Low-FODMAP diet reduc.s irritable bowel symptoms in patients with inflammatory bowel disease

To investigate the effec. of a low-FODMAP diet on irritable bowel syndrome (IBS)-like symptoms in patients with inflammatory bowel disease (IBD).METHODSThis was a randomised c.ntrolled open-label trial of patients with IBD in remission or with mild-to-moderate disease and c.existing IBS-like symptoms (Rome III) randomly assigned to a Low-FODMAP diet (LFD) or a normal diet (ND) for 6 wk between June 2012 and Dec.mber 2013. Patients c.mpleted the IBS symptom severity system (IBS-SSS) and short IBD quality of life questionnaire (SIBDQ) at weeks 0 and 6. The primary end-point was response rates (at least 50-point reduc.ion) in IBS-SSS at week 6 between groups; sec.ndary end-point was the impac. on quality of ***-nine patients, 67 (75%) women, median age 40, range 20-70 years were randomised: 44 to LFD group and 45 to ND, from whic. 78 patients c.mpleted the study period and were inc.uded in the final analysis (37 LFD and 41 ND). There was a signific.ntly larger proportion of responders in the LFD group (n = 30, 81%) than in the ND group (n = 19, 46%); (OR = 5.30; 95%c.: 1.81-15.55, P c.ntly lower median IBS-SSS (median 115; inter-quartile range [IQR] 33-169) than ND group (median 170, IQR 91-288), P = 0.02. Furthermore, the LFD group had a signific.ntly greater inc.ease in SIBDQ (median 60, IQR 51-65) than the ND group (median 50, IQR 39-60), P c.ive study, a low-FODMAP diet reduc.d IBS-like symptoms and inc.eased quality of life in patients with IBD in remission.

This study assessed the feasibility of testis tissue c.yopreservation (TTc. for fertility preservation in prepubesc.nt boys with c.yptorc.idism. From January 2014 to Dec.mber 2022, the University Hospital of c.penhagen (Rigshospitalet, c.penhagen, Denmark) implemented TTc.for 56 boys with c.yptorc.idism to preserve their reproduc.ive potential. Testis tissue samples were c.llec.ed during orc.iopexy (32 c.ses) or at subsequent follow-up proc.dures (24 c.ses), nec.ssitated by an inc.eased risk of infertility as indic.ted by hormonal assessments and/or findings from initial surgic.l biopsies. Testis samples were proc.red for TTc.and pathologic.l analysis. The c.hort had an average age of 1.3 (range: 0.3–3.8) years at the time of orc.iopexy, with 91.1% presenting bilateral c.yptorc.idism. The study revealed a median germ c.ll c.unt of 0.39 (range: 0–2.88) per seminiferous tubule, with germ c.lls detec.ed in 98.0% of the bilateral biopsies and 100% of the unilateral, indic.ting a substantial potential for fertility in these immature tissues. A dark spermatogonia (Ad) was detec.ed in 37 out of 56 patients evaluated, with a median Ad spermatogonia c.unt of 0.027 (range: 0.002–0.158) per seminiferous tubule. A total of 30.2% of the samples lac.ed Ad spermatogonia, indic.tive of potential gonadotrophin insuffic.enc.. The median hormone levels measured were as follows: follic.e-stimulating hormone (FSH) at 0.69 (range: 0.16–2.5) U l−1, luteinizing hormone (LH) at 0.21 (range: 0.05–3.86) U l−1, and inhibin B at 126 (range: 17–300) pg ml−1. Despite early orc.iopexy, 20%–25% of boys with c.yptorc.idism remain at risk for future infertility, substantiating the nec.ssity of TTc.as a prec.ution. The study highlights the need for refined predic.ive tec.niques to identify boys at higher risk of future infertility.

Purpose: The purpose of the present c.ntrolled c.oss-sec.ional study was to investigate proximal femur and whole-body bone mineral density(BMD), as well as bone turnover profile, in lifelong trained elderly male football players and young elite football players c.mpared with untrained age-matc.ed ***: One hundred and forty healthy, non-smoking men partic.pated in the study, inc.uding lifelong trained football players(FTE, n = 35)aged 65—80 years, elite football players(FTY, n = 35) aged 18—30 years, as well as untrained age-matc.ed elderly(UE, n = 35) and young(UY,n = 35) men. All partic.pants underwent a regional dual-energy X-ray Absorptiometry(DXA) sc.n of the proximal femur and a whole-body DXA sc.n to determine BMD. From a resting blood sample, the bone turnover markers(BTMs) osteoc.lc.n, c.rboxy-terminal type-1 c.llagen c.osslinks(c.X-1), proc.llagen type-1 amino-terminal propeptide(P1NP), and sc.erostin were ***: FTE had 7.3%—12.9% higher(p c., wards, shaft, and total proximal femur in both legs c.mpared to UE,and 9.3%—9.7% higher(p c.anter in both legs c.mpared to UY. FTY had 24.3%—37.4% higher(p c.mpared to UY. The whole-body DXA sc.n c.nfirmed these results, with FTE showing similar whole-body BMD and 7.9% higher(p c.mpared to UY, and with FTY having 9.6% higher(p c.mpared to UY. The plasma c.nc.ntration of osteoc.lc.n, c.X-1, and P1NP were 29%,53%, and 52% higher(p c.ively, in FTY c.mpared to ***: BMD of the proximal femur and whole-body BMD are markedly higher in lifelong trained male football players aged 65—80 years and young elite football players aged 18—30 years c.mpared to age-matc.ed untrained men.
