BACKGROUND Visceral leishmaniasis(VL)is a systemic protozoan infection caused by Leishmania donovani(***)and transmitted by sand flies,causing macrophage invasion in the liver,spleen,and bone *** of VL is currently based on clinical signs,symptoms,and specific in-vitro markers and bone marrow ***,VL's specific hematological and bone marrow manifestation in Sudanese pediatric patients is not well *** To examine the blood and bone marrow characteristics in pediatric patients from Sudan who have *** This is a retrospective hospital-based study with a sample of 107 consecutive Sudanese pediatric *** data focused on hematological and bone marrow *** included only the completed records of the pediatric patients with VL in the Tropical Disease Teaching Hospital in Khartoum,Sudan from the period of 2016 to *** The majority of pediatric patients included in this study are below 5-years-old(n=59,55.2%).Moreover,anemia,thrombocytopenia,and leukopenia were among the prevalent characteristics in the population under *** further analyze the data,we developed a machine learning model using boosted forest algorithms to predict *** parasites load,with a mean accuracy of 0.88 for the training dataset and an accuracy of 0.46,0.50,and 0.74 for mild,moderate,and severe *** parasite load in the validation *** This study shows that the most common bone marrow change among Sudanese VL children was increased chronic inflammatory cells(n=88,82.2%)with present macrophage hemophagocytes(n=103,96.3%).While anemia and thrombocytopenia were the most common hematological *** results will hopefully lead to an early diagnosis and hence better management for Sudanese pediatric patients with suspected VL.

Diagnostic Utility of Sago-Like Nodules on Gross Thoracoscopic Appearance in Tuberculous Pleural Effusion and Their Correlation with Final Histo-Microbiologic Findings

Background: Distinguishing tuberculous pleural effusion (TPE) from other causes of exudative effusion is often challenging. Delay in treatment initiation can occur while awaiting histo-microbiologic confirmation owing to the paucibacillary nature of the disease. Sago-like nodules are the most common visual finding on gross thoracoscopic appearance. The primary objective was to determine the diagnostic utility of the presence of sago-like nodules on gross thoracoscopic appearance in TPE to help justify early initiation of tuberculosis (TB) treatment based on their finding while awaiting final histo-microbiologic confirmation. Secondary objective was to study the correlation between the presence of sago-like nodules and the final histo-microbiologic findings in pleural biopsy specimens. Methods: This was a retrospective-descriptive study of all patients with exudative pleural effusion who underwent diagnostic medical thoracoscopy (MT) at Hamad General Hospital during an eight-year period (from January, 2008 to December, 2015). Results: The presence of sago-like nodules on gross thoracoscopic appearance of the pleural surface had a sensitivity of 58%, a specificity of 89% and a positive predictive value of 97% for TPE with a diagnostic accuracy of 62%. There is significant association between the presence of sago-like nodules and demonstration of granulomatous inflammation in pleural biopsy specimens (P = 0.000). There is no association between sago-like nodules and positive TB smear and culture in biopsy specimens. Conclusion: The presence of sago-like nodules on gross thoracoscopic appearance has a high specificity and positive predictive value for TPE and significantly correlates with the presence of granulomatous inflammation. Patients from TB prevalent areas with exudative pleural effusion and sago-like nodules on gross thoracoscopic appearance may be commenced on TB chemotherapy while awaiting final histologic confirmation.

Hematological Alterations in an Eastern Sudanese Chronic Kidney Disease Patient Population

Background: Chronic Kidney Disease (CKD), associated with a slow and progressive loss of kidney function over a period of several years, is an important clinical disaster with an increasing rate of morbidity and mortality especially in the least developed countries. Many hematological parameters are thought to alter dramatically during the course of the disease. These include white blood cells, red blood cells, and platelets. Methods: We tried, retrospectively, to evaluate the peripheral blood hematological alterations in a group of patients undergoing hemodialysis in an eastern Sudan dialysis center to add local medical information. Results: Anemia (Low hemoglobin and hematocrit) was detected in 94% of the patients’ group. Mean Erythrocyte count (3.32vs.4.76 (×109/L)), Hemoglobin concentration (9.4vs.13 (g/dl)), Hematocrit (28.7vs.38.7 (L/L)) and platelet count (296 vs. 238 (×109/L)) were significantly lower in the patients’ group than in the control group (P-values Conclusion: Five out of eight studied parameters (Red cell count, hemoglobin, hematocrit, mean cell hemoglobin concentration, and platelets count) have shown a significant alteration in CKD patients. As the complete blood count (CBC) test is the most utilized test in clinical laboratory practice, these alterations may be considered as early indicators for CKD. Furthermore, all patients with CKD must be routinely checked for these alterations.

The Usefulness of Renal Doppler Parameters in Chronic Kidney Disease: Is There a Cut-Off Value to Estimate End Stage Kidney Disease?

Aims: In this study, Doppler parameters were studied in patients with advanced stage renal disease, and the relationship between Doppler parameters and renal disease stage was investigated. Doppler values were evaluated for a cut-off value between normal and end-stage kidney disease group. Materials and methods: 50 patients with chronic kidney disease and 15 patients belonging to the same age group and with normal serum creatinine levels were comprised of the study and control groups. Resistivity and pulsatality indices were measured at the main renal arteries and interlobular arteries at both sides. Results: Statistically significant differences were found between the PI and RI values obtained from the main renal and interlobular arteries. PI sums were compared with those from the control group, and a cut-off value of 2.15 was found, with a sensitivity of 90% and a specificity of 86.7%. Conclusion: Any increase in the RI and PI values obtained from the main renal and interlobular arteries must bring to mind the possibility of advancing renal damage and interstitial fibrosis. Patients with and without renal parenchymal damage can be differentiated by means of comparing the total PI values obtained from the right and left main renal and interlobular arteries.

Helicobacter pylori is the microbial agent most responsible for gastro-duodenal ulcer and chronic gastritis, which can develop into carcinoma of the stomach. This study was performed in Wad Medani Teaching Hospital, Sudan to detect Helicobacter pylori in stomach samples, and evaluate the performance of the tests used, which were histological stains and PCR. Gastric biopsies were obtained from 105 referred patients during endoscopy, and fixed specimens examined by haematoxylin-eosin and Warthin-Starry silver stains, while DNA was extracted for glmM gene amplification. Epigastric pain was the most common symptom at 78% (82/105) and chronic gastritis recorded with 71% (68/105) of endoscopy results. Warthin-Starry silver stain gave 31% (33/105) as positive for Helicobacter pylori followed by glmM gene 27% (28/105) and haematoxylin-eosin 24% (25/105). The study indicated good performance of histological staining and high specificity of glmM gene in detection of Helicobacter pylori from gastric biopsies.

Border Shifting in Naomi Nye＇s Habibi

This paper investigates how the novel Habibi （1997） by the Arab-American writer Naomi Nye addresses the theme of border shifting from a postmodernist perspective that deconstructs the traditional view of borders meant to maintain exclusion and hegemony and instead considers them as being often flimsy, malleable, and changeable. Drawing upon her experience as a multifarious Arab-American writer whose father was a Palestinian immigrant and whose mother was an American, Nye tries to build bridges across political, national, cultural, and ethnic boundaries. Through a love story between the two protagonists （Liyana, whose father is a Palestinian-American and whose mother is American and omer, whose parents are Jewish Israelis）, Nye endeavors to bring about a sense of harmony and understanding between the politically, ethnically, culturally, and racially separated J home whether in reality ews and Palestinians. At the end, Liyana＇s family realizes they can have more than one or in imagination.

Plank quantum and classical string energy relations seem to be uncorrelated. This work correlated them. The relativistic energy-momentum relation has been used together with plank and de Brogglie hypothesis to prove that the wave group velocity is equal to the particle velocity in both ordinary and curved space. The plank energy relation is shown also to be related to the classical energy relation of an oscillating string. Starting from plank energy relation for n photons and performing integration, the expression of classical string energy was obtained. This means that one can treat electromagnetic waves as a collection of continuous photons having frequencies ranging from zero to w. Conversely, starting from classical string energy relation by differentiating it with respect to angular frequency, the plank quantum energy for n photons has been found. This means that the quanta results from separation of electromagnetic waves to single isolated waves. Each wave consists of n photons or quanta.

针对电网连锁跳闸引起的停电事故以及降低电力系统的有功功率损耗,提出了一种计及网损的电网连锁故障预防方法。该方法综合考虑网损指标和衡量电网安全稳定运行指标,构建了多目标优化模型。该模型的优化目的在于确保均衡权重下安全裕度尽可能大,同时使网损尽可能小。通过粒子群优化算法求解,在IEEE-39节点系统上进行了算例分析。仿真结果表明该优化模型不仅提高了电网安全运行的界限,也实现了降损节能。

Background: Most community and hospital acquired infections are due to Staphylococcus aureus, these infections involve both local suppurative and systemic conditions. There is an increase in the incidence rates of acquiring resistant strains of Staphylococcus aureus in hospitals and community in Sudan. This study aims to detect frequency of Staphylococcus aureus isolated from surgical-sites infections (SSIs) and traumatic wounds, and to determine susceptibility to commonly used drugs. Methods: Prospective cross-sectional laboratory based study was followed, 250 patients admitted to Surgery Departments at Wad Madani Teaching Hospitals during 2019 and 2020 were recruited;with clinical manifestation of SSIs and or/traumatic wounds. Swab sample was taken from each participant and incubated aerobically on mannitol salt agar. Biochemical tests and 16rRNA gene used for Staphylococcus aureus identification. Antimicrobial susceptibility was accomplished using Kirby Bauer disc diffusion technique.
