Endoscopic biliary treatment of unresectable cholangiocarcinoma: A meta-analysis of survival outcomes and systematic review

BACKGROUND Endoscopic radiofrequency ablation(ERFA),percutaneous radiofrequency ablation(PRFA),and photodynamic therapy(PDT),when used in conjunction with conventional biliary stenting,have demonstrated a survival benefit in patients with unresectable *** To compare pooled survival outcomes,adverse event rates,and mean stent patency for those undergoing these *** A comprehensive literature review of published studies and abstracts from January 2011 to December 2020 was performed comparing survival outcomes in patients undergoing ERFA with stenting,biliary stenting alone,PRFA with stenting,and PDT with stenting for unresectable cholangiocarcinoma(CCA).RESULTS Data from four studies demonstrated a pooled mean survival favoring ERFA as compared to biliary stenting alone(12.0±0.9 mo vs 6.8±0.3 mo,Pmproved median survival time(13 mo vs 8 mo,Pmonstrated statistical superiority to biliary stenting alone(Pmparing ERFA to PRFA,pooled data demonstrated overall higher mean survival in the ERFA with stenting cohort as compared to PRFA with stent cohort(12.0+0.9 mo vs 8.1+2.1 mo,Pm two studies demonstrated a pooled median survival favoring ERFA with stenting as compared to PDT with stenting(11.3 mo vs 8.5 mo,P=0.02).CONCLUSION While further prospective,randomized studies are needed to assess efficacy of ERFA,our metaanalysis demonstrated that this technique offers endoscopists a reasonable palliative method by which to treat patients with unresectable CCA that results in longer survival as compared to biliary stenting alone,percutaneous radiofrequency ablation with biliary stenting,and PDT with biliary stenting as well as an acceptable adverse event profile based on available published data.

Background Large-river decision-makers are charged with maintaining diverse ecosystem services through unprecedented social-ecological transformations as climate change and other global stressors intensify. The interconnected, dendritic habitats of rivers, which often demarcate jurisdictional boundaries, generate complex management challenges. Here, we explore how the Resist–Accept–Direct(RAD) framework may enhance large-river management by promoting coordinated and deliberate responses to social-ecological trajectories of change. The RAD framework identifies the full decision space of potential management approaches, wherein managers may resist change to maintain historical conditions, accept change toward different conditions, or direct change to a specified future with novel conditions. In the Upper mississippi River System, managers are facing social-ecological transformations from more frequent and extreme high-water events. We illustrate how RAD-informed basin-, reach-, and site-scale decisions could:(1) provide cross-spatial scale framing;(2) open the entire decision space of potential management approaches; and(3) enhance coordinated inter-jurisdictional management in response to the trajectory of the Upper mississippi River *** The RAD framework helps identify plausible long-term trajectories in different reaches(or subbasins) of the river and how the associated social-ecological transformations could be managed by altering site-scale conditions. Strategic reach-scale objectives may reprioritize how, where, and when site conditions could be altered to contribute to the basin goal, given the basin's plausible trajectories of change(e.g., by coordinating action across sites to alter habitat connectivity, diversity, and redundancy in the river mosaic).Conclusions When faced with long-term systemic transformations(e.g., > 50 years), the RAD framework helps explicitly consider whether or when the basin vision or goals may no longer be achievable,

Acute pancreatitis remains the most common complication of endoscopic retrograde cholangiopancreatography(ERCP).It is reported to occur in 2–10%of unselected patient samples and up to 40%of high-risk *** purpose of this article is to review the evidence behind the known risk factors for post-ERCP pancreatitis,as well as the technical and medical approaches developed to prevent *** have been many advances in identifying the causes of this *** on this knowledge,a variety of preventive strategies have been developed and *** approach to prevention begins with careful patient selection and performing ERCP for specific indications,while considering alternative diagnostic modalities when *** should also be classified by high-risk factors such as young age,female sex,suspected sphincter of Oddi dysfunction,a history of post-ERCP pancreatitis,and normal serum bilirubin,all of which have been identified in numerous research *** pathways of injury that are believed to cause post-ERCP pancreatitis eventually lead to the common endpoint of inflammation,and these individual steps can be targeted for preventive therapies through procedural techniques and medical *** includes the use of a guide wire for cannulation,minimizing the number of cannulation attempts,avoiding contrast injections or trauma to the pancreatic duct,and placement of a temporary pancreatic duct stent in high-risk *** of rectal non-steroidal anti-inflammatory agents(NSAIDs)in high-risk patients is the proven pharmacological measure for prevention of post-ERCP *** evidence for or against numerous other attempted therapies is still unclear,and ongoing investigation is required.

Seepage or loss of the mix-water from the drilling muds into the porous and permeable formations is a common problem during drilling *** drilling mud design requires a good knowledge of sealing integrity and all the factors influencing the mud to bridge through fractures or pore throat of exposed *** circulation materials(LCms)are commonly introduced into the drilling mud to prevent or minimize filtrate *** study investigates silica nanoparticle(SNP)derived from rice husk(RH)termed RH-SNP using the wet-milling method as an LCm inwater-based mud(WBm).The impact of the RH-SNP in the enhancement of rheology and filtrate loss control properties of WBm was ***,the sealing integrity of the RH-SNP in a 1 mm and 2 mm simulated fracture for 7 min was determined using a stainless-steel slotted filter *** performance of the developed RH-SNP was compared with the widely applied *** synthesized RH-SNP at amount of 2.0 wt% significantly enhanced the yield point and plastic viscosity of the WBm by 75% and 386%,respectively,and minimized the fluid loss of the WBm by 47% at 80°*** enhancement is due to the particles ability to spread and interact efficiently with the *** the use of 1 mm and 2 mm simulated fracture for 7 min,the mud loss volume of the base mud reduced by 50%,66.7%,86%,and 90%(for 1 mm)and 40%,65.7%,77.1%,and 80%(for 2 mm)with the inclusion of 0.5 wt%,1.0 wt%,1.5 wt%,and 2.0 wt% of RH-SNP,***,the results showed that RH-SNP enhanced the seal integrity of the drilling mud and was more resistant to deformation compared to the *** findings of this study can help for better understanding of the application of RH-SNP as a loss circulation agent owing to its superior ability to seal fractured formation compared with the often used nutshell.

A diverse range of remarkable boron n itride (BN) nano structures subsumi ng nano-homs, nano-rods, nano-platelets, and clusters of hollow nano spheres (nano-onions, arguably of greatest applied and fun damental in terest) have bee n produced exclusively from crystalline BN precursor powder via lamp ablation. The procedure is safe, devoid of toxic reagents, simple, rapid and scalable—generating some genres of nan oparticles that had previously proved elusive. Product structure and composition were unambiguously assessed by high-resoluti on transmission electron microscopy, energy-dispersive X-ray spectroscopy and electron energy loss spectroscopy.

Superb cryogenic strength of equiatomic CrCoNi derived from gradient hierarchical microstructure

This work demonstrates the effectiveness of a cryogenic torsional pre-straining for significantly improving the cryogenic strength of an equiatomic CrCoNi alloy. The origin of this phenomenon is elucidated by various microstructural characterization tools, which shows that the sequential torsion and tension tests lead to the observed hierarchical microstructure through the activation of different twinning systems and stacking faults. This gives rise to the significant increase in the yield strength from 600 mPa to 1215 mPa,while the fracture strain changes from 68% to 48%. The current study reveals that the incorporation of nanotwins architecture by shear deformation may constitute a viable strategy to tune the mechanical performance and, in particular, to dramatically increase the strength while keeping a good ductility.
