistic target of rapamycin pathway activation,thereby facilitating CRC cell invasion and migration.CONCLUSION These findings imply a crucial involvement of SERPINH1 in the advancement and escalation of CRC,potentially positioning it as a novel candidate for prognostic assessment and therapeutic intervention in CRC management.

Gene cloning,protein expression,and enzymatic characterization of a double-stranded RNA degrading enzyme in Apolygus lucorum

RNA interference(RNAi)is a powerful tool that post-transcriptionally silences target genes in eukaryotic cells.However,silencing efficacy varies greatly among different insect species.Recently,we met with little success when attempting to knock down genes in the mirid bug Apolygus lucorum via dsRNA injection.The disappearance of double-stranded RNA(dsRNA)could be a potential factor that restricts RNAi efficiency.Here,we found that dsRNA can be degraded in midgut fluids,and a dsRNase of A.lucorum(AldsRNase)was identified and characterized.Sequence alignment indicated that its 6 key amino acid residues and the Mg2+-binding site were similar to those of other insects’dsRNases.The signal peptide and endonuclease non-specific domain shared high sequence identity with the brown-winged green stinkbug Plautia stali dsRNase.AldsRNase showed high salivary gland and midgut expression and was continuously expressed through the whole life cycle,with peaks at the 4th instar ecdysis in the whole body.The purified AldsRNase protein obtained by heterologously expressed can rapidly degrade dsRNA.When comparing the substrate specificity of AldsRNase,3 specific substrates(dsRNA,small interfering RNA,and dsDNA)were all degraded,and the most efficient degradation is dsRNA.Subsequently,immunofluorescence revealed that AldsRNase was expressed in the cytoplasm of midgut cells.Through cloning and functional study of AldsRNase,the enzyme activity and substrate specificity of the recombinant protein,as well as the subcellular localization of nuclease,the reason for the disappearance of dsRNA was explained,which was useful in improving RNAi efficiency in A.lucorum and related species.

Taiji-TianQin-LISA network:Precisely measuring the Hubble constant using both bright and dark sirens

In the coming decades,the space-based gravitational-wave(GW)detectors such as Taiji,TianQin,and LISA are expected to form a network capable of detecting millihertz GWs emitted by the mergers of massive black hole binaries(MBHBs).In this work,we investigate the potential of GW standard sirens from the Taiji-TianQin-LISA network in constraining cosmological parameters.For the optimistic scenario in which electromagnetic(EM)counterparts can be detected,we predict the number of detectable bright sirens based on three different MBHB population models,i.e.,popⅢ,Q3d,and Q3nod.Our results show that the TaijiTianQin-LISA network alone could achieve a constraint precision of 0.9%for the Hubble constant,meeting the standard of precision cosmology.Moreover,the Taiji-TianQin-LISA network could effectively break the cosmological parameter degeneracies generated by the CMB data,particularly in the dynamical dark energy models.When combined with the CMB data,the joint CMB+Taiji-TianQin-LISA data offerσ(w)=0.036 in the wCDM model,which is close to the latest constraint result obtained from the CMB+SN data.We also consider a conservative scenario in which EM counterparts are not available.Due to the precise sky localizations of MBHBs by the Taiji-TianQin-LISA network,the constraint precision of the Hubble constant is expected to reach 1.2%.In conclusion,the GW standard sirens from the Taiji-TianQin-LISA network will play a critical role in helping solve the Hubble tension and shedding light on the nature of dark energy.

Enhanced plasticity of spontaneous coagulation cast oxide ceramic green bodies

In the preparation of large-sized ceramics,the use of a green body with relatively high plasticity is crucial to minimize the risk of cracking during processing.To achieve this goal,glycerol and polyethylene glycol(PEG)were utilized as plasticizers in the shaping of green bodies of oxide ceramics through spontaneous coagulation casting(SCC).This study investigated the effects of plasticizers and particle sizes ranging from the submicron to nanoscale on the slurry viscosity,drying shrinkage of wet gels,and mechanical properties of green bodies.The plasticity of the green bodies was assessed by measuring the impact toughness and flexural stress‒strain curves.By incorporating an appropriate plasticizer,the peak width of the flexural stress‒strain curve for dried green bodies from particles of different sizes was nearly twice that without plasticizers,and the impact toughness was enhanced by approximately 71%,34%,and 41%when the particle size decreased from the submicron scale to the nanoscale(0.45μm,0.18μm,and 50 nm,respectively).The drilling test revealed that there was nearly no cracking around the holes in the green bodies with plasticizers.The plasticity mechanism of the green bodies was examined based on ultraviolet–visible(UV–Vis)spectroscopy and scanning electron microscopy(SEM).It was discovered that plasticizers might mitigate the brittleness of green bodies by adjusting the interactions between molecules and modifying the gel network properly.

Alpine grassland degradation intensifies the burrowing behavior of small mammals:evidence for a negative feedback loop

Globally,grassland degradation is an acute ecological problem.In alpine grassland on the Tibetan Plateau,increased densities of various small mammals in degraded grassland are assumed to intensify the degradation process and these mammals are subject to lethal control.However,whether the negative impact of small mammals is solely a result of population size or also a result of activity and behavior has not been tested.In this study,we use plateau pika as a model to compare population size,core area of colony,and the number of burrow entrances and latrines between lightly and severely degraded grassland.We test whether the alleged contribution of pika to grassland degradation is a result of increased population size or increased burrowing activities of individuals in response to lower food abundance.We found that grassland degradation resulted in lower plant species richness,plant height,and biomass.Furthermore,the overall population size of pika was not significantly affected by location in lightly and severely degraded grassland.However,pika core areas in severely grassland degradation were significantly larger and had significantly higher densities of burrows and latrines.Our study provides convincing evidence that habitat-induced changes in the behavior of small,burrowing mammals,such as pika,can exacerbate grassland degradation.This finding has significant implications for managing small mammals and restoring degraded grassland ecosystems.
