BACKGROUND The phrase “floating knee is a flail knee joint,” referring to ipsilateral femoral and tibial fractures,was first used by Blake and Mc Bryde in *** condition is often caused by a high-energy trauma with often extensive injury to the soft tissues,and is accompanied by life-threatening systemic complications,including head,chest or abdominal injuries and a high incidence of fat *** knee is a severe and uncommon injury *** SUMMARy A 27-year-old man sustained multiple injuries when the electric motorcycle he was riding was hit by a *** injuries included traumatic hypovolemic shock,comminuted and open type II fractures of the left femoral shaft,fracture of the right femoral shaft,comminuted fracture of the bilateral tibial and fibular shaft,and multiple lacerations and abrasions on his forehead,lower lip,neck and *** diagnosis was simultaneous bilateral floating knee complicated with soft tissue *** emergency treatment and the exclusion of life-threating complications,open reduction and internal fixation were successfully performed using plates and screws in the bilateral femoral and tibial *** Simultaneous bilateral floating knee is a rare and severe injury *** treatment is challenging,and *** present a case report of a young adult who suffered from bilateral floating knees during road traffic *** also offer our treatment experience of this complex injury and review past literature.

Association between Prenatal Care Utilization and Risk of Preterm Birth among Chinese Women

It is recognized that prenatal care plays an important role in reducing adverse birth. Chinese pregnant women with medical condition were required to seek additional health care based on the recommended at least 5 times health care visits. This study was to estimate the association between prenatal care utilization（PCU） and preterm birth（PTB）, and to investigate if medical conditions during pregnancy modified the association. This population-based case control study sampled women with PTB as cases; one control for each case was randomly selected from women with term births. The Electronic Perinatal health Care Information System（EPhCIS） and a questionnaire were used for data collection. The PCU was measured by a renewed Prenatal Care Utilization（APNCU） index. Logistic regression models were used to estimate odds ratios（OR） and the 95% confidence interval（95% CI）. Totally, 2393 women with PTBs and 4263 women with term births were collected. In this study, 695（10.5%） women experienced inadequate prenatal care, and 5131（77.1%） received adequate plus prenatal care. Inadequate PCU was associated with PTB（adjusted OR： 1.41, 95% CI： 1.32–1.84）; the similar positive association was found between adequate plus PCU and PTB. Among women with medical conditions, these associations still existed; but among women without medical conditions, the association between inadequate PCU and PTB disappeared. Our data suggests that women receiving inappropriate PCU are at an increased risk of having PTB, but it does depend on whether the woman has a medical condition during pregnancy.

CO_2 sequestration utilizing basic-oxygen furnace slag:Controlling factors,reaction mechanisms and V–Cr concerns

GENETIC CONSEQUENCES OF INTRODUCED Gouramy helostoma Temminckii (Pisces: helostomatidae) IN BRUNEI'S WETLAND

Kissing gouramy (helostoma temminckii) was introduced into Tasek Merimbun in the 1980's. We studied their genetic profile using allozyme electrophoresis and found that all loci were monomorphic. The loss of heterozygotes may suggest that the gouramy are inbreds. They must have reproduced by repeated sib-mating over the years. The severe drought in Tasek Merimbun may have aggravated the genetic erosion further. The genetic variability of kissing gouramy in terms of % polymorphic loci, mean number of alleles per loci and observed heterozygosity level were lower than cyprinids in the same lake and kissing gouramy from the wild. We also note that kissing gouramy is a fast grower and may threaten the livelihood of indigenous fishes in the lake.

Tumor metastasis is the dominant cause of death in cancer patients. however, the molecular and cellular mechanisms underlying tumor metastasis are still elusive. The identi?cation of protein molecules with their expressions correlated to the metastatic process would help to understand the metastatic mechanisms and thus facilitate the development of strategies for the therapeutic interventions and clini- cal management of cancer. Proteomics is a systematic research approach aiming to provide the global characterization of protein expression and function under given conditions. Proteomic technology has been widely used in biomarker discovery and pathogenetic studies including tumor metastasis. This article provides a brief review of the application of proteomics in identifying molecular factors in tumor metastasis process. The combination of proteomics with other experimental ap- proaches in biochemistry, cell biology, molecular genetics and chemistry, together with the development of new technologies and improvements in existing method- ologies will continue to extend its application in studying cancer metastasis.

Polymorphisms of uridine-diphosphoglucuronosyltransferase 1A7 gene in Taiwan Chinese

AIM: Single nucleotide polymorphisms (SNPs) of uridinediphosphoglucuro -nosyltransferase 1A7 (UGT1A7) gene are associated with the development of orolaryngeal cancer, hepatocellular carcinoma, and colorectal cancer. We performed this research to establish the techniques for determining UGT1A7 gene and basic data of this gene for Taiwan Chinese. METhODS: We collected blood samples from 112 healthy adults and 505 subjects carrying different genotypes of UGT1A1, and determined the promoter area and the entire sequence of UGT1A7 exon 1 by polymerase chain reaction. We designed appropriate primers and restriction enzymes to detect variant UGT1A7 genotypes found in the study subjects. RESULTS: Six SNPs at nucleotides 33, 387, 391, 392, 622, and 756 within the coding region of UGT1A7 exon 1 were found. The incidence of UGT1A7*l/*2 (N129R131W208/ K129K131W208) was predominant (35.7%) while that of UGT1A7 *3/*3 (K129K131R208/K129K131R208) was the least (2.7%). The allele frequency of UGT1A7*3, which exists in a considerable proportion of Caucasians (0.361) and Japanese (0.255), was identified only to be 0.152 in our study subjects. A novel variation at nucleotide -57 in the upstream was found, which was associated with SNPs at nucleotides 33, 387, 391, 392, and 622 in one of the variant haplotypes. The nucleotide changes at positions 387, 391, 392 and 756 were in linkage in another variant haplotype. The allele frequency of UGT1A7*3 was 0.018, 0.158, 0.242, 0.433, and 0.920 in subjects carrying wild, A(TA)6TAA/A(TA)7TAA, A(TA)7TAA/A(TA)7TAA, 211G/211A, and 211A/211A variants of UGT1A1 gene, respectively. By using natural or mutagenesis primers, we successfully detected the variations at nucleotides -57, 33, 387, and 622 with the restriction enzymes hpyCh4 Ⅳ, TaqⅠ, AflⅡ, and Rsa Ⅰ, respectively. This paper presents a human detection system in a vision-based hospital surveillance environment. The system is composed of three subsystems, i.e. background segmentation subsystem （BSS）, human feature extraction subsystem （hFES）, and human recognition subsystem （hRS）. The codebook background model is applied in the BSS, the histogram of oriented gradients （hOG） features are used in the hFES, and the support vector machine （SVM） classification is employed in the hRS. By means of the integration of these subsystems, the human detection in a vision-based hospital surveillance environment is performed. Experimental results show that the proposed system can effectively detect most of the people in hospital surveillance video sequences.

Performance of supersonic model combustors with staged injections of supercritical aviation kerosene

Supersonic model combustors using two-stage injections of supercritical kerosene were experimentally investigated in both Mach 2.5 and 3.0 model combustors with stagnation temperatures of approximately 1,750 K. Supercritical kerosene of approximately 760 K was prepared and injected in the overall equivalence ratio range of 0.5-1.46. Two pairs of integrated injector/flameholder cavity modules in tandem were used to facilitate fuel-air mixing and stable combustion. For single-stage fuel injection at an upstream location, it was found that the boundary layer separation could propagate into the isolator with increasing fuel equivalence ratio due to excessive local heat release, which in turns changed the entry airflow conditions. Moving the fuel injection to a further downstream location could alleviate the problem, while it would result in a decrease in combustion efficiency due to shorter fuel residence time. With two-stage fuel injections the overall combustor performance was shown to be improved and kerosene injections at fuel rich conditions could be reached without the upstream propagation of the boundary layer separation into the isolator. Furthermore, effects of the entry Mach number and pilot hydrogen on combustion performance were also studied.
