Postpa.tum Depression: Overcoming Menta. Hea.th Cha.lenges during Pa.enthood

Postpa.tum depression a.fects pa.ents a.ross a.l socia., economic, a.d ra.ia. groups. While there a.e ma.y fa.tors tha. ma. contribute to depressive symptoms in the postpa.tum period, under-resourced popula.ions a.d ethnic minorities a.e documented to experience higher ra.es. Tra.itiona. psychothera.eutic methods ha.e shown success in mitiga.ing depressive symptoms;however, these trea.ments a.e not a.wa.s a.cessible a.d/or utilized, pa.ticula.ly in popula.ions with the highest need. Mind-body interventions ha.e become increa.ingly popula. in a.dressing perina.a. menta. hea.th, a.d new a.d innova.ive a.proa.hes a.e currently being eva.ua.ed to determine their effica.y with respect to trea.ing postpa.tum depression. This scoping review highlights published clinica. tria.s, interventions, meta.a.a.yses, a.d reviews conducted in the la.t 20 yea.s a.d exa.ines how ea.h method a.plies to meet the needs of postpa.tum pa.ents, a.d pa.ticula.ly under-resourced popula.ions.

Pregna.t Women’s Perceptions of Pa.ient-Provider Communica.ion for Hea.th Beha.ior Cha.ge during Pregna.cy

a.m: The prima.y a.m of the project wa. to conduct focus groups with pregna.t women to exa.ine their perceptions on pa.ient a.d hea.th ca.e provider (HCP) communica.ion during prena.a. visits perta.ning to hea.th beha.iora. cha.ge. In pa.ticula., to determine wha. types of communica.ion fa.ilita.e or prevent pa.ient enga.ement a.d a.herence to certa.n hea.th beha.iors rela.ed to smoking cessa.ion, enga.ement in physica. a.tivity, hea.thy ea.ing a.d hea.thy weight ga.n, a.d stress ma.a.ement. Methods: Pa.ticipa.ts were recruited from the obstetric a.d midwifery clinics a. the University of Colora.o Hospita.. Twenty-four pregna.t, English-spea.ing women between the a.es of 18 a.d 46 yea.s old, the ma.ority of which ha. full hea.th insura.ce covera.e, pa.ticipa.ed in one of three focus groups tha. were conducted. The tra.scripts were coded for themes a.d pa.terns. Results identified numerous current pra.tices of HCPs, fa.ilita.ors a.d ba.riers in ca.e, a.d pa.ient recommenda.ions rela.ed to effective pa.ient-provider communica.ion. Results: Overa.l ma.y women received ba.ic informa.ion a.out most hea.th beha.iors (i.e. hea.thy ea.ing, physica. a.tivity, a.d smoking cessa.ion) with the exception of stress ma.a.ement from their HCPs via.their introductory informa.ion pa.ket. However, typica.ly there wa. no follow-up beyond receipt of the pa.ket. a. a.result, women sought informa.ion online from numerous sources. Unfortuna.ely, this informa.ion often conflicted with HCP provided informa.ion, a. did the informa.ion provided from multiple HCPs in group ca.e settings. a.ma.or fa.ilita.or of beha.iora. cha.ge perta.ned to building trust a.d ra.port a. it directly enha.ced the perceived qua.ity of pa.ient-provider communica.ion on prena.a. hea.th beha.iors. a.ross a.l beha.iors, women voiced the need for a.a.la.le resources tha. were credible a.d referenced by their HCPs. However, less is known a.out the use of a.tena.a. physica. a.tivity a. a.preventive moda.ity for depression during the perina.a. period. The objective of the present study wa. to determine if there is a. a.socia.ion between a.tena.a. physica. a.tivity a.d PPD. a.cross-sectiona. study using the 2007-2008 Pregna.cy Risk a.sessment Monitoring System (PRa.S) da.a.from Colora.o a.d North Ca.olina.(N = 6026) wa. conducted. PRa.S self-reported da.a.a.e from a.la.ge ra.domized sa.ple collected by the CDC tha. a.sesses ma.erna. demogra.hic, socio-economic, a.d prepregna.cy a.d perina.a. beha.iors. Multiva.ia.le logistic regression wa. used to exa.ine the rela.ionship between a.tena.a. sedenta.y beha.ior a.d PPD. Upon a.justing for ma.erna. a.e, educa.ion, ra.e, ma.ita. sta.us, pa.ity, stress, smoking, drinking, a.d prena.a. ca.e utiliza.ion, women who did not enga.e in a.tena.a. physica. a.tivity were 1.34 times more likely to screen positive for PPD tha. women who exercised 5 or more da.s per week [OR 1.34;95% CI: (1.04, 1.74);p = 0.03]. These findings suggest tha. not enga.ing in a.tena.a. exercise ma. be a.socia.ed with a. increa.e risk of PPD. Further resea.ch is wa.ra.ted to better understa.d the effects of a.tena.a. sedenta.y beha.ior on PPD a.d the potentia. use of physica. a.tivity a. a.preventive moda.ity for PPD.

Objective: Pregna.t women often report a.la.k of knowledge concerning the sa.ety of exercising during pregna.cy. Hea.thca.e providers pla. a. integra. role in providing pregna.t women with the necessa.y knowledge to promote a.tena.a. physica. a.tivity. Thus, the objective of this study wa. to a.sess hea.thca.e providers’ beliefs, a.titudes, knowledge a.d pra.tices rela.ed to a.tena.a. physica. a.tivity counseling. Study Design: 188 Providers (i.e. obstetricia.s, midwives, a.d fa.ily medicine physicia.s) completed a.39 closed-item survey. Cha.a.teristics a.ong hea.thca.e providers’ physica. a.tivity counseling pra.tices a. well a. belief, a.titudes a.d knowledge were explored. Results: The ma.ority of a.l providers a.reed tha. physica. a.tivity during pregna.cy will result in numerous improved hea.th outcomes for mother a.d ba.y. a.proxima.ely ha.f of the providers (48%, n = 89) were not fa.ilia. with the current na.iona. guides recommending tha. women free of obstetric complica.ions should enga.e in a. lea.t 150 minutes of exercise per week. Only 43% of providers believed their pa.ients followed the a.vice they a.e given a.out physica. a.tivity. Over ha.f of the providers reported tha. they provide in-office physica. a.tivity counseling, a.d FMs provide individua.ized counseling less often tha. OBs a.d CNMs (i.e. 33%, 60%, a.d 65%, respectively;p = 0.0014). Importa.tly, 17% (n = 31) of providers reported tha. they never received professiona. tra.ning in a.tena.a. physica. a.tivity counseling a.d of those tha. did receive tra.ning, 69% (n = 107) cla.med their tra.ning wa. “fa.r” or “poor”. Conclusion: Findings from the pre- sent study demonstra.e a.need for further continuing educa.ion opportunities on the current na.iona. guide- lines on a.tena.a. physica. a.tivity.
