Obesity is an important determinant of severity in newly defined metabolic dysfunction-associated fatty liver disease

Background: The recently proposed definition of metabolic dysfunction-associated fatty liver disease(MAFLD) is based on the co-existence of hepatic steatosis with other metabolic disorders, including obesity and metabolic risk abnormalities such as hyperglycemia, high blood pressure and dyslipidemia. This study aimed to assess MAFLD severity according to the presence of metabolic abnormalities and obesity. Methods: Using transient elastography, hepatic steatosis and fibrosis severity were assessed by measuring the controlled attenuation parameter and liver stiffness measurement. A total of 1163 patients with MAFLD were categorized into the following four groups according to metabolic risk abnormalities and obesity presence: non-obese without metabolic risk abnormality group(Group 1;reference group);nonobese with metabolic risk abnormality group(Group 2);obese without metabolic risk abnormality group(Group 3);and obese with metabolic risk abnormality group(Group 4). A multiple logistic regression analysis was performed to determine severe hepatic steatosis and fibrosis risk in each group in both unadjusted and adjusted models. Results: In the adjusted model, the odds ratios(ORs) [95% confidence interval(CI)] for severe hepatic steatosis in Groups 2, 3, and 4 were 1.07(0.61-1.88), 2.43(1.44-4.08), and 4.07(2.56-6.48), respectively( P trend < 0.001). For liver fibrosis, compared with Group 1, Group 2 showed no significant increases in OR, whereas Groups 3 and 4(obese groups) showed significant increases(OR = 4.70, 95% CI: 1.24-17.82 and OR = 6.43, 95% CI: 1.88-22.02, respectively). Conclusions: Obesity, rather than metabolic abnormality, is the principal determinant of severe hepatic steatosis and fibrosis in patients with MAFLD.

Cholecystectomy increases the risk of metabolic syndrome in the Korean population:a longitudinal cohort study

Background:Cholecystectomy is a common surgical procedure to treat symptomatic gallstones;however,the long-term outcomes after cholecystectomy are unknown.Therefore,we aimed to investigate whether incident metabolic syndrome(MetS)is associated with cholecystectomy through a large,population-based,longitudinal study.Methods:Subjects aged≥20 years who underwent cholecystectomy from 2010 to 2014(n=76,485)and controls(n=76,485),matched for age and sex,were identified from the Korean National Health Insurance Corporation.Cox proportional hazards analyses were performed to evaluate the association between cases and incident MetS,and hazard ratios and 95% confidence intervals(CIs)were calculated.Results:A total of 152,970 patients were included.Mean age was 52.47±12.76 years,and 50.65% of participants were male.During the follow-up period,there were 38,979(25.48%)newly diagnosed MetS cases in the study participants.The risk of MetS in the cholecystectomy group was approximately 20% higher than that in the control group[adjusted odds ratio(OR),1.20;95%CI:1.17-1.23].In the fully adjusted models,the corresponding ORs for new-onset high waist circumference(WC),low high-density lipoprotein cholesterol(HDL-C)levels,high triglycerides(TG)levels,high blood pressure(BP),and high blood glucose levels were 1.16(1.13-1.19),1.19(1.16-1.22),1.25(1.22-1.28),1.27(1.23-1.31),and 1.21(1.18-1.24),respectively.Cholecystectomy was an independent risk factor of incident MetS,after adjusting for potential confounding factors.In the subgroup analyses,the cholecystectomy group had a higher risk of MetS than the control group in subjects without hypertension or dyslipidemia,respectively.Conclusions:In this large,population-based study,cholecystectomy was associated with an increased risk of developing MetS,independent of other confounding factors.Therefore,careful monitoring of metabolic variables and long-term follow-up are required to evaluate MetS risk after cholecystectomy.

Health risks of pest control and disinfection workers after the COVID-19 outbreak in South Korea

The exposure patterns of pest control and disinfection workers have changed after the coronavirus disease 2019(COVID-19)outbreak,but the health risks of chemical exposure have not been assessed.We identified these workers’chemical exposure patterns and risks before and after the COVID-19 outbreak.We used data conducted between 2018(pre-pandemic)and 2021(post-pandemic)from three-year cross-sectional surveys on pest control and disinfection workers.Inhalation and dermal exposure concentrations were estimated using equations based on a biocidal product risk assessment model from the Korean National Institute of Environmental Research.The non-carcinogenic and carcinogenic risks of chemicals were calculated using the United States Environmental Protection Agency risk assessment model.We found that the annual work frequency(50^(th)percentile)of foggers using disinfectants increased the most among all the work types,from 140 uses/year to 176 uses/year after the COVID-19 outbreak.Moreover,all chemicals’non-carcinogenic and carcinogenic risks increased regardless of exposure routes.In the worst scenario(95^(th)percentile),the margin of exposure for citric acid,benzethonium chloride,benzyl-C12-16-alkyldimethyl chlorides,and sodium chlorite of inhalation exposure,and isopropyl alcohol and benzyl-C12-16-alkyldimethyl chlorides of dermal exposure were acceptable(>100)before the COVID-19outbreak but became unacceptable(10^(-6))before and after the COVID-19 outbreak but comparatively high after the COVID-19 outbreak.Additionally,significantly more workers experienced health symptoms after the COVID-19outbreak(p<0.05),with the most common being muscle lethargy(31%),skin/face stinging(28.7%),and breath shortness/neck pain(24.1%).

U.S.Preservice Teachers Learning With Multilingual Learners in Korea Through Educational Technology:Bringing Heteroglossic and Global Approaches to TESOL Teacher Education

Purpose This study aimed to explore how educational technology influenced a preparatory teacher education program using heteroglossic and global approaches.Design/Approach/Methods The researchers drew upon the theoretical framework of multilingual digital storytelling(MDST),which emphasizes the intercultural awareness attributes of multilingual learners(MLs)and takes a heteroglossic perspective in linguistic pedagogy.This qualitative case study examined the experiences of 11 U.S.-based preservice teachers(PSTs)and 12 MLs elementary students in Korea in the MDST project of a TESOL methods course.Findings The findings showed that PSTs and MLs enhanced their appreciation for educational technology,multilingualism,and intercultural awareness.Although the project aimed to decenter English as a hegemonic language,both PSTs and MLs maintained traditional discourses that privileged English over MLs’home language and targeted literacy correctness in written English only,moving away from the heterogeneous goals of the course project.PSTs and MLs also faced challenges in navigating technological tools,which negatively affected their perception of the project.Originality/Value This study contributes to heteroglossic approaches in preparatory TESOL teacher education programs and improves the understanding of challenges in educational technology use for global multilingual exchanges to promote global citizenship.

Dear editor,Severe acute respiratory syndrome coronavirus2(SARS-CoV-2)infection can lead to severe outcomes in patients with cancer[1].It has been reported that patients with lung cancers disproportionately manifest severe COVID-19 with a high rate of hospitalization and death[2].Notably,the SARS-CoV-2 Spike(S)protein can induce hyper-inflammation in both epithelial cells and macrophages through toll-like receptor(TLR)1/TLR2 or TLR2/6-dependent nuclear factor-kappaB(NF-κB)path-way[3].However,molecular and cellular evidence on whether the SARS-CoV-2 virus affects the severity of lung cancer patients through TLR1/2 or TLR2/6 signaling remains unclear.

Capsosiphon fulvescens suppresses LPS-stimulated inflammatory responses by suppressing TLR4/NF-κB activation in RAW264.7 murine macrophages

Objective:To evaluate the effects of Capsosiphon fulvescens(C.fulvescens)ethanolic extract on inflammation in lipopolysaccharide(LPS)-induced RAW296.7 macrophages.Methods:The protective effects of C.fulvescens ethanolic extract on LPS-induced inflammation in RAW264.7 macrophages were assessed using biochemical analysis,including enzyme-linked immunosorbent assay,quantitative reverse transcription-polymerase chain reaction,and Western blot analysis.To examine reactive oxygen species(ROS)production,flow cytometry analysis,and immunofluorescence staining were used.Furthermore,the modulatory effect of C.fulvescens ethanolic extract on NF-κB activation was investigated.Results:C.fulvescens ethanolic extract significantly attenuated LPS-induced levels of pro-inflammatory cytokines and notably reduced the secretion and mRNA levels of LPS-mediated matrix metalloproteinases.In addition,C.fulvescens ethanolic extract decreased ROS production and suppressed the TLR4/NF-κB signaling pathway.Conclusions:C.fulvescens ethanolic extract alleviates inflammation as well as oxidative stress by modulating the TLR4/NF-κB signaling in LPS-induced RAW264.7 macrophages.C.fulvescens can be used as a potential therapeutic agent to suppress inflammation and oxidative stress-associated diseases.

Subcarrier BD with Cooperative Communication for MIMO-NOMA System

With the rapid evolution of Internet of things(IoT),many edge devices require simultaneous connection in 5G communication era.To afford massive data of IoT devices,multiple input multiple output non-orthogonal multiple access(MIMO-NOMA)method has been considered as a promising technology.However,there are numerous drawbacks due to error propagation and inter-user interferences.Therefore,proposed scheme aims to improve the reliability of the MIMO-NOMA system with digital beamforming and intracluster cooperative multi point(CoMP)to efficiently support IoT system.In the conventional MIMO-NOMA system,user entities are grouped into clusters.Block diagonalization(BD)is applied to efficiently eliminate the inter-cluster interference of the MIMO-NOMA system.However,since the channel path of the data stream from a single antenna to a single cluster doesn’t hold other cluster’s data,the system can’t fully utilize the selective subcarrier channel states.It indicates that there can be better channel paths for a data stream at a certain subcarrier index.Therefore,proposed scheme allocates data streams to antennas adaptively considering selective channel states.Additionally,intra-cluster CoMP method is adjusted to enhance the reliability of the system in the clusters.The simulation results show that the proposed scheme improves BER and throughput performance compared to the conventional MIMO-NOMA system.

Recurrent intramuscular lipoma at extensor pollicis brevis:A case report

BACKGROUND This report describes and discusses recurrent intramuscular lipoma(IML)of the extensor pollicis brevis(EPB).An IML usually occurs in a large muscle of the limb or torso.Recurrence of IML is rare.Recurrent IMLs,especially those with unclear boundaries,necessitate complete excision.Several cases of IML in the hand have been reported.However,recurrent IML appearing along the muscle and tendon of EPB on wrist and forearm has not been reported yet.CASE SUMMARY In this report,the authors describe clinical and histopathological features of recurrent IML at EPB.A 42-year-old Asian woman presented with a slowgrowing lump in her right forearm and wrist area six months ago.The patient had a history of surgery for a lipoma of the right forearm one year ago with a scar of 6cm on the right forearm.magnetic resonance imaging confirmed that the lipomatous mass,which had attenuation similar to subcutaneous fat,had invaded the muscle layer of EPB.Excision and biopsy were performed under general anesthesia.On histological examination,it was identified as an IML showing mature adipocytes and skeletal muscle fibers.Therefore,surgery was terminated without further resection.No recurrence occurred during a follow-up of five years after surgery.CONCLUSION Recurrent IML in the wrist must be examined to differentiate it from sarcoma.Damage to surrounding tissues should be minimized during excision.

Automatic detection of teeth and dental treatment patterns on dental panoramic radiographs using deep neural networks

Disaster victim identification issues are especially critical and urgent after a large-scale disaster.The aim of this study was to suggest an automatic detection of natural teeth and dental treatment patterns based on dental panoramic radiographs(DPRs)using deep learning to promote its applicability as human identifiers.A total of 1638 DPRs,of which the chronological age ranged from 20 to 49 years old,were collected from January 2000 to November 2020.This dataset consisted of natural teeth,prostheses,teeth with root canal treatment,and implants.The detection of natural teeth and dental treatment patterns including the identification of teeth number was done with a pre-trained object detection network which was a convolutional neural network modified by EfficientDet-D3.The objective metrics for the average precision were 99.1%for natural teeth,80.6%for prostheses,81.2%for treated root canals,and 96.8%for implants,respectively.The values for the average recall were 99.6%,84.3%,89.2%,and 98.1%,in the same order,respectively.This study showed outstanding performance of convolutional neural network using dental panoramic radiographs in automatically identifying teeth number and detecting natural teeth,prostheses,treated root canals,and implants.

Long-term radiofrequency electromagnetic fields exposure attenuates cognitive dysfunction in 5×FAD mice by regulating microglial function

We have previously found that long-term effects of exposure to radiofrequency electromagnetic fields in 5xFAD mice with severe late-stage Alzheimer’s disease reduced both amyloid-βdeposition and glial activation,including microglia.To examine whether this therapeutic effect is due to the regulation of activated microglia,we analyzed mic roglial gene expression profiles and the existence of microglia in the brain in this study.5xFAD mice at the age of 1.5 months were assigned to sham-and radiofrequency electromagnetic fields-exposed groups and then animals were exposed to 1950 MHz radiofrequency electromagnetic fields at a specific absorption rate of 5 W/kg for 2 hours/day and 5 days/week for 6 months.We conducted behavioral tests including the object recognition and Y-maze tests and molecular and histopathological analysis of amyloid precursor protein/a myloid-beta metabolism in brain tissue.We confirmed that radiofrequency electromagnetic field exposure for 6 months ameliorated cognitive impairment and amyloid-βdeposition.The expression levels of Iba1(pan-microglial marker)and colony-stimulating factor 1 receptor(CSF1R;regulates microglial prolife ration)in the hippocampus in 5xFAD mice treated with radiofrequency electromagnetic fields were significantly reduced compared with those of the sham-exposed group.Subsequently,we analyzed the expression levels of genes related to mic rogliosis and microglial function in the radiofrequency electromagnetic fields-exposed group compared to those of a CSF1R inhibitor(PLX3397)-treated group.Both radiofrequency electromagnetic fields and PLX3397 suppressed the levels of genes related to microgliosis(Csf1r,CD68,and Ccl6)and pro-inflammatory cytokine interleukin-1β.N otably,the expression levels of genes related to mic roglial function,including Trem2,Fcgr1α,Ctss,and Spi1,were decreased after long-term radiofrequency electromagnetic field exposure,which was also observed in response to microglial suppression by PLX3397.These results showed that radiofrequency electromagnetic
