Novel non-invasive score to predict cirrhosis in the era of hepatitis c elimination: A population study of ex-substance users in Singapore

Background: chronic hepatitis c infection is common among people with history of substance use. Liver fibrosis assessment is a barrier to linkage to care, particularly among those with history of substance users. The use of non-invasive scores can be helpful in predicting liver cirrhosis in the era of HcV elimination, especially in countries where transient elastography(TE) is not available. We compared the commonly used non-invasive scores with a novel non-invasive score in predicting liver cirrhosis in this population. Methods: HcV patients with history of substance use between 2011 and 2016 were analyzed. All patients had TE for liver fibrosis assessment. clinical performance of established non-invasive scores for fibrosis assessment and novel score were compared. Youden's index was used to determine optimal cut-off of the novel score. Results: A total of 579 patients were included. In multivariate logistic regression, cirrhosis on TE was associated with age( P = 0.002), aspartate aminotransferase(AST)( P = 0.004), and platelet count( P components of modified AST-toplatelet ratio index(APRI) score. Modified APRI was superior to APRI in predicting cirrhosis(AUROc, 0.796 vs. 0.770, P = 0.007), but not fibrosis-4 score(FIB-4)( P = 1.00). Modified APRI at cut-off of 4 has sensitivity, specificity and negative predictive value(NPV) of 94.4%, 26.9% and 92.6%, respectively, and at 19, has sensitivity, specificity and positive predictive value(PPV) of 33.3%, 96.2% and 77.1%, respectively. FIB-4 has a NPV and PPV of 88.6%, 41.8% and 78.5%, 77.6%, at cut-off of 1.45 and 3.25, respectively. Using the cut-off of 4 and 14 for modified APRI, 32.5% of patients can be correctly classified and misses out only 5.6% of cirrhosis patients. conclusions: Modified APRI score is superior in predicting cirrhosis in HcV population, with 32.5% of the population being correctly classified using cut-off of 4 and 14.

The metanalysis by Tantai and collaborators highlights the role of sarcopenia in cirrhosis by demonstrating that the presence of muscular alterations is independently related with a mortality(HR 2.61)(1).Sarcopenia is confirmed as a condition with high prevalence in patients with cirrhosis(37.5%),being more representative in males than females(41.9%vs.37.8%)and in subjects with alcoholic etiology(49.6%)or with more advanced degree of liver disease(child Pugh c 46.7%).Interestingly,authors demonstrate that sarcopenia was associated with mortality in subjects with MELD15(HR 2.34)as well evaluating only on studies that excluded patients with hepatocellular carcinoma(Hcc),risk of mortality was similar to that of the main analysis(2.35)(1).

The entomopathogenic fungus Isaria fumosorosea Wize, formerly known as Paecilomyces fumosoroseus is reported as a promising biocontrol agent for controlling subterranean termites, particularly those belonging to the family Rhinotermitidae. In Malaysia, the family Rhinotermitidae includes two species of subterranean termites with extremely high economic importance; namely coptotermes curvignathus Holmgren, and the Asian SLubterranean Termite (coptotermes gestroi Wasmann). To comprehend the potential control of this soil-dwelling fungus against these subterranean termites in Malaysia, an investigation was carried out by testing the pathogenecity of 11 isolates against these termite species. All isolates showed pathogenic potential against the termite (Mortality rate of c. curvignathus:84.4%; c. gestroi:67.3%). IsolatePF49 was the most effective against both species of termites and was further tested for its virulence and *** Lc_(50) values of PF49 against c. curvignathus and *** were 7.55×103 and 1.09×102 conidia/ml,respectively. The average number of days required to complete the mycosis process in c. curvignathus and *** were 4.7 and 8 days, respectively. These fungi are believed useful for protecting living trees, plants, wood,wood structures, and other cellulosic materials susceptible to termite infestation and damage.

MYB transcription factors regulate diverse aspects of plant development and secondary metabolism,often by partnering in transcriptional regulatory ***,we harness genomic resources to identify novel MYBs,thereby producing an updated eudicot MYB phylogeny with revised relationships among subgroups as well as new information on sequence variation in the disordered c-terminus of anthocyanin-activating ***®and hidden Markov model scans of gene annotations identified a total of 714 MYB transcription factors across the genomes of four crops that span the eudicots:apple,grape,kiwifruit and *** model-based phylogenetic inference identified novel members of previously defined subgroups,and the function of specific anthocyanin-activating subgroup 6 members was assayed transiently in tobacco *** conservation within subgroup 6 highlighted one previously described and two novel short linear motifs in the disordered c-terminal *** novel motifs have a mix of hydrophobic and acidic residues and are predicted to be relatively ordered compared with flanking protein *** of motifs with the Eukaryotic Linear Motif database suggests roles in protein–protein *** of motifs and their flanking regions from strong anthocyanin activators into weak activators,and vice versa,affected *** conclude that,although the MYB c-terminal sequence diverges greatly even within MYB clades,variation within the c-terminus at and near relatively ordered regions offers opportunities for exploring MYB function and developing superior alleles for plant breeding.

Introduction Internal carotid artery termination(IcAT)and proximal A1 aneurysms can be challenging for open surgical clipping or endovascular *** with flow diversion covering the middle cerebral artery(McA),an end vessel supplying a terminal circulation,has not been *** A prospective,Institutional Review Board-approved database was analysed for patients with pipeline embolisation device(PED)placement from the anterior cerebral artery(AcA)to the IcA during cerebral aneurysm *** Nine cases were identified,including five proximal A1,three posterior communicating artery and one IcAT aneurysm *** aneurysm size was 8.3 mm(range 3-17),with 67%saccular and 78%*** indication for treatment was significant dome irregularity(44%),recurrence or enlargement(33%),underlying collagen vascular disorder(11%)and traumatic pseudoaneurysm(11%).Preservation of the ipsilateral AcA(with PED placed in A1)was performed when the anterior communicating artery(67%)or contralateral A1(33%)were absent on *** coiling was done in four cases(44%).There was one major stroke leading to mortality(11%)and one minor stroke(11%).clinical follow-up was 27 months on ***-up digital subtraction angiography(average interval 15 months)showed complete aneurysm obliteration(88%)or dome occlusion with entry remnant(12%).The jailed McA showed minimal or mild delay(primarily anterograde flow)in 75%of cases and significant delay(reliance primarily on AcA and external carotid artery collaterals)in 25%.conclusions covering the McA with a flow diverting stent should be reserved for select rare *** attention to blood pressure augmentation during the periprocedural period is necessary to minimise potential ischaemic compromise.
