The role of genetic factors in HBV-related HCC:perspectives from local genetic backgrounds and clinical epidemiology

Aim:Chronic persistent hepatitis B virus carriers are generally asymptomatic until the advanced stage of the ***"Hepatitis B-Carrier Clinics"of Chang Gung Memorial Hospital has been using alpha-fetoprotein(AFP)and liver ultrasound for early detection of hepatocellular carcinoma(HCC)in hepatitis B surface antigen(HBsAg)carriers since ***:We evaluated the results of surveillance between 1980 and 2012 by collecting clinic data,matched cancer registry status,and national mortality database ***:Of 15,235 HBsAg carriers,238 instances of HCC(1.5%or 156.2/100,000 person-years)were detected over a mean follow-up period of 10.0±7.6 *** were more men(89.1%)and patients with liver cirrhosis(70.2%)in the HCC group(P<0.001),and both the initial and maximal alanine aminotransferase(ALT)levels were higher in this group(P<0.001).One hundred and thirty cases of HCC(54.6%)were identified during regular follow-up sessions,55(23.1%)were detected after the regular schedule had lapsed("out-of-schedule"),and 53(22.3%)were lost to follow-up *** mean tumor size was smaller in the regular group than in the out-of-schedule group(2.72 cm vs.4.59 cm,P<0.001),and the survival rate was higher(43.8%vs.30.9%,P<0.001).Conclusion:The incidence of HCC was relatively low in the HBsAg-Carrier Clinics *** for early diagnosis of HCC improved the survival of high-risk HBsAg *** ensure cost-effectiveness,we suggest using different screening strategies according to the individual risk of hepatocarcinogenesis.

Effects of sex and generation on hepatitis B viral load in families with hepatocellular carcinoma

AIM To explore factors associated with persistent hepatitis B virus (HBV) infection in a cohort of hepatocellular carcinoma (HCC)-affected families and then investigate factors that correlate with individual viral load among hepatitis B surface antigen (HBsAg)-positive relatives. METHODS questionnaire. Demographics, relationship to index case, HBsAg status of mothers and index cases were evaluated for association with the HBV persistent infection or viral load by generalized estimating equation analysis. RESULTS Among 729 relatives enrolled, parent generation (P = 0.0076), index generation (P = 0.0044), mothers positive for HBsAg (P = 0.0007), and HBsAg-positive index cases (P = 5.98 x 10(-8)) were associated with persistent HBV infection. Factors associated with HBV viral load were evaluated among 303 HBsAg-positive relatives. Parent generation (P = 0.0359) and sex (P = 0.0007) were independent factors associated with HBV viral load. The intra-family HBV viral load was evaluated in families clustered with HBsAg-positive siblings. An intra-family trend of similar HBV viral load was found for 27 of 46 (58.7%) families. Male offspring of HBsAg-positive mothers (P = 0.024) and older siblings were associated with high viral load. CONCLUSION Sex and generation play important roles on HBV viral load. Maternal birth age and nutritional changes could be the reasons of viral load difference between generations.

Contribution of the Alternative Respiratory Pathway to PSII Photoprotection in C3 and C4 Plants

The mechanism by which the mitochondrial alternative oxidase （AOX） pathway contributes to photosystem II （PSII） photoprotection is in dispute. It was generally thought that the AOX pathway protects photosystems by dissipating excess reducing equivalents exported from chloroplasts through the malate/oxaloacetate （Mal/OAA） shuttle and thus preventing the over-reduction of chloroplasts. In this study, using the aoxla Arabidopsis mutant and nine other C3 and C4 plant species, we revealed an additional action model of the AOX pathway in PSII photoprotection. Although the AOX pathway contributes to PSII photoprotection in C3 leaves treated with high light, this contribution was observed to disappear when photorespiration was suppressed. Disruption or inhibition of the AOX pathway significantly decreased the photorespiration in C3 leaves. Moreover, the AOX pathway did not respond to high light and contributed little to PSII photoprotection in C4 leaves possessing a highly active Mal/OAA shuttle but with little photorespiration. These results demonstrate that the AOX pathway contributes to PSII photoprotection in C3 plants by maintaining photo- respiration to detoxify glycolate and via the indirect export of excess reducing equivalents from chloro-plasts by the MaI/OAA shuttle. This new action model explains why the AOX pathway does not contribute to PSII photoprotection in C4 plants.

Effects of a Single-Dose of Mind-Body Practice on the Heart Rate Variability, Brain Activity and Shooting Performance in Archers: A Pilot Study

Objective: Factors affecting performance of archery are categorized into psychological, skill, and fitness perspectives. Increase in attention, relaxation and parasympathetic system activity is believed to enhance the shooting performance of archers during competition. This pilot study aims to investigate the effects of single-dose of tai Chi practice on archer’s performance and the feasibility of an investigation protocol under a competition scenario. Methods: This was a randomized controlled trial (RCT) with a crossover study design. Pre- and post-intervention measurements were conducted to evaluate the effects of tai Chi practice on the archer’s autonomic nervous system activity, attention and relaxation through electroencephalography investigation, and shooting performance when compared with stretching as control. Results: The protocol of the study was found to be feasible during a competition setting. No significant difference was detected in the outcome measures under the two intervention conditions. Participants after practicing tai Chi were found to improve significantly in their shooting performance (p = 0.021). Conclusion: This pilot study showed that a real-time measurement of autonomic nervous system and brain wave activities in the competition field was feasible and it did not hinder the archers’ performance. There was a significant improvement observed in the shooting performance of archers after tai Chi practice when compared with baseline. The findings of this pilot study provide the basis for the conduction of a larger RCT study to examine the potential beneficial effects of tai Chi practice on archers.
