A rat model of multicompartmental traumatic injury and hemorrhagic shock induces bone marrow dysfunction and profound anemia

Background:Severe trauma is associated with systemic inflammation and organ *** rodent trauma models are the mainstay of postinjury research but have been criticized for not fully replicating severe human *** aim of this study was to create a rat model of multicompartmental injury which recreates profound traumatic ***:Male Sprague-Dawley rats were subjected to unilateral lung contusion and hemorrhagic shock(LCHS),multicompartmental polytrauma(PT)(unilateral lung contusion,hemorrhagic shock,cecectomy,bifemoral pseudofracture),or na?ve ***,plasma toll-l ike receptor 4(TLR4),hemoglobin,spleen to body weight ratio,bone marrow(BM)erythroid progenitor(CFU-GeMM,BFU-e,and CFU-e)growth,plasma granulocyte colony-stimulating factor(G-CSF)and right lung histologic injury were assessed on day 7,with significance defined as p valuesesults:Polytrauma resulted in markedly more profound inhibition of weight gain compared to LCHS(p=0.0002)along with elevated plasma TLR4(per hemoglobin(penlarged spleen to body weight ratios(p=0.004).Both LCHS and PT demonstrated suppression of CFU-e and BFU-e growth compared to naive(plasma G-CSF was elevated in PT compared to both na?ve and LCHS(pemonstrated significant histologic right lung injury with poor alveolar wall integrity and interstitial ***:Multicompartmental injury as described here establishes a reproducible model of multicompartmental injury with worsened anemia,splenic tissue enlargement,weight loss,and increased inflammatory activity compared to a less severe *** may serve as a more effective model to recreate profound traumatic injury to replicate the human inflammatory response postinjury.

Background Resuscitation can sometimes be futile and making a do-not-resuscitate(DNR)decision is in the best interest of the *** electronic poor outcome screening(ePOS)score was developed to predict 6-month poor outcomes of critically ill *** explored the diagnostic accuracy of the ePOS score in predicting DNR decisions in the intensive care unit(ICU).Methods This study was conducted at the ICU of a tertiary referral hospital in Saudi Arabia between March and May ***,we calculated ePOS scores for all eligible consecutive admissions after 48 h in the ICU and recorded the DNR *** ability of the score to predict DNR was explored using logistic ***'s ideal cut-off value was calculated using the DeLong method,and different diagnostic accuracy measures were generated with corresponding 95%confidence intervals(CIs).Results We enrolled 857 patients,125 received a DNR order and 732 did *** average ePOS score of DNR and non-DNR patients was 28.2±10.7 and 15.2±9.7,*** score,as a predictor of DNR order,had an area under receiver operator characteristic(AUROC)curve of 81.8%(95%CI:79.0 to 84.3,Pen's ideal cut-off value>17 was associated with a sensitivity of 87.2(95%CI:80.0 to 92.5,Pecificity of 63.9(95%CI:60.3 to 67.4,Pe predictive value of 29.2(95%CI:24.6 to 33.8,Pegative predictive value of 96.7(95%CI:95.1 to 98.3,Pe ePOS score performed well as a diagnostic test for patients who will be labeled as DNR during their ICU stay.A cut-off score>17 may help guide clinical decisions to withhold or commence resuscitative measures.

Background:Physiological and biochemical processes across tissues of the body are regulated in response to the high demands of intense physical activity in several occupations,such as firefighting,law enforcement,military,and sports.A better understanding of such processes can ultimately help improve human performance and prevent illnesses in the work ***:To study regulatory processes in intense physical activity simulating real-life conditions,we performed a multi-omics analysis of 3 biofluids(blood plasma,urine,and saliva)collected from 11 wildland firefighters before and after a 45 min,intense exercise *** profiles ***-exercise were compared by Student’s t-test followed by pathway analysis and comparison between the different omics ***:Our multi-omics analysis identified and quantified 3835 proteins,730 lipids and 182 metabolites combining the 3 different types of *** blood plasma analysis revealed signatures of tissue damage and acute repair response accompanied by enhanced carbon metabolism to meet energy *** urine analysis showed a strong,concomitant regulation of 6 out of 8 identified proteins from the renin-angiotensin system supporting increased excretion of catabolites,reabsorption of nutrients and maintenance of fluid *** saliva,we observed a decrease in 3 pro-inflammatory cytokines and an increase in 8 antimicrobial peptides.A systematic literature review identified 6 papers that support an altered susceptibility to respiratory ***:This study shows simultaneous regulatory signatures in biofluids indicative of homeostatic maintenance during intense physical activity with possible effects on increased infection susceptibility,suggesting that caution against respiratory diseases could benefit workers on highly physical demanding jobs.

The application of a gate voltage to control the superconducting current flowing through a nanoscale superconducting constriction,named as gate-controlled supercurrent(GCS),has raised great interest for fundamental and technological *** gain a deeper understanding of this effect and develop superconducting technologies based on it,the material and physical parameters crucial for the GCS effect must be ***-down fabrication protocols should also be optimized to increase device scalability,although studies suggest that top-down fabricated devices are more resilient to show a ***,we investigate gated superconducting nanobridges made with a top-down fabrication process from thin films of the noncentrosymmetric superconductor niobium rhenium with varying ratios of the constituents(NbRe).Unlike other devices previously reported and made with a top-down approach,our NbRe devices systematically exhibit a GCS effect when they were fabricated from NbRe thin films with small grain size and etched in specific *** observations pave the way for the realization of top-down-made GCS devices with high *** results also imply that physical parameters like structural disorder and surface physical properties of the nanobridges,which can be in turn modified by the fabrication process,are crucial for a GCS observation,providing therefore also important insights into the physics underlying the GCS effect.

Gastric cancer is a leading cause of cancer incidence and death *** with advanced gastric cancer benefit from a multi-modality treatment *** oncologic resection with negative margins and complete lymphadenectomy plays a crucial role in long-term survival for patients with resectable *** utilization of minimally invasive techniques for gastric cancer has been slowly increasing and is proving to be both technically and oncologically *** chemotherapy is the current standard of care for advanced gastric cancer.A variety of chemotherapy regimens have been used with the combination of docetaxel,oxaliplatin,5-fluorouracil,and leucovorin being the current recommendation given its superior ability to induce a complete pathologic response and prolong *** use of radiation has been more controversial with its optimal place in the treatment sequence being *** are current ongoing studies assessing the impact of radiation as an adjunct or in place of *** treatments(e.g.,trastuzumab for human epidermal growth factor receptor 2 positive tumors and pembrolizumab for programmed death-ligand 1 positive tumors)are showing promise and are part of a continued emphasis on individualized *** chemotherapy may also play a role in preventing peritoneal recurrences for patients with high risk *** treatment of patients with advanced gastric cancer in the West continues to advance and improve with a better understanding of optimal treatment sequences and the utilization of personalized treatment regimens.

Optimizing outcomes for patients with gastric cancer peritoneal carcinomatosis

Peritoneal carcinomatosis (PC) from gastric cancer has traditionally been considered a terminal progression of the disease and is associated with poor survival out-comes. Positive peritoneal cytology similarly worsens the survival of patients with gastric cancer and treatment options for these patients have been limited. Recent ad-vances in multimodality treatment regimens have led to innovative ways to care for and treat patients with this disease burden. One of these advances has been to use neoadjuvant therapy to try and convert patients with positivecytologyorlow-volume PC to negative cytolo-gy with no evidence of active peritoneal *** strategies include the use of neoadjuvant systemic chemotherapy alone,using neoadjuvant laparoscopic heated intraper itoneal chemotherapy(NLHIPeC)after systemic chemotherapy,or using neoadjuvant intra-peritoneal and systemic chemother apy(NIPS)in a bi-dir ectional manner. For patients with higher volume PC,cytoreductive surgery (CRS) and hyperthermic intrape-ritoneal chemotherapy(HIPeC)have been mainstays of treatment. When used together, CRS and HIPeC can improve overall outcomes in properly selected patients,but overall survival outcomes remain unacceptably *** extent of peritoneal disease, commonly measured by the peritoneal carcinomatosis index (PCI), and the com-pleteness of cytor eduction,has been shown to greatly impact outcomes in patients undergoing CRS and *** uses of NLHIPeC and NLHIPeC plus NIPS have both been shown to decrease the PCI and thus increase the opportunity for complete cytoreduction. Association between high deductible health plan coverage and age at pediatric umbilical hernia repair

Background High deductible health plans(HDHPs)are associated with the avoidance of both necessary and unnecessary *** hernia repair(UHR)is a procedure that is frequently unnecessarily performed in young children,contrary to best practice *** hypothesized that children with HDHPs,as compared with other types of commercial health plans,are less likely to undergo UHR before 4 years of age but are also more likely to have UHR delayed beyond 5 years of *** Children aged 0-18 years old residing in metropolitan statistical areas(MSAs)who underwent UHR in 2012-2019 were identified in the IBM Marketscan Commercial Claims and encounters Database.A quasi-experimental study design using MSA/year-level HDHP prevalence among children as an instrumental variable was employed to account for selection bias in HDHP ***-stage least squares regression modeling was used to evaluate the association between HDHP coverage and age at *** A total of 8601 children were included(median age 5 years,IQR 3-7).Univariable analysis revealed no differences between the HDHP and non-HDHP groups in the likelihood of UHR being performed before 4 years of age(27.7%vs 28.7%,p=0.37)or after 5 years of age(39.8%vs 38.9%,p=0.52).Geographical region,metropolitan area size,and year were associated with HDHP *** variable analysis demonstrated no association between HDHP coverage and undergoing UHR atears of age(p=0.76)or>5 years of age(p=0.87).Conclusions HDHP coverage is not associated with age at pediatric *** studies should investigate other means by which UHRs in young children can be avoided.

Behavioral interactions between a coexisting rodent Micaelamys namaquensis and macroscelid elephantulus myurus

A number of mechanisms are known to influence coexistence in small mammal communities. However, the role ofbehavioral interactions in promoting species coexistence is under-represented in the literature. We studied the behavioral interactionsof two coexisting small mammals, the Namaqua rock mouse Micaelamys namaquensis (Rodentia) and the Rock sengi elephantulusmyurus (Macroscelidae), which have high dietary and microhabitat overlap. Using wild-caught individuals, intra- andinterspecific dyadic encounters were staged in tanks on a neutral rocky outcrop. Interspecific dyads displayed significantly moreactive avoidance behavior. There was no support for the prediction that the larger, more specialist species, e. myurus, is behaviorallydominant over M. namaquensis. Intraspecific encounters of e. myurus showed significantly more passive avoidance andamicable behavior, whereas the behavior of M. namaquensis did not differ between intra- and inter-specific encounters, both ofwhich were characterized by low levels of aggression and amicability. Thus, due to the lack of aggression, direct competition appearsweak and instead mutual avoidance may reduce or minimize interspecific interactions, potentially promoting coexistence.
