We report a case of ileo-colonic Histoplasmosis without apparent respiratory involvement in a patient who had previously undergone an orthotopic liver transplant(OLT) for primary biliary cholangitis 15 years earlier. The recipient lived in the United Kingdom,a non-endemic region for Histoplasmosis. However,she had previously lived in rural southern Africa prior to her OLT. The patient presented with iron deficiency anaemia,diarrhoea,abdominal pain and progressive weight loss. She reported no previous foreign travel,however,it later became known that following her OLT she had been on holiday to rural southern Africa. On investigation,a mild granulomatous colitis primarily affecting the right colon was identified,that initially improved with mesalazine. Her symptoms worsened after 18 mo with progressive ulceration of her distal small bowel and right colon. Mycobacterial,Yersinia,cytomegalovirus and human immunodeficiency virus infections were excluded and the patient was treated with prednisolone for a working diagnosis of Crohn's disease. Despite some early symptom improvement following steroids,there was subsequent deterioration with the patient developing gram-negative sepsis and multi-organ failure,leading to her death. Post-mortem examination revealed that her ileo-colonic inflammation was caused by Histoplasmosis.

Racial residential segregation in the United States is generally associated differences in health outcomes among Blacks and Whites due to differential exposures to physical, social and economic resources. While several studies have explored the association between segregation and several chronic conditions in the US, few have specifically examined diabetes using a nationally representative population-based sample. The current study relies on 2005 data from both the Behavioral Risk Factor Surveillance Survey (BRFSS) and the American Community Survey (ACS) to explore the association between segregation, socioeconomic status and diabetes. Using multilevel mixed-effects logistic regression, we present models that account for clustering of individuals within metropolitan areas and are adjusted for objective environmental measures (including segregation) and potential individual-level confounders (including education, employment, and income) among a sample of 121,321 adults who were at least 18 years old. After controlling for individual-level factors, Blacks residing in highly segregated areas have the same odds of being diagnosed with diabetes as Whites. Whites are more likely to be diagnosed with diabetes in areas where there are higher percentages of Blacks than in areas with low Black representation. Moreover, in this national sample, there is no statistical association between Blacks residing in highly segregated neighborhoods and diabetes risk. However, the increased prevalence of diabetes among Whites living in Black metropolitan areas suggests that future studies are needed to explore the linkages between levels of segregation and diabetes risk.

Incorporation of near-real-time hospital occupancy data to improve hospitalization forecast accuracy during the COVID19 pandemic

Public health decision makers rely on hospitalization forecasts to inform COVID-19 pandemic planning and resource *** forecasts are most relevant when they are accurate,made available quickly,and updated *** rapidly adapted an agent-based model(ABM)to provide weekly 30-day hospitalization forecasts(i.e.,demand for intensive care unit[ICU]beds and non-ICU beds)by state and region in North Carolina for public health decision *** ABM was based on a synthetic population of North Carolina residents and included movement of agents(i.e.,patients)among North Carolina hospitals,nursing homes,and the *** assigned SARSCoV-2 infection to agents using county-level compartmental models and determined agents’COVID-19 severity and probability of hospitalization using synthetic population characteristics(e.g.,age,comorbidities).We generated weekly 30-day hospitalization forecasts during MayeDecember 2020 and evaluated the impact of major model updates on statewide forecast accuracy under a SARS-CoV-2 effective reproduction number range of *** the 21 forecasts included in the assessment,the average mean absolute percentage error(MAPE)was 7.8%for non-ICU beds and 23.6%for ICU *** the major model updates,integration of near-real-time hospital occupancy data into the model had the largest impact on improving forecast accuracy,reducing the average MAPE for non-ICU beds from 6.6%to 3.9%and for ICU beds from 33.4%to 6.5%.Our results suggest that future pandemic hospitalization forecasting efforts should prioritize early inclusion of hospital occupancy data to maximize accuracy.
