The role of serial physical examinations in the management of angioedema involving the head and neck:A prospective observational study

Objective:To elucidate the progression of angioedema of the head and neck with routine management and to assess the utility of serial physical exams and fiberoptic laryngos-copy in its management. br> Methods:This study was a prospective observational research. From 2013 to 2014, a prospec-tive observational study was conducted at a tertiary referral center. Forty patient were ap-proached, 7 refused, 33 (18e90 years old) were enrolled. Patients presented with angioedema involving the head and neck over a 12 month period were asked to participate in the study. Physical examination and fiberoptic laryngoscopy were performed at presentation and then repeated at least 1 h later. br> Results:Thirty-three patients with head and neck angioedema from any cause were enrolled (mean age 58, range 23e89 years). The upper lip was the most commonly involved site (58%). On reevaluation, 82% of patients reported subjective improvement in symptoms. Theassociation between subjective improvement and the physical exam, including fiberoptic laryngoscopy findings, was statistically significant (Pbr> Conclusion:In stable patients with angioedema of any head and neck subsite, self-reported symptoms are associated with clinical stability or improvement as assessed by physical signs and fiberoptic laryngoscopy. Patients’ symptoms may be an appropriate surrogate to monitor clinical status without the need for routine serial physical examinations or fiberoptic laryngos-copy, though further study is needed. br> Copyright a 2016 Chinese Medical Association. bicuculline-insensitive receptors for the inhibitory neurotransmitter γ-aminobutyric acid (GAbA), GAbAb receptors, are a distinct subclass of receptors that mediate depression of synaptic transmission and contribute to neu-ronal inhibition. When activated, these receptors reduce transmission at excitatory and inhibitory synapses, as a result of an increase in K+ conductance, or a decrease in voltage-dependent Ca2+ currents. They are also linked to G-proteins, or intracellular effector systems in a very complex manner. The recent development of highly specific and potent agonists and antagonists for these receptors has led to a much better understanding of their physiology and pharmacology, including their heterogeneity, as well as their molecular biology. Over the past year, expression and cloning studies have contributed to major advances in characterizing GAbAb receptor structure, with the discovery of the amino acid sequences of GAbAbRla/R1b splice variants and GAbAbR2 receptors. An efficient in vitro method for rapid vegetative propagation of bienertia sinuspersici, one of four terrestrial species of family Chenopodiaceae capable of performing C4 photosynthesis within a single cell, was developed. Cuttings of b. sinuspersici were used to examine the effects of Murashige and Skoog (MS) media strength and auxins on adventitious root formation. Half-strength MS medium was determined to be ideal for adventitious root formation in bienertia cuttings. Although cuttings cultured in medium containing 5.0 mg/L α-naphthalene acetic acid (NAA) promoted the highest number of adventitious roots, cuttings cultured in medium supplemented with 1.0 mg/L indole-3-butyric acid (IbA) produced the longest adventitious roots and had the highest survival rate upon transplanting to soil. Histological analysis revealed variations in the root anatomy generated by the various auxins which may affect adventitious root formation and subsequent establishment of cuttings in soil. Overall, the established procedure provides a simple and cost-effective means for the rapid propagation of the single-cell C4 species b. sinuspersici.

Clickers and Deep Learning： An Oxymoron？

The use of clicker technology as a tool for promoting learning, let along deep learning, is hotly debated and has its proponents and opponents. With the question still open, this paper examines the use of this technology in fostering critical thinking and other higher-order learning and team-building skills, in the context of two entry-level psychology courses. The results obtained indicate that from the perspectives of both the student and the instructor, the integration of this technology was positively perceived to enhance the learning process and the acquisition of these skills. It would seem these concepts are, after all, not antithetical.

Hepatitis due to hepatitis b virus(HbV) reactivation can be severe and potentially fatal, but is preventable. HbV reactivation is most commonly reported in patients receiving cancer chemotherapy, especially rituximabcontaining therapy for hematological malignancies and those receiving stem cell transplantation. All patients with hematological malignancies receiving anticancer therapy should be screened for active or resolved HbV infection by blood tests for hepatitis b surface antigen(Hbs Ag) and antibody to hepatitis b core antigen(antiHbc). Patients found to be positive for Hbs Ag should be given prophylactic antiviral therapy to prevent HbV reactivation. For patients with resolved HbV infection, no standard strategy has yet been established to prevent HbV reactivation. There are usually two options. One is pre-emptive therapy guided by serial HbV DNA monitoring, whereby antiviral therapy is given as soon as HbV DNA becomes detectable. However, there is little evidence regarding the optimal interval and period of monitoring. An alternative approach is prophylactic antiviral therapy, especially for patients receiving highrisk therapy such as rituximab, newer generation of anti-CD20 monoclonal antibody, obinutuzumab or hematopoietic stem cell transplantation. This strategy may effectively prevent HbV reactivation and avoid the inconvenience of repeated HbV DNA monitoring. Entecavir or tenofovir are preferred over lamivudine as prophylactic therapy. Although there is no well-defined guideline on the optimal duration of prophylactic therapy, there is growing evidence to recommend continuing prophylactic antiviral therapy for at least 12 mo after cessation of chemotherapy, and even longer for those who receive rituximab or who had high serum HbV DNA levels before the start of immunosuppressive therapy. AIMTo evaluate efficacy/safety of hepatitis C virus (HCV) protease inhibitor boceprevir with pegylated interferon (PEG-IFN) alfa and weight-based ribavirin (RbV) in a phase 3 trial. METHODSA prospective, multicenter, phase 3, open-label, single-arm study of PEG-IFN alfa, weight-based RbV, and boceprevir, with a PEG-IFN/RbV lead-in phase was performed. The HCV/human immunodeficiency virus coinfected study population included treatment naïve (TN) and treatment experienced (TE) patients. Treatment duration ranged from 28 to 48 wk dependent upon response-guided criteria. All patients had HCV Genotype 1 with a viral load > 10000 IU/mL. Compensated cirrhosis was allowed. Sample size was determined to establish superiority to historical (PEG-IFN plus RbV) rates in sustained viral response (SVR). RESULTSA total of 257 enrolled participants were analyzed (135 TN and 122 TE). In the TN group, 81.5% were male and 54.1% were black. In the TE group, 76.2% were male and 47.5% were white. Overall SVR12 rates (HCV RNA P = 0.002). Among the TN, SVR12 was 42.1% among whites and 27.4% among blacks (P = 0.09). CONCLUSIONThe trial met its hypothesis of improved SVR compared to historical controls but overall SVR rates were low. All-oral HCV treatments will mitigate these difficulties.
