Treatment of intractable orbital implant exposure with a large conjunctival defect by secondary insertion of the implant after preceding dermis fat graft

AIM: To report a procedure and results of a two-stage operation to manage intractable extensive orbital implant exposure with a large conjunctival defect which was difficult to treat with dermis fat grafts due to repeated graft necrosis. METHODS: A retrospective chart review of four patients who had extensive orbital implant exposures with large conjunctival defects and had past histories of repeated autologous or preserved dermis graft failures was done. As a first-stage operation, the problematic pre-existing orbital implants were removed and autologous dermis fat grafts alone were performed on the defect area. Four months later, new orbital implants were secondarily inserted after confirmation of graft survival. The size of the conjunctival defects and state of the extraocular muscles were checked preoperatively. Success of the operations and complications were ***: The mean size of the conjuctival defects was 17.3mm×16.0mm, and the mean time from the initial diagnosis of orbital implant exposure to implant removal and autologous dermis fat graft was 20.8 months. After implant removal and autologous dermis fat graft, no graft necrosis was observed in any patients. Also, implant exposure or fornix shortening was not observed in any patients after new orbital implant insertion. CONCLUSION: The secondary insertion of a new orbital implant after pre-existing implant removal and preceding dermis fat graft is thought to be an another selective management of intractable orbital implant exposure in which dermis fat grafts persistently fail.

A novel 50 kW fast charger was proposed for electric vehicles. The proposed fast charger is divided into two main sections an AC-DC converter performing a PFC function and a DC-DC converter performing a charging function. A transformer including leakage inductances was used in the AC-DC converter in order to obtain isolation and inductance. A series-connection topology was used in the DC-DC converter between the DC-bus and outlet. This topology enables high power conversion efficiency up to 95% for the DC-DC converter. In order to reduce the impact of the 50 kW charging on the AC grid, the proposed fast charger system includes a buffering battery unit between the two main power conversion units. This leads to reductions in the power installation costs of power companies and to improvements in the power quality were verified through simulations and experimental results. on the AC grid. The effects of Mg enhancement and heat treatment on the microstructures and tensile properties of Al_2Ca-added ADC12 die casting alloys were investigated. 0.3% and 0.5% Mg in the form of a master alloy including a trace amount of Al_2Ca were added to conventional ADC12（383 and AlSi10Cu2Fe） alloy with an initial Mg-content of 0.3% to increase the Mg content to 0.6% and 0.8%, respectively. To avoid heat treatmentinduced surface blisters, shortened solution treatment for 15 min at 490 ℃ and artificial aging for 6 h at 150 ℃ was undertaken. The results show that a 10% improvement in the shape factor of eutectic Si particles was achieved for Al_2Ca-added ADC12 with 0.8% Mg compared to the conventional ADC12 in the as-aged condition. Al_2Ca-added ADC12 with 0.8% Mg exhibited a yield strength of 289 MPa, a tensile strength of 407 MPa, and an elongation of 4.22%.

Hepatic artery pseudoaneurysm(HAP) is a very rare disease but in cases of complication,there is a very high *** most common cause of HAP is iatrogenic trauma such as liver biopsy,transhepatic biliary drainage,cholecystectomy and *** may also occur with complications such as infections or inflammation associated with septic *** has been reported rarely in patients with acute *** far as we are aware,there is no report of a case caused by acute idiopathic pancreatitis,*** report a case of HAP caused by acute idiopathic pancreatitis which developed in a 61-year-old *** woman initially presented with acute pancreatitis due to unknown *** conservative management,her symptoms seemed to have *** eight days after admission,abdominal pain abruptly became worse *** computed tomography(CT) was rechecked and it detected a new HAP that was not seen in a previous abdominal *** retrograde cholangiopancreatography(ERCP) was performed because of a suspicion of hemobilia as a cause of aggravated abdominal *** confirmed hemobilia by observing fresh blood clots at the opening of the ampulla and several filling defects in the distal common bile duct on *** any particular treatment such as embolization or surgical ligation,HAP thrombosed *** months after discharge,abdominal CT demonstrated that HAP in the left lateral segment had disappeared.

AIM:To assess the indications,findings,therapeutic procedures,safety,and complications of endoscopic retrograde cholangiopancreatography(ERCP) performed in Korean ***:The demographic characteristics,indications for ERCP,findings,therapeutic procedures,and complications of 122 pediatric patients who underwent 245 ERCPs in the Asan Medical Center between June 1994 and March 2008 were ***:The mean age of the 122 patients was 8.0 ± 4.2 *** were biliary pathology in 78(64.0%),pancreatic pathology in 43(35.2%),and chronic abdominal pain in *** indications included choledochal cysts in 40,choledocholithiasis in 24,suspected sclerosing cholangitis in 8,trauma in 2,and other conditions in *** indications includedacute pancreatitis in 7,acute recurrent pancreatitis in 11,chronic pancreatitis in 20,trauma in 3,and pancreatic mass in *** the 245 ERCPs,success rate was 98.4% and 190(77.6%) were for therapeutic purposes,including endoscopic nasal drainage(51.8%),biliary sphincterotomy(38.0%),pancreatic sphincterotomy(23.3%),stent insertion(15.1%),stone extraction(18.8%),and balloon dilatation(11.0%).Complications were postERCP pancreatitis in 16(6.5%),ileus in 23(9.4%),hemorrhage in 2(0.8%),perforation in 2(0.8%),sepsis in 1(0.4%),and impacted basket in 1(0.4%).There were no procedure-related deaths,and most complications improved under supportive ***:This study showed that there is a high incidence of choledochal cyst and diagnostic and therapeutic ERCP for the management of various biliary and pancreatic diseases was safe and effective in Korean children.

A method based on die casting experiments and mathematic modeling is presented for the determination of the heat flow density （HFD） and interfacial heat transfer coefficient （IHTC） during the high pressure die casting （HPDC） *** were carried out using step shape casting and a commercial magnesium alloy,*** profiles were measured and recorded using thermocouples embedded inside the die. Based on these temperature readings,the HFD and IHTC were successfully determined and the calculation results show that the HFD and IHTC at the metal-die interface increases sharply right after the fast phase injection process until approaching their maximum values,after which their values decrease to a much lower level until the dies are *** patterns of heat transfer behavior were found between the die and the casting at different *** thinner the casting was,the more quickly the HFD and IHTC reached their steady ***,the values for both the HFD and IHTC values were different between die and casting at different thicknesses.
