High-Field MRI Contribution in Tolosa-Hunt Syndrome (THS). Report of Two Cases and Review of Literature

background: Tolosa Hunt Syndrome is an inflammatory condition of unknown etiology of the cavernous sinus and superior orbital fissure. because of the difficulty in establishing histological evidence, his diagnosis is based on a set of arguments established by the International Headache Society. MRI allows indirect visualization of the granuloma and plays a key role in diagnosis and follow-up. Aim: To illustrate High-field MRI contribution in Tolosa-Hunt Syndrome (THS). Cases Presentation: Two patients, a 25-year-old female and a 40-year-old male were recruited in this retrospective case report study at the Radiology service of Fann University Hospital (Dakar Senegal). The first patient has been suffering from a right retro-orbital pain and diplopia for 2 months and the second from a painful oculomotor nerve palsy for 3 months. blood tests, lumbar puncture, thyroid hormone levels and an infectious screen were done. Screening for converting enzymes, and serum antibodies were also done. They underwent a high field MRI (Siemens 1.5T) with T1, T2, FLAIR, T2*, diffusion b1000, TOF polygon, CISS 3D and T1 gadolinium sequences in the 3 planes space. No significant abnormality was detected in blood tests or CSF analysis. Screening for converting enzymes and serum antibodies screen were also negative. For each patient, MRI examinations showed a non tumoral thickening of the right cavernous sinus, suggesting a granulomatous involvement. Tolosa Hunt Syndrome was evoked firstly. They were put on corticotherapy at high doses with a spectacular regression of symptoms. The Criteria of the International Headache Society of THS were met in both patients. Conclusion: High-field MRI is a significant diagnostic tool in the assessment of painful ophthalmoplegia. It allows a direct visualization of the granuloma of the cavernous sinus and assesses its course throughout the disease.

In the early Toarcian stage of early Jurassic(～183 million years ago(Ma)),a marine mass extinction took place,represented by the T-OAE(Toarcian Ocean Anoxic Event).This event was marked by a rapid and ample(2.5-7‰) negative carbon-isotope excursion,the worldwide deposition of organic black shale,indicating *** this time,a period of global warming has been recognized based on faunal,floral and sedimentary *** the T-OAE is well studied in the marine realm,accompanying climatic and environmental change on the terrestrial ecosystem is poorly *** we use foliar morphologies,leaf mass per area(LMA) and CO concentration change associated with global mean land surface temperature(GMLST) to reconstruct the environmental conditions during Toarcian based on South China Xiangxi Flora materials from Hubei *** evidences showed that coincident with the negative carbon-isotope excursion p CO is drawn down by 900-1175 ppm and then elevated by 1,259-1441 ppm,and infer a global lower temperature and then higher temperature of 4.43-5.46 °C and 5.74-6.28 °*** and foliar morphologies results showed that the climate of bed b11 of the XIangxi Formation is a hot dry condition,beds A2,b10,b12 are warm and humid *** A-b is hot and bed b9 is a warm *** addition,both the pCO and temperature trends during the Toarcian in the eastern Danish and southern China showed a generally agreement with each ***,based on the GMLST from Hubei Province,we did not recognize a global cooling episode during the initial part of the carbon isotopic excursion.

ONCE in a restaurant specializing in vegetarian food in Hubei Province, I saw some dishes on the menu which did not sound like vegetarian food, such as chicken, duck, fish and pork. Only after I ordered and tasted did I understand that these were all made of bean products. Simply speaking, the Chinese vegetarian diet is comprised of grains, vegetables, bean products and fruits. Real-world cure rates for hepatitis C virus treatments that include simeprevir and/or sofosbuvir are comparable to clinical trial results

AIM To assess the real-world effectiveness and cost of simeprevir(SMV), and/or sofosbuvir(SOF)-based therapy for chronic hepatitis C virus(HCV) *** The real-world performance of patients treated with SMV/SOF ± ribavirin(RbV), SOF/RbV, and SOF/RbV with pegylated-interferon(PEG) were analyzed in a consecutive series of 508 patients with chronic HCV infection treated at a single academic medical center. Patients with genotypes 1 through 4 were included. Rates of sustained virological response-the absence of a detectable serum HCV RNA 12 wk after the end of treatment [sustained virological response(SVR) 12]-were calculated on an intention-to-treat basis. Costs were calculated from the payer's perspective using Medicare/Medicaid fees and Redbook Wholesale Acquisition Costs. Patient-related factors associated with SVR12 were identified using multivariable logistic *** SVR 12 rates were as follows: 86%(95%CI: 80%-91%)among 178 patients on SMV/SOF ± RbV; 62%(95%CI: 55%-68%) among 234 patients on SOF/RbV; and 78%(95%CI: 68%-86%) among 96 patients on SOF/PEG/RbV. Mean costs-per-SVR 12 were $174442(standard deviation: ± $18588) for SMV/SOF ± RbV; $223003(± $77946) for SOF/RbV; and $126496(± $31052) for SOF/PEG/RbV. Among patients on SMV/SOF ± RbV, SVR12 was less likely in patients previously treated with a protease inhibitor [odds ratio(OR): 0.20, 95%CI: 0.06-0.56]. Higher bilirubin(OR: 0.47, 95%CI: 0.30-0.69) reduced the likelihood of SVR12 among patients on SOF/RbV, while FIb-4 score ≥ 3.25 reduced the likelihood of SVR 12(OR: 0.18, 95%CI: 0.05-0.59) among those on SOF/PEG/RbV. CONCLUSION SVR 12 rates for SMV and/or SOF-based regimens in a diverse real-world population are comparable to those in clinical trials. Treatment failure accounts for 27% of costs.

目的:观察草酸艾司西酞普兰联合曲唑酮治疗抑郁症患者的临床疗效。方法:收集126例抑郁症患者,随机数字表法将患者分成三组,分别给予草酸艾司西酞普兰(A组,n=43),曲唑酮(b组,n=43)及草酸艾司西酞普兰联合曲唑酮(C组,n=43)治疗2个月,采用汉密尔顿焦虑(HAMA)量表和汉密尔顿抑郁(HAMD)量表评定治疗后疗效;使用副反应(TESS)量表检测药物的副作用;生活质量综合评定(GQOLI)量表检测患者治疗后的生活质量。结果:与治疗前相比,三组患者治疗后HAMA评分和HAMD评分均显著降低,差异具有统计学意义(Pb组相比,HAMA评分和HAMD评分降低更明显(Pb组相比,TESS评分降低明显,GQOLI评分升高更明显(P<0.05)。结论:草酸艾司西酞普兰联合曲唑酮能够有效改善患者焦虑和抑郁症状,联合用药效果显著优于各自单独用药,且药物不良反应少,生活质量改善更明显。

The Influence of Storage Conditions on the Microbial Quality of Daucus carots(Carrots)and Capsicum annuum(Green Pepper)

The influence of different storage conditions on the microbial quality carrot(Daucus carots)and green pepper(Capsicum annuum)was determined using standard microbiological method from day zero to day *** bacteria count for carrot stored at room temperature ranged from Log10Cfu/g3.22 to 7.45 and for carrot stored at refrigeration temperature ranged from Log10 Cfu/g *** bacteria count for green pepper stored at room temperature ranged from Log10Cfu/g 4.22 to 7.45 and for green pepper stored at room temperature ranged from 1.12 to 4.14 for refrigeration *** isolated includes ***(4%),bacillus sp.(8%),Pseudomonas(16%),Proteus vulgaris(4%),Staphylococcus sp.(28%),Klebsiella(8%),Salmonella(12%),Micrococcus sp.(12%)and Acinetobacter(8%).Fungal count for carrot at room temperature ranged from Log102.22 to 2.54 Cfu/g and 2.01 to 2.34 Cfu/g for refrigeration *** count for green pepper at room temperature ranged from Log10 Cfu/g 3.02 to and 7.45,Log101.81 Cfu/g to 3.34 for refrigeration *** isolated includes Penicillium(33.3%),Aspergillus(53.3%),and Candida(13.4%).Proximate composition indicates that moisture,ash,carbohydrates,lipid and fibre are lower at room temperature compared to refrigeration *** and storage duration have been said to affect the content of fruits and vegetables,therefore constant temperature and appropriate storage condition should be maintained.
