Long-term irritable bowel syndrome symptom control with reintroduction of selected FODMAPs

To investigate the long-term effect of dietary education on a low fermentable oligosaccharide, disaccharide and polyol (FODMAP) diet on irritable bowel syndrome (IBS) symptoms and quality of life (QoL). METHODSParticipants with IBS (Rome III) were randomized to two groups. Group I commenced a low FODMAP diet at baseline. At three months, group II, so far a comparator group, crossed over to a low FODMAP diet while group I started re-challenging foods. All patients completed the IBS SSS (IBS symptom severity scoring system, 0-500 points increasing with severity), IBS QoL questionnaire (0-100 increasing with QoL), a FODMAP specific food frequency questionnaire and provided a stool sample at baseline, three and six months for microbiome analysis. RESULTSFifty participants were enrolled into group I (n = 23) or group II (n = 27). Participants in both groups were similar in baseline values but with more men in group I. There was a significantly lower IBS SSS (275.6 ± 63.6 to 128.8 ± 82.5 vs 246.8 ± 71.1 to 203.6 ± 70.1) (P nd increased QoL (68.5 ± 18.0 to 83 ± 13.4 vs 72.9 ± 12.8 to 73.3 ± 14.4) (P n group I vs group II at 3 mo. The reduced IBS SSS was sustained at 6 mo in group I (160 ± 102) and replicated in group II (124 ± 76). Fiber intake decreased on the low FODMAP diet (33 ± 17 g/d to 21 ± 8 g/d) (P nd after re-introducing FODMAP containing foods increased again to 27 ± 9 g/d. There was no change seen in the intestinal microbiome when participants adopted a low FODMAP diet. COnCLUSIOnThis study demonstrated that a reduction in FODMAPs improves symptoms in IBS and this improvement can be maintained while reintroducing FODMAPs.

A comparison of survival and pathologic features of non-alcoholic steatohepatitis and hepatitis C virus patients with hepatocellular carcinoma

AIM： To compare the clinical outcome and pathologic features of non-alcoholic steatohepatitis （nASH） patients with hepatocellular carcinoma （HCC） and hepatitic C virus （HCV） patients with HCC （another group in which HCC is commonly seen） undergoing liver transplantation. METHODS： Patients transplanted for HCV and nASH at our institution from January 2000 to April 2011 were analyzed. All explanted liver histology and pre-trans- plant liver biopsies were examined by two specialist liver histopathologists. Patient demographics, disease free survival, explant liver characteristics and HCC features （tumour number, cumulative tumour size, vascular invasion and differentiation） were compared between HCV and nASH liver transplant recipients. RESULTS： A total of 102 patients with nA^SH and 283 patients with HCV were transplanted. The incidence of HCC in nASH transplant recipients was 16.7% （17/102）. The incidence of HCC in HCV transplant recipients was 22.6% （64/283）. Patients with nASH-HCC were statisti- cally older than HCV-HCC patients （P 〈 0.001）. A signif- icantly higher proportion of HCV-HCC patients had vas- cular invasion （23.4% vs 6.4%, P = 0.002） and poorly differentiated HCC （4.7% vs 0%, P 〈 0.001） compared to the nASH-HCC group. A trend of poorer recurrence free survival at 5 years was seen in HCV-HCC patients compared to nASH-HCC who underwent a Liver trans- plantation （P = 0.11）. COnCLUSIOn： Patients transplanted for nASH-HCC appear to have less aggressive turnout features com- pared to those with HCV-HCC, which likely in part ac- counts for their improved recurrence free survival.

Jailing polymer jacketed guide-wires during bifurcation coronary interventions is associated with procedural myocardial infarction

AIM To study the relationship of jailed polymer jacketed guide wires(PGW) with procedural myocardial infarction(PMI) after bifurcation coronary *** Consecutive bifurcation interventions performed from January 2010 to October 2014 were included in the study. Chart review was performed to obtain demographic, clinical and procedural data. PMI was defined as Creatine Kinase MB > 3 × upper reference limit of normal. Multivariate logistic regression was used to ascertain relationship of PGW use with *** Two hundred and ninety-three patients(age 63.5 ± 12.3 years; 33.8% diabetic) were included in the study. Eighty point two percent(n = 235) were true bifurcation lesions use of PGW was associated with PMI on univariate analysis(OR = 4.1; P = 0.002). This association remained significant after adjusting for other possible risk factors(OR = 3.5; P = 0.02).COnCLUSIOn Our results suggest that PGW use for side branch protection may be associated with PMI. Randomized studies are needed to validate these findings.

碳电极表面的伯胺电嫁接被广泛地应用于电极表面改性.本文通过比较发现,在比例为4:1的乙腈和0.1mol·L^(-1) 碳酸氢钠溶液混合物中,玻碳电极表面嫁接上叔丁氧羟基-乙二胺的效率比在单纯的乙腈中显著提高.有碳氢酸钠存在时,循环伏安测得电极的初始电流增大,表明电极表面胺嫁接层的形成更快速,从而使得电极表面更容易钝化,导致乙二胺-叔丁氧羟基膜层更严重地阻碍[Fe(Cn)_6]^(3-)的反应.通过去除叔丁氧羟基保护层,在蒽醌-2-羧酸上接自由胺,可获得较高的蒽醌表面覆盖度.采用简单动力学模型模拟了电嫁接反应,结果表明,模拟得到的循环伏安曲线与实验测得的循环伏安曲线相一致.对比在单纯乙腈和乙腈/碳酸氢钠溶液混合物中模型拟合得到的参数值可知,胺自由基与碳表面的反应和在均相溶液中的反应相互竞争,更有利于乙腈与碳氢酸钠的表面反应.
