Necrotic stercoral colitis:Importance of computed tomography findings

AIM: To study the computed tomography (CT) signs in facilitating early diagnosis of necrotic stercoral colitis (NSC). METHODS: Ten patients with surgically and pathologically confirmed NSC were recruited from the Clinico-Pathologic-Radiologic conference at chang Gung Memorial Hospital, Taoyuan, Taiwan. Their CT images and medical records were reviewed retrospectively to correlate CT findings with clinical presentation. RESULTS: All these ten elderly patients with a mean age of 77.1 years presented with acute abdomen at our Emergency Room. Nine of them were with systemic medical disease and 8 with chronic constipation. Seven were with leukocytosis, two with low-grade fever, two with peritoneal sign, and three with hypotensive shock. Only one patient was with radiographic detected abnormal gas. Except the crux of fecal impaction, the frequency of the CT signs of NSC were, proximal colon dilatation (20%), colon wall thickening (60%), dense mucosa (62.5%), mucosal sloughing (10%), perfusion defect (70%), pericolonic stranding (80%), abnormal gas (50%) with pneumo-mesocolon (40%) in them, pericolonic abscess (20%). The most sensitive signs in decreasing order were pericolonic stranding, perfusion defect, dense mucosal, detecting about 80%, 70%, and 62.5% of the cases, respectively. CONCLUSION: Awareness of NSC and familiarity with the CT diagnostic signs enable the differential diagnosis between NSC and benign stool impaction.

Automatic fovea detection and choroid segmentation for choroidal thickness assessment in optical coherence tomography

MET receptor tyrosine kinase promotes the generation of functional synapses in adult cortical circuits

Noise-induced hearing loss is the primary non-genetic factor contributing to auditory ***,there are currently no effective pharmacological interventions for patients with noise-induced hearing ***,we present evidence suggesting that the lysine-specific demethylase 1 inhibitor–tranylcypromine is an otoprotective agent that could be used to treat noise-induced hearing loss,and elucidate its underlying regulatory *** established a mouse model of permanent threshold shift hearing loss by exposing the mice to white broadband noise at a sound pressure level of 120 d B for 4 *** found that tranylcypromine treatment led to the upregulation of Sestrin2(SESN2)and activation of the autophagy markers light chain 3B and lysosome-associated membrane glycoprotein 1 in the cochleae of mice treated with *** noise exposure group treated with tranylcypromine showed significantly lower average auditory brainstem response hearing thresholds at click,4,8,and 16 k Hz frequencies compared with the noise exposure group treated with *** findings indicate that tranylcypromine treatment resulted in increased SESN2,light chain 3B,and lysosome-associated membrane glycoprotein 1 expression after noise exposure,leading to a reduction in levels of 4-hydroxynonenal and cleaved caspase-3,thereby reducing noise-induced hair cell ***,immunoblot analysis demonstrated that treatment with tranylcypromine upregulated SESN2 expression via the autophagy *** treatment also reduced the production of NOD-like receptor family pyrin domaincontaining 3(NLRP3)*** conclusion,our results showed that tranylcypromine treatment ameliorated cochlear inflammation by promoting the expression of SESN2,which induced autophagy,thereby restricting NLRP3-related inflammasome signaling,alleviating cochlear hair cell loss,and protecting hearing *** findings suggest that inhibiting lysine-specific demethylase 1 is a

The origin and evolution of sponges have attracted interest, not only because they are among the most basal metazoans, but also because they act as important ecological engineers in the ocean. Previous research suggested that sponge groups diversified quickly during the Cambrian Series 2, while Terreneuvian sponge fossils are conspicuously rare and poorly substantiated. Yet, an increasing number of sponge spicules have been reported from the Ediacaran-Cambrian(E-C, ca.551–523 Ma) transitional carbonates/cherts worldwide, particularly in South China, which calls for updated discussions. Ocean oxygenation has been widely proposed to play a critical role in organismal evolution, but its relationship with sponges is unclear and falls into a “chicken-egg” dilemma. In this study, we summarize reported sponge spicule distributions from five E-C sections in South China to investigate the early evolutionary pattern of siliceous sponges. According to current stratigraphic correlation,monaxons first appear in the late Ediacaran in the intrashelf basin-slope area, and then potentially expand to the continental shelf and deep basin in the lower Cambrian. In these sections, the abundance, size, and diversity of monaxons increase upwards,followed by the appearance of diaxons, triaxons and then polyactines. Despite the uncertain affinity of the sponge fossils, it is possible that siliceous hexactinellids evolved later than siliceous demosponges, given that the earliest spicule assemblages are exclusively composed of monaxons, which commonly form bundles comparable to modern demosponges. New geochemical data from the Sifangjing section, Guizhou Province and the Yanziqiao section, Hunan Province, South China show that the appearance of sponges usually corresponds to anoxic environment. 为探究深地工程开挖诱导岩体损伤破裂及渗透性演化规律,开展了同步测定波速与渗透率的循环加载与卸围压组合扰动三轴试验,研究了不同条件下红砂岩的损伤与渗透性演化规律。结果表明:①岩石在不同组合扰动阶段具有不同的应力应变曲线形态,其末端形态取决于在何种扰动阶段破坏,初始应力比为决定组合扰动诱发岩石破坏难易的主要因素;②总峰值轴向应变随初始应力比增大而增大,其中组合扰动轴向应变占比始终较低,总峰值体积应变绝对值随初始应力比增大而先增大后减小,其中组合扰动体积应变占比较高;③在初始加载阶段,波速增大,渗透率降低,在组合扰动阶段,波速下降,渗透率增加,且损伤与体积应变近似呈线性关系,损伤与渗透率随扰动次数增加均呈先慢后快的增长趋势,且拐点分别近似在一条直线上;④岩石在组合扰动卸围压阶段破坏为单剪切破坏模式,在组合扰动循环加载阶段破坏为共轭剪切破坏模式;⑤双高(高初始应力比与高初始围压)条件将显著加剧岩体开挖失稳破坏风险。

Reduction of the ages at diagnosis and operation of biliary atresia in Taiwan: A 15-year population-based cohort study

AIM: To describe the ages at diagnosis and operation of biliary atresia(BA) and its incidence over a 15-year period in ***: This was a population-based cohort study. BA cases were identified from the Taiwan National Health Insurance Research Database based on the International Classification of Diseases,Ninth Revision(ICD-9) code of BA 751.61 plus Kasai operation(ICD-9 procedure code 51.37) or liver transplantation(LT,ICD-9 procedure code 50.5). The patients' characteristics including sex,age at diagnosis,age at receiving Kasai operation and age at receiving LT were compared among three birth cohorts:(1) 1997 to 2001;(2) 2002 to 2006; and(3) 2007 to ***: There were a total of 540 BA cases(275 females) with an incidence of 1.62 per 10000 live births. No seasonality of BA was noted. The mean ages at diagnosis of three cohorts were 57.9,55.6 and 52.6 d.A linear regression model demonstrated a decreasing trend of the mean age at diagnosis(1.27 d per year). The proportion of BA cases that received the Kasai operation within 60 d of age increased from 76% to 81%. A total of 189(35%) BA patients underwent LT. The mean age at LT was reduced from 3-year-old to 1-year-old. The rates of LT were 25.6% and 32.3% in patients who received the Kasai operation within 60 d or after 60 d of age,respectively. All patients who did not undergo a Kasai operation eventually required ***: The ages at diagnosis and operation in BA cases have decreased over time. Kasai operation performed at younger age reduces the need for LT. The incidence of BA in Taiwan fluctuates,but without certain trend.

Fractionated evaluation of immunohistochemical hormone receptor expression enhances prognostic prediction in breast cancer patients treated with tamoxifen as adjuvant therapy

Objective： To compare the prognostic prediction between dichotomized and fractionated evaluations of hormone receptor expressions. Methods： Patients with stages Ⅰ-Ⅲ breast cancers, who received adjuvant tamoxifen, were enrolled. The expression of estrogen receptor （ER） and progesterone receptor （PR） was evaluated by immu- nohistochemistry （IHC）. A fractionated score （F score）, the percentage of positive-staining nuclei （0=none, 1=1%-10% 2= 11%-30%, 3=31%-50%, 4=51%-70%, and 5=71%-100%）, was assigned to each case. The dichotomized scoring method defines an F score 〉1 as positive. The prognostic values of both scores were compared by multiple Cox＇s proportional hazard models of disease-free survival （DFS） and overall survival （OS）. Results： Four hundred and six- teen patients with a median follow-up of 78.0 months were included. F scores for ER and PR correlated directly with DFS and OS. Although both the dichotomized and fractionated ER and PR scores were significantly associated with DFS and OS in univariate analyses, only fractionated ER and PR scores remained as independent prognostic factors of DFS and OS in the final multiple Cox＇s proportional hazard models. Conclusion： Fractionated IHC hormone receptor expression evaluation enhances the prognostic prediction compared with a dichotomized assessment.

Human Detection for Video Surveillance in Hospital

This paper presents a human detection system in a vision-based hospital surveillance environment. The system is composed of three subsystems, i.e. background segmentation subsystem （BSS）, human feature extraction subsystem （HFES）, and human recognition subsystem （HRS）. The codebook background model is applied in the BSS, the histogram of oriented gradients （HOG） features are used in the HFES, and the support vector machine （SVM） classification is employed in the HRS. By means of the integration of these subsystems, the human detection in a vision-based hospital surveillance environment is performed. Experimental results show that the proposed system can effectively detect most of the people in hospital surveillance video sequences.
