Management of borderline resectable pancreatic cancer

Pancreatic cancer is the fourth most common cause of cancer death in the United States. Surgery remains the only curative option; however only 20% of the patients have resectable disease at the time of initialpresentation. The definition of borderline resectable pancreatic cancer is not uniform but generally denotes to regional vessel involvement that makes it unlikely to have negative surgical margins. The accurate staging of pancreatic cancer requires triple phase computed tomography or magnetic resonance imaging of the pancreas. Management of patients with borderline resectable pancreatic cancer remains unclear. The data for treatment of these patients is primarily derived from retrospective single institution experience. The prospective trials have been plagued by small numbers and poor accrual. Neoadjuvant therapy is recommended and typically consists of chemotherapy and radiation therapy. The chemotherapeutic regimens continue to evolve along with type and dose of radiation therapy. Gemcitabine or 5-fluorouracil based chemotherapeutic combinations are administered. The type and dose of radiation vary among different institutions. With neoadjuvant treatment, approximately 50% of the patients are able to undergo surgical resections with negative margins obtained in greater than 80% of the patients. Newer trials are attempting to standardize the definition of borderline resectable pancreatic cancer and treatment regimens. In this review, we outline the definition, imaging requirements and management of patients with borderline resectable pancreatic cancer.

Yu et al’s study in the World Journal of Gastroenterology(2023)introduced a novel regimen of Vonoprazan-amoxicillin dual therapy combined with Saccharomyces boulardii(***)for the rescue therapy against helicobacter pylori(***),a pathogen responsible for peptic ulcers and gastric *** is a potassium-competitive acid blocker renowned for its rapid and long-lasting acid suppression,which is minimally affected by *** to proton pump inhibitors,which bind irreversibly to cysteine residues in the h+/K+-ATPase pump,Vonoprazan competes with the K+ions,prevents the ions from binding to the pump and blocks acid *** with increasing antibiotic resistance,effects on the gut microbiota,patient compliance,and side effects have led to the advent of a dual regimen for *** studies suggested that *** plays a role in stabilizing the gut barrier which improves *** eradication *** an acceptable safety profile,the dual-adjunct regimen was effective regardless of prior treatment failure and antibiotic resistance profile,thereby strengthening the applicability in clinical ***,*** comes in various formulations and dosages,warranting further exploration into the optimal dosage for supplementation in rescue ***,larger,randomized,double-blinded controlled trials are warranted to confirm these promising results.

Background Non-interventional large-scale research on real-world patients who had a stroke requires the use of multiple data sources ensuring access to longitudinal data from large populations with clinically-detailed *** sought to establish a framework for longitudinal research on patients hospitalised with stroke by linking information-rich,deidentified inpatient data from the Paul Coverdell National Acute Stroke Program(PCNASP)to commercial and Medicare Advantage longitudinal claims *** All stroke admissions in PCNASP between 2008 and 2015 were evaluated for linkage to longitudinal claims from a commercial insurer using an algorithm based on six available common data fields(patient age,gender,admission date,discharge date,discharge diagnosis and state)and a hospital *** evaluated the linkage quality(via the percentage of unique records in the linked dataset)and the representativeness of the linked *** also described medical history,stroke severity and patterns of medication use among the PCNASP-claims linked *** The linkage produced uniqueness equal to 99.1%.We identified 5644 linked and 98896 unlinked patients who had an ischaemic stroke hospitalisation in claims *** patients were younger than unlinked(69.7 vs 72.5 years),but otherwise similar by medical history,prestroke medication use or lab *** severity was mild and most patients were discharged *** and discharge use of antihypertensive and statins in the PCNASP were greater than their use as measured by filled prescriptions in *** high-quality linkage between the PCNASP and commercial claims data is *** linkage identified differences between reported or recommended versus actual out-of-hospital medication utilisation,highlighting the importance of longitudinal data availability for research aimed to improve the care of patients who had a stroke.

how Sugars Might Coordinate Chloroplast and Nuclear Gene Expression during Acclimation to high Light Intensities

The concept of retrograde control of nuclear gene expression assumes the generation of signals inside the chloroplasts, which are either released from or sensed inside of the organelle. In both cases, downstream signaling path- ways lead eventually to a differential regulation of nuclear gene expression and the production of proteins required in the chloroplast. This concept appears reasonable as the majority of the over 3000 predicted plastidial proteins are encoded by nuclear genes. hence, the nucleus needs information on the status of the chloroplasts, such as during acclimation responses, which trigger massive changes in the protein composition of the thylakoid membrane and in the stroma. here, we propose an additional control mechanism of nuclear- and plastome-encoded photosynthesis genes, taking advantage of pathways involved in sugar- or hormonal signaling. Sugars are major end products of photosynthesis and their con- tents respond very sensitively to changes in light intensities. Based on recent findings, we ask the question as to whether the carbohydrate status outside the chloroplast can be directly sensed within the chloroplast stroma. Sugars might syn- chronize the responsiveness of both genomes and thereby help to coordinate the expression of piastome- and nuclear- encoded photosynthesis genes in concert with other, more specific retrograde signals.

Effect of solution condition on hydroxyapatite formation in evaluating bioactivity of B2O3 containing 45S5 bioactive glasses

The effects of testing solutions and conditions on hydroxyapatite(hAp)formation as a means of in vitro bioactivity evaluation of B2O3 containing 45S5 bioactive glasses were systematically *** glass samples prepared by the traditional melt and quench process,where SiO2 in 45S5 was gradually replaced by B2O3(up to 30%),were *** solutions:the simulated body fluid(SBF)and K2hPO4 solutions were used as the medium for evaluating in vitro bioactivity through the formation of hAp on glass surface as a function of *** was found that addition of boron oxide delayed the hAp formation in both SBF and K2hPO4 solutions,while the reaction between glass and the K2hPO4 solution is much faster as compared to *** addition to the composition and medium effects,we also studied whether the solution treatments(e.g.,adjusting to maintain a ph of 7.4,refreshing solution at certain time interval,and no disturbance during immersion)affect hAp *** transform infrared spectrometer(FTIR)equipped with an attenuated total reflection(ATR)sampling technique and scanning electron microscopy(SEM)were conducted to identify hAp formation on glass powder surfaces and to observe hAp morphologies,*** results show that refreshing solution every 24 h produced the fastest hAp formation for low boron-containing samples when SBF was used as testing solution,while no significant differences were observed when K2hPO4 solution was *** study thus suggests the testing solutions and conditions play an important role on the in vitro bioactivity testing results and should be carefully considered when study materials with varying bioactivities.

Large-diameter trees and deadwood correspond with belowground ectomycorrhizal fungal richness

Background: Large-diameter trees have an outsized influence on aboveground forest dynamics, composition, and structure. Although their influence on aboveground processes is well studied, their role in shaping belowground fungal communities is largely unknown. We sought to test if (i) fungal community spatial structure matched aboveground forest structure;(ii) fungal functional guilds exhibited differential associations to aboveground trees, snags, and deadwood;and (iii) that large-diameter trees and snags have a larger influence on fungal community richness than smaller-diameter trees. We used MiSeq sequencing of fungal communities collected from soils in a spatially intensive survey in a portion of Cedar Breaks National Monument, Utah, USA. We used random forest models to explore the spatial structure of fungal communities as they relate to explicitly mapped trees and deadwood distributed across 1.15 ha of a 15.32-ha mapped subalpine forest. Results: We found 6,177 fungal amplicon sequence variants across 117 sequenced samples. Tree diameter, dead-wood presence, and tree species identity explained more than twice as much variation (38.7% vs. 10.4%) for ectomy-corrhizal composition and diversity than for the total or saprotrophic fungal communities. Species identity and dis-tance to the nearest large-diameter tree (≥ 40.2 cm) were better predictors of fungal richness than were the identity and distance to the nearest tree. Soil nutrients, topography, and tree species differentially influenced the composition and diversity of each fungal guild. Locally rare tree species had an outsized influence on fungal community richness. Conclusions: These results highlight that fungal guilds are differentially associated with the location, size, and species of aboveground trees. Large-diameter trees are implicated as drivers of belowground fungal diversity, particularly for ectomycorrhizal fungi.

Methanotrophic bacteria are currently used industrially for the bioconversion of methane-rich natural gas and anaerobic digestion-derived biogas to valuable *** bacteria may also serve to mitigate the negative effects of climate change by capturing atmospheric greenhouse *** genetic tools have previously been developed for genetic and metabolic engineering of ***,the available tools for use in methanotrophs are significantly underdeveloped compared to many other industrially relevant bacteria,which hinders genetic and metabolic engineering of these *** such,expansion of the methanotroph genetic toolbox is needed to further our understanding of methanotrophy and develop biotechnologies that leverage these unique microbes for mitigation and conversion of methane to valuable ***,we determined the copy number of three broad-host-range plasmids in Methylococcus capsulatus Bath and Methylosinus trichosporium OB3b,representing phylogenetically diverse Gammaproteobacterial and Alphaproteobacterial methanotrophs,***,we show that the commonly used synthetic Anderson series promoters are functional and exhibit similar relative activity in *** and *** OB3b,but the synthetic series had limited ***,we mutagenized the native *** particulate methane monooxygenase promoter and identified variants with activity that expand the activity range of synthetic,constitutive promoters functional not only in ***,but also in Escherichia ***,the tools developed here advance the methanotroph genetic engineering toolbox and represent additional synthetic genetic parts that may have broad applicability in Pseudomonadota bacteria.

Exercise training and antioxidant supplementation independently improve cognitive function in adult male and female GFAP-APOE mice

Purpose： The purpose of this study was to determine if antioxidant supplementation, moderate exercise, and the combination of both treatments could ameliorate cognitive performance in adult mice and whether the apolipoprotein E （APOE） genotype as well as sex could influence the functional outcomes of the treatments. Methods： For a period of 16 weeks, separate groups of male and female mice expressing either the human APOE3 or APOE4 isoforms were fed either a control diet （NIh-31） or the control diet supplemented with vitamins E and C （1.12 IU/g diet a-tocopheryl acetate and 1.65 mg/g ascorbic acid）. The mice were further separated into a sedentary group or a group that followed a daily exercise regimen. After 8 weeks on the treatments, the mice were administered a battery of functional tests including tests to measure cognitive and affective function. Results： There was no effect of genotype or treatment on the learning performance in the Morris water maze. In the discriminated avoidance task, APOE4 mice performed better in learning the discrimination component of the task. Overall, exercise improved performance of APOE4 and APOE3 mice on various aspects of the active avoidance task. Antioxidant supplementation improved performance only in the APOE4 mice. On the test for anxiety, APOE4 mice spent more time in the open arms and supplementation with antioxidant reversed that effect. Conclusion： Exercise was the most effective treatment at improving cognitive function in both genotypes and sex, while antioxidants seemed to be effective only in the APOE4. In young adult mice only non-spatial learning and memory were improved. The combination of the two treatments did not yield further improvement in cognition, and there was no antagonistic action of the antioxidant supplementation on the beneficial effects of exercise.
