AIM： To study the immunological effects of physiologica doses of purple sweet potato leaves （PSPL）. METHODS： The randomized crossover study （two periods, each lasting for 2 wk） involved 16 healthy non-smoking adults of normal weight. The 6-wk study consisted of a run-in （wk 1） PSPL diet （daily consumption of 200 g PSPL） or a control diet （low polyphenols, with the amount of carotenoids adjusted to the same level as that of PSPL）（wk 2-3）, washout diet （wk 4）, and switched diet （wk 5-6）. Fasting blood was collected weekly in the morning. T-lymphocyte function was assessed via the proliferation and secretion of immunoreactive cytokines. Salivary IgA secretion and the specific cytotoxic activities of cytotoxic T lymphocytes and natural killer （NK） cells were determined. ResULTS： The plasma 13-carotene level increased with time in both groups, while the plasma polyphenol level decreased in the control group, and no significant difference was detected between the two groups. Although p^asma polyphenol levels did not significantly increase in the PSPL group at the end of the study, they were significantly elevated in urine. PSPL consumption produced a significant increase in proliferation responsiveness of peripheral blood mononuclear cells （PBMC） and their secretion of immunoreactive IL-2 and IL-4. As well, lytic activity in NK cells was elevated in a time-dependent fashion. Salivary IgA secretion significantly decreased in control group after 2 wk, and returned to baseline following dietary switch to PSPL. CONCLUSION： Consumption of PSPL modulates various immune functions including increased proliferation responsiveness of PBMC, secretion of cytokines IL-2 and IL-4, and the lytic activity of NK cells. The responsible determinants of PSPL remain to be elucidated, as does the biological significance of the present observations.

Cloud computing becomes a hot research topic recently Before the concept of cloud computing was introduced, similar concepts or computing models have been proposed such as distributed computing and grid computing. Cloud computing is composed of a front end, a back end, cloud computing services, and cloud computing technologies. Though several institutes and companies, such as NIST, Google, Microsoft, IBM, and Wikipedia, define cloud computing with different definitions, could computing must provide the following four properties. （1） Users can access services via the Internet. （2） Resources can be dynamically managed and arranged. （3） There exists a distributed virtual architecture. （4） Because of requested services and demands, possible fees may be charged.

AIM： To evaluate blood pressure（BP） changes during phacoemulsification（PC） and femtosecond laser（FSL）-assisted cataract ***： A retrospective chart review was performed for all patients who received traditional phacoemulsification surgery（PC group） and FSL-assisted cataract surgery（FS group） from July 2013 to December 2014. Totally 206 eyes from 133 patients receiving the two types of procedures were *** characteristics（age, gender, and hypertension history）, pre- and post-operative BPs were ***： The pro-operative systolic and diastolic BPs（mm Hg） were 124.89 ±20.48 vs 126.98 ±16.85, and71.88 ±9.81 vs 73.56 ±10.03, in PC and FS groups,respectively. While the post-operative systolic and diastolic BPs（mm Hg） were 130.13 ±22.59 vs 134.77 ±17.52, and 73.41 ±11.62 vs 78.89 ±12.2, in PC and FS groups, respectively. Paired-sample t-tests showed obvious systolic and diastolic BP elevations in FS group after surgery（P =0.001 and 0.007） and no reliability in PC group（P =0.094 and 0.359）. A linear regression model revealed systolic and diastolic BP elevations,which were related to longer surgical times for FS group（P =0.008 and 0.021）. Age, gender, and hypertension history were not correlated with blood pressure elevation in either ***： BP increases but at a limited level after FSL-assisted cataract surgery compared to traditional phacoemulsification

采用柱淋洗实验模拟攀枝花地区降雨条件,分别探讨钒钛磁铁矿尾矿及其在周边农田土壤表面沉积0、1、2、5 cm的条件下,几种重金属的迁移淋滤特征.通过淋洗-干旱-淋洗,共94 d的淋洗实验,研究结果表明,土壤、尾矿及土壤-尾矿复合系统淋滤液中Cr的浓度没有显著差异,但随着尾矿沉积厚度的增加,尾矿沉积显著增加土壤-尾矿复合系统淋滤液Mn、Ni和Zn的浓度及累积释放量,具有较大环境风险;土壤中Mn、Zn的累积释放量显著高于尾矿,可能与尾矿较高的pH及这几种金属在土壤及尾矿中的存在形态不同有关;土壤中各金属的最大释放率为Mn>Zn>Ni>Cr,尾矿中为Ni>Cr=Mn>Zn;干湿交替对土壤和尾矿中Mn、Zn的淋滤没有显著影响,但会改变土壤和尾矿中Cr和Ni的释放特征,增加土壤和尾矿中Cr、Ni的环境风险.

Background: In Taiwan, emergency departments are so busy that it is not uncommon for staffs (doctors and nurses) to be deprived of their lunch breaks or breaks to go to the restroom. As a consequence, during their busy shifts some emergency department healthcare workers choose to limit their water intake and no time to micturate which are both risk factors of urinary tract infection or urolithiasis. Hematuria is a common laboratory finding due to urinary tract infection and urolithiasis. Objective: To assess the prevalence of hematuria among the emergency department healthcare workers, we conducted a retrospective study in a district hospital in Taiwan. Documented data included those of healthcare workers who worked in the emergency department, as well as other departments of the same hospital. Methods: We reviewed the three-year records of general body checkup of hospital staffs who served in the emergency room and other departments of the same hospital between January 1, 2009 and December 31, 2011. Statistical software SPSS statistical analysis of survey data recovery is applied;data and test results by the questionnaire analysis patterns are associated with the urinary system abnormalities to ANOVA analysis to explore the hospital group whether there is a significant sex differences, and the other to analyze and verify the correlation analysis to the Scheffe post-hoc comparison method. Results: A higher prevalence of hematuria was observed among emergency department healthcare workers when compared with their colleagues who work in other departments of the same hospital. Female workers have higher risk of urinary system abnormalities than male workers. Nurses in the urinary system abnormalities are higher than other positions category. Shift work system has a negative effect on the urinary system. 高载荷飞行仿真中载人离心机本身的转动惯量较大,在飞行高过载状态离心机的延时控制成为控制系统要解决的主要问题之一。针对这一问题,通过基于T-S模糊逻辑推理建立DFS系统高载荷飞行阶段的模糊规则,经对相关操纵输入量的模糊聚类辨识,在采样点附近采用广义预测控制方法得到当前控制规律,从而实现对飞行仿真的预测控制。仿真试验表明了该方法的科技较好的调节控制规律。

In this review,we discuss the chronic care model(CCM)in relation to the diabetes pay-for-performance(P4P)program in *** first introduce the 6 components of the CCM and provide a detailed description of each of the activities in the P4P program implemented in Taiwan,mapping them onto the 6 components of the *** each CCM component,the following three topics are described:the definition of the CCM component,the general activities implemented related to this component,and practical and empirical practices based on hospital or local government *** then conclude by describing the possible successful features of this P4P program and its challenges and future *** conclude that the successful characteristics of this P4P program in Taiwan include its focus on extrinsic and intrinsic incentives(i.e.,shared care network),physician-led P4P and the implementation of activities based on the CCM ***,due to the low rate of P4P program coverage,approximately 50%of patients with diabetes cannot enjoy the benefits of CCMrelated activities or receive necessary *** addition,most of these CCM-related activities are not allotted an adequate amount of incentives,and these activities are mainly implemented in hospitals,which compared with primary care providers,are unable to execute these activities *** of these issues,as well as insufficient implementation of the e-CCM model,could hinder the advanced improvement of diabetes care in Taiwan.

Stress during prenatal development is correlated with detrimental cognitive and behavioral outcomes in offspring. However, the long-term impact of prenatal stress(PS) and disrupted glucocorticoid signaling on bone mass and strength is not understood. In contrast, the detrimental effect of lead(Pb) on skeletal health is well documented. As stress and Pb act on common biological targets via glucocorticoid signaling pathways and co-occur in the environment, this study first sought to assess the combined effect of stress and Pb on bone quality in association with alterations in glucocorticoid signaling. Bone parameters were evaluated using microCT, histomorphometry, and strength determination in 8-month-old male mouse offspring subjected to PS on gestational days 16 and 17, lifetime Pb exposure(100 p.p.m. Pb in drinking water), or to both. Pb reduced trabecular bone mass and, when combined with PS, Pb unmasked an exaggerated decrement in bone mass and tensile strength. Next, to characterize a mechanism of glucocorticoid effect on bone, prednisolone was implanted subcutaneously(controlled-release pellet, 5 mg·kg^(-1) per day) in 5-month-old mice that decreased osteoblastic activity and increased sclerostin and leptin levels. Furthermore, the synthetic glucocorticoid dexamethasone alters the anabolic Wnt signaling pathway. The Wnt pathway inhibitor sclerostin has several glucocorticoid response elements, and dexamethasone administration to osteoblastic cells induces sclerostin expression. Dexamethasone treatment of isolated bone marrow cells decreased bone nodule formation, whereas removal of sclerostin protected against this decrement in ***, these findings suggest that bone loss associated with steroid-induced osteoporosis is a consequence of sclerostin-mediated restriction of Wnt signaling, which may mechanistically facilitate glucocorticoid toxicity in bone.
