＜正＞ We examined POMC mRNA and beta - endorphin expression in mouse hypothalamus elicited by intraplantar formalin. POMC mRNA increased at 2 hr after injection,and returned to control level at 10 hr in the arcuate nu. of hypothalamus. In the tail - flick test,formalin attenuated opioids - induced antinociception. Increase of POMC was also observed after systemic morphine and attenuated by naltrexone. Thus,formalin - induced increase of hypothalamic POMC may be mediated by endogenous opioid system. We further examined alterations of signal molecules. pERK was increased within 30 min and remained at a high level up to 10 hr in the arcuate nu..pCaMKII was increased within 2 hr but decreased at 10 hr. However. POMC mRNA expression was reduced by pretreatment with PD98059 or KN93. Furthermore. pIkB was increased at 2 hr and remained at high level up to 10 hr. Using confocal IF. we confirmed that cells contain beta - endorphin after formalin also express pERK. pCaMKII and pIkB. In conclusion. POMC mRNA expression in arcuate nu. of hypothalamus induced by intraplantar formalin may be mediated by pERK as well as pCaMKII. Furthermore. NFkB pathway may play an important role in the regulation of POMC gene expression.

瞄准：为干扰素(IFN ) 证实预兆的因素为有丙肝的长期的肝炎病人的 -alpha 和 ribavirin 联合治疗病毒(HCV ) 遗传型 1b。方法：从感染 HCV 遗传型 1b 的 50 个病人的 HCV RNA 被决定区域(ISDR ) 的干扰素敏感克隆并且定序学习， PKR-eIF2alpha 磷酸化相同领域(PePHD ) 。病人们为 6 瞬间与 IFN-alpha 和 ribavirin 被对待并且由治疗的有效性组织了。许多因素被分析。结果：我们的数据证明年龄， HCV RNA 效价，和 ISDR 类型能为联合 IFN-alpha 和 ribavirin 功效被用作预兆的因素。表示特性地，仅仅当联合治疗是有效的时，在 PePHD 的变化出现了。另外的因素，例如性别和丙氨酸， aminotransferase (中高音) 铺平，不与它的功效有关。好久调整并且 HCV RNA titer 显示 ISDR 类型是最有势力预兆的因素。结论：HCV RNA ISDR 类型是为在朝鲜病人预言 IFN-alpha 和 ribavirin 联合治疗的功效的一个重要因素。

AIM: To investigate whether, or how, DA-9601, which is a new gastroprotective agent, inhibits TNF-α-induced inflammatory signals in gastric epithelial AGS cells. METHODS: Cell viability was determined by MTT assay. IL-8 and CCL20 promoter activities were determined by a luciferease reporter gene assay. NF-κB-dependent transcriptional activity was determined by I-κBαdegradation, NF-κB p65 nuclear translocation and a luciferase activity assay. IL-8 and CCL20 gene expression and protein secretion were determined by RT-PCR and an enzymelinked immunosorbent assay (ELISA). Total and phos-phorylated forms of mitogen-activated protein kinases (MAPKs) were determined by Western blot. RESULTS: Treatment of AGS cells with DA-9601 reduced TNF-α-induced IL-8 and CCL20 promoter activities, as well as their gene expression and protein release. TNF-αalso induced NF-κB-dependent transcriptional activity in AGS cells. In contrast, in cells treated with DA-9601, TNF-α-induced NF-κB activity was significantly blocked. Although all three MAP kinase family members were phosphorylated in response to TNF-α, a selective inhibitor of p38 kinase SB203580 only could inhibit both NF-κB-dependent transcriptional activity and IL-8 and CCL20 production, suggesting a potential link between p38 kinase and NF-κB-dependent pathways in AGS cells. Interestingly, DA-9601 also selectively inhibited p38 kinase phosphorylation induced by TNF-α. CONCLUSION: DA-9601 blocked TNF-α-mediated inflammatory signals by potentially modulating the p38 kinase pathway and/or a signal leading to NF-κBdependent pathways in gastric epithelial cells.

AIM:To evaluate the efficacy of stents in treating patients with anastomotic site obstructions due to cancer recurrence following colorectal ***:The medical records of patients who underwent endoscopic self-expanding metal stents(SEMS)insertion for colorectal obstructions between February2004 and January 2014 were retrospectively *** the study period,a total of 218 patients underwent endoscopic stenting for colorectal *** identified and examined the patients who underwent endoscopic stenting for obstructions caused by cancer recurrence at the anastomotic site following colorectal surgeries for primary colorectal ***:Five consecutive patients[mean age,56.4years(range:39-82 years);4 women,1 man]underwent endoscopic stenting for obstructions caused by cancer recurrence at the anastomotic site following colorectal surgeries for primary colorectal *** and clinical success was achieved in all 5 patients,without any early *** follow-up,3patients did not need further intervention,prior to their death,after the first stent insertion;thus,the overall success rate was 3/5(60%).Perforations occurred in 2patients who required a second SEMS insertion due to re-obstruction;none of the patients experienced stent ***:SEMS placement is a promising treatment option for patients who develop obstructions of their colonic anastomosis sites due to cancer recurrence.

Optimal initiation of Helicobacter pylori eradication in patients with peptic ulcer bleeding

AIM:To evaluate when Helicobacter pylori(***)eradication therapy(ET)should be started in patients with peptic ulcer bleeding(PUB).METHODS:Clinical data concerning adults hospitalizedwith PUB were retrospectively collected and ***,sex,type and stage of peptic ulcer,whether endoscopic therapy was performed or not,methods of *** detection,duration of hospitalization,and specialty of the attending physician were *** influencing the confirmation of *** infection prior to discharge were determined using multiple logistic regression *** *** eradication rates of patients who received ET during hospitalization and those who commenced ET as outpatients were ***:A total of 232 patients with PUB were evaluated for *** infection by histology and/or rapid urease *** these patients,53.7%(127/232)had confirmed results of *** infection prior to *** multivariate analysis,duration of hospitalization and ulcer stage were factors independently influencing whether *** infection was confirmed before or after *** the patients discharged before confirmation of *** infection,13.3%(14/105)were lost to *** the patients found to be ***-positive after discharge,41.4%(12/29)did not receive *** was no significant difference in the *** eradication rate between patients who received ET during hospitalization a n d t h o s e w h o c o m m e n c e d E T a s o u t p a t i e n t s[intention-to-treat:68.8%(53/77)vs 60%(12/20),P=0.594;per-protocol:82.8%(53/64)vs 80%(12/15),P=0.723].CONCLUSION:Because many patients with PUB who were discharged before *** infection status was confirmed lost an opportunity to receive ET,we should confirm *** infection and start ET prior to discharge.

AIM:To elucidate the endoscopic features that predict the cancer following endoscopic submucosal dissection(ESD)in patients with high-grade neoplasia(HGN).METHODS:We retrospectively analyzed the medical records of patients who underwent ESD of gastric neoplasms from January 2007 to September *** was performed in 555 cases involving 550 patients.A total of 112 lesions from 110 consecutive patients were initially diagnosed as HGN without cancer by forceps biopsy,and later underwent *** classified lesions into two groups according to histologic discrepancies between the biopsy and ESD *** adenoma in the final diagnosis by ESD specimens were defined as adenoma *** with coexisting cancer after ESD were defined as cancer ***:The mean age was 65.3 years,and 81 patients were *** was no significant difference in the age or gender distribution between the adenoma(n=52)and cancer(n=60)***-six of these lesions(32.1%)showed histologic concordance between the forceps biopsy and ESD specimens,16(14.3%)showed a downgraded histology(low-grade neoplasia),and 60(53.6%)showed an upgraded histology(cancer).A red color change of the mucosal surface on endoscopy was found in 27/52(51.9%)of cases in the adenoma group and in 46/60(76.7%)of cases in the cancer group(P=0.006).Ulceration of the mucosal surface on endoscopy was found in 5(9.6%)of 52 lesions in the adenoma group and in 17(28.3%)of 60 lesions in the cancer group(P=0.013).In the multivariate analysis,a reddish surface color change and mucosal ulceration were significant predictive factors correlated with cancer after ESD of the HGN by forceps ***:HGN with a red color change or mucosal ulceration correlated with the presence of gastric *** finding may help to guide the diagnosis and treatment.
