背景对接受低剂量阿司匹林治疗发生消化道溃疡出血患者,采取内镜下止血治疗后是否应继续使用阿司匹林。目的检验内镜下控制溃疡出血后继续采用阿司匹林与质子泵抑制剂联合治疗并不劣于停用阿司匹林治疗,基于患心血管或脑血管病的成年复发性溃疡出血的患者。设计平行随机安慰剂对照试验。试验中患者和临床医生对指定的治疗双盲,从2003—2006年,将计算机生成的编号密封。设施一家三级内镜中心。患者接受低剂量阿司匹林的消化道溃疡出血者。干预78例患者接受阿司匹林80mg/d,78例接受安慰剂8周,立即采取内镜治疗。所有患者输注泮托拉唑(pantoprazole)治疗72h,接着口服泮托拉唑。对所有患者进行跟踪随访。测量初级终点为用内镜确定30日内再发溃疡出血。次极终点为8周内所有原因和特发原因所致的死亡率。结果 56例患者纳入治疗意向分析。3例在做跟踪前退出了试验。阿司匹林组30日内再发溃疡出血的发生率为10.3%,安慰剂组为5.4%〔相差4.9%,95%CI(3.6%～13.4%)〕,所有原因死亡率,接受阿司匹林治疗的患者比用安慰剂的患者要低〔1.3%vs12.9%;相差11.6%,95%CI(3.7%～19.5%)〕。与安慰剂组患者比较,将阿司匹林组患者死亡率较低的原因归因于心血管、脑血管或胃肠道并发症较少〔1.3%vs10.3%,相差9%,95%CI(1.7%～16.3%)〕。局限性样本量相对较小;仅做了低剂量阿司匹林80mg;安慰剂组有2例再发出血患者未做进一步内镜检查。结论接受低剂量阿司匹林治疗者发生消化道溃疡性出血后,继续用阿司匹林治疗可增加再发出血的风险,但有可能减少死亡率。证实这些发现还需做大量的实验研究。

Abdominal pain is the most common symptom of chronic pancreatitis(CP)and is often debilitating for patients and very difficult to *** date,there exists no cure for the *** strategies focus on symptom management and on mitigation of disease progression by reducing toxin exposure and avoiding recurrent inflammatory *** treatment protocols start with medical management followed by consideration of procedural or surgical intervention on selected patients with severe and persistent *** incorporation of adjuvant therapies to treat comorbidities including psychiatric disorders,exocrine pancreatic insufficiency,mineral bone disease,frailty,and malnutrition,are in its early *** clinical studies and animal models have been designed to improve investigation into the pathophysiology of CP pain,as well as to improve pain *** the array of tools available,many therapeutic options for the management of CP pain provide incomplete *** still remains much to discover about the neural regulation of pancreas-related *** this review,we will discuss research from the last 5 years that has provided new insights into novel methods of pain phenotyping and the pathophysiology of CP *** discoveries have led to improvements in patient selection for optimization of outcomes for both medical and procedural management,and identification of potential future therapies.

Anaerobic microbial corrosion of iron-containing metals causes extensive economic *** microbes are capable of direct metal-to-microbe electron transfer(electrobiocorrosion),but the prevalence of electrobiocorrosion among diverse methanogens and acetogens is poorly understood because of a lack of tools for their genetic *** studies have suggested that respiration with 316l stainless steel as the electron donor is indicative of electrobiocorrosion,because,unlike pure Fe^(0),316l stainless steel does not abiotically generate H_(2) as an intermediary electron ***,we report that all of the methanogens(Methanosarcina vacuolata,Methanothrix soehngenii,and Methanobacterium strain IM1)and acetogens(Sporomusa ovata and Clostridium ljungdahli)evaluated respired with pure Fe^(0)as the electron donor,but only ***,***,and *** were capable of stainless steel *** electrobiocorrosive methanogens re-quired acetate as an additional energy source in order to produce methane from stainless *** of *** and *** demonstrated how acetogens can provide acetate to methanogens during *** only was Meth-anobacterium strain IM1 not capable of electrobiocorrosion,but it also did not accept electrons from Geobacter metal-lireducens,an effective electron-donating partner for direct interspecies electron transfer to all methanogens that can directly accept electrons from Fe^(0).The finding that ***,***,and *** are capable of electrobiocorrosion,despite a lack of the outer-surface c-type cytochromes previously found to be important in other electrobiocorrosive microbes,demonstrates that there are multiple microbial strategies for making electrical contact with Fe^(0).

Background:Newly diagnosed breast cancer patients experience symptoms that may affect their quality of life,treatment outcomes,and *** and managing breast cancer-related symptoms soon after diagnosis is *** purpose of this study was to investigate the associations between health-related fitness(HRF)and patient-reported symptoms in newly diagnosed breast cancer ***:This study utilized baseline data from the Alberta Moving Beyond Breast Cancer Cohort Study that were collected within 90 days of *** measures included peak cardiopulmonary fitness(peak volume of oxygen consumption(VO_(2peak))),maximal muscular strength and endurance,flexibility,and body *** measures included depression,sleep quality,and *** multivariable logistic regression was performed for ***:Of 1458 participants,51.5%reported poor sleep quality,26.5%reported significant fatigue,and 10.4%reported moderate *** multivariable-adjusted models,lower relative VO_(2peak)was independently associated with a greater likelihood of all symptom measures,including moderate depression(pleep quality(p=0.009),significant fatigue(p=0.008),any symptom(pltiple symptoms(pld associations with all symptom measures such that all 3 lower quartiles exhibited similar elevated risk compared to the highest *** strength of the threshold associations varied by the symptom measure with odds ratios ranging from-1.5 for poor sleep quality to-3.0 for moderate depression and multiple ***,lower relative upper body muscular endurance was also independently associated with fatigue in a dose-response manner(p=0.001),and higher body weight was independently associated with poor sleep quality in an inverted U pattern(p=0.021).Conclusion:Relative VO_(2peak)appears to be a critical HRF component associated with multiple patient-reported symptoms in newly diag

Aicardi-Goutieres syndrome(AGS)is a systemic inflammatory disorder caused by mutations in any one of the nine different genes,whose deficiency provokes a type I(interferon)IFN response probably central to pathogenesis.^(1) ADAR1,one of the genes mutated in AGS(AGS6),encodes for an enzyme that belongs to the ADAR family(ADAR1,ADAR2,and ADAR3)that catalyzes the conversion of adenosine to inosine within double-stranded RNAs(dsRNAs)(RNA editing A-to-1).
