Predictors for difficult cecal insertion in colonoscopy: The impact of obesity indices

AIM To identify the factors influencing cecal insertion time(CIT) and to evaluate the effect of obesity indices on CIT. METHODS We retrospectively reviewed the data for participants who received both colonoscopy and abdominal computed tomography(CT) from February 2008 to May 2008 as part of a comprehensive health screening program. Age, gender, obesity indices [body mass index(BMI), waist-to-hip circumference ratio(WHR), waist circumference(WC), visceral adipose tissue(VAT)volume and subcutaneous adipose tissue(SAT) volume on abdominal CT], history of prior abdominal surgery, constipation, experience of the colonoscopist, quality of bowel preparation, diverticulosis and time required to reach the cecum were analyzed. CIT was categorized as longer than 10 min(prolonged CIT) and shorter than or equal to 10 min, and then the factors that required a CIT longer than 10 min were *** A total of 1678 participants were enrolled. The mean age was 50.42 ± 9.931 years and 60.3% were men. The mean BMI, WHR, WC, VAT volume and SAT volume were 23.92 ± 2.964 kg/m2, 0.90 ± 0.076, 86.95 ± 8.030 cm, 905.29 ± 475.220 cm3 and 1707.72 ± 576.550 cm3, respectively. The number of patients who underwent abdominal surgery was 268(16.0%). Colonoscopy was performed by an attending physician alone in 61.9% of cases and with the involvement of a fellow in 38.1% of cases. The median CIT was 7 min(range 2-56 min, IQR 5-10 min), and mean CIT was 8.58 ± 5.291 min. Being female, BMI, VAT volume and involvement of fellow were significantly associated with a prolonged CIT in univariable analysis. In multivariable analysis, being female(OR = 1.29, P = 0.047), lower BMI(< 23 kg/m2)(OR = 1.62, P = 0.004) or higher BMI(≥ 25 kg/m2)(OR = 1.80, P < 0.001), low VAT volume(< 500 cm3)(OR = 1.50, P = 0.013) and fellow involvement(OR = 1.73, P < 0.001) were significant predictors of prolonged CIT. Applications of a novel curve-fitting technique are presented to efficiently predict the motion of the vortex filament, which is trailed from a rigid body such as wings and rotors. The gov- erning equations of the motion, when a Lagrangian approach with the present curve-fitting method is applied, can be transformed into an easily solvable form of the system of nonlinear ordinary dif- ferential equations. The applicability of Bezier curves, B-spline, and Lagrange interpolating polyno- mials is investigated. Local Lagrange interpolating polynomials with a shift operator are proposed as the best selection for applications, since it provides superior system characteristics with minimum computing time, compared to other methods. In addition, the Gauss quadrature formula with local refinement strategy has been developed for an accurate prediction of the induced velocity computed with the line integration of the Biot-Savart law. Rotary-wing problems including a vortex ring problem are analyzed to show the efficiency, accuracy, and flexibility in the applications of the pro- posed method.

Effects of PWHT on Microstructure and Mechanical Properties of Weld Metals of Ni-Based Superalloy 617 and 263 for Hyper-Supercritical Power Plants

It is well known that solid solution-strengthened alloy 617 and γ′ precipitation-strengthened alloy 263 have excellent mechanical properties and corrosion resistance at high temperatures. Hyper-supercritical power plants work at temperatures above 700 ℃, and these superalloys are considered candidate materials for steam turbines components of these power plants. In this study, gas tungsten arc weldability of these superalloys was evaluated, and the effects of post- weld heat treatment （PWHT） on the microstructural characteristics and the mechanical properties of their weld metals were investigated. Scanning transmission electron microscopy, energy-dispersive spectroscopy and electron probe microanalysis were utilized for the investigation. The experimental results confirmed that these weld metals had different characteristics in microstructure and mechanical properties. PWHT resulted in the precipitation of intergranular carbides, γ′ particles and an increase in tensile strength of these superalloy weld metals. Furthermore, fine γ′ particles, which were not detected in the as-welded metal of alloy 263, were precipitated after PWHT and those particles were the reason for the drastic increase in tensile strength.

Users, especially the non-expert users, commonly experience problems when connecting multiple devices with interoperability. While studies on multiple device connections are mostly concentrated on spontaneous device association techniques with a focus on security aspects, the research on user interaction for device connection is still limited. More research into understanding people is needed for designers to devise usable techniques. This research applies the Research-through-Design method and studies the non-expert users＇ interactions in establishing wireless connections between devices. The ＂Learning from Examples＂ concept is adopted to develop a study focus line by learning from the expert users＇ interaction with devices. This focus line is then used for guiding researchers to explore the non-expert users＇ difficulties at each stage of the focus line. Finally, the Research-through-Design approach is used to understand the users＇ difficulties, gain insights to design problems and suggest usable solutions. When connecting a device, the user is required to manage not only the device＇s functionality but also the interaction between devices. Based on learning from failures, an important insight is found that the existing design approach to improve single-device interaction issues, such as improvements to graphical user interfaces or computer guidance, cannot help users to handle problems between multiple devices. This study finally proposes a desirable user-device interaction in which images of two devices function together with a system image to provide the user with feedback on the status of the connection, which allows them to infer any required actions.

Impact of nonalcoholic fatty liver disease on the risk of gallbladder polyps in lean and non-obese individuals:A cohort study

Background:The association between non-obese or lean nonalcoholic fatty liver disease(NAFLD)and gallbladder polyps(GBPs)has not yet been *** aimed to determine whether NAFLD is an independent risk factor for the development of GBPs,even in non-obese and lean ***:We analyzed a cohort of 331208 asymptomatic adults who underwent abdominal ultrasonography(US).The risk of GBP development was evaluated according to the obesity and NAFLD ***:The overall prevalence of NAFLD and GBPs≥5 mm was 28.5%and 2.9%,*** prevalence of NAFLD among 160276 lean,77676 overweight and 93256 obese participants was 8.2%,31.2%,and 61.1%,*** with NAFLD had a significantly higher incidence of GBPs with a size of≥5 mm[adjusted odds ratio(OR)=1.18;95%confidence interval(CI):1.11–1.25].A higher body mass index and its categories were also significantly associated with an increased risk of GBPs≥5 ***,risk of GBPs≥5 mm was significantly increased even in NAFLD individuals who are not obese(lean:adjusted OR=1.36,95%CI:1.19-1.54;overweight:adjusted OR=1.14,95%CI:1.03–1.26,respectively).Conclusions:Non-obese/lean NAFLD is an independent risk factor for GBP development,suggesting that NAFLD may play an important role in the pathogenesis of GBPs regardless of the obesity ***,a more thorough evaluation for GBPs may be necessary when hepatic steatosis is detected on abdominal US,even in non-obese or lean individuals.

This study investigated the impact of multiple-Doppler radar data and surface data assimilation on forecasts of heavy rainfall over the central Korean Peninsula;the Weather Research and Forecasting(WRF) model and its three-dimensional variational data assimilation system(3DVAR) were used for this purpose. During data assimilation,the WRF 3DVAR cycling mode with incremental analysis updates(IAU) was used. A maximum rainfall of 335.0 mm occurred during a 12-h period from 2100 UTC 11 July 2006 to 0900 UTC 12 July *** radar data showed that the heavy rainfall was due to the back-building formation of mesoscale convective systems(MCSs).New convective cells were continuously formed in the upstream region,which was characterized by a strong southwesterly low-level jet(LLJ).The LLJ also facilitated strong convergence due to horizontal wind shear,which resulted in maintenance of the *** assimilation of both multiple-Doppler radar and surface data improved the accuracy of precipitation forecasts and had a more positive impact on quantitative forecasting(QPF) than the assimilation of either radar data or surface data *** back-building characteristic was successfully forecasted when the multiple-Doppler radar data and surface data were *** data assimilation experiments,the radar data helped forecast the development of convective storms responsible for heavy rainfall,and the surface data contributed to the occurrence of intensified low-level *** surface data played a significant role in enhancing the thermal gradient and modulating the planetary boundary layer of the model,which resulted in favorable conditions for convection.

Carbon super-heterostructures with high nitrogen contents from the covalent hybrid precursors of covalent triazine frameworks(CTFs)and zeolitic imidazolic frameworks(ZIFs)are scarcely explored because of CTF's ordered structure and toxic superacid that dissolves or destabilizes the metal *** solve this problem,herein,we report a straightforward two-step pathway for the covalent hybridization of disordered CTF(d–CTF)–ZIF composites via preincorporation of an imidazole(IM)linker into ordered CTFs,followed by the imidazole-site-specific covalent growth of *** carbonization of these synthesized d–CTF−IM−ZIF hybrids results in unique hollow carbon super-heterostructures with ultrahigh nitrogen content(>18.6%),high specific surface area(1663m^(2)g^(−1)),and beneficial trace metal(Co/Zn NPs)contents for promoting the redox *** proof of concept,the obtained carbon super-heterostructure(Co–Zn–NC_(SNH)–800)is used as a positive electrode in an asymmetric supercapacitor,demonstrating a remarkable energy density of 61Wh kg^(−1)and extraordinary cyclic stability of 97%retention after 30,000 cycles at the cell *** presynthetic modifications of CTF and their covalent hybridization with ZIF crystals pave the way toward new design strategies for synthesizing functional porous carbon materials for promising energy applications.
