Limited endoscopic sphincterotomy plus large balloon dilation for choledocholithiasis with periampullary diverticula

AIM: To investigate the effectiveness and safety of limited endoscopic sphincterotomy (EST) plus large balloon dilation (LBD) for removing choledocholithiasis in patients with periampullary diverticula (PAD). METHODS: A total of 139 patients with common bile duct (CBD) stones were treated with LBD (10-20 mm balloon diameter) after limited EST. Of this total, 73 patients had PAD and 66 patients did not have PAD (controls). The results of stone removal and complications were retrospectively evaluated. RESULTS: There were no significant differences between the PAD and the control groups in overall successful stone removal (94.5% vs 93.9%), stone removal in first session (69.9% vs 81.8%), mechanical lithotripsy (12.3% vs 13.6%), and complications (11.0% vs 7.6%). Clinical outcomes were also similar between the types of PAD, but the rate of stone removal in first session and the number of sessions were significantly lower and more frequent, respectively, in type B PAD (papilla located near the diverticulum) than controls [23/38 (60.5%) vs 54/66 (81.8%), P = 0.021; and 1 (1-2) vs 1 (1-3), P = 0.037, respectively] and the frequency of pancreatitis was significantly higher in type A PAD (papilla located inside or in the margin of the diverticulum) than in controls (16.1% vs 3.0%, P = 0.047). CONCLUSION: Limited EST plus LBD was an effective and safe procedure for removing choledocholithiasis in patients with PAD. However, some types of PAD should be managed with caution.

In multicellular organisms, controlling the timing and extent of asymmetric cell divisions （ACDs） is crucial for correct patterning. During post-embryonic root development in Arabidopsis thaliana, ground tissue （GT） maturation involves an additional ACD of the endodermis, which generates two different tissues： the endo- dermis （inner） and the middle cortex （outer）. It has been reported that the abscisic acid （ABA） and gibberellin （GA） pathways are involved in middle cortex （MC） formation. However, the molecular mechanisms under- lying the interaction between ABA and GA during GT maturation remain largely unknown. Through transcriptome analyses, we identified a previously uncharacterized C2H2-type zinc finger gene, whose expression is regulated by GA and ABA, thus named GAZ （GA- AND ABA-RESPONSIVE ZINC FINGER）. Seedlings ectopically overexpressing GAZ （GAZ-OX） were sensitive to ABA and GA during MC formation, whereas GAZ-SRDX and RNAi seedlings displayed opposite phenotypes. In addition, our results indicated that GAZ was involved in the transcriptional regulation of ABA and GA homeostasis. In agreement with pre- vious studies that ABA and GA coordinate to control the timing of MC formation, we also confirmed the unique interplay between ABA and GA and identified factors and regulatory networks bridging the two hor- mone pathways during GT maturation of the Arabidopsis root.

＜正＞This paper is concerned with the structural design of domed roof, Gwang-Myeong cyclodrom. The concepts behind the 20,000m2 roof are a plan image of wheel of bicycle and making flow truss from finding road of water. It is made flow truss from guiding force flowing as a water flowing. And it is possible to domed action form the guarantee of stifmess due to making tensing and compression ring truss. It is the dome action that generates the geometric stiffness and is fundamental to its structural action in resisting gravity loads. Whole structural analysis is necessary to ensure the stability. Issues addressed in this paper include the decision of the roof shape, structural analysis of whole roof and detail analysis. The domed roof construction was completed in January 2005. The building was completed in December 2005.

The paper presents an experimental study on the behavior of axial loaded concrete-filled steel square-tube stub columns with high strength fiber reinforced concrete until *** specimens were tested to investigate the effect of high strength concrete on the load carrying capacity of the concrete-filled steel square-tube stub *** effect of the presence of steel fiber in high strength concrete which filled in the steel tube was also *** main parameters in the tests were:(1)the strength of concrete(30 Mpa and 100 Mpa),and(2)the use of reinforcing steel fiber in concrete(plain high strength concrete and steel fiber high strength concrete).The main purpose of these tests were three-step:(1)to describe a series of tests on composite stub columns,(2)to analyze the influence of several parameters,and(3)to compare the accuracy of the predictions by using the specifications in the code(ACI and EC4 etc.)for the design of high-strength composite *** results indicate that the high strength of concrete and use of steel-fiber in concrete had significant influence on both the axial compressive load capacity and the ductile of the steel square-tube stub columns.

Randomized controlled trial of sodium phosphate tablets vs polyethylene glycol solution for colonoscopy bowel cleansing

AIM:To compare efficacy,patient compliance,acceptability,satisfaction,safety,and adenoma detection rate of sodium phosphate tablets(NaP,CLICOLONTM)to a standard 4 L polyethylene glycol(PEG)solution for bowel cleansing for adults undergoing ***:In this multicenter,randomized,prospective,investigator-blind study,the relatively young(19-60years)healthy outpatients without comorbidity were randomly assigned to one of two *** colonoscopy were scheduled in the *** NaP group was asked to take 4 tablets,5 times the evening before and4 tablets,3 times early on the morning of the *** PEG group was asked to ingest 2 L of solution the evening before and 2 L early in the morning of the *** of bowel preparation was scored using the Boston bowel preparation ***:No significant differences were observed between the NaP group(n=158)and PEG group(n=162)in bowel cleansing quality(adequate preparation93.0%vs 92.6%,P=0.877),patient compliance(P=0.228),overall adverse events(63.3%vs 69.1%,P=0.269),or adenoma detection rate(34.8%vs 35.2%,P=0.944).Patient acceptability,satisfaction,and patient rating of taste were higher in the NaP group than in the PEG group(P<0.001).CONCLUSION:NaP tablets,compared with PEG solution,produced equivalent colon cleansing,did not cause more side effects,and had better patient acceptability and satisfaction in the relatively young(age<60years)healthy individuals without *** oral tablet formulation could make bowel preparation less burdensome,resulting in greater patient participation in screening programs.

Genevestigator analysis has indicated heat shock induction of transcripts for NADPH-thioredoxin reduc-tase, type C （NTRC） in the light. Here we show overexpression of NTRC in Arabidopsis （NTRC°E） resulting in enhanced tolerance to heat shock, whereas NTRC knockout mutant plants （ntrcl） exhibit a temperature sensitive phenotype. To investigate the underlying mechanism of this phenotype, we analyzed the protein＇s biochemical properties and protein structure. NTRC assembles into homopolymeric structures of varying complexity with functions as a disulfide reductase, a foldase chaperone, and as a holdase chaperone. The multiple functions of NTRC are closely correlated with protein structure.. Complexes of higher molecular weight （HMW） showed stronger activity as a holdase chaperone, while low molecular weight （LMW） species exhibited weaker holdase chaperone activity but stronger disulfide reductase and fol-dase chaperone activities. Heat shock converted LMW proteins into HMW complexes. Mutations of the two active site Cys residues of NTRC into Ser （C217/454S-NTRC） led to a complete inactivation of its disulfide reductase and foldase chaperone functions, but conferred only a slight decrease in its holdase chaperone function. The overexpression of the mutated C217/454S-NTRC provided Arabidopsis with a similar degree of thermotolerance compared with that of NTRC°E plants. However, after prolonged incubation under heat shock, NTRC°E plants tolerated the stress to a higher degree than C217/454S-NTRC°E plants. The results suggest that the heat shock-mediated holdase chaperone function of NTRC is responsible for the increased thermotolerance of Arabidopsis and the activity is significantly supported by NADPH.

Dramatic Improvement of Long Lasting Post-Inflammatory Hyperpigmentation by Oral and Topical Tranexamic Acid

Post-inflammatory hyperpigmentation is common problem, but its treatment still remains challenging. Tranexamic acid has been used to treat or prevent excessive bleeding loss in various medical conditions. There have been some reports of the effect of oral and topical tranexamic acid for treatment of pigmented disorder. Herein we report on a case of female patient who showed improvement of PIH after oral and topical tranexamic acid administration.
