Background Central blood pressure （BP） is pathophysiologically more important than peripheral BP for the pathogenesis of cardiovascular disease. Arterial stiffness is also a good predictor of cardiovascular morbidity and mortality. The effects of benidipine, a unique dual L-IT-type calcium channel blocker, on central BP have not been reported. This study aimed to compare the effect of benidipine and Iosartan on the central BP and arterial stiffness in mild to moderate essential hypertensives. Methods This 24 weeks, multi-center, open label, randomized, active drug comparative, parallel group study was designed as a non-inferiority study. The eligible patients （n=200） were randomly assigned to receive benidipine （n=101） or Iosartan （n=99）. Radial artery applanation tonometry and pulse wave analysis were used to measure the central BP, pulse wave velocity （PWV） and augmentation index （AIx）. We also measured the metabolic and inflammatory markers. Results After 24 weeks, the central BP decreased significantly from baseline by （16.8±14.0/10.5±9.2） mmHg （1 mmHg =0.133 kPa）（systolic/diastolic BP; P 〈0.001） in benidipine group and （18.9±14.7/12.1±10.2） mmHg （P 〈0.001） in Iosartan group respectively. Both benidipine and Iosartan groups significantly lowered peripheral BP （P 〈0.001） and AIx （P 〈0.05）, but there were no significant differences between the two groups. The mean aortic, brachial and femoral PWV did not change in both groups after 24-week treatment. There were no significant changes of the blood metabolic and inflammatory biomarkers in each group. Conclusion Benidipine is as effective as Iosartan in lowering the central and peripheral BP, and improving arterial stiffness.

Lymphangioma, a benign neoplasm of the lymphatic system, is common in children but rare in adults. Its clinical manifestations include abdominal pain, nausea, vomiting and a palpable mass. However, abdominal sonography or abdominal computed tomography （CT） scan can also incidentally reveal lymphangioma. A larger or symptomatic lymphangioma is treated with total resection to prevent recurrence, infection, torsion and enlargement. Although lymphangioma rarely becomes malignant, its prognosis is generally good. We report a cystic lymphangioma of the spleen and retroperitoneum, which was incidentally found in a 56-year-old man who was hospitalized due to a colon mass. Physical examination showed no specific findings. Abdominal CT revealed a 5.7 cm, non-enhanced multilobulated cystic mass with multiple sepia in the spleen and a 10 cm lobulated cystic mass in the paraaortic area. Splenectomy and retroperitoneal resection of the cystic mass were conducted. The endothelium of splenic and retroperitoneal cyst was immunohistochemically stained with D2-40 antibody. The patient was finally diagnosed with splenic cystic and retroperitoneal cavernous lymphangioma.

A Study on Transmission Loss Characteristics of Honeycomb Panel for Offshore Structures

Honeycomb panel is consisted of 3 layers that are double-faced sheets and honeycomb-shaped core. It is highly desirable for ship, railway, and aerospace industry. The reason is that honeycomb panel excels in strength and in its weight. However in terms of insulation, it is a little bit insufficient to commonly use sandwich-panel. In this paper, Moor’s theory is used to predict sound transmission loss (STL). The theory is assumed that core layer is homogeneous orthotropic. And to calculate STL, it is evaluated in terms of the symmetric and anti-symmetric panel impedances, and the characteristic impedance of air. After that predicted data are compared with experiment data.

Very High Cycle Fatigue Behaviors and Surface Crack Growth Mechanism of Hydrogen-Embrittled AISI 304 Stainless Steels

The influence of hydrogen embrittlement on the fatigue behaviors of AISI 304 stainless steel is investigated. The fatigue endurance limits of the untreated and hydrogen-embrittled materials were almost the same at 400 MPa, and hydrogen embrittlement had little influence even though the sample contained about 8.1 times more hydrogen. Thus, the sensitivity of hydrogen gas in this material is very low. A surface crack initiation, growth, coalescence, and micro ridge model is proposed in this study. Slip line formation?⇒microcrack formation?⇒increases in the crack width, and blunting of the crack tip as it grows?⇒formation of many slip lines because of deformation in the shear direction?⇒growth of the crack in the shear direction, forming micro ridges, coalescence with adjacent cracks ⇒?continuous initiation, growth, coalescence, and ridge formation of surface cracks and specimen breakage.

Treatment of Bone Defect with Modular Metal Augmentation Using a Downsized Block in Total Knee Arthroplasty: Technical Note and Report of 17 Cases

In total knee arthroplasty (TKA), a treatment of bone defect varies depending on the location and extent of defect and requires proper surgical procedure. Metal augmentation is readily available for both femoral and tibial bone defects. We report the operative technique of modular metal augmentation using a downsized block for ambiguous proximal tibial and distal femoral bone defects in primary and revision TKA. Regarding bone defects, bone loss can be minimized by using a different size of metal augmentation, and suitable reinforcement for bone defects can be achieved. Once our technique is properly used, it will be very helpful in treating bone defects.

Randomized clinical trial comparing fixed-time split dosing and split dosing of oral Picosulfate regimen for bowel preparation

AIM To compare the efficacy of fixed-time split dose and split dose of an oral sodium picosulfate for bowel *** This is study was prospective, randomized controlled study performed at a single Institution(2013-058). A total of 204 subjects were assigned to receive one of two sodium picosulfate regimens(i.e., fixed-time split or split) prior to colonoscopy. Main outcome measurements were bowel preparation quality and subject *** There was no statistical difference between the fixedtime split dose regimen group and the split dose regimen group(Ottawa score mean 2.57 ± 1.91 vs 2.80 ± 2.51, P = 0.457). Cecal intubation time and physician's satisfaction of inspection were not significantly different between the two groups(P = 0.428, P = 0.489). On subgroup analysis, for afternoon procedures, the fixed-time split dose regimen was equally effective as compared with the split dose regimen(Ottawa score mean 2.56 ± 1.78 vs 2.59 ± 2.27, P = 0.932). There was no difference in tolerability or compliance between the two groups. Nausea was 21.2% in the fixed-time split dose group and 14.3% in the split dose group(P = 0.136). Vomiting was 7.1% and 2.9%(P = 0.164), abdominal discomfort 7.1% and 4.8%(P = 0.484), dizziness 1% and 4.8%(P = 0.113), cold sweating 1% and 0%(P = 0.302) and palpitation 0% and 1%(P = 0.330), respectively. Sleep disturbance was two(2%) patients in the fixed-time split dose group and zero(0%) patient in the split dose preparation(P = 0.143) *** A fixed-time split dose regimen with sodium picosulfate is not inferior to a split dose regimen for bowel preparation and equally effective for afternoon colonoscopy.

AIM:To investigate the efficacy of hepatic arterial infusion chemotherapy(HAIC) using floxuridine(FUDR) in patients with advanced hepatocellular carcinoma(HCC) confined to the ***:Thirty-four patients who had advanced HCC with unresectability or unsuccessful previous therapy in the absence of extrahepatic metastasis were treated with intra-arterial FUDR chemotherapy at ourhospital between March 2005 and May *** the 34 patients,9 patients were classified as Child class C,and 18 patients had portal vein tumor thrombus(PVTT).One course of chemotherapy consisted of continuous infusion of FUDR(0.3 mg/kg during day 1-14) and dexamethasone(10 mg on day 1,4,7 and 11),and this treatment was repeated every 28 ***:Two patients(5.9%) displayed a complete response,and 12 patients(35.3%) had a partial *** tumor control rate was 61.8%.The median overall survival times were 15.3 mo,12.4 mo and 4.3 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0392).The progression-free survival was 12.9 mo,7.7 mo and 2.6 mo for the patients who were classified as Child class A,Child class B and Child class C,respectively(P = 0.0443).The cumulative survival differed significantly according to the Child-Pugh classification and the presence of *** addition to hepatic reserve capacity and PVTT,the extent of HCC was an independent factor in determining a poor *** most common adverse reactions to HAIC were mucositis,diarrhea and peptic ulcer disease,but most of these complications were improved by medical treatment and/or a delay of ***:The present study demonstrates that intra-arterial FUDR chemotherapy is a safe and effective treatment for advanced HCC that is recalcitrant to other therapeutic modalities,even in patients with advanced cirrhosis.

茶多糖是红茶中重要的活性成分之一,红茶多糖的生理活性与其组成及结构有关,而这种组成与结构受加工过程的影响,因此本论文研究不同的红茶加工过程对红茶多糖的影响。以鲜叶为原料进行相同的萎凋处理后,经不同揉捻程度和不同氧气浓度发酵加工处理,通过水提醇沉法得到红茶多糖(BTPS),分析不同加工方式红茶多糖的理化性质和三种体外抗氧化活性。实验结果如下:1.不同揉捻程度红茶多糖BTPS-R1(揉捻0h)、BTPS-R2(揉捻0.5h)、BTPS-R3(揉捻1h)、BTPS-R4(揉捻1.5h)及BTPS-R5(揉捻2h)的得率依次为1.17%、0.99%、1.07%、1.26%、1.21%,BTPS-R4的得率最高。5种BTPS的化学成分主要由蛋白、中性糖、糖醛酸和茶多酚组成,各BTPS单糖组分含有Man、Rha、Glc A、Gal A、Glc、Gal、Ara、Fuc。BTPS-R4红茶多糖的粒径和分子量显著降低,稳定性也最好。5种BTPS通过紫外光谱测定发现均含有蛋白质,且通过红外光谱扫描分析可知,各BTPS都含有共价结合蛋白复合物的官能图特征。在SEM低倍镜下观察,BTPS表面上呈片状结构或折叠和卷曲形状,在SEM高倍镜下观察,呈无规则颗粒状的聚集体。随着揉捻程度加深,BTPS-R1可能具有三螺旋结构,BTPS-R2、R3、R4、R5不具有三螺旋结构。2.不同氧气浓度发酵的红茶多糖BTPS-O1(氧气浓度5%)、BTPS-O2(氧气浓度21%)、BTPS-O3(氧气浓度40%)和BTPS-O4(氧气浓度60%)的得率次为1.17%、1.34%、1.65%、1.78%,BTPS-O4的得率最高。4种BTPS主要由四类物质组成(蛋白、中性糖、糖醛酸和茶多酚),其含量之和都大于80%。4种BTPS都含有8种单糖,其中Gal含量最高,Ara、Gal A和Glc含量较高,Fuc的含量比较低。氧气浓度达到21%以上时,可能使茶多糖分子更紧密地聚集,糖醛酸含量增加,因此氧发酵程度越高,BTPS粒径和分子量呈上升趋势。但不同氧气浓度发酵差异对稳定性没有影响。在SEM低倍镜下观察,BTPS表面是片状结构,在SEM高倍镜下观察,BTPS图片上显示颗粒状的聚集体。对不同氧气浓度发酵红茶原料中多糖高级结构进行分析,通过紫外光谱和红外光谱测定发现4种红茶多糖均含有蛋白质,具有共价结合蛋白复合物的官能图特征。不同氧气浓度发酵的4种BTPS都没有三螺旋结构。***具有优异的抗氧化能力,本实验主要通过三个抗氧化体系,不同揉捻程度BTPS的三种抗氧化活性能力依次为:BTPS-R5>BTPS-R4>BTPS-R3>BTPS-R2>BTPS-R1,不同氧气浓度发酵程度BTPS的三种抗氧化活性能力依次为:BTPS-O3>BTPS-O4>BTPS-O2>BTPS-O1。由于加工过程的不同,各BTPS的抗氧化能力存在着明显差异,糖醛酸含量、蛋白含量和茶多酚含量对BTPS的抗氧化有影响。Man、Glc A、Gal A、Glc、Ara和Fuc 6个单糖组成均为影响不同揉捻程度的BTPS抗氧化活性的重要单糖组成成分,Rha、Glc A和Ara均为影响不同氧气浓度发酵BTPS抗氧化活性的重要单糖组成成分。综上所述,红茶在揉捻和发酵加工过程中进行酶促氧化作用。化学成分含量剧烈变化,揉捻程度越来越高,抗氧化活性也越来越好,而在发酵过程中化学成分含量没有显著差异,氧气浓度发酵达到40%左右,抗氧化活性最强,氧气浓度发酵超过40%以上化学成分含量却下降,抗氧化活性效果也降低。

BRAF mutation in multiple primary cancer with colorectal cancer and stomach cancer

Aims:Recently,BRAF mutation testing has been introduced as a marker in differentiating Lynch syndrome from sporadic colorectal cancers or in predicting colorectal cancers with worse *** with hereditary predisposition to cancer development are at an increased risk of developing multiple primary *** purpose of this study is to identify mutation in the BRAF gene in multiple primary cancers with colorectal cancer and stomach ***:BRAF mutation was analysed in 45 patients with colorectal cancer and stomach cancer,synchronously or ***:Mean age was 64.07 years(range:47–83 years).For the colorectal cancer,tumors were located at the sigmoid colon in eight patients(17.8%)and at the rectum in 22 patients(48.9%).Twenty-three patients(51.1%)had synchronous *** patients(8.9%)had family members with *** mutation was identified in three patients(6.7%).All three of these patients had metachronous *** colorectal cancers were located in the sigmoid colon(1 patient)and the rectum(2 patients).Conclusions:BRAF mutation rate was low in the multiple primary cancer with colorectal cancer and stomach *** only BRAF gene study,it was not possible to identify any correlation with family history of colorectal *** study means considering other genes–MSI,MSH2,MLH1,MSH6.
