Short-term outcomes after the loading phase of intravitreal bevacizumab and subthreshold macular laser in non-center involved diabetic macular edema

AIM： To compare the effectiveness of intravitreal bevacizumab and subthreshold macular photocoagulation （SMP）, for the treatment of non-center involved diabetic macular edema （non-CI DME）. METhODS： Prospective, randomized, controlled clinical study included patients with type 2 diabetes, non-CI DME and best-corrected visual acuity （BCVA） of 0.30 logMAR or better. Each eye was randomized into three groups： group 1, monthly intravitreal bevacizumab; group 2, single SMP; group 3, single SMP and monthly bevacizumab. Main outcome measures were BCVA, and macular thickness measured with optical coherence tomography as macular central subfield thickness （CST）, macular area of greater thickness （MAGT） and total macular volume （TMV）. Results were analyzed after 3mo. RESULTS： A total of 32 eyes were included. Group 3 improved in BCVA （0.19±0.16 to 0.12±0.14 logMAR; P=0.041） and in TMV （7.90±0.57 to 7.65±0.73 mm3; P=0.025）. Group 1 improved in MAGT （325±26.26 to 298.20±44.85 μm; P=0.022） and TMV （7.79±0.57 to 7.50±0.56 mm3, P=0.047）. Group 2 didn’t show significant improvement of any variable. CONCLUSION： The loading phase of bevacizumab as monotherapy or combined with SMP is superior to SMP as monotherapy in providing short-term visual and anatomical improvement in non-CI DME.

AIM:To determine whether or not the use of antioxidant supplementation aids in the prevention of post-endoscopic retrograde cholangiopancreatography ***:A systematic review of randomized controlled trials(RCTs) was made to evaluate the preventive effect of prophylactic antioxidant supplementation in post-endoscopic retrograde cholangiopancreatography pancreatitis(PEP).The inclusion criteria included:acute post-endoscopic retrograde cholangiopancreatography pancreatitis in adults; randomized clinical trials with the use of any antioxidant as an intervention compared with placebo,to reduce *** outcome measure was the incidence and severity of *** RCTs involving 3110 patients since 1999 *** antioxidants used were selenite,β-carotene,and pentoxifylline(each one in one trial),N-acetylcysteine(NAC) in three trials,and allopurinol in six *** group of patients treated with NAC received different doses; either oral or intravenous,and allopurinol-treated patients received five different oral doses in two different administration *** results are expressed with raw numbers,proportions,as well as mean and standard *** incidence of pancreatitis between groups was analyzed with Pearson's χ2 test or Fisher's exact test(F).The main outcome is expressed as relative risks and 95%***:The incidence of pancreatitis in all antioxidant treatment groups was 8.6%,whereas it was 9.7% in the control *** antioxidants used were selenite,β-carotene,and pentoxifylline(each one in one trial),NAC in three trials,and allopurinol in six *** allopurinol trials,three different dosifications were used; two trials reported a low dosage(of less than 400 mg),two trials reported a moderate dose(600 mg) and the remaining two employed higher doses(more than 900 mg).Supplementation was not associated with a significant reduction in the incidence of PEP [relative risk(RR) = 0.93; 95%CI:0.82-1.06; P = 0.28].In addition,the incid

BACKGROUND: Low-frequency and high-risk situations, such as neonatal resuscitation, are the ideal targets for simulation-based learning. The aim of this paper is to present the structure of our internal neonatal resuscitation training program, using a realistic, simulated delivery room, and to present the participants' opinions about teamwork, emotional stress, and their subjective ability to face a resuscitation. METhODS: We administered a training course to 24 doctors and midwives. One of the simulation classrooms was modified to appear similar to a real delivery room. Four scenarios were conducted using a previously designed checklist of primary and secondary goals. Upon completion, all students participated in a debriefing session with the help of a video review. RESULTS: Students rated the achievement of their previously defined goals on a scale of 1 to 5. Grouping together the percentages of the highest ratings (Categories 4 and 5), 83.4% (20/24) of the students considered the course useful for acquiring clinical skills. For 87.5% (21/24) of the students, the scenarios simulated real clinical situations, the room properly simulated a real delivery room, and the course improved the students' ability to work in a team. For 66.6% (16/24) of the students, the course improved their stress in confronting neonatal resuscitation. Initially, only 33.3% (8/24) of the students considered themselves very capable or fully able to cope with a resuscitation. After the course, that percentage rose to 62.5% (15/24). CONCLUSIONS: The incorporation of simulation-based learning into neonatal resuscitation teaching programs, using realistic scenarios, is useful and offers the possibility of acquiring technical skills, but it also allows for the improvement of teamwork and the adoption of different roles and positive attitudes towards emotional stress.

This study presents the simulation of a MFC with Pseudomonas aeruginosa based on a metabolic flux analysis (MFA) which arises as a linear programming model that served as input for the fuel cell model. The linear model was implemented in Xpress MP? and the coupling model in Comsol Multiphysics?. The in silico model predicted maximum potentials of 0.135 V which were consistent with those obtained in the experimental cell. Afterwards, an optimization platform based on multiobjective optimization approach was implemented aimed to determine genes that increase the average cell power. cysA, cysP and rpoS mutants which were predicted to increase the power of the cell, were experimentally tested with an air cathode fuel cell finding an augment up to 35-fold in the average power density for the rpoS mutant. Power densities were obtained through experimentation in the range of [0.13 - 5] mW/m2. Electron shuttle rise was qualitative corroborated trough cyclic voltammetry tests, which allowed to visualize the augment of the peaks for rpoS mutation.

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.
