Positive attitudes towards priority setting in clinical guidelines among Danish general practitioners: A web based survey

Aims: Increasing focus on improvement and optimisation of the treatment in primary care and reduction of healthcare costs emphasize the need to understand which factors determines adherence and non-adherence to clinical guidelines. In the present study, we examined attitudes towards clinical guidelines in Danish general practitioners (GPs). Methods: We conducted a survey among Danish GPs from all five regions of Denmark. In total, 443 GPs answered the web-based questionnaire that contained questions about attitudes and barriers to clinical guidelines. Results: More than 90% of the GPs reported that they have good k.owledge of the guidelines and in general follows the guidelines. A majority of the GPs (81%) found it acceptable that economic considerations are part of the guidelines. The most important factors for non- adherence to guidelines were “need of adjustment to clinical practice” and “lack.of confidence in guidelines”. The attitudes to clinical guidelines were not significantly associated with practice characteristics such as gender, years of experience, practice organisation and localisation. Conclusions: Our findings show that clinical guidelines are an integrated or internalised part of everyday practice among GPs in Denmark. Furthermore, the findings indicate that Danish GPs are positive towards applying priority setting in their practice. This is decisive in the light of rising healthcare costs due to development of new expensive technologies and ageing populations that puts pressure on the healthcare system in general and primary healthcare in particular.

The abundance of ammonia-oxidizing bacteria and archaea and their amo A genes from the aerobic activated sludge tank.,recycled sludge and anaerobic digesters of a full-scale wastewater treatment plant(WWTP)was *** chain reaction and denaturing gradient gel electrophoresis were used to generate diversity profiles,which showed that each population had a consistent profile although the abundance of individual members *** the aerobic tank.,the ammonia-oxidizing bacterial(AOB)population was more than 350 times more abundant than the ammonia-oxidizing archaeal(AOA)population,however in the digesters,the AOA population was more than 10 times more *** the activity of the amo A gene expression of the two populations using RT-PCR also showed that the AOA amo A gene was more active in the digesters than in the activated sludge *** batch reactors and dd PCR,amo A activity could be measured and it was found that when the AOB amo A activity was inhibited in the anoxic reactors,the expression of the AOA amo A gene increased *** suggests that these two populations may have a cooperative relationship for the oxidation of ammonia.

Short turn radius colonoscope in an anatomical model: Retroflexed withdrawal and detection of hidden polyps

AIM: To evaluate the new Retro View^(TM) colonoscope and compare its ability to detect simulated polyps "hidden" behind colonic folds with that of a conventional colonoscope, utilizing anatomic colon ***: Three anatomic colon models were prepared,with twelve simulated polyps "hidden" behind haustral folds and five placed in easily viewed locations in each model. Five blinded endoscopists examined two colon models in random order with the conventional or Retro View^(TM) colonoscope, utilizing standard withdrawal technique. The third colon model was then examined with the Retro View^(TM) colonoscope withdrawn initially in retroflexion and then in standard withdrawal. Polyp detection rates during standard and retroflexed withdrawal of the conventional and Retro View^(TM) colonoscopes were determined. Polyp detection rates for combined standard and retroflexed withdrawal(combination withdrawal) with the Retro View^(TM) colonoscope were also ***: For hidden polyps, retroflexed withdrawal using the Retro View^(TM) colonoscope detected more polyps than the conventional colonoscope in standard withdrawal(85% vs 12%, P = 0.0001). For hidden polyps, combination withdrawal with the Retro View^(TM) colonoscope detected more polyps than the conventional colonoscope in standard withdrawal(93% vs 12%, P ≤ 0.0001). The Retro View^(TM) colonoscope in "combination withdrawal" was superior to other methods in detecting all(hidden + easily visible) polyps, with successful detection of 80 of 85 polyps(94%) compared to 28(32%) polyps detected by the conventional colonoscope in standard withdrawal(P < 0.0001) and 67(79%) polyps detected by the Retro View^(TM) colonoscope in retroflexed withdrawal alone(P < 0.01). Continuous withdrawal of the colonoscope through the colon model while retroflexed was achieved by all endoscopists. Complex topography buffers forests against deforestation in mountainous regions. However, it is unk.own if terrain also shapes forest distribution in lowlands where human impacts are lik.ly to be less constrained by terrain. In such regions, if important at all, to- pographic effects will depend on cultural-historical factors and thus be human-driven （an- thropogenic） rather than natural, except in regions where the general climate or extreme soils limit the occurrence of forests. We used spatial regression modeling to assess the extent to which topographic factors explain forest distribution （presence-absence at a 48x48 m resolu- tion） in a lowland agricultural region （Denmark. 43,075 k.2） at regional and landscape scales （whole study area and 10x10 k. grid cells, respectively）, how landscape-scale for- est-topography relationships vary geographically, and which potential drivers （topographic heterogeneity, forest cover, clay content, coastal/inland location） determine this geographic heterogeneity. Given a moist temperate climate and non-extreme soils all landscapes in Denmark.would naturally be largely forest covered, and any topographic relationships will be totally or primarily human-driven. At regional scale, topographic predictors explained only 5% of the distribution of forest. In contrast, the explanatory power of topography varied from 0%-61% at landscape scale, with clear geographic patterning. Explanatory power of topog- raphy at landscape scale was moderately dependent on the potential drivers, with topog- raphic control being strongest in areas with high topographic heterogeneity and little forest cover. However, these conditioning effects were themselves geographically variable. Our findings show that topography by shaping human land-use can affect forest distribution even in flat, lowland regions, but especially via localized, geographically variable effects.

Bait mark.rs are indispensable for ecological research but in small mammals,most mark.rs are invasive,expensive and do not enable quantitative analyses of ***-iophenoxic acid(Et-IPA)is a non-toxic,quantitative bait mark.r,which has been used for studying bait uptak. in several carnivores and *** developed a bait with Et-IPA,assessed its palatability to common voles(Microtus arvalis),and determined the dose-residue-relation for this important agricultural pest rodent ***-IPA concentrations of 40 to 1280μg Et-IPA per g bait were applied to wheat using sunﬂower oil or polyethylene glycol 300 as potential *** a laboratory study,common voles were offered the bait and blood samples were collected 1,7,and 14 days after *** samples were analyzed with LC-ESI-MS/MS for blood residues of ***ﬂower-oil was the most suitable bait ***-IPA seemed to be palatable to common voles at all test ***-dependent residues could be detected in blood samples in a dose-dependent manner and up to 14 days after uptak. enabling generation of a calibration curve of the dose-residue ***-IPA was present in common vole blood for at least 14 days,but there was dissipation by 33–37%depending on ***-IPA meets many criteria for an“ideal”quantitative bait mark.r for use in futureﬁeld studies on common voles and possibly other small mammal species.
