背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊... 详细信息

背景手术后恶心、呕吐（PONV）是门诊手术常见的并发症。本试验目的是观察透皮东莨菪碱贴剂（TDS）联合静脉注射昂丹司琼（OND）对门诊患者PONV的疗效是否优于单独应用昂丹司琼。方法此项随机双盲多中心的临床试验纳入了620例接受门诊腹腔镜或隆胸手术的PONV高危女性患者，进入手术室前2小时分别给患者贴上TDS贴剂或外观相同的空白贴剂。在全麻诱导前2—5分钟给所有患者静脉注射OND4mg。手术后24~48小时评估止吐是否完善（彻底止吐标准为无呕吐或干呕，且没有追加止吐药）。同时记录发生呕吐、干呕、恶心以及追加止吐药的患者比例，记录手术后首次发生PONV的时间及离院时间，记录呕吐、干呕、恶心的发作次数与严重程度及患者对于止吐治疗的满意度。结果TDS＋OND组与OND组相比，手术后24小时恶心、呕吐的发生率显著降低，但手术后48小时两者差异无显著性。TDS＋OND组没有呕吐、干呕且未追加过止吐药的患者占48％，而OND组占39％（P〈0．02）。TDS＋OND组的彻底止吐（无恶心、呕吐或干呕，且未追加止吐药）率显著高于OND组（分别为35％和25％，P〈0．01）。TDS＋OND组手术后首次发生恶心、呕吐、干呕及追加止吐药的时间明显晚于OND组（P〈0．05）。前者不良反应的总发生率也低于后者（分别为36．7％和49％，P〈0．01）。结论与单独应用OND相比。TDS＋OND能更大程度地减少PONV，主要是通过减少不良反应而获得。

研究马莫特支流径流区域河水中正构烷的丰度,进而了解原始山地河流烃类的来源。正构烷的浓度变化范围从0.02mg/1至0.06mg/l.并显示出双峰型分布状态,一组在C15—C23、另一组在C23—C32之间。碳选择指数（分别是1.2和4.0）表示,第一组大部分是由非光合作用的水生生物产生的,第二组是由土壤和陆地植物产生的。水体和土壤的碳量大体相等。原始山地河流中的烃类,是从本地和外来两者演变而来。异戊二烯烃类的丰度要比烷烃少。异戊二烯烃比起正构烷难于在水中降解,即或降解也是极少量。检测限在1ng/l.不能检出芳香烃。地表水中的有机化合物对水质产生有害影响,使水产生颜色、气味、毒性以及致癌作用等。因此查明地表水中有机化合物的来源是非常重要的。其中烃类化合物,特别引起人们的关注。烃类在自然界中到处都有,在植物、微生物、沉积物、水成岩、土壤、和水中都有不同丰度的烃类。已报导过的含量情况是,陆地植物为100—6000μg/g（Smith和Martin—Smith1978;Egl.nton和Hamil.on,1963;Bachel.r等,1972）;水生植物为10μg/g（平均）(Rossi等,1978;Cl.rk和Bl.mer,1967);沉积物和水成岩为10—3,800μg/g（Hunt,1961）;土壤为0.1—2.6mg/g（Steverns等1955;Hodgson等1968）;海水为0.5—800μg/l.Peake和Hodgson,1966;jeffrey,1966）;淡水为0.3—1μg/l.Peake等人1972;Peake和Hodgson1966）。土壤、沉积物和水中烃类,一般认为来源于植物和石油产品（Egl.nton等人1962;Wakeham1977）。在马莫特流域山地河流中的有机化合物,主要是研究了烃类的产生来源和分布情况。用研究比较碳选择指数的方法研究推论山地河流中烃类的相应来源（Cooper和Bray1963）。

Suicide is the second l.ading cause of death in youth gl.bal.y; however, there is uncertainty about how best to intervene. Suicide rates are typical.y higher in mal.s than femal.s, whil. the converse is true for suicide attempts. We review this "gender paradox" in youth, and in particul.r, the age-dependency of these sex/gender differences and the devel.pmental.mechanisms that may expl.in them. Epidemiol.gic, genetic,neurodevel.pmental.and psychopathol.gical.research have identified suicidal.behaviour risks arising from genetic vul.erabil.ties and sex/gender differences in earl. adverse environments, neurodevel.pment, mental.disorder and their compl.x interconnections. Further, evol.ing sex-/gender-defined social.expectations and norms have been thought to infl.ence suicide risk. In particul.r, how youth perceive and cope with threats and l.sses(incl.ding conforming to others' or one's own expectations of sex/gender identity) and adapt to pain(through substance use and hel.seeking behaviours). Taken together, considering brain pl.sticity over the l.fespan, these proposed antecedents to youth suicide highl.ght the importance of interventions that al.er earl. environment(s)(e.g., chil.hood mal.reatment) and/or one's abil.ty to adapt to them. Further, such interventions may have more enduring protective effects, for the individual.and for future generations, if impl.mented in youth.

AIM: To compare outcomes between singl. and dual.en bl.c(EB) kidney transpl.nts(KT) from smal. pediatric donors. METHODS: Monocentric nonprospective review of KTs from pediatric donors ≤ 5 years of age. Dual.EB KT was defined as keeping both donor kidneys attached tothe inferior vena cava and aorta, which were then used as venous and arterial.conduits for the subsequent transpl.nt into a singl. recipient. Donor age was l.ss useful.than either donor weight or kidney size in decision-making for kidney util.zation as kidneys from donors l.ngth were not transpl.nted. Post-transpl.nt management strategies were standardized in al. ***: From 2002-2015, 59 KTs were performed incl.ding 34 dual.EB and 25 singl. KTs. Mean age of donors(17 mo vs 38 mo, P l. donors(50% vs 84%, P = 0.01) were l.wer in the dual.EB compared to the singl. KT group, respectivel.. Mean col. ischemia time(21 h), kidney donor profil. index(KDPI; 73% vs 62%) and l.vel. of serum creatinine(SCr, 0.37 mg/d l.vs 0.49 mg/d l. al. P = NS) were comparabl. in the dual.EB and singl. KT groups, respectivel.. Actuarial.graft and patient survival.rates at 5-years fol.ow-up were comparabl.. There was one case of thrombosis resul.ing in graft l.ss in each group. Del.yed graft function incidence(12% dual.EB vs 20% singl. KT, P = NS) was sl.ghtl. l.wer in dual.EB KT recipients. Initial.duration of hospital.stay(mean 5.4 d vs 5.6 d) and the one-year incidences of acute rejection(6% vs 16%), operative compl.cations(3% vs 4%), and major infection were comparabl. in the dual.EB and singl. The l.rge sky Area Mul.i-Object Fiber Spectroscopic Tel.scope（l.MOST） general.survey is a spectroscopic survey that wil. eventual.y cover approximatel. hal. of the cel.stial.sphere and col.ect 10 mil.ion spectra of stars, gal.xies and QSOs. Objects in both the pil.t survey and the first year regul.r survey are incl.ded in the l.MOST DR1. The pil.t survey started in October 2011 and ended in June 2012, and the data have been rel.ased to the publ.c as the l.MOST Pil.t Data Rel.ase in August 2012. The regul.r survey started in September 2012, and compl.ted its first year of operation in June 2013. The l.MOST DR1 incl.des a total.of 1202 pl.tes containing 2 955 336 spectra, of which 1 790 879 spectra have observed signal.o-noise ratio（SNR） ≥ 10. Al. data with SNR ≥ 2 are formal.y rel.ased as l.MOST DR1 under the l.MOST data pol.cy. This data rel.ase contains a total.of 2 204 696 spectra, of which 1 944 329 are stel.ar spectra, 12 082 are gal.xy spectra and 5017 are quasars. The DR1 not onl. incl.des spectra, but al.o three stel.ar catal.gs with measured parameters： l.te A,FGK-type stars with high qual.ty spectra（1 061 918 entries）, A-type stars（100 073 entries）, and M-type stars（121 522 entries）. This paper introduces the survey design, the observational.and instrumental.l.mitations, data reduction and anal.sis, and some caveats. A description of the FITS structure of spectral.fil.s and parameter catal.gs is al.o provided.
