Chronic intestinal ischemia and splanchnic blood-flow:Reference values and correlation with body-composition

AIM:To determine the splanchnic blood flow and oxygen uptake in healthy-subjects and patients and to relate the findings to ***:The total splanchnic blood flow (SBF) and oxygen uptake (SO2U) were measured in 20 healthy volunteers (10 women) and 29 patients with suspected chronic intestinal ischemia (15 women),age 40-85 years,prior to and after a standard *** method is based on the Fick principle using the continuous infusion of an indicator (99mTechnetium-labelled mebrofenin) and catheterization of an artery and the hepatic *** angiography of the intestinal arteries was performed during the same investigation.A whole-body dual-energy x-ray absorptiometry scan was performedin healthy volunteers to determine body ***:Angiography revealed no atherosclerotic lesions in the intestinal *** mean baseline SBF was 1087mL/min (731-1390),and this value increased significantly to 1787 mL/min after the meal in healthy volunteers (P<0.001).The baseline SBF in patients was 1080 mL/min,which increased to 1718 mL/min postprandially (P<0.001).The baseline SBF was independent of age,sex,lean body mass and percentage of body *** mean meal-induced increase in SBF was equal to 282mL/min + 5.4mL/min × bodyweight,(P=0.025).The SO2U in healthy volunteers and patients was 50.7mL/min and 48.0mL/min,respectively,and these values increased to 77.5 mL/min and 75mL/min postprandially,*** baseline and postprandial SO2U were directly related to lean body *** and sex exerted no impact on ***:A direct correlation between body weight and the postprandial increase in SBF was *** effect of body weight should be considered in the diagnosis of chronic intestinal ischemia.

Low-FODMAP diet reduces irritable bowel symptoms in patients with inflammatory bowel disease

To investigate the effect of a low-FODMAP diet on irritable bowel syndrome (IBS)-like symptoms in patients with inflammatory bowel disease (IBD).METHODSThis was a randomised controlled open-label trial of patients with IBD in remission or with mild-to-moderate disease and coexisting IBS-like symptoms (Rome III) randomly assigned to a Low-FODMAP diet (LFD) or a normal diet (ND) for 6 wk between June 2012 and December 2013. Patients completed the IBS symptom severity system (IBS-SSS) and short IBD quality of life questionnaire (SIBDQ) at weeks 0 and 6. The primary end-point was response rates (at least 50-point reduction) in IBS-SSS at week 6 between groups; secondary end-point was the impact on quality of ***-nine patients, 67 (75%) women, median age 40, range 20-70 years were randomised: 44 to LFD group and 45 to ND, from which 78 patients completed the study period and were included in the final analysis (37 LFD and 41 ND). There was a significantly larger proportion of responders in the LFD group (n = 30, 81%) than in the ND group (n = 19, 46%); (OR = 5.30; 95%CI: 1.81-15.55, P < 0.01). At week 6, the LFD group showed a significantly lower median IBS-SSS (median 115; inter-quartile range [IQR] 33-169) than ND group (median 170, IQR 91-288), P = 0.02. Furthermore, the LFD group had a significantly greater increase in SIBDQ (median 60, IQR 51-65) than the ND group (median 50, IQR 39-60), P < *** a prospective study, a low-FODMAP diet reduced IBS-like symptoms and increased quality of life in patients with IBD in remission.

A proteomic analysis of the effect of radiation therapy on wound healing in women reconstructed with the TRAM flap

The incidence of breast cancer is still increasing, and with improved cancer treatment, more women live longer with the side effects of their treatment. The response of normal tissue to radiation continues for years after the treatment is completed. The influence of radiotherapy on the outcome of breast reconstructtive surgery remains unpredictable. The combination of two surgical sites of which one is previously irradiated, is rarely encountered in humans and thus compiles a unique opportunity to study the implications of irradiation followed by surgery. The aim of this study was to examine the long-term effect of radiation therapy on the proteins expressed in the wound tissue after a breast reconstruction. Ten patients were included in the study, all treated with radiotherapy after a mastectomy and breast reconstruction with a contralateral pedicled TRAM flap. Expanded poly-tetrafluoretylene polymer tubes were implanted for 10 days, subcutaneously, below the inframammary fold and below the donor site. The protein from the newly synthesized granulation tissue in the tubes was extracted and analyzed for differences in protein expression with 2D gel electrophoresis and mass spectrometry. A total of 676 proteins were detected;of these, 4 proteins changed significantly and were successfully identified. TPM4 and APOA4 from the radiation treated tissue were shown to be significantly decreased, whereas IGKC and VDAC1 were found to be significantly increased. The proteomic technique combined with the ePTFE tube wound model can elucidate some of the molecular alterations in the wound healing induced by radiation therapy. The protein modifications of TPM4, APOA4, IGKC and VDAC1 may influence the cell proliferation, apoptosis and the inflammation of the tissue repair process.
