经过20余年的相关研究,MRI现已成为临床公认的用于检出和诊断乳腺疾病的影像检查手段之一。笔者简要阐述了乳腺动态增强(DCE)磁共振成像技术及其临床应用。详细介绍了动态增强中有关药代动力学的测量方法、各种相关参数的定性分析、以及Ktrans和kep等反应药代动力学特征参数的定量分析。笔者不但就当前"是否有必要对每位患者进行动脉输入函数进行测量"等有争议的话题进行了讨论,还在对当前广泛应用的各种动态增强MRI后处理软件进行了较详尽的描述之后,进一步对动态增强MRI在乳腺癌的诊断及疗效监测等方面的临床应用情况进行了详尽的回顾。作者认为:总体来讲,对于乳腺癌的诊断,关键在于关注病灶中最具侵袭性部分的MRI特征。而对于乳腺癌疗效的监测,则应该对整个病灶进行分析。最后,笔者就乳腺MRI对乳腺癌高危人群的筛查以及风险的处理等问题进行了讨论。笔者预测乳腺MRI在女性乳腺疾病尤其是乳腺癌的筛查、诊断、治疗及疗效监测等一系列问题上将继续发挥不可替代的重要作用。

Background The onsets of needling sensation introduced by acupuncture stimulus can vary widely from subject to subject. This should be explicitly accounted for by the model blood oxygenation-level dependent （BOLD） time course used in general linear model （GLM） analysis to obtain more consistent across-subject group results. However, in standard GLM analysis, the model BOLD time course obtained by convolving a canonical hemodynamic response function with an experimental paradigm time course is assumed identical across subjects. Although some added-on properties to the model BOLD time course, such as temporal and dispersion derivatives, may be used to account for different BOLD response onsets, they can only account for the BOLD onset deviations to the extent of less than one repetition time （TR）. Methods In this study, we explicitly manipulated the onsets of model BOLD time course by shifting it with -2, -1, or 1 TR and used these temporally shifted BOLD model to analyze the functional magnetic resonance imaging （fMRI） data obtained from three acupuncture fMRI experiments with GLM analysis. One involved acupuncture stimulus on left ST42 acupoint and the other two on left GB40 and left BL64 acupoints. Results The model BOLD time course with temporal shifts, in addition to temporal and dispersion derivatives, could result in better statistical power of the data analysis in terms of the average correlation coefficients between the used BOLD models and extracted BOLD responses from individual subject data and the T-values of the activation clusters in the grouped random effects. Conclusions The GLM analysis with ordinary BOLD model failed to catch the large variability of the onsets of the BOLD responses associated with the acupuncture needling sensation. Shifts in time with more than a TR on model BOLD time course might be required to better extract the acupuncture stimulus-induced BOLD activities from individual fMRI data.

BACKGROUND Status epilepticus is an emergent and critical condition which needs management without hesitation. Nonconvulsive status epilepticus (NCSE) tends to be less recognized, and its diagnosis is delayed in comparison with overt status epilepticus because of the absence of specific clinical signs. It is often difficult to make a diagnosis, particularly in patients with hepatic encephalopathy. CASE SUMMARY A 38-year-old man with a history of alcoholic liver cirrhosis presented with altered mental status;the initial diagnosis was hepatic encephalopathy. Although optimal treatment for hepatic encephalopathy was administered, the patient's mental status did not improve. A final diagnosis of NCSE was made by continuous electroencephalogram (EEG) monitoring. Treatment with levetiracetam and propofol pump was immediately started. The patient’s consciousness gradually improved after discontinuation of propofol therapy, and no further epileptic discharge was observed by EEG monitoring. After 1 wk, the patient returned to full consciousness, and he was able to walk in the hospital ward without assistance. He was discharged with minimal sequela of bilateral conjunctivitis. CONCLUSION In cases of persistent altered mental status without reasonable diagnosis, NCSE should be considered in hepatic encephalopathy patients with persistently altered levels of consciousness, and EEG monitoring is very important. We also recommend propofol as a safe and efficient therapy for NCSE in liver cirrhosis patients.

High-performance organic friction modifiers(OFMs)added to lubricating oils are crucial for reducing energy loss and carbon *** establish a new class of OFMs,we measured the friction and wear properties of N-(2,2,6,6-tetramethyl-1-oxyl-4-piperidinyl)dodecaneamide referred to as *** effect of its head group chemistry,which is characterized by a rigid six-membered ring sandwiched by an amide group and a terminal free oxygen radical,was also investigated with both experiments and quantum mechanical(QM)*** measurement results show that C12Amide-TEMPO outperforms the conventional OFMs of glyceryl monooleate(GMO)and stearic acid,particularly for load-carrying capacity,wear reduction,and stability of friction over *** friction and wear reduction effect of C12Amide-TEMPO is also greatly superior to those of C12Ester-TEMPO and C12Amino-TEMPO,in which ester and amino groups replace the amide group,highlighting the critical role of the amide *** QM calculation results suggest that,in contrast to C12Ester-TEMPO,C12Amino-TEMPO,and the conventional OFMs of GMO and stearic acid,C12Amide-TEMPO can form effective boundary films on iron oxide surfaces with a unique double-layer structure:a strong surface adsorption layer owing to the chemical interactions of the amide oxygen and free radical with iron oxide surfaces,and an upper layer owing to the interlayer hydrogen-bonding between the amide hydrogen and free radical or between the amide hydrogen and ***,the intralayer hydrogen-bonding in each of the two layers is also *** suggest that in addition to strong surface adsorption,the interlayer and intralayer hydrogen-bonding also increases the strength of the boundary films by enhancing the cohesion strength,thereby resulting in the high tribological performance of *** findings in this study are expected to provide new hints for the optimal molecular design of OFMs.

Plastic scintillators(PSs)embedded with wavelength-shifting fibers are widely used in high-energy particle physics,such as in muon taggers,as well as in medical physics and other *** this study,a simulation package was built to evaluate the effects of the diameter and layout of optical fibers on the light yield with different *** optimal optical configuration was designed based on simulations and validated using two PS prototypes under certain experimental condi-tions.A top veto tracker(TVT)for the JUNO-TAO experiment,comprising four layers of 160 strips of PS,was designed and *** threshold was evaluated when the muon tagging efficiency of a PS strip was>99%.The efficiency of three layer out of four layer of TVT is>99%,even with a tagging efficiency of a single strip as low as 97%,using a threshold of 10 photoelectrons and assuming a 40%silicon PM photon detection efficiency.

Altered energy metabolism of cancer cells shapes the immune cell response in the tumor microenvironment that faclitates ***,we reported the novel of tumor cell-expressed Piwi Like RNA-Mediated Gene Silencing 1(PWL1)in mediatingthe crosstlk of fatty acid metabolism and immune response of human hepatocellular carcinoma(HCC).PlWIL1 expression in HCCwas increased compared to normal hepatic tissues and was positively correlated with the proliferation rate of HCC cell ***1overexpression accelerated in vitro proliferation and in vivo growth of HCC tumors,while PIWL1 knockdown showed ***1 increased oxygen consumption and energy production via fatty acid metabolism without altering aerobic *** of fatty acid metablism abolished PWIL1-induced HCC prolferation and ***-seq analysis revealed that immunesystem regulation might be involved,which was echoed by the experimental observation that PIWL1-overexpressing HCC cellsattracted myeloid-derived suppressor cells(MDSCs)into the tumor *** depletion reduced the prolferationand growth of PlWIL1-overexpressing HCC *** C3,whose secretion was induced by PIWL1 in HCC cells,mediatesthe interaction of HCC cells with MDSCs by activated p38 MAPK signaling in MDSCs,which in turn initiated expression of immunosuppressive cytokine *** lL10 secretion reduced the immunosuppressive activity of MDSCs in the microenvironment of PIWL1-overexpressing *** together,our study unraveled the critical role of PIWL1 in initiating theinteraction of cancer cell metabolism and immune cell response in *** cell-expressed PlIWL1 may be a potential target forthe devellopment of novel HCC treatment.

Betulinic acid, a natural product, was extracted from a broad range of Chinese medicinal herbs. Subsequent researches show when subjected to betulinic acid, tumours derived from multiple tissues manifest apoptotic features. This study aims to investigate the role of betulinic acid in apoptosis and autophagy of hepatocellular carcinoma(HCC) as well as the crosstalk between betulinic acid-induced apoptotic and autophagic cell death. We employed luciferase-tagged hepatoma cell line MHCC97L to establish orthotopic HCC implanted mice. Hepatocellular carcinoma cells PLC/PRF/5 and MHCC97L were used as in vitro models. The apoptotic mechanism behind betulinic acid function was examined using flow cytometry, immunoblotting, and real-time PCR techniques. Autophagic regulation was monitored by transmission electron microscopy, fluorescence spectroscopy, and MTT assays. We found that betulinicacid induced targeted degradation of inhibitor of apoptosis proteins(IAPs) family(cIAP-1, cIAP-2, XIAP and survivin),which are generally overexpressed in tumours. Meanwhile, autophagy was modulated in betulinic acid-treated hepatoma cellsas evidenced by induction of autophagic flux. Administration of autophagy inhibitor had no significant influence on molecules associated with apoptosis(IAPs), but could reversebetulinic acid-induced hepatoma cell death. Above all, the mode of action underlying betulinic acid-induced anti-cancer efficacy in vitro and in vivo was due, at least in part, to autophagy activation as well as pro-apoptotic responses mediated by modulation of IAPs family.

AIM： Autologous blood donation （ABD） is mainly used to reduce the use of banked blood. In fact, ABD can be regarded as acute blood loss. Would ABD 2-3 d before operation affect the CVP level and subsequently result in less blood loss during liver resection was to be ***： Eighty-four patients undergoing living donor left hepatectomy were retrospectively divided as group Ⅰ （GⅠ） and group Ⅱ （GⅡ） according to have donated 250-300 mL blood 2-3 d before living donor hepatectomy or not. The changes of the intraoperative CVP, surgical blood loss,blood products used and the changes of perioperative hemoglobin （Hb） between groups were analyzed and compared by using Mann-Whitney ***： The results show that the intraoperative CVP changes between GⅠ （n = 35） and GⅡ （n = 49） up to graft procurement were the same, subsequently the blood loss,but ABD resulted in significantly lower perioperative Hb levels in ***： Since none of the patients required any blood products perioperatively, all the predonated bloods were discarded after the patients were discharged from the hospital. It indicates that ABD in current series had no any beneficial effects, in term of cost, lowering the CVP, blood loss and reduce the use of banked blood products, but resulted in significant lower Hb in perioperative period.

AIM: Hyperglycemia commonly seen in liver transplantation (LT) has often been attributed to the dextrose in the storage solution of blood transfusion products. The purpose of the study is to compare the changes of the blood glucose levels in transfused and non-transfused patients during LT. METHODS: A retrospective study on 60 biliary pediatric patients and 16 adult patients undergoing LT was carried out. Transfused pediatric patients were included in Group Ⅰ (GⅠ), those not transfused in Group Ⅱ (GⅡ). Twelve adult patients were not given transfusion and assigned to Group Ⅲ(GⅢ); whereas, four adult patients who received massive transfusion were assigned to Group Ⅳ (GⅣ). The blood glucose levels, volume of blood transfused, and the volume of crystalloid infused were recorded, compared and analyzed. RESULTS: Results showed that the changes in blood glucose levels during LT for both non-transfused and minimally transfused pediatric groups and non-transfused and massively-transfused adult groups were almost the same. CONCLUSION: We conclude that blood transfusion does not cause significant changes in the blood glucose levels in this study.
